ObjectiveTo determine the bispectral index and dose of propofol at which 50 % of patients fall into unconsciousness during induction (BIS50 and EDs0 ). MethodsThirty unpremedicated ASA Ⅰ-Ⅱ patients scheduled for elective surgery were included in this study. The patients did not take any sedatives or hypnotics and had no psychiatric disease. Loss of consciousness and changes in BIS during induction with propofol were studied by sequential experimental method. The loss of responses to verbal command and eyelash stimulation was used as an induction of unconsciousness. The data obtained were analyzed by linear regression and calculated to determine BIS50 and ED50 using weighted averaging method. ResultsThe BIS50 and ED50 for propofol were 79.17 and 1.0192 ng/kg with the 95％ confidence intervals of 72.08-88.55 and 0.94-1.148 mg/kg respectively. Conclusions The sequential experimental method for determination of ED50 is simple, efficient and reliable and also suitable for determination of BIS50.

OBJECTIVETo assess the efficacy of Yishen Jiejing Decoction (YJD) in treating poststroke shoulder-hand syndrome (SHS) patients of yin deficiency yang hyperactivity with blood stasis stagnation collaterals syndrome.

METHODSTotally 60 SHS patients of yin deficiency yang hyperactivity with blood stasis stagnation collaterals syndrome were randomly assigned to two groups, the treatment group and the control group, 30 cases in each group. Conventional rehabilitation training and therapeutics were applied in all patients. Besides, patients in the treatment group took 50 mL YJD, twice a day. One month without interruption consisted of one course of treatment. The curative effects of each group were evaluated respectively before treatment and at one month after treatment. The neurologic impairment, TCM syndrome, and the improvement of upper limbs movement were assessed by the neurologic impairment integral, scoring for TCM syndrome diagnostics, Fugl-Meyer Assessment (U-FMA). Adverse reactions were observed at the same time.

RESULTSThe effective rate of stroke was 86.67% and the effective rate of SHS was 90.00% in the treatment group, higher than those of the control group (P < 0.05). Both groups got improvement in neurologic impairment, stroke induced blood stasis syndrome, yin deficiency yang hyperactivity syndrome, and the improvement of upper limbs movement after treatment (all P < 0.05). Besides, all the improvement was obviously superior in the treatment group (P < 0. 05). No adverse reaction occurred during the course of treatment.

CONCLUSIONThe curative effect of YJD combined with conventional rehabilitation training was confirmative and superior to the control group.

Objective To observe the clinical therapeutic effect of danshenfenzhen and nutritional support in treating congestive heart failure accompanied with malnutrition on the basis of common treatment.Methods 42 cas- es of congestive heart failure accompanied with malnutrition were randomly divided into 2 groups:test group 21 cases (danshenfenzhen and nutritional support) and control group 21 cases(free diet),in the test group intravenous drip danshenfenzhen and vein nutrition were given if the internal nutrition can not provide enough energy on the basis of count of energy consumption,the serum albumin,total blood lymph cells and body weight were measured.Results After 14 days' treatment in the test group,the serum albumin,body weight and the total lymph cell increased signifi- cantly while in the control group,the serum albumin,body weight and the total lymph cells decreased significantly. Conclusion Intravenous drip danshenfenzhen and nutritional support can improve nutritional status and cardiac function of patients with congestive heart failure accompanied with malnutrition.

AIM:To investigate the effect of different cutting centers on the visual acuity, refractive diopter and visual quality of patients undergoing laser assisted in situ keratomileusis (LASIK).METHODS: A total of 80 patients (160 eyes) with myopia treated by elective LASIK were divided into two groups.Thirty-six cases (72 eyes) with visual axis corneal reflection point (VACRP) as the cutting center were included into the VACRP group while 44 cases (88 eyes) with pupil center (PC) as the cutting center were included into the PC group.The uncorrected visual acuity (UCVA), the best corrected visual acuity (BCVA), refractive diopter, corneal aberration [total corneal and anterior corneal surface higher-order aberrations (HOA), spherical aberration (Z40), vertical coma (Z3-1), horizontal coma (totZ31) and offset of cutting centers were determined before surgery and 1mo after surgery.RESULTS: There was no difference in the probability of UCVA ≥ 0.1, BCVA and refractive diopter between the two groups at 1mo after surgery (P>0.05).The astigmatism and cutting center deviation of VACRP group were lower than those of PC group (P<0.05).The totHOA, totZ40, totZ3-1, totZ31, froHOA, froZ3-1、froZ31 and froZ40 were lower in VACRP group than PC group at 1mo after surgery (P<0.05).CONCLUSION: The UCVA of patients treated with both cutting centers for LASIK is good but VACRP has more advantages in reducing the offset of cutting center and improving postoperative visual quality.

In this article we analyzed the current development of moxibustion treating rheumatoid arthritis from the usefulness, advancement, synergistic effect as well as the variance between different kind of moxibustion. We concluded that moxibustion was an effective intervention for treating RA, and the methods used in moxibustion were searched in clinic. But the clinical tralls has a long way to go, we should pay more attention to the critical issues while in the use of moxibustion.

Objective To investigate the protective effects of 1-hydroxy-2, 3, 5-trimethoxyxanthone (QGS) on acute lung injury of mice induced by ip lipopolysaccharide (LPS). Methods Mice were pretreated with QGS for 7 d. Murine models of acute lung injury were duplicated by injection of LPS 20 mg/kg intraperitoneally. In 12 h, the lung weight index was observed and the NO level in the bronchoalveolar lavage fluid (BALF) was measured with kits. Objective To study the MR imaging technology and 3D reconstruction of the articular cartilage of knee.MethodsConventional,2D,3D and multiparameter MR imaging scans of knee were performed in ninety patients with trauma(n=34)andosteoarthritis(n=56) of knee and 10 healthy volunteers.The articular cartilages on 3D images were reconstructed using maximum intensity projection(MIP).The mean thickness of articular cartilage was measured and analysed statistically.Results Articular cartilages of knee were clearly depicted by MR imaging technique and 3D.On 2D FSE sequence,about 50% of articular cartilage appeared as three laminar apearance.On 3D imaging,the three-dimensional morphology of articular cartilage was showed clearly.Conclusion MR imaging and 3D reconstruction of the articular cartilages of knee is helpful in early diagnosis and treatment of disordered cartilages.

BACKGROUND: Mesenchymal stem cells are few in human bone marrow, and their number will decrease with aging or body weakening, so a large amount of amplification is necessary. However, the biological characteristics of human mesenchymal stem cells of each passage remain poorly understood.OBJECTIVE: To analyze and compare the biological characteristics of each passage of bone marrow-derived mesenchymal stem cells (MSCs) so as to provide a basis for clinical demands of tissue engineering.DESIGN,TIME AND SETTING: Cytological observation in vitro. The experiment was performed at the Department of Orthopedics and Central Laboratory, Dongzhimen Hospital, Beijing University of Chinese Medicine from March to October 2008.MATERIALS: From bone marrow of patients with non-hematopoietic disease, MSCs were provided by Department of Orthopedics, Dongzhimen Hospital, Beijing University of Chinese Medicine.METHODS: Bone marrow was collected form posterior superior iliac spine of patient, MSCs were isolated and cultured by Percoll method. When the cells were confluent at 90%, they were trypsinized and observed by inverted miscroscopy. The second passage of cells were collected for index detection.MAIN OUTCOME MEASURES: Cell morphological characteristics and immunophenotype; cell activity was detected by MTT; cell division and apoptosis in the proportion of necrosis were analyzed by flow cytometry analysis.RESULTS: The passaged MSCs exhibited uniform appearance in fusiform shape, and their growth was slowed down after 9 passages, exhibiting cytoplasm vacuolization and body enlarging. The second passage of MSCs was positive for CD44, CD106,and CD105, but negative for CD34 and CD45. MTT values peaked at passage 9, and gradually decreased since passage 10. At passage 11, the number of MSCs at division stage was increased, but from the sixth passage, the number of apoptotic cells increased significantly, reaching more than 60% at passage 8.CONCLUSION: According to biological characteristics analysis of MSCs at each passage, the third to the sixth passage cells are recommend for clinical therapy.
