1.Computer Virtual Screening of Farnesoid X Receptor Inhibitors Based on the TCM Database

OBJECTIVE: To seek for the farnesoid X receptor (FXR) inhibitors from the TCM database using chemoinformatics.METHODS: Three-dimensional structure active fraction of FXR-fexaramine compound (PDB code: 1OSH) was analyzed using Cerius2 4.10 software package developed by Accerlrys company.Ligandfit module was applied for molecular docking.RESULTS: 10 compounds which were predicted to have good interactions with FXR were screened using docking score of original inhibitor (fexaramine) and the receptor as threshold value.CONCLUSION: The study could be helpful for development of new lipid-lowering drug and further study of mechanism of FXR and pharmaceutical molecular.
2.The life-saving emergency thoracic endovascular aorta repair management on suspected aortoesophageal foreign body injury

BACKGROUND: Fatal aortic rupture caused by esophageal foreign body (EFB), is associated
with a high mortality, but can be prevented by thoracic endovascular aorta repair (TEVAR)
that performed increasingly as technology improves. This study aims to investigate the cause,
management and prognosis of suspected penetrating aortoesophageal foreign body injury.
METHODS: Twelve cases who met the criteria were enrolled in this study. The demographic
and clinical data were reviewed for evaluating the characteristics of EFB.
RESULTS: Among 12 cases enrolled, 7 were males and 5 were females, with an age 27–86
years. The distance of EFB from aorta (DFA) of 7 cases were less than or equal to 0 mm, 5 cases were
0–2 mm. Eleven cases were managed with TEVAR, only one case was with open surgery standby but
fi nally treated by fl exible endoscopy (FE) successfully, without TEVAR. In group with TEVAR, EFB of
7 cases were successfully removed by rigid endoscopy (RE), and one of them was failed at the first
RE treatment. EFB of 2 cases were successfully removed by open surgery with TEVAR, and other 9
cases were managed by endoscopies with TEVAR. The mean length of stay of hospitalization (LOS)
and length of ICU stay of patients treated by open surgery with TEVAR (18.50±2.12 days and 5.50±0.71
days) was signifi cantly longer than those of patients treated by endoscopy with TEVAR (7.00±2.74 days
and 1.33±1.12 days, P<0.001 and P=0.001, respectively). Five cases had severe complications.
CONCLUSION: Rational application of TEVAR can be a life-saving management for
aortoesophageal foreign body injury, and jointed with endoscopy is safe and effective with a shorter
length of ICU or total hospital stay.
3.Investigation of ECG Holter System Based on SD Card

Objective To develop ECG Holter system with SD card as storage medium. Methods Data that used MSP430F449 SCM to acquire 3-channel ECG signal to record SD card through serial peripheral interface were reviewed and analyzed on computer. Results Portable ECG Holter System based on SD card is realized. The using of SD card can enhance storage performance. Conclusion Low-cost, and high-performance solution program is observed.
4.Clinical application of preop erative ultrasound-guided guide-wire localization in diagnosing tiny breast lesions

Objective To discuss the clinical application of ultrasound-guided preoperative guide-wire localization in diagnosing tiny breast lesions. Methods A total of 56 patients with impalpable tiny breast lesions were enrolled in this study. Preoperative guide-wire localization of the lesion was performed under high-frequency ultrasound guidance, which was followed by tracking resection of the lesion. The clinical data were retrospectively analyzed. Results Successful localization and complete resection of the lesion was accomplished in all 56 patients. The lesions included invasive ductal carcinoma (n=11), intraductal carcinoma (n=5), ductal carcinoma in situ (n=2), mucinous carcinoma (n=2), fibroadenoma (n=24), atypical hyperplasia (n=6), intraductal papilloma (n=4) and plasma-cell mastitis (n=2). Conclusion Ultrasound-guided preoperative guide-wire localization is a safe and reliable technique for the diagnosis of impalpable tiny breast lesions. It can precisely localize the lesion, which is very helpful for making a complete resection of the lesion and avoiding unnecessary breast injury. Therefore, this technique should be recommended in clinical practice.
5.Hemodynamic changes detected by transcranial doppler ultrasonography in iron deficiency anemia

Objective To analyze the hemodynamic characteristics in patients with iron deficiency anemia(IDA) using transcranial Doppler ultrasonography(TCD),and to discuss the diagnostic value of it.Methods There were 46 patients with IDA,among them 28 patients had moderate or severe anemia,and 18 patients had mild anemia.Hemodynamic changes in patients were observed with TCD,and compared with 40 case healthy controls. Results In addition to 3 case results of TCD were normal,the increase of velocity were found in the others patients.The cerenbral blood flow velocity of the moderate or severe anemia group were significantly higher than that of the mild anemia group(P<0.05),and these were significantly higher than that of the control group(P<0.01).The pulsatility index of the moderate or severe anemia group were significantly lower than that of the control group(P<0.01).2 case artery stenosis in 46 patients were found by TCD,there were 5 case with syncope in 46 patients.Conclusion The severness of anemia can be observed by the increased cerenbral blood flow velocities.TCD may be evaluat Effestiveness of blood flow,and TCD may be possible to raise the discover rate of artery stenosis in anemia.

7.Study on effect of Xiaoaiping in enhancing efficacy of neoadjuvant chemotherapy for breast cancer and its mechanism.

Traditional Chinese medicine (TCM) Xiaoaiping shows a pharmacological activity in treatment of breast cancer. Although neoadjuvant chemotherapy has been more and more widely used in treatment of breast cancer in recent years, no report has been made about the clinical efficacy and mechanism of the combined application of neoadjuvant chemotherapy and Xiaoaiping in treatment of breast cancer. In this study, 66 patients with breast cancer were selected and divided into the control group and the treatment group evenly with the random number table method. All patients received TEC neoadjuvant chemotherapy. On that basis, the treatment group also received the adjuvant therapy of Xiaoaiping injection (60 mL, i. v. , qd). The short-term response rate and the follow-up survival rate of the two groups were observed and compared. Surgical specimens of the patient were collected to observe and compare their expressions of estrogen receptor ER-α36 in breast cancer tissues with the immunohistochemical method. According to the findings, the overall response rate of the treatment group was 78.79%, which was significantly higher than that of the control group (57.58% , χ2 = 5.48, P < 0.05). Compared with the control group, the treatment group showed significant increases in the disease-free survival (DFS) rate and the total survival rate at the 3rd year and 5th year (all P < 0.05) , and a notable reduction in ER-α36 expression in breast cancer tissues (P < 0.05). Based on the our results, Xiaoaiping can significantly enhance short-term ad long-term efficacies of neoadjuvant chemotherapy for breast cancer. Its mechanism may be correlated with the inhibition of ER-α 36 expression in breast cancer tissues.

8.The study of piper futokadsura neoligans on brain protection after cerebral ischemia and reperfusion in rats

AIM To compare the effect of Piper Futokadsura Neoligans with Ginkgolides, a platelet activiting factor receptor antagonist, on cerebral ischemic area,region cerebral blood flow(rCBF) and neuroligic function deficits following focal cerebral ischemia/reperfusion in rats. METHODS The middle cerebral artery (MCA) focal cerebral ischemia and reperfusion rat models were used. The cerebral ischemic area, rCBF and neural function deficits were measured by TTC staining,hydyogen clearance and neurologic function scoring, respectively. RESULTS A significant reduction in infarct volume were found in the Piper Futokadsura Neoligans treated group as well as in the Ginkgolides treated group compared with the control group.Furthermore, Piper Futokadsura Neoligans induced significant amelioration of the rCBF decrease and neurologic dysfunction following focal cerebral ischemia and reperfusion in rats. CONCLUSION Both Piper Futokadsura Neoligans and Ginkgolides have remarkable brain protective effects.
9.Astrocyte proliferation in the rat hippocampus after middle cerebral artery occlusion

AIM: To study rat astrocyte proliferation in ipsilateral hippocampus following focal cerebral ischemia. METHODS: Ischemia was induced by temporary middle cerebral artery occlusion (MCAO). In hippocampus of rats at 3, 7 and 30 days after MCAO, the numbers and anatomic distribution of glial fibrillary acidic protein (GFAP) were detected by immunohistochemistry. The protein expression of GFAP and proliferating cell nuclear antigen (PCNA) in the ipsilateral hippocampus were analyzed by Western blot analysis. RESULTS: Astrocytes appeared hypertrophic, with increased process thickness and numbers at 7 days after MCAO, and the highest density of astrocytes were seen at 30 days in the CA1, CA2 regions of the ipsilateral hippocampus. Western blot analysis revealed that GFAP levels were normal at 3 days, but increased by 7 days and remained elevation at 30 days. Western blot analysis of PCNA protein also revealed identified upregulation PCNA at 3 days after MCAO and the expression peaked at 7 days. CONCLUSION: This study demonstrates that focal cerebral ischemia in the rat results in a rapid response, a process often referred to as reactive astrogliosis or glial scarring, from resident astrocytes of the ipsilateral hippocampus to the side of ischemia. 10.Correlation of blood transfusion volume to motility rate in the patients with known duration of transfusion-acquired HIV infection in China

Objective To investigate the association of blood transfusion volume with early death among recipients with known duration of infection in rural central China. Methods From January 1, 1999 to December 30, 2003, 76 individuals with duration of blood transfusion who met the study criterion had been found to be infected in the HIV/AIDS notified system in rural of Yuncheng Prefecture of Shanxi Province, China. A retrospective study was conducted to investigate Socio-demographic characteristics, transfusion volume, and mortality. Cox' proportional hazard regression model was used to analyze the data. Results By December 30, 2003, among 76 subjects with transfusion-acquired HIV infection, 24 recipients had died. Mortality rate and median survival was 45.64 and 8.3 per 1000 person years, respectively. Transfusion volume (HR=3.12, 95% CI: 1.18~8.25; P=0.0218) was significantly associated with mortality in recipients in the multiply Cox' proportional hazard regression model. Conclusions This study suggests that more units of transfusion in recipients with known duration of HIV infection can influence time to death in recipients in China.
