2.Dose-response relationship and muscle relaxant residual induced by rocuronium in children with cleft lip and palate repair

Objective:To investigate the effects of rocuronium in the intravenous anesthesia in children with cleft lip and palate repair.Methods:According to random number table,96 children with cleft lip and palate aged 1-3 years underwent general anesthesia for surgery were divided into 3 groups (n =32).The children in group A,B and C received intravenous injection of rocuronium at 0.6,0.9 and 1.2 mg/kg,respectively.The anesthesia effects were observed.Accelerometry (TOF-Watch(@) SX) was used to monitor neuromuscular transmission.Results:Intubating conditions ranged from good to excellent in all children.The onset time of anesthesia in Group C was significantly shorter(P < 0.05),the duration of neuromuscular block was significantly longer in group C than in group A and B (P < 0.05).The recovery index in group C was significantly longer than that in group A and B (P < 0.05,A vs B,P > 0.05).Muscle relaxant residual incidence in group C was higher than that in group A and B (P < 0.05,A vs B,P > 0.05).Conclusion:Rocuronium at 0.6 mg/kg or 0.9 mg/kg are effective for excellent tracheal intubation,with faster onset time and recovery time,low incidence of postoperative residual curarization(PORC) in children with cleft lip and palate repair.
3.Single ectopic ureter(report of 19 cases)

Objective To elucidate the entity of single ectopic ureter and improve efficacy of its clinical treatment and to explore the correlation between the site of the ectopic orifice and the degree of the dysplastic kidney it drains. Methods The clinical data of 19 cases with single ectopic ureter were closely studied.And the correlation between the site of the ectopic orifice and the size of the kidney was analyzed. Results Thirteen of the 19 dysplastic kidneys were detected by combined use of ultrasound and IVU.The site of the ectopic orifice had no correlation with the degree of the dysplastic kidney.All patients were cured by means of one procedure. Conclusions The epidemiology and clinical features of single ectopic ureter have racial and regional differences.Appropriate intervention can obtain an excellent outcome.
4.Effects ofTongbicapsule on knee joint function and synovial fluid levels of matrix metalloproteinase-3,-9 in patients with knee osteoarthritis

Objective To investigate the effects ofTongbicapsule on the knee joint function and the synovial fluid levels of matrix metalloproteinase (MMP)-3,-9 in patients with knee osteoarthritis.Methods A total of 76 patients with knee osteoarthritis were randomly divided into a control group and an observation group according to the random number table, 38 in each group. Patients in the control group were treated with glucosamine hydrochloride capsules, while patients in the observation group were givenTongbicapsule on the basis of the control group. Both groups were treated for 12 weeks. The Western Ontario and McMaster Universities Osteoarthritis Index was used to measure the knee joint function, and enzyme-linked immunosorbent assay was used to determine the synovial fluid levels of MMP-3 and MMP-9.Results The knee joint function in the observation group was significantly improved than that in the control group (94.74%vs. 78.95%;χ2=4.146, P<0.05). The synovial fluid levels of MMP-3 (67.58 ± 22.35 ng/mlvs.93.51 ± 27.84 ng/ml;t=4.477,P<0.01) and MMP-9 (36.24 ± 10.56 ng/mlvs.51.87 ± 12.35 ng/ml;t=5.930,P<0.01) in the observation group patients were significant lower than those in the control group after treatment.ConclusionsTongbicapsule was effective in treating knee joint function in patients with knee osteoarthritis, and its mechanism may be related to its reducing the synovial fluid levels of MMP-3 and MMP-9.
5.The value of fMRI for predicting the clinical outcome of passive movement of a hemiplegic upper limb after stroke

Objective To study the potential of functional MRI (fMRI) during passive finger extension (FE) for predicting the motor function recovery of hemiplegic stroke survivors.Methods Nineteen survivors of a 1st cerebral infarction in the non-M1 cortex with complete paralysis on one side were selected for study.Within 1 week after the stroke,fMRI was performed while one of the patient's paralyzed fingers was passively flexed and extended.fMRI signals were acquired from the sensorimotor cortex (SMC),the supplementary motor area (SMA) and the premotor area (PM).Fugl-Meyer (FM) scores of arm motor function were recorded one week,one month and three months after the stroke.Results The early fMRI results were used to classify cortex activation performances as type Ⅰ (activation mainly on the affected side,but SMC and SMA activated bilaterally),type Ⅱ (activation of the SMC and SMA only on the affected side) or type Ⅲ (only activation of the SMC on the affected side).At one week after stroke the average FM scores of the three groups were not significantly different.At one month the three average FM scores were 47.1 ± 5.5 points,36 ±6.7 points and 11.2 ± 3.1 points,for groups Ⅰ,Ⅱ and Ⅲ respectively,each significantly different from the others.At three months the average FM ratings of groups Ⅰ (61.1 ± 3.8 points) and Ⅱ (59 ± 5.2 points) were not significantly different,but both were significantly higher than the average score of group Ⅲ (10.8 ± 5.6 points).Conclusion The early stage fMRI characteristics of hemiplegic patients can be used for predicting motor outcomes.
6.Detecting mesorectal margin micrometastasis in total mesorectal excision patients with rectal carcinoma

Objective To evaluate the impact of mesoreetal margin micrometastasis on local recurrence of rectal carcinoma after total mesorectal excision, and the relation between mesorectal margin mierometastasis and local recurrence. Methods Mesorectal margin specimens from 52 cases of rectal cancer after total mesorectal excision were studied by reverse transcriptase-polymerse chain reaction (RT-PCR) to detect the expression of CK20 mRNA. These patients were divided into CK20 mRNA positive and negative group and followed up for 3 years until local recurrence developed. Results In 52 patients with rectal cancer,21 cases were found to express CK20 mRNA in mesorectal margin, the expression rate of CK20 mRNA in Dukes A,B and C was 17% ,30% and 54% (P<0.05), respectively. The positive rate of well differentiated, moderately differentiated, poorly differentiated and undifferentiated carcinomas were 43%, 38% ,40% and 50% ,respectively(P>0.05 ). The incidence of local recurrence in CK20 mRNA positive and negative group was 24% and 3% , respectively. The difference was statistically significant between the two groups (P <0.05 ). Conclusion Detection of mesorectal margin CK20 mRNA as micrometastasis by RT-PCR in rectal carcinoma patients was a sensitive and effective way in predicting local recurrence. It is necessary for patients with positive mesorectal margin CK20 mRNA to undergo postoperative radiotherapy after total mesorectal excision.

8.Cloning, expression, and purification of c-di-AMP metabolism-related genes from Porphyromonas gingivalis.

OBJECTIVETo clone, express, and purify cyclic diadenosine monophosphate (c-di-AMP) metabolism-related genes from Porphyromonas gingivalis (P. gingivalis) ATCC33277.
METHODSPolymerase chain reaction (PCR) from the genome of P. gingivalis ATCC33277 amplified, the coding regions of pgn0523, pgn1187, and pgn2003 genes. The amplified DNA fragments were ligated with a prokaryotic expression vector pET28a to construct the recombinant expression plasmids pET-pgn0523, pET-pgn1187, and pET-pgn2003. These recombinant plasmids were transformed into Escherichia coli (E. coli) BL21 (DE3) competent cells. The expression of recombinant proteins was induced by isopropyl-β-D-thiogalactoside and detected by sodium dodecyl sulfate-polyacrylamide gel electrophoresis. Proteins were purified using a Ni²⁺ matrix column, and their concentrations were determined by a BCA Protein Quantitative Kit.
RESULTSThe c-di-AMP metabolism-related genes from P. gingivalis ATCC33277 were amplified successfully with the correct molecular size. The recombinant expression vectors were constructed by ligating enzyme-digested PCR products and pET28a vector, and verified by PCR and sequencing. After induction and purification, recombinant proteins were expressed successfully and obtained with the correct molecular size (19.5 x 10³, 39.9 x 10³, 66.0 x 10³). The final protein concentrations were 0.708, 0.523, and 0.861 mg · mL⁻¹ after dialysis.
CONCLUSIONThe c-di-AMP metabolism-related genes from P. gingivalis ATCC33277 are cloned successfully, and their coding products are expressed correctly in E. coli. High-purity proteins are finally obtained. The cloning and purification of these important proteins will help us to further investigate the physiological function and regulatory mechanism of c-di-AMP signaling system in P. gingivalis.

9.The research progress of Aurora-B kinase and its inhibitors.

Aurora-B as an important kinase to adjust the cell normal mitosis is a potent target for cancer treatment. Aurora-B is overexpressed in a broad range of tumor and tumor cells are more sensitive while Aurora-B is inhibited. Due to the key role of the Aurora-B in cell mitosis, the development of its inhibitors is becoming more and more important. Several small molecules inhibit with a similar efficacy both Aurora-A and Aurora-B, however, in most cases the effects resemble Aurora-B disruption by genetic methods, indicating that Aurora-B represents an effective therapeutic target. There were several Aurora-B kinase inhibitors which had entered the clinics and displayed good antitumor activity. In this review, we will outline the functions of Aurora kinase B in normal cell division and in malignancy. We will focus on recent preclinical and clinical studies that have explored the mechanism of action and clinical effect of Aurora-B inhibitors in cancer treatment.
10.The analysis ofβ-lactam antibiotics interference in fructosamine testing

Objective To evaluate the interference of 13 kinds of β‐lactam antibiotics in fructosamine detection .Methods A se‐ries of solution of antimicrobial drugs were formulated with water for injection ,and added into the mixture fresh serum volume by 5% .Interfere with antimicrobial drugs on the test results of fructosamine were evaluated .Results When the blood concentrations of cefathiamidine and ticarcillin sodium/clavulanate potassium were up to 37 .5 mg/L and 247 .5 μg/mL respectively ,they began to produce positive interference on fructosamine detection .However ,the degree of interference was not correlated to the serum concen‐tration of fructosamine .Conclusion Cephathiamidine and ticarcillin sodium/clavulanate potassium have positive interference on fructosamine detection .
