1.Systematic review and meta-analysis of influencing factors for poor prognosis of coronavirus disease 2019 in people living with human immunodeficiency virus

Objective:To investigate the clinical features and their relationship with mortality of coronavirus disease 2019 (COVID-19) in people living with human immunodeficiency virus (HIV).Methods:A thorough literature review was conducted about peer-reviewed publications including cohort study, cross-sectional research, and case series on HIV/2019 novel coronavirus (2019-nCoV) coinfection from January to August 2020. Systemic review and meta-analysis were used to investigate the correlation between mortality and clinical features including age, comorbidities, CD4 + T lymphocyte count, HIV RNA level, and anti-retroviral therapy. Stata 15.0 software was used for meta-analysis. Results:Twenty-four articles were included with a total of 939 HIV/2019-nCoV coinfected patients. Overall mortality was 10.3% (97/939). Advanced age and comorbidities, including hypertension, diabetes mellitus, renal insufficiency, chronic obstructive pulmonary disease/asthma and tumor were all significantly associated with mortality (95% confidence interval 0.005-0.050, 0.042-2.294, 0.390-2.754, 0.513-2.848, 0.348-3.743 and 1.943-7.101, respectively, P=0.021, 0.043, 0.012, 0.008, 0.022 and 0.005, respectively). There were no correlations between mortality and CD4 + T lymphocyte count <200/μL or >500/μL, HIV RNA was below the lower limit of detection, or anti-retroviral drug (including tenofovir) (all P>0.05). Conclusions:The overall prognosis of COVID-19 in people living with HIV is similar to general population. Increased mortality correlates with advanced age and comorbidities including hypertension, diabetes mellitus, renal insufficiency, chronic obstructive pulmonary disease/asthma and tumor. Low CD4 + T lymphocyte count does not affect mortality. Preliminary results indicate that anti-retroviral drugs have no protective effect on COVID-19.
2.Miccoli thyroidectomy

Objective To explore the feasibility and a dv antages of video-assisted minimally invasive thyroidectomy by using the ultrason ic scalpel. Methods A total of 14 patients underwent cervical mini-incision video-assisted minimally invasive partial or subtotal thyroidectom y by using the ultrasonic scalpel. Results All the operations were successfully completed, without conversions to open surgery. The operation time was 30~70 min (mean, 45 min). Postoperative pathological findings showed no dular goiter. All the patients recovered uneventfully, without wound infection o r subcutaneous effusion. The cervical incision was 2.5 cm or so in length. Follo w-up for 3~17 months revealed barely visible scars in the 14 patients. Conclusions Miccoli thyroidectomy is simple to perform and offers excel lent cosmetic results.
3.Manipulative reduction and percutaneous pin fixation for the treatment of severely displaced distal tibial fractures in children.

OBJECTIVETo study the therapeutic efficacy of manipulative reduction and percutaneous pin fixation for the treatment of severely displaced distal tibial fractures in children.
METHODSFrom May 2009 to December 2012,56 children with severely displaced distal tibial fractures were analyzed, who had been treated with manipulative reduction and percutaneous pin fixation. Preoperative fractures were confirmed as severely displaced fractures by X-ray apparatus. There were 33 boys and 23 girls,ranging in age from 3 to 14 years, with an average of 10.1 years. All the fractures were closed without vascular or nerve injuries. According to the bone fracture type, under C-arm fluoroscopy, functional reduction was achieved by manipulative reduction. The fractures were fixed with percutaneous pins. Postoperative X-ray confirmed the functional reduction. Follow-up indexes were recorded: intra-operative and postoperative complications,postoperative radiographic examination, lower extremity length and range of ankle motion. Ankle score system of Teeny was used to evaluate ankle function.
RESULTSAll the patients were followed up, and the duration ranged from 3 to 46 months, with an average duration of 19.4 months. According to the Teeny score standard, 35 patients got an excellent result, 7 good and 3 fair. Pin track reaction was found in 4 cases. The X-ray showed all the fractures healed without shortening deformity or epiphyseal arrest at early stage. All the patients could participate in the normal physical activities, having a normal range of motion and excellent strength of the ankle joint.
CONCLUSIONThe method of manipulative reduction and percutaneous pin fixation is a safe and convenient treatment for severely displaced distal tibial fractures in children. It has several advantages as follow: micro-trauma, tiny tissue damage, firm fixation, and the patients can exercise the function of ankle early, suggesting that it is an effective treatment method.

4.Treatment type C fracture of the distal radius with locking compression plate and external fixators.

OBJECTIVETo compare efficacy of unilateral external fixators and locking compression plates in treating type C fractures of the distal radius.
METHODSFrom January 2009 to June 2010, 76 patients with distal radius fracture were treated with LCP and external fixators, 54 patients were followed up. Among them, 29 cases were male and 25 cases were female with an average age of 45.31 (ranged, 24 to 68) years old. There were 29 patients in LCP group. According to AO classification, 8 cases were type C1, 7 cases were type C2 and 14 cases were type C3. There were 25 cases in external fixators group. According to AO classification, 6 cases were type C1, 8 cases were type C2 and 11 cases were type C3. Radial height, volar tilt and radial inclination were compared, advanced Gartland-Werley scoring were used to assessed wrist joint function after 6 and 12 months' following up.
RESULTSTwo cases were suffered from nail infection in external fixators group. Fifty-four patients were followed up from 12 to 24 months with an average of 21.3 months. Radial height was (9.60 +/- 0.72) mm, volar tilt was (9.55 +/- 0.80) degrees and radial inclination was (21.40 +/- 0.78) degrees in LCP group,while those were (9.40 +/- 0.70) mm, (9.47 +/- 0.71) degrees and (21.20 +/- 0.73) degrees in external fixtors group, and with no statistical significance (P>0.05). Advanced Gartland-Werley score after 6 months' following up was 3.31 +/- 1.17 in LCP group, 5.56 +/- 1.58 in external fixtors group, and with significant difference (t=-5.99,P<0.05); after 12 months' following up, advanced Gartland-Werley score was respectively 2.66 +/- 1.01 and 3.08 +/- 1.00, but with no statistical meaning (t=-1.55, P>0.05).
CONCLUSIONLCP and external fixtors can receive good curative effects in treating type C distal radius fracture, and LCP can obtain obviously short-term efficacy, while there is no significant difference between two groups in long-term results. For serious distal radius comminuted fracture which unable to plate internal fixation, external fixators is a better choice.

5.Evaluation of the performance of a DNA microarray method for detecting HBV mutations conferring antiviral resistance to lamivudine and adefovir

Objective To evaluate the performance of a DNA microarray method for detecting HBV antiviral drug-resistant mutations. Methods Two hundred and twenty four serum samples from patients with CHB were tested in parallel by DNA microarray and direct sequencing for the mutations within the HBV reverse transcriptase (rt) region, which included rtL180, rtA181, rtM204 and rtN236. Samples with discrepant results were retested by clonal sequencing. Results Complete concordance between DNA microarray and direct sequencing results was observed in 214 out of 224 samples (95. 5% ). The presence of mixed viral populations in the other 10 samples detected by DNA microarray but not by direct sequencing was confirmed by clonal analysis. The DNA microarray could detect minor viral populations which constituted 5.0%-15. 0% of the total viral load. Conclusion DNA microarray is highly consistent with direct sequencing in detecting HBV mutations conferring drug resistance and more sensitive in detecting mixed mutant and wild-type sequences than direct sequencing, which makes it a useful tool for early detection of drug resistance early.

7.Analysis of survey results of iodine deficiency disorders in Huai'an City Jiangsu Province in 2011

Objective To evaluate the status of iodine deficiency disorders(IDD) in Huai'an City in 2011 and to provide a scientific basis for adopting target prevention strategies timely and adjusting the intervention policy scientifically.Methods According to The National Project of Surveillance on IDD,probability sampling method (PPS) was used to select 30 counties in Huai'an City,and one primary school was selected randomly from each county,then 40 children aged 8 to 10 were randomly selected in each school.Children's thyroids were examined by palpation and the iodine in salt samples collected in their home was tested by direct titration.At the same time,urine samples from 12 children of the 40 selected children were collected and urinary iodine was tested by As(Ⅲ)-Ce4+ catalytic spectrophotometry.Questionnaire survey of IDD was conducted among 30 students of grade 5 and 5 housewives around that school.Results A total of 1200 children aged 8 to 10 were examined,and the goiter rate was 1.3%(15/1200).All the goiters were level Ⅰ.The goiters of each age group was 0.8%(3/390),1.8%(7/390) and 1.3%(5/420),respectively.The 360 urine samples were collected from children aged 8-10,and the median of urinary iodine was 171.5 μg/L with the proportions of < 20 μg/L,< 50 μg/L and < 100 μg/L were 0(0/360),1.1% (4/360) and 5.6% (20/360),respectively.A total of 1200 salt samples were tested in children's family.The coverage rate of iodized salt was 99.4% (1193/1200) and the qualified rate of iodized salt was 97.7% (1165/1193).The consuming rate of qualified iodized salt was 97.1%(1165/1200),and non iodized salt rate was 0.6%(7/1200).The knowledge rates of IDD were 82.7% (2234/2700) and 69.8% (314/450) in 900 students and 150 housewives,respectively.Conclusions In Huai'an City,except the knowledge rate of IDD,the goiter rate and urinary iodine of children aged 8-10,iodized salt consumption maintain at the stage of the target level.In addition to strengthen the monitoring of iodized salt and to supply qualified iodized salt,it should also increase the focus on health education propaganda in the future.
8.Study on the clearance effect of the cross-linked agar entrapping attapulgite clay capsules on the hemoperfusion in vitro

Objective To investigate the adsorptive capacity of cross-linked agar entrapping attapulgite clay (CAA) capsule, a novel adsorbent for hemoperfusion. Methods We selected some drugs such as phenothiazines, methylene blue, and diazepam as the model components to test the adsorptive capacity of CAA in human blood in vitro by means of static stirred absorption experiment and repeated hemoperfusion. Results CAA was of strong adsorption for phenothiazines and methylene blue but of little effect on diazepam. The corresponding adsorptive mechanisms were also discussed here. Conclusion CAA could be used as a highly effective adsorbent for the cation drugs in human blood.
9.Effects of Highly Concentrated Potassium Chloride via Central Venous Catheterization on Severe Hypokalemia

Objective To investigate the clinical effects and feasibility of highly concentrated Potassium Chloride via central venous catheterization by micro pump on severe hypokalemia patients in EICU.Methods A totlal of 120 severe hypokalemia patients in our department were randomly divided into experimental group (treated with highly concentrated Potassium Chloride) and control group (normal treatment group) respectively,and treated with Potassium Chloride liquid of different concentration.Potassium levels in blood were checked every hour and the time for reaching standard potassium level (4.0mmol/L) and the total volumes of infusion fluid within 24 hours in the two groups was compared.Results The mean time for reaching standard potassium level and the total volumes of infusion fluid within 24 hours in the experimental group,(12.83 ± 3.57) h and (402.56 ± 54.61) ml respectively,were significantly less than those in the control group (P <0.01),(23.18 ±4.98) h and (2875.2 ± 206.26) ml respectively.Conclusion Highly-concentrated potassium chloride injection via central venous catheterization by micro-pump is a safe,effective and feasible treatment on the patients with severe hypokalemia,especially on the patients with volume-overloaded heart and severe hypokalemia,which is worthy of further clinical research.
10.Comparative analysis of Gp120 sequences of enterotropic and neurotropic SIV strains

Objective To study the mutations of Env sequence of SIVmac239 after infection of Chinese rhesus monkeys, and compare the differences and characteristics of Gp120 sequences of enterotropic and neurotropic SIV strains. Methods Six strains of simian immunodeficiency virus were analyzed in this study: four separated from peripheral blood mononuclear cells of SIVmac239-infected monkeys and two neurotropic SIVmac251 strains.Isolated and cultured monoclonal virus was obtained by limiting dilution assay.Gp120 sequences were amplified after the RNA extraction and phylogenetic analysis was processed thereafter.So did the Gp120 amino acid sequence and N-glycosylation sites analysis of the enterotropic and neurotropic strains.Results SIVmac239 had different mutations in four rhesus monkeys.The diversity in amino acid sequences of the enterotropic and neurotropic strains concentrated in the V1 and V4 regions of Gp120.The enterotropic strains had an addition of glycosylation site in V4 but the glycosylation site changes of neurotropic strains were located in the conservative regions of C1, C2 and C3.Conclusions The addition of one glycosylation site in V4 region of GP120 and loss of one glycosylation site in C1 region are associated with enhanced enterotropism and neurotropism.The differences between the enterotropic and neurotropic strains are not dipicted in Gp120 V3 region which is closely related with the tropism of strains.
