1.Curative effect analysis of D2 lymphadenectomy for distal gastric cancer with pancreas as the central landmark

Objective To investigate the feasibility and clinical significance of D2 lymphadenectomy for distal gastric cancer in which the pancreas were regarded as the central landmark. Methods 26 patients of advanced distal gastric cancer were performed D2 lymphade-nectomy centered on the pancreas from March 2012 to February 2013. The operation time, amount of bleeding,number of lymph nodes,and postoperative complications were recorded. Results All the 26 patients underwent the operation successfully. The amount of bleeding was 50~200 mL(averagely 110 mL),and the number of lymph nodes which were cleaned was 16~35 (averagely 26). One patient had intra-ab-dominal hemorrhage and one patient had lymphorrhagia after the operation,and they were cured finally. There was no local recurrence,no dis-tant metastasis,and no death case occurred during the follow-up. Conclusion D2 lymphadenectomy for distal gastric cancer with pancreas as the central landmark is safe and feasible, and it is of lower incidence of complications .
2.A model-based meta-analysis to compare urate-lowering response rate of febuxostat and allopurinol in gout patient.

This study aims to compare the urate-lowering response rate of febuxostat and allopurinol in gout patient using a model-based meta-analysis. The literature search identified 22 clinical trials of gout with a total of 43 unique treatment arms that met our inclusion criteria, and a total of 6 365 gout patients were included in the study. The response rates of allopuriol and febuxostat were characterized by Tmax model and Emax model respectively, and the effect of baseline serum uric acid (sUA) and patient type on the drug effect was tested. The results showed that allopurinol can reach an average maximum response rate of 50.8% while febuxostat can reach a 100% response rate within a very short time, and the ED50 was 34.3 mg. Covariate analysis revealed that baseline sUA has a negative effect on response rate of allopurinol, and a positive effect on the predicted ED50 of febuxostat. For patients who had shown inadequate response to prior allopurinol treatment, the average response rate was about half that of the allopurinol responder patients.

3.Effects of Jinmaitong serumon the expression of β-catenin, GSK-3β and myelin protein zeroin Schwann cells of rats cultured in high glucose medium

Objective To investigate the effect of serum containing different concentrations of traditional Chinese medicine Jinmaitong on β-catenin, GSK-3β and P0 in Schwann cells cultured in high glucose medium.MethodsTwenty-five male Sprague-Dawley rats were randomly treated with 5, 10, 15 and 20 times of Jinmaitong and distilled water.Schwann cells were divided into six groups, which are control group, high glucose group, 5 times group, 10 times group, 15 times group, 20 times group.72 hours later, the proliferative activity of SCs cells were detected by CCK, the mRNA and protein expression of β-catenin, P0 and GSK-3β was detectived by rt-PCR and Western blot.Results High glucose medium could inhibit the proliferation of Schwann cells, down-regulate the expression of β-catenin and P0(P<0.01), and up-regulate the expression of GSK-3β(P<0.05) mRNA significantly.But Jinmaitong can invert the results (P<0.01, P<0.05).Conclusions High glucose medium will injure the proliferation of Schwann cells, but Jinmaitongcan increase the proliferation activity of Schwann cells, and promotes the secretion of P0 partially dependent on up-regulating the activity of β-catenin and down-regulating the activity of GSK-3β.
5.Differentiation of muscle-derived stem cells into neuron-like cells induced by ciliary neurotrophic factor and Compound Salvia Miltiorrhiza Injection in vitro

BACKGROUND: The low survival rate of neuron cells is one of the main mechanisms of stem cell allograft, which might lead to the failure of allograft. Nuclear factor-KB (NF-kB) is one of main transcription factors for cell signaling transduction and participates in call proliferation and differentiation.OBJECTIVE: To study the differentiation of muscle-derived stem cells (MDSCs) into neuron-like cells induced by ciliary neurotrophic factor (CNTF) and Compound Salvia Miltiorrhiza Injection in vitro, and the expression of NF- kB.DESIGN, TIME AND SETTING: The experiment at cell molecular level was performed at the Oral Science Laboratory of the Second Affiliated Hospital of Liaoning Medical College from March to May 2008.MATERIALS: A total of 10 Sprague Dawley neonatal rats aged 7 days were supplied by Experimental Animal Center, Liaoning Medical University. CNTF (Sigma, USA) and Compound Salvia Miltiorrhiza Injection (Chiatai Qingchunbao Pharmacantical Co.,Ltd., China) were used in this study.METHODS: Rat MDSCs were harvested in vitro, pudfied by differential adherence and enzyme digestion, and incubated in 6-well plate. Samples in the induction group were incubated in DMEM containing CNTF for 24 hours. The medium was changed.Subsequently, samples were dnsed three times, and then incubated in serum-free DMEM supplemented with Compound Salvia Miltiorrhiza Injection for 5 hours. Samples in the control group were treated with serum-free DMEM.MAIN OUTCOME MEASURES: Neurofilament protein and NF-KB inhibitor protein expression were detected using reverse transcription-polymerase chain reaction (RT-PCR) and Westem blotting.RESULTS: No neurofilament protein expression was found in MDSCs before induction, and neurofilament protein-positive MDSCs were detected following induction. 6.Clinical research on the effect of abdominal Tuina and acupuncture in treating Swiss generalized anxiety disorder

Objective To observe the clinical effect of abdominal Tuina and acupuncture in treating Swiss generalized anxiety disorder (GAD). Methods 80 patients of GAD caused by liver depression-qi stagnation were randomly recruited into a treatment group and a control group, 40 patients in each. The treatment group was treated with abdominal tuina & acupuncture; while the control group was treated with acupuncture exclusively. Clinical effects and hamilton anxiety scale (HAMA) score were observed in both groups before and after the treatment. Results ①There was significant difference in total efficacy between the two groups CP<0.05), and the treatment group was better than the control group. ②The HAMA score of the two groups after the treatment was lower than that before the treatment, and the difference was significant (P< 0.05). There was also significant difference of HAMA score between the two groups after the treatment (/><0.05), with the treatment group showing better results than the control group. Conclusion Abdominal tuina combined with acupuncture has sound effects in treating Swiss GAD due to liver depression-qi stagnation.
7.Factors influencing long-distance transportation and medical rescue of trauma patients attributable to China Wenchuan earthquake

Objective To explore the influencing factors during long-distance transportation and medical rescue of trauma patients in Wenchuan earthquake. Methods After Wenchuan earthquake, based on prehospital care scheme, we organized medical coordinators and psychologically trained medical staff in emergency medicine, cardiovascular medicine and logistics to transport trauma patients and pro-vide early psychological intervention during whole course of transportation. Results A total of 162 trauma patients were safely transported from Sichuan province to Chongqing relying on prehospital care scheme, equipment supplies, integrated rescue mode and psychological intervention during the transporta-tion. Conclusions Medical rescue mode and psychological intervention indicate that it is necessary to conduct psychological intervention for better transportation of patients for further medical treatments. In the meantime, we should establish medical rescue system for coping with psychological stress, improve rescue preparation scheme and professional medical care team for long-distance transportation and guaran-tee sufficient medical supplies in station hospitals.

9.Signal transduction pathway ERK1/2 involved in cytotoxicity of NK cell lines

Objective:To determine the roles of constitutively activated signal transduction pathway ERK1/2(p44/42 MAPK)in controlling cytotoxicity or proliferation activity of natural killer cell lines.Methods:Whole cell extracts from human NK cell lines(YT,IL 2 independent;NK 92,IL 2 dependent) was used in Western blot to detect the constitutively activated signal transduction pathway(s) in human NK cell lines. Specific inhibitor PD098059 was used to abrogate phosphorylation of ERK1/2 for further evaluation. A MTT based method was applied to analyze the cytotoxicity and proliferation capability of NK cell lines before and after specific inhibition of ERK1/2 activation by PD098059.RT PCR protocol was applied to analyze the cytotoxic related molecules possibly engaged with the ERK1/2 signal transduction pathway.Results:Western blot showed that signal transduction pathway ERK1/2,NF ?B and STAT3 was constitutively phosphorylated in two representative human NK cell lines YT and NK 92 which killed NK sensitive K562 target efficiently,other sigal pathways(STAT1,STAT6,p38 MAPK and PI 3K) were not activated.ERK1/2 inhibitor PD098059 apparently inhibited cytotoxicity of NK cell lines but did not influence their proliferation potential, RT PCR analysis revealed that the expression of lytic related molecules including IFN?,FasL,and perforin was downregulated to distince degree by PD098059 as it blocked the phosphorylation of ERK1/2 in NK cell lines.Conclusion:The constitutively phosphorylated signal pathway ERK1/2 in NK cell lines mainly involved in transducing signals controlling the cytotoxic capacity but not the proliferation potential of NK cells,ERK1/2 regulated the lytic capacity of NK cells by inducing the expression level of lytic related molecules including IFN?、FasL and perforin. Hormone replacement therapy (HRT), involving giving sex steroid hormones such as estrogen alone or with a progestogen, is widely used in postmenopausal women.HRT helps to relieve menopausal symptoms and has also been shown to prevent osteoporosis.Although most observational studies have showed that HRT can reduce the risks of cardio-cerebrovascular diseases, the subsequent randomized controlled trials were inconsistent with the results.This article reviews the relationship between HRT and stroke from drug type, route of administration, estrogen dosage, and initiation time.
