1.Mental Health of Teachers and the Relationship Between Mental Health and Achievement Motivation

Objective: To study the relationship between mental health and achievement motivation of teachers.Methods: 322 high school teachers completed SCL-90 and Achievement Motivation Scale.Results: The percent of high-score teachers with at least one SCL-90 subscale scored more than 3 is more than 5%.There is obvious correlation between the score of SCL-90 and the AMS's.Conclusion: The high level of achievement-seeking or the high level of failure-avoiding is harmful to the mental health of teachers.
2.Variation and clinical signiifcance of serum galectin-3 in children with chronic heart failure

Objective To explore the variation of serum galectin-3 and its correlation with ventricular remodeling in children with chronic cardiac failure (CHF). Methods Forty-ifve children with CHF were included and divided into cardiac function II group (n=10 ), III group (n=18 ), and IV group (n=17 ) according to the severity of CHF. The subjects were also divided into endocardial fibroelastosis (EFE) group (n=21 ) and dilated cardiomyopathy (DCM) group (n=24 ) according to primary disease. Thirty health children were included as control group. The level of serum galectin-3 was detected by ELISA. The level of serum NT-pro BNP was measured by radio immunoassay. The index of ventricular remodeling was detected by ultrasonic cardiogram. The correlation of the level of serum galectin-3 with ventricular remodeling and the level of serum NT-pro BNP were analyzed. Results In 45 children ( 19 males and 26 females) with CHF, the mean age was 3 . 42 ± 1 . 89 years. The levels of serum galectin-3 and NT-pro BNP were higher in cardiac function II group, III group, and IV group than those in control group (all P0 . 05 ) and signiifcant differences in the levels also existed between cardiac function II group, III group, and IV group (P all?0 . 05 ). The level of serum galectin-3 was not different between EFE group and DCM group (P>?0 . 05 ). Spearman rank correlation analysis showed that the level of serum galectin-3 was positively correlated with the left ventricular end diastolic diameter, the left ventricular mass, the left ventricular mass fraction, and the level of serum NT-pro BNP (all P?0 . 05 ). Meanwhile the level of serum galectin-3 was negatively correlated with the left ventricular ejection fraction and the left ventricular fractional shortening (all P?0 . 05 ). Conclusion Serum galectin-3 may be helpful in clinical diagnosis and assessment of CHF in children.
3.Effect of two different suture methods on the degree of pain and corneal epithelium healing condition in pterygium excision combined with autologous conjunctival flap graft transplantation

AIM: To assess the effect of two different suture methods on the degree of pain and corneal epithelium healing condition after pterygium excision combined with autologous conjunctival flap graft transplantation. ·METHODS: Retrospective case-series study. According to the suture method, a total of 92 patients (92 eyes) with pterygium who received treatment in the First Affiliated Hospital of Huzhou University from June 2015 to June 2016 were divided into two group. There were 48 patients ( 48 eyes) in Group A were received intermittent suture, and 44 patients ( 44 eyes ) in Group B were received continuous interlocking suture. The degree of pain after surgery were evaluated between the two groups at 2h, 1d and 1wk after surgery by visual analogue score ( VAS). The healing status of corneal epithelium were observed between the two groups at 1d and 1wk after surgery by fluorescent staining. ·RESULTS: There was no significant difference in the average pain value 2h after surgery between Group A and Group B (P>0. 05). The average pain values 1d and 1wk after surgery in Group B was lower than that in Group A respectively (P<0. 01). Mean scores of corneal epithelium healing condition at 1wk were significantly better in Group B than in Group A (0. 54 ± 0. 32 vs 0. 86 ± 0. 34, P<0. 05), while not significantly different at 1d after the surgery (4. 04±1. 46 vs 4. 30±1. 42, P>0. 05). · CONCLUSION: Compared to intermittent suture, continuous interlocking suture can release pain response after pterygium excision combined with autologous conjunctival flap graft transplantation and promote the healing status of corneal epithelium.
4.Thrombolytic therapy for treatment of Moyamoya disease and acute cerebral infarction: a successful case report

Moyamoya disease is a chronic progressive cerebrovascular disease. It got its nickname, the moyamoya disease, because the image of abnormal blood vascular net at the skull base in the encephaloangiography of such patients is very similar to smog. The moyamoya disease can be divided into two types: ischemic and hemorrhagic according to its clinical manifestations and imaging characteristics. In the ischemic type of moyamoya disease, when the blood vascular lumens are not narrow enough to completely block the blood flow, the chief manifestation is the cerebral blood circulation insufficiency; when the salvage is not in time, the disease will further progress to develop vascular obstruction or thrombosis, resulting in cerebral infarction, finally hemiplegia, aphasia, etc. irreversible symptoms and signs occur. In the primary hospital, the first choice for treatment of acute cerebral infarction (ACI) is drug thrombolysis. However, the main treatment for moyamoya disease with ACI is chiefly revascularization. This article reported a patient with moyamoya disease and ACI successfully treated by intravenous reteplase for thrombolysis in People's Hospital of Pucheng County.
5. Plasma concentration of 5-fluorouracil predicts the efficacy and safety of capecitabine plus cisplatin as first-line chemotherapy in patients with advanced gastric cancer: A single-center clinical trial

Objective: To assess the value of plasma concentration of 5-FU (5-fluorouracil) predicting the efficacy and safety in patients with advanced gastric cancer receiving capecitabine plus cisplatin as first-line chemotherapy. Methods: The clinical data from 113 advanced gastric cancer patients receiving capecitabine plus cisplatin in Shanghai First People's Hospital from July 2007 to December 2010 were retrospectively collected. The plasma concentration of 5-FU was examined at each cycle of chemotherapy. The relationships between plasma concentration of 5-FU and the OS (overall survival), PFS (progression-free survival) and RR (response rate) were determined by statistical analysis. Results: The mean plasma concentration of 5-FU was 25.8 μg/L. The patients were divided into group A (plasma concentration of 5-FU > 25 μg/L, n = 59) and group B (plasma concentration of 5-FU < 25 μg/L, n = 54). The median OS time of group A and group B were 14.4 and 9.6 months, respectively (hazard ratio = 0.36, P = 0.000). The median PFS time of group A and group B were 8.0 and 4.9 months, respectively (hazard ratio = 0.37, P = 0.000). The RR of group A and group B were 55.9% and 33.3%, respectively (P = 0.023). All toxicities occurred more frequently in group A than in group B. The grade III/IV mocositis and hand-foot syndrome were significantly increased in group A as compared with group B. Conclusion: For advanced gastric cancer, high plasma concentration of 5-FU might predict efficacy and toxicities of capecitabine plus cisplatin. Copyright © 2013 by TUMOR.
6. The relationship between the dose of fluorouracil and its plasma concentration and the survival of patients with advanced colorectal cancer

Objective: To investigate the relationship between 5-FU (5-fluorouracil) dose (based on body surface area or not) and its plasma concentration and the overall survival of patients with advanced colorectal cancer receiving 5-FU-based chemotherapy regimens as standard first- and second-line treatment. Methods: One hundred and ninety-two patients with advanced colorectal cancer received 5-FU-based chemotherapy regimens of FOLFOX (5-FU, calcium folinate and oxaliplatin) and FOLFIRI (5-FU, calcium folinate and irinotecan ) as first-line and second-line chemotherapy, respectively or in a reverse sequence. Plasma concentration of 5-FU at steady state was examined when the dose of 5-FU was adjusted according to the toxicity in each patient. Results: The plasma concentration of 5-FU was not associated with the dose of continuous infusion of 5-FU (r = 0.066, P = 0.146), and also not associated with the dose of infusional 5-FU based on body surface area (r = 0.056, P = 0.217). Median overall survival was 31.4 months for patients with high plasma concentration of 5-FU (> 30 μg/L) versus 14.5 months for patients with low plasma concentration of 5-FU (< 30 μg/L) (P = 0.001). The adverse effects including grades 3-4 neutropenia and hand foot syndrome were more frequent in patients with high plasma concentration of 5-FU. Conclusion: The monitoring of plasma concentration of 5-FU may be a basis for dose adjustment of 5-FU in safe and effective range for patients with advanced colorectal cancer. 7. Comparison of the 7th and 6th edition AJCC TNM staging systems for gastric cancer: Analysis of prognosis in a retrospective review of 918 patients at a single institution in China

Objective: To evaluate the prognostic priority of the 7th edition AJCC (American Joint Committee on Cancer) TNM staging system for gastric carcinoma compared with the 6th edition in Chinese population. Methods: A total of 918 patients with gastric carcinoma from a single institution between January 2003 and December 2008 in China were staged using the 6th edition and the 7th edition AJCC TNM staging systems. The univariate and multivariate analyses were applied to evaluate the value of the two staging systems in prognostic prediction for gastric carcinoma. The AIC (Akaike's Information Criterion) was used to determine which is better between the two staging systems. Results: Significant differences in overall survival were observed according to the T, N and M substages using the 7th edition AJCC TNM staging system. The Kaplan-Meier survival curves showed a good discriminatory ability among stages I through IV in both the 6th and 7th edition staging systems (P = 0.000). There were similar survival curves between substages IA and IB (P = 0.643), IB and IIA (P = 0.267), IIB and IIIA (P = 0.534), IIIA and IIIB (P = 0.124), and IIIB and IIIC (P = 0.174) when the patients were classified into eight substages according to the 7th edition AJCC TNM staging system. The AIC value of the 7th edition staging system was smaller than the 6th edition staging system. Conclusion: The 7th edition AJCC TNM staging system is superior to the 6th edition staging system. But the advantage of the new 7th edition staging system is very limited and it presents some deficiencies. Copyright © 2012 by TUMOR.
9.Invasive Pulmonary Aspergillosis in Hemopathy Patients: Its Diagnosis and Treatment

OBJECTIVE To assess the clinical characters,diagnosis and treatment of invasive pulmonary(aspergillosis) in hemopathy patients.METHODS The hospitalized patients with hemopathy from Jul 2004 to Jun 2005 was retrospectivly analyzed.RESULTS Among 155 hemopathy cases,3 cases developed invasive pulmonary(aspergillosis),from them 2 died and 1 cured.CONCLUSIONS Overimmnosuppression is one of the main risk factors for invasive pulmonary aspergillosis.Amphotericin B is still the best choice for the treatment of aspergillosis and its gradual,low concentration administration can ease the side effects.
10.Urgent reoperation early after coronary artery bypass graft

Objective To explore the cause of acute myocardial ischemia early after coronary artery bapass graft (CABG) and surgical management on it. Methods From 2001 to 2009, 28 patients underwent urgent reoperation early after CABG due to acute myocardial ischemia. The incidence of reoperation is about 0.02%. The cause of reoperation inclouded early graft occlusion (10 cases) ,IMA damage or injury during harvesting(9 cases), inexactitude distal anastomosis(2 cases)and radial artery spasm(4 cases). The mean interval time between two operations was 8 hours. Reoperation was done under offpump bypass in 2 patients and on-bypass used in other patients. Unsatisfactoey graft were substituted with new graft material and thrombotic was removed. If LIMA was the reson for myocardial ischemia, an additional vein graft was inserted. The spasm radial artery were substituted with new vein graft. Completely revascularization was used in re-do CABG. Results Two patients died during reoperation. 8 patients was died between 1 day and 14 days after reoperation. IABP was used in 16 patients,which 2 patients received ECMO suppord and 2 patient received LVAD suppord at mean time. Conclusion There have very high mortality in acute myocardial ischemia early after CABG. The early diagnosis and correct surgical management can improve the rates of survival. The active prevent should be emphasized during the first CABG.
