Objective To study the difference in occurrence of complications and health economics between early enteral nutrition (EN) and total parenteral nutrition (TPN) in patients with alimentary tract carcinoma after surgical operation.Methods Totally,164 patients with alimentary tract carcinoma hospitalized at the Department of General Surgery in China-Japan Friendship Itospital,Beijing during September 2003 to December 2005 with nutritional intervention,84 cases with early EN and 80 with TPN within 24 to 48 hours after surgical operation,were reviewed.Nutrition index,complications,recovery of gastrointestinal movement,length of hospital stay after operation and total cost of the patients were analyzed. 3.The diagnostic value of endoscopy assisted with laparoscopy in patient with small intestinal bleeding

Objective To study the diagnostic value of endoscopy assisted by laparoscopy in patients with small intestinal bleeding. Methods In case of small intestinal bleeding it is hard to define the accurate etiological factor by routine examination. From 1994 to 2002 there were 14 patients with chronic me-lena or persisted melena complicated with chronic anemia and 2 cases of acute alimentary tract hematorrhea complicated with obvious hypovolemia. Results Endoscopies assisted by laparoscopy were accomplished in all of the 16 cases. Lesions were detected in 5 cases with laparoscopy directly, and in 11 cases the pathological changes were discovered by endoscopies assisted with laparoscopy . The pathologic results were leiomyo-sarcoma 1 cases, leiomyoma 4 cases, adenoma of small intestine , hemangioma , vascular malformation and capillary telangiectasia 2 cases each, and Meckels diverticulitis with hemorrhage 3 cases. Conclusion The diagnostic value of endoscopy assisted with laparoscopy is superior to the routine examination in detecting the small intestinal bleeding. This technique has the advantage of simple, less-traumatic and high sensitivity. It is worthy to popularize in clinical practice.

5.Comparison of the effect of various herniorraphy in inguinal hernia

Objective To evaluate the effect of various herniorraphy in treating inguinal hernia. Methods The clinical data of 108 patients who underwent conventional Bassini herniorraphy (BH), tension free herniorraphy (TH), or laparoscopic herniorraphy (LH),respectively were analysed and comparated. Results The operation time in TH was less than that in BH, and in LH was longer than that in TH. The ability recovery, hospita stay and postoperative complication in TH and LH were less than those in BH.All the patients were followed up for at least 1 year.Of them, 1 recurred after BH, but there was no recurrence after TH and LH. The blood neutrophils, serum level of fibrin, C-reactive protein and alpha-1 antitrypsin increased significantly in the patients undergoing herniorraphy, which were significantly higher in the patients underwent TH and LH. Conclusions The curative effect of TH and LH is much better than that of BH in treating ingumal hernia. Application of marlex for herniorraphy might increase inflammation response, but it will not increase the incidence of infection.
6.Application of hands chopping cataract surgery in patients with microcoria

AIM: To discuss the safety of hands chopping phacoemulsification in patients with microcoria cataract.METHODS:Hands chopping phacoemulsification with intraocular lens implantation was used for the microcoria cataract of 30 patients ( 32 eyes ) . Their visual acuity, pupil, and complication were observed in postoperative 1d,1wk and 1mo.
RESULTS: Postoperative naked vision be or more than 0. 3 were in 27 eyes (84. 4%) at one day, be or more than 0. 3 were in 30 eyes (93. 8%) at one week, be or more than 0. 5 were in 28 eyes (87. 5%) at one month. All pupil returned to round or oval. No synechia happened in postoperative 1mo.
CONCLUSION:Hands chopping nucleus operation is safe and effective for uveitis combined with microcoria phacoemulsification.
7.Application of BODE grading system in evaluating clinical prognosis of chronic obstructive pulmonary disease

Objective The aim of this study is to evaluate clinical application of BODE (body mass index, airflow obstruction, dyspnea, and exercise capacity) index in patients with chronic obstructive pulmonary disease (COPD). Methods Forty patients with stable COPD in Xuanwu Hospital of Beijing during September 2003 to December 2007, were recruited in the study, including 25 men and 15 women, aged 38 to 85 years (with a mean of 62 ± 13 years). BODE index was measured for each patient, one year and three years after recruitment, respectively. Severity of COPD, including number and duration of episodes, length of each hospital stay and times of hospitalization due to acute exacerbation of COPD (AECOPD), were recorded. All data were statistically analyzed with SPSS 13.0 software for windows. Results ①There was a remarkable difference in COPD severity between patients with BODE score equal to or greater than five and these with less than five (P <0.01 ). ①BODE score inversely correlated with forced expired volume at the first second ( FEV1 ) in COPD patients (P < 0.01 for those with BODE score equal to or greater than five and P <0.05 for these with BODE score less than five, respectively). ③BODE score positively correlated with number and duration of acute exacerbation, length of hospital stay and times of hospitalization due to AECOPD (P <0. 01 for those with BODE score equal to or greater than five and P < 0. 05 for those with BODE score less than five, respectively). For patients with COPD, their BODE score in the first year positively correlated with that in the third year ( r = 0.834, P < 0.01 ). Conclusions BODE index is a better indicator to evaluate their condition severity in patients with COPD.
8.Normal lung dose-volume histogram varieties in the former and inter period of three dimensional conformal radiation therapy plans and their clinical significance in non-small cell lung cancer

Objective To analyze the normal lung dose-volume histogram(DVH) varieties in the former and later period(P1 and P2)of three dimensional conformal radiation therapy(3DCRT) plans and the compound (Pc) plan in non-small cell lung cancer(NSCLC),and to access the feasibility to modify the target volume during the treatment course.Methods Twenty-one NSCLC patients who had received accelerated hyper-frationation 3DCRT in P2 were included in the study.Both of the P1 and P2 plans were redesigned to a total dose of 70 Gy with V20 smaller than 35%.When the target volume was modified and P2 plan was rede signed using accelerated hyper-frationation 3DCRT of 30 Gy after P1 plan of 40 Gy,the Pc plan was compoun ded by transmitting the parameters(such as target volume,irradiation field and dose) of P1 plan into P2 plan. Total lung volume and target volumes(GTV,PTV) of P1 and P2 were evaluated.MLD,V5,V10,V20 and V30 of P1,P2 and Pc were calculated.Results The total lung volume in P1 and P2 plans was not significantly dif ferent(t = 0.19,P = 0.850).The volumes of GTV,PTV in P2 were obviously smaller than P1 (t = 2.88,P = 0.009 ; t = 4.01 ,P = 0.001) .When comparing P2 with P1 ,MLD were 16.5 Gy Vs 17.8 Gy (t = 2.60, DOI:10.3760/cma.j.issn.1004-4221.2009.01.057 P = 0.017),V30 was significantly decreased (t = 2.19,P = 0.041),but V5,V10 and V20 had no significant difference.Similar differences were found in MLD,V5 ,V10 ,V20 and V30 when comparing Po to P1.P2 plans had significantly smaller MLD,Vs,V10,V20 and V30 than Pc plans.Fourteen patients with decreased PTV were further analyzed.The V30 and MLD decreased significantly (t = 3.00,P = 0.0 I 0;t = 2.38,P = 0.033), but V5 ,V10,V20 had no difference when comparing P1 and P2 plans.Among these 14 patients,the V10 and V30 decreased significantly(t = 2.76,P = 0.033 ; t = 3.60,P = 0.011) when P2 plans were generated using the same field number and beam angles in P1 plans in 7 patients.The parameters were similar in P1and Pc plans,but increased significantly when comparing to P2.Various parameters were the same among P1,P2 and Pc plans when P2 plans were designed using 1-2 different fields and angles in the other 7 patients.The differ ences were not significant between P1 and P2 plans in 7 patients with the same or increased PTV.Expansion or contraction of PTV significantly influenced MLD and V2o (r =-0.62,P = 0.03 ; r = O.48,P = 0.029). Conclusions When the tumor regresses,the high dose volume of the lung decreases with modifying the tar get volume and replanning in the later period using accelerated hyper-frationation 3DCRT.The low dose vol ume of the lung may decrease if the field orientations are same throughout the treatment.It is rational to eval uate the normal lung DVH of the whole plan when the physical parameters of the later period plan are the same as the former one.
9.Analysis on the medication rules of state medical masterYan Zhenghua from the prescriptions with Angelicae Sinensis Radix based on data mining

Objective To explore the medication rules of State Medical MasterYan Zhenghua. Methods The prescriptions including Angelicae Sinensis Radix that built by Pro. Yan were collected to build a database based on Traditional Chinese Medicine (TCM) inheritance assist system(V2.0.1). After analyzed by the statistical reports module and the data analysis module which were from TCM inheritance assist system, and the methods of data-mining that including association rules and apriori algorithm, the frequency of single medicine, the frequency of drug combination, the association rules between drugs and core drug combinations which all were including Angelicae Sinensis Radix can be get from the database.Results The prescriptions including Angelicae Sinensis Radix were commonly used to treat stomach-ache, arthralgia syndrome, irregular menstruation and other syndromes. The highest frequency used drugs were Radix Paeoniae Rubra, Radix Paeoniae Alba, Radix Salviae Miltiorrhizae, Pericarpium Citri Reticulatae, Rhizoma Cyperi, and so on. The most frequency drug combinations were “Angelicae Sinensis Radix, Radix Paeoniae Rubra”, “Radix Paeoniae Alba, Angelicae Sinensis Radix”, and “Radix Salviae Miltiorrhizae, Angelicae Sinensis Radix”. The drug association rules that the confidence was more than 0.9 were “Fructus Jujubae-Angelicae Sinensis Radix”, “Fructus Amomi Villosi-Angelicae Sinensis Radix”, “Radix Codonopsis-Angelicae Sinensis Radix”, and “Os Draconis-Concha Ostreae”, and so on.Conclusion The drugs in the prescriptions containing Angelicae Sinensis Radix that built by Pro. Yan mostly had the effects of cooling blood, replenishing blood, and promoting blood circulation for removing blood stasis, which reflected the clear thought when making prescriptions.

Objective To understand the knowledge of tuberculosis in general hospital patients with respiratory diseases.Methods Face to face questionnaire survey 821 respiratory patients, the main contents include: the tuberculosis awareness, common symptoms of tuberculosis, tuberculosis with or without contagious, persistent cough expectorant willing to go to where the treatment of tuberculosis, national policy, treatment of the course of treatment, tuberculosis prevention measures, and whether the knowledge of tuberculosis drug resistance.Results 92.1% of patients were aware of TB and 98.7% of patients knew TB was contagious, and 99% knew that TB was transmitted by the respiratory tract.Only 4.5% of patients with symptoms appear willing to go to tuberculosis treatment of patients.94.9% of the patients were aware of a specific TB control facility, 85% knew that the basic TB treatment was free, 99% thought to be good treatment of tuberculosis patients, 55% of patients know that tuberculosis treatment for a long time, to regular medication.30% of people know that mycobacterium tuberculosis resistance.90% of patients know to isolate tuberculosis patients, 40% of patients that protect susceptible populations, and 95% believed that BCG could prevent tuberculosis.Conclusion Patients have a certain understanding of tuberculosis, the timely treatment of indifference to the treatment of treatment is probably not impressed, very little knowledge of tuberculosis resistance.
