B lymphocyte stimulator (BLyS), a tumor neurosis factor ligand superfamily, is an important factor of B cell survival and activation. However, BLyS also regulates T cell activation and survival, playing key roles in T cell-mediated autoimmune disorders. In the paper, we introduced the mechanisms of BLyS and a proliferation-inducing ligand (APRIL) regulating T cell responses and their roles in rheumatoid arthritis (RA).

Drug treatment is one of the most important parts in t he therapeutics of periodontal disease. This review discusses the purpose, signifi cance、pharmacological mechanisms, administration methods and announcements of c linic medication in periodontal disease and summarized clinical commonused drug as well as the reseach progress in drug therapeutics of periodontal disease.

Objective: To evaluate the effectiveness of minocycline at subantimicrobial dosage in the treatment of experimentally induced periodontitis in rats.Methods: Periodontitis was induced by silk ligature technique on 4 second molars in each of 16 adult male Sprague-Dawley rats and the animals were fed with 100 g/L sucrose drink. The rats were divided into 2 groups: (1)periodontitis group without treatment;(2)treatment group, the rats with periodontitis were treated by systemic administration of minocycline at 5 mg/(kg?d).Another 8 normal rats were used as the controls. Assessment was carried out at day 28 and 56 using a number of different visual, histological and ultrastructure approaches.Visual assessment included tooth mobility(TM), gingival index(GI), and alveolar bone loss. Histological examination included monocyte effusion,resorption lacunae with osteoclasts and percentage of the periodontal collagen.The collected data were statistically analyzed using variance test.Results: Systemic administration of minocycline at subantimicrobial dosage can significantly reduce GI, TM, resorption lacunae with osteoclasts and alveolar bone loss either at day 28 or at day 56; significantly inhibited monocyte effusion and the collagen degradation in the periodontium at day 56 in rats with periodontitis. Conclusion: Minocycline at subantimicrobial dosage may decrease alveolar bone loss and osteoclasts formation in periodontium with periodontitis.

The diagnosis of the hip diseases and the judgment of the treatment are based on the measurement parameters with X-rays that mainly defined as anterposterior pelvis radiographs of the hip joint.The data of the measurement changes in different ages due to the growth and development of children's bodies.There are varies of parameters attempt to improve the evaluation of the hip joint.The review focuses on the Chinese and English literature since 1930s,and summarizes the introduction of the history and current condition,modified progress of methods and data analysis of measuring results.

Objective:To compare the clinical factors and physical parameters between 3-fields and 4-field 3-dimensional con-formal radiotherapy(3DCRT)for mid-thoracic or upper-thoracic esophageal carcinoma.Methods:Patients with mid-thoracic or upper-thoracic esophageal carcinoma were chosen at random and treated with 3DCRT.Contouring and treatment planning were performed with VARIAN Eclipse treatment planning system(TPS).The target volume was determined according to guidelines from ICRU Report No.50.The three-beams treating plan and the four-beams treating plan were performed and a 8mm isotropic mar-gin was added for the planning target volume(PTV)in beam setting.The different plans were evaluated with dose volume histograms(DVH)in terms of dose coverage of PTV and protection of organs at risk(OAR).The prescribed dose was 60 Gy/30 F to the PTV.Results:There was no significant difference in the dose coverage to PTV and the explored dose to spinal cord with the two model of 3DCRT.When the three-beams treating plan was compared with four-beams treating plan,lungs V20 and V30 were significantly better,and at the same time lungs V5 was higher.Conclusions:3-fields 3DCRT is better than 4-fields 3DCRT with lower lungs V20 and V30 so as to prevent acute radial injury of lungs in radiotherapy of mid-thoracic or upper-thoracic esophageal carcinoma.

Objective To study the effects of laparoscopic radical gastrectomy for gastric cancer on serum inflammatory cytokines and intestinal barrier function in patients.Methods Retrospective analysis was used to investigate a total of one hundred and forty gastric cancer patients in Ezhou Central Hospital from January 2014 to September 2016,including 66 cases with laparoscopic radical gastrectomy (laparoscopic group) and 74 cases with traditional open radical gastrectomy (traditional group),the perioperative indexes,inflammatory factors and intestinal barrier function of the two groups were compared.Results In the laparoscopic group,the amount of bleeding loss in the laparoscopic group ((86.8 ± 15.2) ml),the incision length ((6.2 ± 1.4) cm),the first anal exhaust time ((3.0 ± 0.6) d),the fluid diet time ((4.6 ± 0.9) d),the postoperative hospitalization ((8.8 ± 1.7) d) were all lower than those in the traditional group ((158.2±25.0) ml,(16.3±2.7) cm,(3.6±0.7) d,(5.1±0.8) d,(10.3±2.5) d),the differences were statistically significant (P=0.000,0.000,0.000,0.014,0.001);the operation time of the traditional group was (196.5±27.8) min,the number of lymph nodes was (21.6±2.8),compared with those in the laparoscopic group ((192.5±30.3) min,(22.0±3.1),P=0.421,0.137).The levels of WBC,IL-6,CRP,TNF-ɑ,COR,plasma D-lactic acid and diamine oxidase in the laparoscopic group were lower than those in the traditional group (P=0.000).Conclusion The effect of laparoscopic radical gastrectomy on serum inflammatory cytokines and intestinal barrier function in patients is not obvious.

Objective To investigate the effect of lead aVR on recent prognosis in patients with non -ST -elevation acutecoronary syndrome(NSTE -ACS).Methods 195 patients with NSTE -ACS were divided into ST segment elevation in lead aVR group(n =54 cases)and non -ST segment elevation in lead aVR group(n =141 ) according to ST segment elevation in lead on admission;clinical data,laboratory index and coronary artery lesion were recorded,and major adverse cardiac (MACE)in hospitalization time were also recorded.Results Compared with non -ST segment elevation in the lead aVR group,the heart rate of ST segment elevation in the lead aVR group was faster[(85.4 ±12.0)beats per minute vs.(79.3 ±13.2)beats per minute,t =2.959,P <0.05],systolic blood pressure and diastolic blood pressure were higher[(145.0 ±20.3)mmHg vs.(136.0 ±19.4)mmHg,t =2.874,P <0.05];NT -proBNP[(743.9 ±125.8)pg/mL vs.(416.4 ±95.3)pg/mL,t =19.625,P <0.05],cTnI[(3.4 ± 1.0)ng/mL vs.(1.2 ±0.4)ng/mL,t =21.994,P <0.05],CK -MB[(31.5 ±9.8)ng/mL vs.(19.8 ±6.5)ng/mL, t =9.682,P <0.05]were higher (P <0.05);LVEF were lower[(50.1 ±5.2)% vs.(53.8 ±5.7)%,t =4.153, P <0.05];the rate and amplitude of low ST segment in outside lead aVR also was higher[(9.5 ±3.8)mm vs. (7.4 ±2.8)mm,t =4.224,P <0.05];the rate of left main and three -vessel disease in ST segment elevation in lead aVR group were higher (P <0.05).Compared with non -MECE group,proportion of ST segment elevation in lead AVR on admission was higher (P <0.05).Logistic regression analysis showed that heart rate(OR =1.472),NT -proBNP(OR =1.732),cTnI(OR =1.893),left main coronary artery(OR =2.478)and three -vessel disease (OR =2.310)were risk for ST segment elevation in lead AVR(P <0.05).Logistic regression analysis also showed that ST segment elevation in lead AVR were risk for MACE(OR =1.474).Conclusion ST segment elevation in lead AVR has close relationship with large area of myocardial ischemia,and patients prognosis is poorer,which may be independ-ent risk factor for the recent outcomes of patients with NSTE -ACS.Early record of the change in the case of aVR lead ECG ST segment NSTE -ACS patients is of important value in evaluation of the condition and prognosis.

Objective To find the accurate incidence of vacuum phenomenon in the hip and the best projection position produced the vacuum phenomenon on plain radiograph of hip in children.Methods All pediatric anteroposterior view and frog-leg position plain radiographs of hip joints were retrieved in our hospital between January 2003 and March 2013.All subjects was between 0 and 14 years old (mean age,4.2 years old).All the plain radiographs showing the crescent,linear,and irregular lucency between the femoral head and acetabulum were included in the present study.Results A total of 16 749 cases,including 12 422 cases (5 912 boys and 6 510 girls) only taken AP view plain radiographs and 4 327 cases (1 537 boys and 2 790 girls) taken both AP view and frog-leg position plain radiographs,were taken in our hospital between January 2003 and March 2013.None of AP view plain radiographs of hips could find the vacuum phenomenon.Two hundred and fifty-eight cases (321 hips) with vacuum phenomenon of the hips were found only in the frog-leg position plain radiographs of 4 327 cases,and the constituent ratio of vacuum phenomenon in the hip joint is 5.96％ in frog-leg position plain radiographs.One thousand seven hundred and thirty-eight normal children were included in 4 327 frog-leg position radiographs,and 150 cases of vacuum phenomenon were included in 1 738 normal children,and incidence of vacuum phenomenon in normal children is 8.63％.Two thousand three hundred and sixty children with developmental dysplasia of the hip (DDH) were included in 4 327 frog-leg position radiographs,and 98 cases of vacuum phenomenon were included in 2 360 children with DDH,and incidence of vacuum phenomenon in children with DDH is 4.15％.258 cases with vacuum phenomenon included 150 cases of normal hips (58.14％),98 cases of developmental dysplasia of the hip (37.98％),5 cases of Legg-Calvé-Perthes disease (1.94％),and 1 each in solitary eosinophilic granuloma of ischium,polyostotic fibrous dysplasia of the bilateral proximal femur,hereditary multiple exostoses,congenital coxa vara and fracture of femoral neck after surgery.Three hundred and twenty-one hips in 258 cases were classified into complete type (121 hips,37.69％) and partial type (200 hips,62.31％) according to the proportion of lucency area in the hip joints and linear (159 hips,49.53％),crescent (151 hips,47.04％) and irregular (11 hips,3.43％) lucency according to the shape of lucency area in the hips.Conclusion Vacuum phenomenon of the hip in children is not very rare in frog-leg position plain radiograph.It is easier to find the vacuum phenomenon in normal hip compared to that of developmental dysplasia of the hip.The frog-leg position plain radiograph is a better projection position to obtain the vacuum phenomenon of the hip in children compared to the anteroposterior view plain radiograph.

Objective To investigate the expression of proliferating cell nuclear antigen (Ki67) in hepatitis B virus (HBV) related hepatocellular carcinoma (HCC) tissue and its correlation with clinicopathological parameters of HBV related HCC. Methods HBV-related HCC patients (n=134) were divided according to Ki67 index (the percentage of Ki67-posi?tive cells) into Ki67 index<10%group (negative group, n=30) and Ki67 index≥10%group(positive group, n=104). The cor?relation of Ki67 index with age, gender, glypican 3 (GPC3), histological grade, BCLC (Barcelona Clinic liver cancer) staging, portal involvement, AFP (AFP), liver function related child-pugh classification data were calculated in both groups. Ki 67 in?dex is set according to the interval score to assess the relationship between Ki67 score and BCLC classification. Results (1) SP staining showed, Ki67 present a trend of increase from normal liver tissue to well-differentiated to medium differentiat?ed tissue to poorly differentiated tissues intensity. (2) In positive group, Ki67 index was correlated with BCLC staging, portal metastasis, histological grade, GPC3 and AFP (P<0.05);By contrast, in negative group, Ki67 index was not correlated with any clinical parameter (P>0.05). (3) Ki67 score increases with the progress of BCLC classification. Conclusion In Ki67 positive group, Ki67 index combined with GPC3, histological grade, BCLC staging, portal involvement, AFP and liver func?tion child-pugh classification can assess the severity of the patients with HCC, which is expected to become an important in?dicator for the HBV-related HCC early diagnosis.
