Objective To investigate the expression of Oct-4 as a stem cell marker in gastric carcinoma tissues and its clinical significance. Methods From June 1996 to March 2006, 63 paraffin samples of gastric carcinoma tissues were obtained, and the expression of Oct-4 was examined by immunohistochemical staining. Another 58 normal tissues adjacent to gastric carcinoma and 10 normal gastric mucosa tissues were served as controls. The clinieopathologieal data of 63 patients with gastric carcinoma were retrospectively analysed, and their relationship with the expression of Oct-4 was analysed. Fifty of these 63 patients were followed up for 8 years, Kaplan-Meier method and Log-rank teat were employed to explore the correlation between Oct-4 expression and survival, and Cox Regression analysis was performed to evaluate the possibility of Oct-4 expression as an independent prognostic factor for gastric carcinoma. Results The positive expression rate of Oct-4 in gastric carcinoma tissues was 80.95% (51/63), significantly higher than that of normal tissues adjacent to gastric carcinoma (5.2%, 3/58) and normal gastric mucosa tissues (0) (P < 0.01). The expression of Oct-4 was positively correlated to the clinical stage of gastric carcinoma(P<0.01). There was no significant difference in the expression of Oct-4 among patients with different differentiation, age, and gender (P>0.05). Patients with higher expression of Oct-4 had significantly shorter survival time (P<0.05). The expression of Oct-4 was not significantly correlated with the prognosis of gastric carcinoma(RR, 0.554; 95% CI, 0.209-1.466). Conclusion The expression of Oct-4 is correlated with the clinical stage and survival time of patients with gastric carcinoma, however, it is not an independent prognostic factor for gastric carcinoma.

Objective To evaluate four tumor markers of insulin-like growth factor 1((IGF-1), CEA, cytokeratin fragment antigen 21-1 (CYFRA21-1), neuron-specific enolase (NSE) for the diagnosis and prediction of treatment response in human lung cancer. Methods Serum samples were taken from three groups: 91 patients with lung cancer, 30 healthy adults and 15 patients with benign pulmonary diseases. Serum IGF-1 was assayed by radioimmunoassay and CEA, CYFRA21-1, and NSE by electrochemiluminescence immunoassay. The differences among the three groups were determined by Kruskal-Wallis one-way analysis of variance (ANOVA) and with Mann-Whitney rank-sum test. Diagnostic efficacy was evaluated by ROC curves. Results The four serum tumor marker levels were significantly higher in lung cancer group, as compared with the benign and the healthy (IGF-1:χ2=26.95,P<0.001, CEA:χ2=49.11,P<0.001; CYFRA21-1:χ2=40.63,P<0.001; NSE:χ2=14.76;P<0.001). The diagnostic sensitivities of IGF-1, CEA, CYFRA21-1 and NSE was 75.6% (34/45), 53.3% (24/45), 66.7% (30/45) and 42.2% (19/45) respectively for lung cancer. The diagnostic sensitivity of IGF-1 combined with CYFRA21-1 was 95.5 %( 43/45) and that of IGF-1 combined with CEA and CYFRA21-1was 97.8%(44/45). Only IGF-1 and CYFRA21-1 showed significant changes before and after treatment (IGF-1: χ2=5.99,P=0.014; CYFRA21-1:χ2=4.99, P=0.025) in cancer group. Conclusions Serum IGF-1, CEA, CYFRA21-1 and NSE are all valuable for lung cancer diagnosis and the combination of those parameters can enhance the diagnostic efficiency. Serum IGF-1 and CYFRA21-1 may also be useful for evaluating the treatment response in lung cancer.

Objective To learn the incidence of the inherited metabolic diseases in Beijing. Methods Urine samples were analyzed by gas chromatography-mass spectrometry(GC-MS)for inherited metabolic diseases in high risky infants in Beijing . Results Urine samples from 411 high risky infants were analyzed by gas chromatography-mass spectrometry. 269 cases (65.5%) were detected to have metabolic abnormalities, including 19 cases (4.6%) diagnosed of inherited metabolic diseases in which there were 15 cases of methylmalonic academia and 1 case each of propionic academia, hyperphenylalaninemia, urea cycle abnormality and pyroglutamic aciduria. There were 22 suspected cases (5.4%) of inherited metabolic diseases including 13 cases of lactic acidosis, 5 cases of primary glycerol aciduria, 4 cases of fatty acid metabolic disorders including 1 case each of Citrin defects, tyrosinemia, galactosemia 3-methylcrotonoyl coenzyme A carboxylase deifciency and maple syrup urine disease. There were also 228 cases (55.5%) of metabolic abnormalities, such as increasing urine levels of lactic acid, sucrose, lactose, galactose, N-acetyl tyrosine, succinic acid, dicarboxylic acid and abnormal serine/threonine ratio. Conclusions Methylmalonic academia might be the most common inherited metabolic diseases in high risky infants in Beijing. For infants with clinical manifestations but unclear etiology, GC-MS should be performed. MS-MS and gene analysis could be combined if necessary.
