Objective: To study the relationship between the uncomfortable symptoms and psychological state of post CABG(Coronary Artery Bypass Grafting) patients.Methods: 65 post CABG patients answered the questionaires(Symptom Inventory,Self-Rating Anxiety Scale,Self-Rating Depressing Scale).Results: The Symptom Inventory has positive correlation with the total standard points of SAS and SDS.The higher the standard scores the worse the patient's psychological state.Conclusion: Use of proper health education,psychological care and drugs can release the patients of their uncomfortable symptoms.
2.Significance of HIF-1α, MVD and EGFR expression in detection of nasal inverted papilloma and clinical effect of taxol

Objective:To explore the significance of the expression of HIF-1α,MVD and EGFR on diagnostic of nasal inverted papilloma and the clinical effect of taxol .Methods:The expression of HIF-1α,MVD and EGFR were detected by immunohistochemistry in nasal inverted papilloma .Results:The expression of HIF-1α,MVD and EGFR were enhanced with the degree of atypical hyperplasia in epithelium.The research also showed that the expression of HIF-1α,MVD and EGFR were increased dramatically in severe atypical hyperplasia with a significant statistical differences ( P<0.01 ) .The expression of EGFR and MVD showed a positive correlation with HIF-1α.The administration of Taxol alleviated the enhanced expression of HIF-1α, MVD and EGFR with a significant statistical difference(P<0.01),but with on statistical difference with moderately severe tissue (P>0.05).Conclusion:HIF-1α,MVD and EGFR are involved in the pathogenesis and deteriorate of nasal inverted papilloma and can be taken as biomarkers for the diagnosis of the dete -rioration of nasal inverted papilloma .
3.Effects of diammonium glycyrrhizinate on the expression of Toll-like receptor 4 in rat's kidney with acute kidney injury induced by paraquat

Objective To investigate the effects of diammonium glycyrrhizinate (DG) on the expression of Toll-like receptor 4 in rat' s kidney poisoned by paraquat (PQ).Methods The PQ (100 mg/kg) were administered via a gastric tube to make an animal model of acute kidney injury in rats.Then DG (50 mg/kg per 24 h) were administered immediately by intraperitoneal injection in the DG group.The ELISA assay was applied to measuring the IL-6 and IL-10 in kidneys and in blood.And the protein expressions of TLR-4, Myd88 and NF-κB P65 were examined by Immunohistochemistry.The gene expressions of TLR-4, Myd88 and NF-κB on kidneys were detected by RT-PCR.Results The levels of serum Cr, BUN , IL-6 and renal IL-6 in PQ group were significantly higher than those in NS group (F =266.014, 168.160, 63.279, 192.997, respectively, P <0.01).While, the levels of serum Cr, BUN , IL-6 and renal IL-6 in DG group were significantly lower than those in PQ group (F =266.014, 168.160, 63.279, 192.997, respectively, P < 0.01).And the levels of serum and renal IL-10 in PQ group were higher than those in NS group (F =87.511, 79.473, respectively, P < 0.01).But the levels of serum and renal IL-10 in DG group were lower than those in PQ group (F =87.511, 79.473, respectively, P <0.01).The results of PCR confirmed that the expressions of TLR-4 mRNA , myd88 mRNA and NF-κB mRNA in PQ group were higher than those in NS group (F =84.408, 60.683, 86.272, respectively, P <0.01), while, the expressions of TLR-4 mRNA , myd88 mRNA and NF-κB mRNA in DG group were lower than those in PQ group (F =84.408, 60.683, 86.272, respectively, P < 0.01).The protein levels of TLR-4 , Myd88 and NF-κB P65 in PQ group were higher than those in NS group (F =79.139, 61.799,112.740, respectively, P <0.01).While the protein levels of TLR-4, Myd88 and NF-κB P65 in DG group were lower than those in PQ group (F =79.139, 61.799, 112.740, respectively, P < 0.01).Conclusions Diammonium Glycyrrhizinate can attenuate rat's acute kidney injury caused by paraquat poisoning and decrease the expressions of TLR-4, Myd88 and NF-κB.
4.Analysis of risk factors for postoperative nausea and vomiting in spinal anesthesia patients

Objective To investigate the risk factors for postoperative nausea and vomiting (PONV) in spinal anesthesia patients.Methods A total of 841 patients received spinal anesthesia were visited after operation.Data were analyzed using univariate analysis and multivariate Logistic regression to identify risk factors related to PONV.Results PONV occurred in 94 patients (11.2%,94/841).Univariate analysis showed that PONV was unrelated with gender,age,ASA classification,anesthesia mode (P > 0.05),related with operation department (P =0.026),body mass index (P =0.020),education level (P =0.000),history of previous surgery anesthesia (P =0.005),history of PONV (P =0.000),history of kinesia (P =0.002),smoke (P =0.019),intraoperative using of tramadol (P =0.018).Multivariate analysis showed that operation department (OR =4.039,95% CI 1.331-12.259,P=0.048),education level (OR =3.504,95% CI 1486-8.260,P=0.015),history of PONV (OR =5.113,95% CI 1.790-14.606,P =0.002),intraoperative using of tramadol (OR =5.316,95% CI 1.091-25.908,P =0.039) were identified as independent risk factors for PONV.Conclusions The independent factors associated with PONV following spinal anesthesia include operation department,education level,history of PONV,intraoperative using of tramadol.Identifying patients who are at high risk for PONV will enable the formation of more timely management project.
5.A model of connective tissue disease-associated pulmonary arterial hypertension in rat

ObjectiveTo investigate the possibility of establishing a model of connective tissue disease-associated pulmonary arterial hypertension (PAH) in rats.MethodsPAH was induced by a single intraperitoneal injection of monocrotaline solution at a dose of 50 mg/kg.The rats of the control group were injected the same volume of 2:8 ethanol saline.After 2 and 4 weeks,a polyvinyl catheter was inserted into the pulmonary artery,and the hemodynamic variables were monitored continuously by Maclab/8 s.The pulmonary vascular pathologic remodelling was examined with hematoxylin and lectin perfusion.Thickness and area indices were calculated.ResultsFour weeks after intra-peritoneal injection of monocrotaline,the systolic,diastolic,and mean pulmonary arterial pressures [(41 ±6),(24.3±3.8),(29.8±4.2) mm Hg] were obviously increased in the experimental group [(23 ±3),(8.5 ±2.4),( 17.1 ±2.5) mm Hg] (P<0.05) when compared with those of the control groups.Histological examination showed that the thickness of arterial wall increased,the lumen became narrowed,and the thickness and area index(TI:0.723±0.034 vs 0.314±0.023,AI:0.912±0.203 vs 0.414±0.021 ) increased in small pulmonary arteries(P<0.05).ConclusionThe model of connective tissue disease-associated pulmonary hypertension could be established by intraperitoneal injection of monocrotaline solution.The model has been demonstratedthe characteristic morphologic changes in the pulmonary vasculature.This method may provide an economic,easy,and stable animal model for the study of the mechanisms of pulmonary vascular remodeling in connec-tive tissue disease-associated pulmonary hypertension.
6.The clinical feature analysis of acute ischemic bowl disease

Objective To explore the clinical features of acute ischemic bowl disease in order to guide clinical treatment and avoid the severe complications.Methods 25 cases diagnosed as ischemic bowl disease were enrolled retrospectivly analysed the clinical features of symptoms,signs,laboratory test results,abbominal enhanced CT and CTA,enteroscopes of these patients.Results Among the 25 cases accorrding to first presentation of first contacts,the cardinal symptoms were spectively abdomial pain 20 (80%),abdomial distension 16 (64%),diarrhea 18 (72%),vomiting 13 (52%),hemafecia 6 (24%),bloody purulent stool 8 (32%),watery stool 7 (28%),fever 11 (44%) and physical signs were spectively local tenderness 12(48%),peritonitis sign 9(36%),active bowl sound 7 (28%),weak or disappeared bowl sound 5 (20%).22 of 25 cases were positive with ocult blood test of stool and 23 of 25 cases showed elevated D-dimer concentration(more than 500μg/L) within 24 hours after first contacts.All the 25 cases were dignosed with CTA and 1 case was performed with enteroscopy which showed that local mucosa of sigmoid colon was congestive,edema,submucosal extravasated blood and some part was bleeding.Conclusion The patient with high risk factors who suffered from the tetralogy of severe abdominal pain,intense evacuation symptoms,highly elevated D-dimer concentration and positive ocult blood test,is stongly suggested to be a ischemic bowl disease and should be performed the abdomial CTA or DSA examination in time to avoid missing the golden opportunity to cure.
7.Establishment of mice breast cancer model in BALB/c mice with 4T1 cells

Objective:To establish a model of mice breast cancer of 4T1 cells in BALB/c mice and choose the best model making method.Methods:Ninety mice were divided into three groups randomly ,with 30 in each group injected by 4T1 cells suspension of 1×106 ml-1 ,1×107 ml-1 ,1×108 ml-1 respectively.Each group of mice were randomly divided into two groups which were inoculated on the chest wall and lateral abdominal wall respectively.Tumor formation time ,tumor growth rate and the 8 week survival rate in each group were compared ,and pathological character was observed by C-erbB-2 immunohistochemistry staining.Results:Tumor growth rate in cells suspension of 1×107 ml-1 was high and grow steadily.Tumor growth rate wasn′t correlative with 4T1 cells injection in different parts.The result of C-erbB-2 immunohistochemistry staining was positive.Conclusion: Injection of 4T1 cells suspension with 1 ×107 ml-1 is the best way to male tumor model in three suspension.
8.Determination of optimal left ventricular pacing site for cardiac resynchronization therapy by tissue Doppler imaging

Objective To investigate whether the left ventricular delayed contraction site determined by tissue Doppler imaging might be an optimal left ventricular lead position for improved outcomes of cardiac resynchronization therapy (CRT) in patients with non-ischemic cardiomyopathy. Methods Thirty-three patients subjected to CRT were selected, and all were performed conventional ultrasound cardiography and tissue Doppler examinations before operation. The left ventricular delayed contraction site was determined according to the interval between the onset of QRS and the peak systolic velocity. Retrograde coronary venography was performed during operation, and the left ventricular lead site was selected according to the left ventricular delayed contraction site determined by tissue Doppler examination before operation. The coronary sinus lead site was determined under the guidance of X ray of dorsaventral, lateral, right anterior oblique and left anterior oblique positions. Patients were divided into group A(n=20, the left ventricular lead site was in line with the delayed contraction site) and group B (n=13, the left ventricular lead site was not in line with the delayed contraction site). Results There was no significant difference in age, NYHA grading, left ventricular end-systolic volume(LVESV), left ventricular ejection fraction(LVEF), pulmonary arterial systolic pressure, QRS width and Ts-SD between the two groups before operation(P> 0.05). Six months after CRT, there was no significant difference in NYHA grading, LVESV and mitral regurgitation(MR) grading between the two groups(P>0.05), while the increase in LVEF and decrease in LVESV of group A were more significant than those of group B (P<0.01). Conclusion In patients with non-ischemic cardiomyopathy, CRT significantly improves left ventricular performance, and the more favourable outcomes are achieved in those pace at the delayed contraction site. Tissue Doppler imaging may help to guide the implant of left ventricular lead.
9.Isotretinoin erythromycin gel in the treatment of acne vulgaris: a multicenter randomized parallel-controlled clinical study

Objective To compare the clinical efficacy and safety of isotretinoin erythromycin gel, a gel containing isotretinoin (0.05%) and erythromycin (2%), versus adapalene gel in the treatment of mild to moderate acne vulgaris. Methods A multicenter, randomized, open, parallel-controlled clinical study was conducted. A total of 192 patients with mild to moderate (Grade Ⅰ -Ⅲ ) acne vulgaris were enrolled in this study according to the grading criteria for acne severity in guidelines for the treatment of acne in China. Efficacy analysis was carried out in 169 patients and safety analysis in 190 patients. The patients were classified into trial group (n = 86) and control group (n = 83 ) to be treated with isotretinoin erythromycin gel or adapalene gel once a night for 6 weeks. Patients were evaluated at the baseline, on week 2, 4 and 6 during the treatment for the count of comedones (both open and closed), inflammatory papules and pustules, severity of acne and local or general adverse effects. Results After the start of treatment, the response rate gradually increased and severity of acne decreased in both groups. On week 6, the total response rate was 51.16% in the trial group and 40.96% in the control group (P > 0.05), while a greater reduction in the count of pustules and inflammatory lesions was observed on week 4 and 6 in the trial group with a lower severity grade of acne compared with the control group (P < 0.05 or 0.01 ). Adverse reactions were similar in both groups and manifested as tolerable local irritation. Conclusions The efficacy of isotretinoin erythromycin gel is similar to that of adapalene gel in the treatment of mild to moderate acne vulgaris, however, isotretinoin erythromycin gel seems superior to adapalene gel in reducing inflammatory lesions and rapidly improving severity of acne vulgaris.
10.Characteristics of long-term survivors on peritoneal dialysis

ObjectiveTo evaluate the characteristics of patients on long-term peritoneal dialysis (PD). MethodsPatients who started PD since 1994 and received PD for at least one year were included in this study. According to dialysis duration, patients were divided into two groups. Group A (long-term) was defined as patients survived on PD for more than 5 years. Group B (short-term) was defined as patients who died or switched to bemodialysis within less than 5 years. Demography, biochemical indexes, dialysis prescription and adequacy were compared between two groups. ResultsThere were 68 patients in group A and 98 patients in group B. Mean followed-up period of group A and B was (84.80±19.42) months and (27.25±12.31) months, respectively. Younger, fewer episodes of diabetic comorbidity (group A 3/68 vs group B 18/98, P <0.05) and coronary heart disease (group A 6/68 vs group B 22/98, P<0.05) were found in group A. Compared to group B, the level of serum albumin at the beginning of PD was much higher in group A [(35.56±4.74) g/L vs (33.69±5.45) g/L, P<0.01). The levels of blood sugar, TC, TG, hemoglobin, calcium, phosphate and iPTH were not significantly different between two groups. Estimated GFR, renal Kt/V and renal Ccr at the beginning of dialysis were much higher in group A, however there was no significant difference in urinary volume between two groups. Both estimated GFR and urinary volume decreased more slowly in group A compared to group B. Peritonitis mobidity was lower in group A (1/81.22 months vs 1/29.03 months, P<0.01). Conclusions In comparison to short-term survivors, long-term PD patients are characterized by being younger, less diabetic and coronary heart disease, fewer episodes of peritonitis, higher level of serum albumin, higher estimated GFR and less loss of residual renal function.
