1. Effect of atorvastatin on the adverse events after middle cerebral artery stenting

Objectives: To investigate the effect of atorvastatin on the adverse events after middle cerebral artery stenting. Methods: Twenty-four patients who successfully completed middle cerebral artery stenting (restenosis rate ≥ 70%) were randomly assigned to 2 groups; treatment group (taking atorvastatin 40 mg + plavix 75mg + aspirin enteric-coated tablet 300 mg, once a day) and control group (taking plavix 75mg + aspirin enteric-coated tablet 100 mg, once a day) (n = 12 in each group). The patients were followed up at 1, 3, 6, and 12 months after procedure. New adverse events (transient ischemic attack, cerebral infarction, restenosis, and reintervention) were recorded. Results: Compared with the control group, the blood lipid level and C-reactive protein level were decreased significantly at all time points in the treatment group (P < 0.05 or P < 0.01), except the LDL-C level at 1 month. The total incidence of adverse events in the treatment group was lower than that in the control group. There was statistically significant difference (P =0.00017); however, the C-reactive protein level was higher than that before procedure, and it was lower than that before procedure until 12 months. The blood lipid level after procedure was significantly lower than that before procedure in the treatment group. Conclusion: Atorvastatin treatment after middle cerebral artery stenting may effectively prevent the occurrence of adverse events after stenting.
2.Determination of Paeonol and Imperatorin in Cangzhi Nose Spray by HPLC

OBJECTIVE:To establish a HPLC method for determination of paeonol and imperatorin in Cangzhi nose spray. METHODS:HPLC system consisted of ODS C 18 column(150mm?4.6mm,5?m)methanol-water(6∶4)mixture as mobile phase,with detection wavelength at310nm,flow rate1.0ml/min.RESULTS:The average recoveries of paeonol and imperatorin were99.4%(RSD=0.69%)and99.9%(RSD=2.66%)respectively.CONCLUSION:The method is sensitive,rapid and ac?curate.
4. Porphyromonas gingivalis infection induced reproductive abnormalities in mice

Objective To establish a pregnant mouse model infected with Porphyromonas gingivalis (P.g), and investigate the relationship of P.g infection to prematurity and associated birth abnormalities. Methods Fifty two female mice were randomly divided into P.g infection group (n=26) and control group (n=26). Mice in P.g infection group were anesthetized, the pulp cavity of the first molar was opened and directly injected with W83 strain P.g, and the tooth was then filled. Six weeks after infection, the mice were mated with males and the formation of vagina plug was recorded as 0d. The P.g extracted from the granulation tissue in tooth root was cultivated. The pregnant days and the connatal body weight of infant mouse were recorded, the serum and placental tissue were collected to assess the systemic and local conditions during pregnancy. Results After periodontal P.g infection, the TNF-α, IL-17, IL-6 and IL-1 levels in peripheral blood sera increased significantly. The average gestation was shorter in P.g infection group (18.25d) than in control group (20.45d, P<0.01), and the connatal body weight of infant mouse was also less in the former than in the latter (P<0.01). Immunohistochemistry and PCR revealed the existence of P.g in placenta tissue. P.g infection caused premature rupture of membranes, placental abruption, degeneration and necrosis of trophoblastic and endothelial cells; significantly increased the number of neutrophils and macrophages in placenta tissues, and increased the expression of local TNF-α and COX-2 inflammatory factors at the same time. In P.g infection group, the expressions of CD-31 in endothelial cells of placenta tissues and the apoptotic factor caspase-3 decreased, and the DNA oxidative damage index 8-OHdG increased. Conclusions P.g infection in female mice may cause premature birth and lower connatal body weight of infant mouse, and increase the expression of serous and local inflammatory factors in the placenta. It is suggested that timely removal of periodontitis or gingivitis can prevent some of reproductive abnormalities.
5.Application of double joystick technique in reduction-internal fixation for femoral shaft fracture in adults.

OBJECTIVETo study the applied value of double joystick technique in reduction-internal fixation for femoral shaft fracture in adults.
METHODSThirty-four patients (24 males and 10 females) with femoral shaft fractures were treated with reduction assisted by double joystick technique and internal fixation with interlocking intramedullary nail from September 2010 to June 2013. The average age of the patients was 41 years old, ranged from 17 to 65 years old. The duration of the disease course ranged from 3 to 7 days, with a mean of 5 days. The fractures belonged to AO types 32A (5 cases), 32B (20 cases) and 32C (9 cases) and located in left femur for 18 patients and right femur for 16 patients. The patients were followed up, and fracture healing and complications were observed. The curative effect were evaluated according to Thorsen femur fracture evaluation standard.
RESULTSThe operative time ranged from 40 to 110 min (mean 75 min) and intraoperative blood loss ranged from 200 to 300 ml (mean 250 ml). All the patients obtained a good fracture reduction and were followed up for 12 to 24 months (mean 18 months) after the surgery. All the fractures united between 4 and 8 months with a mean of 5 months. No complications such as breakage of nail, infection, osteofascial compartment syndrome, refracture and fracture malunion were found. According to Thorsen femur fracture evaluation standard, 30 patients obtained an excellent result, 3 good and 1 fair.
CONCLUSIONIn surgery of reduction-internal fixation for femoral shaft fracture in adults, the use of double joystick technique obtains good reduction result, short operative time, less injury, high healing rate of bone fractures, less complications and good limbs function, so it is worthy of popularizing in clinic.

6.Research progress of novel bispecific monoclonal antibody Faricimab in the treatment of diabetic macular edema and age-related macular degeneration

Diabetic macular edema(DME)and age-related macular degeneration(ARMD)are the leading causes of visual impairment and blindness worldwide, and their common pathological features are increased vascular permeability and abnormal neovascularization, in which cytokines such as vascular endothelial growth factor(VEGF)and angiopoietin-2(Ang-2)play an important role. Intravitreal injection of anti-VEGF agents significantly changed the clinical management of DME and ARMD, but limitations such as the non-responsive cases, the treatment burden and risks caused by frequent injections need to be overcome. Faricimab, a novel bispecific monoclonal antibody that simultaneously targets VEGF-A and Ang-2, can effectively reduce vascular permeability, decrease the number of neovascularization and alleviate retinal edema. Registered clinical studies have shown that Faricimab is effective in improving vision and reducing retinal edema, which is non-inferior to Aflibercept and Ranibizumab, maintains a long dosing interval, and has a high safety profile. This article reviews the latest advances in the treatment of DME and ARMD with Faricimab.
8.Clinical study on the effects of Buyang-Huanwu decoction combined with western medicine on early renal injury of patients with type 2 diabetes

Objective To investigate the effects of Buyang-Huanwu decoction combined with western medicine on early diabetic renal injury.Methods 200 cases of early diabetic renal injury were randomly recruited into study group and control group,with 100 cases in each group.The control group was treated with insulin injection and oral benazepril hydrochloride,and the blood pressure,blood lipid changes were closely monitored; while the study group was additionally treated with Buyang-Huanwu decoction on the basis of the control group.Results The total therapeutic effects in the study group was significantly higher than the control group (x2=15.62,P<0.01).The values of FBG,2 h PG,DBP,SBP,HbAlc in both groups were significantly reduced after the treatment (P<0.05,t=4.23,4.47,3.59,3.87,3.99 in the study group,t=4.34,4.76,4.05,3.78,3.69 in the control group) ; but the difference between the two groups had no significance (P>0.05).Before treatment rnALB,Urea,Cr,TG and TC were not significantly different (P>0.05) ; the values of mALB,Ureaand Cr were significantly reduced in the study group after the treatment (P<0.05,t=4.56,3.73,3.28);especially mALB in the study group was obviously lower than the control group (P<0.05,t=5.01).Conclusion Buyang-Huanwu decoction combined with western medicine was effective to treat early renal damage of type 2 diabetes,with the functions of reducing urinary protein,improving microcirculation and protecting renal function.
9.Hematologic effects of linezolid in advanced aged patients

Objective To evaluate the hematologic effects of linezolid in advanced aged patients with hospital onset of infection.Methods The clinical characteristics and complete blood routine examination results of 45 elderly patients aged(92.9±5.2)years treated with linezolid over 600 mg of lowest dose everyday for more than 72 hours were retrospectively analyzed.Results Among the 45patients,20 patients(44.4%)had substantially lower platelet,6 patients(13.3%)lower haemoglobin,1 patient(2.2%)lower absolute neutrophil,and the linezolid treatment in 19 patients were discontinued due to hematological system adverse reactions.The significant drop in platelet occurred at(10.2±3.3)days after treatment.The significant drop in hemoglobin occurred at(9.8±3.0)d after treatment.The lowest count of platelet and hemoglobin took place at(13.1±3.9)d and (10.5±3.5)d after treatment,respectively.The drops in platelet and hemoglobin of some patients took place after cease of treatment.Stepwise logistic regression analysis revealed that treatment duration(OR =1.407,P<0.05)and creatinine clearance rate(OR=0.732,P<0.05)were the risk factors for thrombocytopenia.Conclusions The elderly patients using linezolid more likely suffer from adverse reactions of hematological system,particularly thrombocytopeniawhich is significantly more common in patients with renal insufficiency and prolongation of treatment time.The blood routine examination should be monitored closely when elderly patients are treated with linezolid.
10.Influence of atorvastatin on ischemic adverse events following cerebral middle artery stent implantation: A 12-month follow-up in 24 cases

BACKGROUND: Intravascular stent implantation is one of the main methods for intra-extracranial artery stenosis. The safety and efficacy of stent implantation has been elevated, and symptoms or ischemic advert events with artery stenosis has been reduced. However, risks of thrombosis, cerebralhemorrhage, restenosis, and hyperperfusion still existed. OBJECTIVE: To investigate the influence of atorvastatin on ischemic adverse events following cerebral middle artery stent implantation.METHODS: Twenty-four patients received cerebral middle artery stent implantation were randomly divided into 2 groups. In the treatment group, patients were received atorvastatin 40 mg + plavix 75 mg + aspirin enteric-coated tablet 300 mg, once per day; in the control group, patients were received plavix 75 mg + aspirin enteric-coated tablet 300 rag, once per day. Ischemic adverse events, such as in-stent restenosis, transient ischemic attack, cerebral infarction, or re-interventional therapy were observed by transcranial Doppler, digital subtraction angiography, nuclear magnetic resonance imaging, diffusion weighted imaging, blood lipid level and C-reactive protein level examinations prior to and at months 1, 3, 6 and 12 after stent implantation.RESULTS AND CONCLUSION: Compared with the control group, the levels of blood lipid and C-reactive protein were obviously decreased at months 1, 3, 6 and 12 after treatment (P<0.05, P<0.01 ), with dramatically declined ischemic adverse events (P<0.05, P<0.01 ). Compared with before operation, the levels of blood lipid and C-reactive protein were decreased after operation in the treatment group (P<0.05, P<0.01). No server adverse events occurred in patients treated by atorvastatin, which showed 40 mg was safe for patients. The results revealed that atorvastatin combined with antiplatelet therapy can prevent ischemic adverse events following cerebral middle artery stent implantation.
