With the development of molecular biology and molecular genetics,more attention has been taken to the advanced techniques which can be used to detect minority mutations.In recent years,many techniques such as amplification refractory mutation system,PCR clamping,pyrophosphorolysis-activated polymerization,co-amplification at lower denaturation temperature-PCR and digital PCR have been developed,which will make the hereditary disease diagnosis,cancer detection,noninvasive antenatal diagnosis step into a new stage in the near future.This review presents an overview of the principles,aoolications,advantages and disadvantages of these techniques.

Objective[WT5”BZ] To evaluate the effect of hepatectomy under direction of ultrasound guided segmental dye staining combined with intraoperative chemotherapy following segmental portal vein branch occlusion by a balloon catheter.[WT5”HZ]Methods[WT5”BZ] 12 cases of hepatic carcinoma underwent this novel therapy(group A).Results were compared with that of 21 cases treated by hepatectomy (group B).AFP,CT and MRI were used regularly during the follow up period of 24 months to evaluate the outcome.[WT5”HZ]Results[WT5”BZ] The average disease free survival time is 21 months in group A, and 11 months in group B. Recurrent carcinoma was found in 2 cases during the fellow up period in group A, and in 13 cases in group B.[WT5”HZ]Conclusions[WT5”BZ] The preliminary result showes that our new approach to the treatment of hepatic carcinoma is more effective than traditional hepatectomy. As one of exploratory data analysis method, the clustering analysis can provide technical support for objectivity and standardization ofTCM. This paper dealt with the application of the clustering analysis in the research of Chinese medcine, TCM pattern differentiation and the regularity of TCM herbal formulae. The clustering analysis showed great value on classifying imformation and finding out regularity from data, only if the characteristics of TCM data were considered.

Background and purpose:Zhongshan City is one of the areas with the highest NPC incidence worldwide and nationwide. Although its incidence rate had been analyzed thoroughly, but its mortality rate was not studied further until now. This study was to explore NPC mortality status in Zhongshan City from 1970 to 2010, and to provide scientiifc information for its control and prevention.Methods:NPC death data in Zhongshan City from 1970 to 2010 came from Zhongshan Cancer Registry Ofifce. Such indices as its death number, crude rate, age standardized rate (ASR) and truncated rate etc were calculated and analyzed. Results:There were 5 267 NPC death cases in Zhongshan City from 1970 to 2010, its crude world ASR and truncated rates were 11.03/105, 12.34/105 and 29.31/105 respectively. Its world ASRs increased before 1999 but no statistical signiifcance (P=0.64) and decreased thereafter with obvious statistical signiifcance (P=0.001). But overall, there were a decreasing trend for its ASRs (P=0.001) from 1970 to 2010 in Zhongshan City. Moreover, NPC mortality rate in male was remarkably higher than in female, the death peak age was 55-59 age group, and the different age groups were with different trends fro 1970 to 2010. Conclusion:Although with declining trend from 1970 to 2010, NPC mortality rate in Zhongshan City was at high level worldwide. It suggested that NPC control and prevention should be enhanced.

Objective To investigate the clinical value and effect of peripherally inserted central eatheter(PICC) in patients with tuberculous meningitis. Methods 56 patients with tuberculous meningi-tis were randomly divided into the treatment group and the control group with 28 cases in each group. The treatment group was medicated by PICC, while the patients of the control group were medicated through pe-ripheral superficial vein. The curing rate, degree of anxiety, infusion response, satisfaction with nursing and related expense were recorded. Results The curing rate and satisfaction with nursing of the treatment group were higher than those of the control group. The anxiety degree, infusion response and related ex-pense of the treatment group were lower than those of the control group.Their difference had statistical sig-nificance. Conclusions PICC has clinical value in patients with tuberculous meningitis,which is worth promoting.

Objective To assess the effect of clomiphene citrate(CC)on the proliferative stage of endometrium by the addition of low-doge estrogen(progynova,PGV)and aspirin.Methods Sixty women with unexplained infertility were divided into 4 groups randomly.Untreated group,CC treated group(C group),CC+PGV treated group(CE group)and CC+PGV+aspirin treated group(CEA group),each group Was 15 cases.Endometrium thickness,pattern of endometrium and resistant index(RI)of uterine artery were detected by ultrasound,all at end proliferative stage of endometrium.Results The endometrium thickness,the rate of the endometrium thickness≥8 mm and the rate of A-endometrium in C group were significantly less than those in other 3 groups(P<0.05).The rate of A-endometrium in CE group was significantly less than that in untreated group and CEA group(P<0.05).The number of follicle diameter≥15 mm in untreated group(1.01±0.25)was signifieantly less than that in other 3 groups(1.56±0.64,1.52±0.70,1.61±0.67)(P<0.os).RI of uterine artery in C group(0.93±0.07)and CE group(0.95±0.04)were significantly higher than that in untreated group(0.81±0.05)and CEA group(0.83±0.03)(P<0.05).Conclusion Low-dese estrogen can reverse the deleterious effect of CC on.endometrial development,and low-dose estrogen and aspirin can promote uterine perfusion and improve uterine compatibility.

Objective To observe the effect of different doses of propofol injection and propofol medium/long-chain fat emulsion injection in short time infusion on plasma ketone body ratio,to eva-lute its effecton hepatic energy metabolism.Methods Forty patients,aged 18-50 years old,ASA Ⅰ orⅡ undergoing selective surgery were randomly divided into 4 groups with 10 cases in each;propofol injection 4 mg·kg-1·h-1 maintain anesthesia (group L4 ),propofol injection 6 mg·kg-1·h-1 maintain anesthesia (group L6 ),propofol medium/long-chain fat emulsion injection 4 mg·kg-1·h-1 maintain anesthesia (group M4 ),propofol medium/long-chain fat emulsion injection 6 mg·kg-1·h-1 maintain anesthesia (group M6 ).MAP,HR,SpO2 and PET CO2 were recorded before anesthesia induction (T0 ),after tracheal intubation (T1 ),after 2 hours infusion of propofol (T2 )and operation completed (T3 ).The blood samples were collected at T1 and T2 to detect the level of acetoacetate,β-hydroxybu-tyrate and to calculate the blood ketone body ratio (the ratio of acetoacetate andβ-hydroxybutyrate). Results MAP,HR,SpO2 ,PET CO2 at T0-T3 and acetoacetate,β-hydroxybutyrate,blood ketone body ratio at T1 ,T2 showed no significant statistic difference.Conclusion Different doses of propofol and different doses of propofol medium/long-chain fat emulsion injection in short time continuous in-fusion has no obvious effect on hepatic energy metabolism;same dose of propofol injection and propo-fol medium/long-chain fat emulsion injection in short time continuous infusion has no obvious effect on hepatic energy metabolism.

OBJECTIVE:To compare the cost-effectiveness of4antibacterials in the prevention of post-operative in-fection in patients with uterine-incision delivery(UID).METHODS:A total of1066cases with UID were ascribed to receive cefoperazone sodium/sulbactam sodium+arilin(therapy A),ampicillin sodium/sulbactam sodium+arilin(B),cefradine+ar-ilin(C)and cefoperazone sodium/sulbactam sodium(D),respectively for the prevention of infection.The cost-effectiveness of the4groups was analyzed retrospectively.RESULTS:The costs of the4schemes were4954.13yuan,4752.77yuan,3870.16yuan and3988.22yuan,respectively;The effective rates were98.5%,87.4%,84.6%and96.8%,respectively;The cost-effectiveness ratios were50.30,54.38,45.75and41.20,respectively;The incremental cost-effectiveness ratios of scheme A,B and D were77.98,315.22and9.68,respectively as against scheme C.CONCLUSIONS:Scheme D is more economical and reasonable in the prevention of post-operative infection in patients with UID.

AlM:To investigate the effect of peripheral vision control technology for delaying the development of juvenile myopia.METHODS:A total of ninty-nine cases of 12 ~18 year-old myopic patients were randomly divided into two groups. The experimental group was peripheral vision control technology group whose members wore the special lenses which can help correct the hypermetropic defocus of peripheral retina. The other was control group whose members wore ordinary monofocal lenses. All the subjects needed to accept re-examination every 3mo and be recorded the data of dioptre, corneal curvature and axial length.RESULTS: After 18mo, the dioptre and axial length of two groups had increased in varying degree. But the data's different quantity of the corneal curvature in each group had no statistical significance ( P > 0. 05 ). Themyopia deepen quantity in experimental group was-0. 65±0. 65D and its axial growth was 0. 23±0. 22mm, and the myopia deepen quantity in control group was -1. 17 ± 0. 50D and its axial growth was 0. 41 ± 0. 17mm. Under the circumstance of valid data, the increment of myopia and axial length in experimental group was lower than that in control group (P<0. 05).CONCLUSlON: Peripheral vision control technology can delay the development of juvenile myopia effectively.

Objective To analyze the clinical data of Staphylococcus aureus sputum culture positive cases and the antimicrobial susceptibility of Staphylococcus aureus strains isolated from them and provide the evidence for clinical diagnosis and treatment. Methods Retrospectively analyze the clinical data of Staphylococcus aureus sputum culture positive cases of our hospital from January 2014 to December 2014. Results The Staphylococcus aureus sputum culture positive cases was 111 in all, while antimicrobial sensitive test was performed in 110 cases, including MSSA in 27 cases (24.55%), MSRA in 83 cases (74.45%), community infection MSRA in 13 cases (15.66%), hospital infection MRSA in 70 cases (84.34%). The cases were mainly from the department of respiratory diseases, department of neurosurgery, ICU, the department of neurology, accounting for 75.68%. The risk rate of community acquired MRSA in patients from nursing center were significantly higher than that of hospital infection MRSA and MSSA (P = 0.004). The antimicrobial susceptibility rate of community infection MRSA to clindamycin, gentamicin, erythromycin, azithromycin was higher than that of hospital infection MRSA. Conclusion The susceptibility rate in non-β-lactams of community infeciton MRSA was higher than that of hospital infection MRSA.
