1.Comparison between Vaginal and Abdominal Myomectomy

Objective To investigate the clinical value of vaginal myomectomy. Methods From March 2005 to May 2008,78 women with abnormal uterine bleeding were diagnosed with submucous myoma by hysteroscopy combined with ultrasonography. All the myomas were located in the lower uterine segment,and the diameter of the tumor ranged from 30 mm to 63 mm. The patients were divided into two groups to underwent vaginal myomectomy (vaginal group,36 cases) or abdominal electrotomy (open group,42 cases). The operation time,intraoperative blood loss,postoperative hospital stay and recovery of the two groups were compared. Results The vaginal operation was accomplished in all of the 36 cases. Compared with the open group,the vaginal group had significantly shorter operation time [(64.7?10.4) min vs (71.1?11.3) min,t=-2.599,P=0.011],less intraoperative blood loss [(200.0?38.4) ml vs (253.6?47.6) ml,t=-5.412,P=0.000],quicker recovery of bowel movement [(18.5?4.3) h vs (30.9?4.7) h,t=-12.078,P=0.000],and shorter postoperative hospital stay [(4.9?1.2) d vs (7.0?1.3) d,t=-7.368,P=0.000]. Follow-up was available in all of the 78 cases for 3 to 40 months (mean,28 months),during which no relapse occurred,and no statistical differences in pregnancy rate between the two groups was revealed [2.8% (1/36) vs 4.8% (2/42),?2=0.000,P=1.000]. The operation satisfaction rates in both of the two groups were up to 100%. Conclusions Vaginal myomectomy has advantages in less hemorrhage,fast recovery,short postoperative hospital stay,and low complication rate. For the tumors located in the lower uterine segment or those larger than 30 mm in diameter,vaginal myomectomy is an alternative to hysteroscopic electrotomy.

5.Desferroxamine preconditioning protects against hypoxia in neurons

Objective To investigate the possible effects and underlying mechanisms of desferroxamine (DFO) preconditioning against hypoxia in neurons. Methods Cortical neurons were cultured in DFO under ischemia condition of oxygen-glucose deprivation (OGD). Cell viability was determined by cell counting kit-8 (CCK-8) method; apoptotic cell ratio was examined with Hoechst 33342 staining; the morphological change was observed. Middle cerebral artery was occluded with or without DFO administration to establish the cerebral ischemia rat model. Infarct sizes were examined by TIC staining, and the neurological severity score was evaluated. Meanwhile immunofluorescent staining was employed to detect the protein synthesis of hypoxia inducible factor-1 (HIF-1) and erythropoietin (EPO), RT-PCR was performed to detect the mRNA expression of HIF-1 and EPO as well Results Neuronal viability kept in 49% (OGD group was 25%, t =8. 544, P<0. 05), the rate of apoptosis was 38% (OGD group was 30%, t = 4. 409, P <0.05 ) after administration of DFO (post-DFO) , the morphology of neurons improved. In the model of focal cerebral ischenfia of 30 mg/kg group, neurological severity score was reduced, the percentage of brain infarct decreased 8.5% (t=4.649, P<0.05) 3 days post-DFO(vs control). In the 100 mg/kg group, neurological severity score was 7.44 ±0.39 (t=2.903, P<0.05 ) ,5.60±0.47 (t=10.143, P < 0.01 ) ,6.97 ±0.73 (t=3.142, P<0.05 ), the percentage of brain infarct decreased 12. 0% (t=5.056, P<0.05), 32.3% (t =10.993, P<0.01), 10.6% (t =4.385, P<0.05)2,3 and7 days post-DFO(vs control), respectively. Immunofluorescent staining found synthesis of HIF-1α and EPO in cultured cortex neurons after DFO pretreated; HIF-1α and EPO were upregulated in the neurons of rat brain after DFO pretreated. The mRNA of HIF-1α and EPO upregulated in vivo and in vitro. Conclusion DFO preconditioning can protect the brain against ischemic damage, which is related to the protective effect on neurons. The mechanism of DFO preconditioning may be involved in the expression of HIF-1α and EPO in vivo and in vitro.
6.LEEP conization on future obstetrics outcome

Objective To evaluate the implication of LEEP cervical conization on the outcome of subse-quent pregnancy. Methods The study group comprised 85 women who had a LEEP in Renmin Hoapital of Wuhan University during Jan. 2005 and Jan. 2007 ,and then had a subaequent pregnancy. 109 control women were extracted from outpatient clinic who received antenatal care in the same period with no history of cervical surgery, matching by age, health condition and perinatal stage. The pregnancy outcome of two groups were analyzed. Results Women who had a LEEP were more likely to give preterm delivery than controla (9.88% va 3.70%). But there waa no differ-ence in preterm delivery(χ2=2.97, P>0.05). So were low birth weight infants, preterm premature rupture of mem-branes (pPROM) or cesarean section. On a further study, we found that the time interval between cervical conization and subsequent pregnancy was associated with risk of preterm birth. The shorter time interval, especially shorter than 6 months,the higher risk of preterm birth. Conclusions LEEP cervical conization is not associated with an in-creased risk of preterm delivery, low birth weight infants, pPROM or cesarean section. LEEP conization is a more sol-id choice for women who want to preserve reproductive function. But it would be better for them to have pregnancy plan six months later.

8.Expression of SGK in uterine decidua of patients with unexplained recurrent spontaneous abortion

Objective To observe the expression of SGK in decidua in patients with unexplained recurrent spontaneous abortion(URSA) and to investigate its role in the course of apoptosis.Methods The expression of SGK was detected by immunohistochemistry in decidua of URSA(abortion group,n =50) and normal first trimester pregnant women(control group,n =30).The apoptotic index was detected by TUNEL.Results Compared with the control group,the positive expression of SGK decreased significantly in the abortion group,and the difference was statistically significant(x2 =6.78,P < 0.05).The average apoptotic index of the abortion group was (8.19 ± 3.58) %,which was significantly higher than (2.87 ±1.07)% of the control group(t =7.94,P<0.05).Conclusion Intensive apoptosis,the decreasing positive expression rate of SGK in decidua cells may play critical roles in URSA.
9.Preventive Effect of Jie-Gu-Tang on Osteoporosis Model Using Zebrafish Induced by Prednisolone

Prednisolone-induced zebrafish osteoporosis model was used to explore the bone-strengthening effect of Jie-Gu-Tang (JGT). Zebrafish larvae of 5 days post fertilization (d.p.f.) were co-exposed with 25 μmol·L-1 pred-nisolone and a series of JGT solutions with a range of concentrations (0.025, 0.25, 2.5, 25 and 100 mg crude herb per liter). The 25 μmol·L-1 prednisolone was selected as the model group. Etidronate disodium (15 and 30 mg·mL-1) with 25 μmol·L-1 prednisolone was used as the positive group. And 0.5% DMSO was used as the vehicle control group. All groups were incubated in 24-well plates (28.5℃) until 10 d.p.f. Zebrafish skeleton at 10 d.p.f. was anes-thetized and fixed for staining with alizarin red. Quantitative analysis of the stained area was performed by microscop-ic inspection and digital imaging methods to reflect the amount of zebrafish head skeleton mineralization. The results showed that prednisolone group at 25 μmol·L-1 concentration can obviously decrease the staining area and the stain-ing optical density values when compared with the vehicle control group (0.5% DMSO). Compared with the model group, both etidronate disodium (15 and 30 mg·mL-1) and JGT (2.5, 25 and 100 mg crude herb per liter) can in-crease the mineralized matrix and integrated optical density (IOD) of zebrafish head skeleton significantly with dose-effect relationship. It was concluded that zebrafish osteoporosis model was successfully used in the evaluation on bone loss prevention and bone formation promotion of JGT, which provided basis for the reliability and reasonability of zebrafish model.
