OBJECTIVETo explore the clinical effects of minor bone anchors and palmaris longus tendon graft in treating chronic mallet fingers deformity.

METHODSFrom January 2008 to June 2013, 26 patients with chronic mallet fingers deformity were treated with minor bone anchors and palmaris longus tendon graft. There were 18 males and 8 females, aged from 18 to 52 years old with an average of (32.0±1.3) years. Among them, 8 cases caused by machine injury, 6 cases by fall injury, 6 cases by sprain from fight, 4 cases by tendon spontaneous rupture, 2 cases by knife trauma. There was no tendon attachment of extensor tendon check in 16 cases, and with 0.3 to 0.5 cm tendon attachment in 10 cases. All patients had the flexion deformity and the disability of dorsiflexion activity. During operation, the distal interphalangeal joint was fixed in 10° to 20° dorsiflexion by a Kirshner wire, the minor bone anchor was used to reconstruct the extensor tendon insertion, the palmaris longus tendon slice was transplanted the decayed area of extensor tendon insertion. Four weeks postoperatively, the Kirshner wire was removed and the plaster external fixation was used, and the patient began function exercises. Postoperative complications were observed and fingers functions were assessed according to Dargan standard.

RESULTSThe patients were followed up from 6 to 14 months with an average of (5.0±0.3) months. Wound superficial infection occurred in 2 cases, the skin pressure ulcer in 2 cases, joint activities disability in 1 case; these symptoms got improvement after symptomatic treatment. Traumatic arthritis occurred in 2 cases, 1 case was improved after treatment, and 1 case had chronic pain for a long time. No internal fixation loosening or breakage and tendon rupture were found. According to Dargan standard to evaluate the finger function, 17 cases got excellent results, 8 good, and 1 poor.

CONCLUSIONIt is an effective way to treat the chronic mallet finger deformity using minor bone anchors and palmaris longus tendon graft, and the method has advantages of reliable fixation, easy operation, satisfactory effect and less complication.

The plastic splint PCL,which is introduced in this article,is a new type of medical exter- nal immobilization material.During the course of its development,the burden has been optimality seeked with regressive orthogonality after screening of the material.Its function has attained the international levels during the eighties.This splint may be used not only in the treatment of burns and various orthopedic conditions,but also in the immobilization of fracture and rehabilation treatment of orthopedic patients etc.

To study the influence of foreign plasmid DNA on spleen metabolism in immune-stimulated mice via the gastrointestinal tract. Mice were oral administered by pipette with 200?g of plasmid pcDNA3 and spleen were isolated at 4h and 18h after oral administration. Total RNA were extracted from spleen and spleen gene expression profile of Balb/c mice was analyzed by using Affymetrix oligonucleotide genechip after oral plasmid pcDNA3 administration. Functional cluster analysis was conducted by Genmapp and MAPPFinder software. By functional cluster analyzing the genes which were up-regulated more than twofolds, Genmapp results showed that plenty of metabolic pathway were induced in spleen after oral administration of plasmid pcDNA3. These metabolic process included purine metabolism, pyrimidine metabolism, protein synthesis, cholesterol synthesis, fatty acid synthesis, Glycolysis, TCA cycle and mitochondria oxidative phosphorylation pathway. The similar results also took place at 18h after oral administration. The result indicated that foreign plasmid DNA can modulate metabolism process in spleen of mice via the gastrointestinal tract, and may help understand the mechanism of action of foreign plasmid DNA uptaked via the gastrointestinal tract.

Objective To evaluate remedy therapy for biliary restenosis after repair of bile duct injury.Methods Clinical data of 60 patients with bile duct injury including 16 patients with restenosis after repair admitted to Renji Hospital affiliated to Shanghai Jiaotong University School of Medicine from January 2000 to December 2012 was retrospectively analyzed.Results 16 patients suffering from biliary duct restenosis included 3 cases of type Ⅱ 1 d,10 cases of type Ⅱ2 d,2 cases of type Ⅱ 3 d and 1 case of type Ⅱ 4 d.The reoperative procedures included hepatic hilar biliary plasty with bilioenteric anastomosis in 15 cases,right hemihepatectomy with left hepatic bilioenteric anastomosis in 1 case.Postoperative bile leakage in 3 cases and pleural effusion in 10 cases were cured by watchful therapy.All of the 16 cases were followed up with an average time of 5.2 years.No occurrence of cholangitis and elevated liver enzymes were observed up to now in 8 patients,increased γ-GT and ALP,no cholangitis but anastomotic stenosis as showed by MRCP in 6 patients with 2 patients neccesitating reoperation to address repeated cholangitis.Conclusions Restenosis after bile duct repair was closely associated with injury type,repair opportunity,repair methods and the surgeon's expertise.Precise preoperative evaluation,the choice of rational surgical approach,the clinging to mucosa-to-mucosa bilioenteric anastmosis principle and the establishment of postoperative long-term followup system centered on ALP,γ-GT and life quality score are required in the reoperation of stenosis after bile duct repair.

AIM: To research the correlation factor of the anterior segment values of eyes with different axial lengths.METHODS: In this study, 697 cases 697 eyes were included.According to axial length we divided all the cases into short axial length group, normal axial length group, mild long axial length group, moderate long axial length group and severe long axial length group.The relevant indicators of the anterior segment values of patients in the five groups was comparatively analyzed.RESULTS: In the short axial length group, the anterior chamber volume (ACV) showed a significantly negative correlation with age;anterior chamber depth (ACD) showed a significantly negative correlation with age.In the normal axial length group, the ACV showed a significantly negative correlation with age and a significantly positive correlation with axial length(AXL);the anterior chamber angle (ACA) showed a significantly negative correlation with age and a significantly positive correlation with AXL, and a significantly positive correlation with corneal curvature;the ACD showed a significantly negative correlation with age and a significantly positive correlation with AXL.In the mild long axial length group, the ACV showed a significantly negative correlation with age and a significantly positive correlation with AXL, and a significantly negative correlation with corneal curvature;the ACA showed a significantly negative correlation with age and a significantly positive correlation with AXL, and a significantly positive correlation with corneal curvature;the ACD showed a significantly negative correlation with age and a significantly positive correlation with AXL.In the moderate long axial length group, the ACV showed a significantly negative correlation with age;the ACA showed a significantly positive correlation with AXL;the ACD showed a significantly positive correlation with AXL.In the severe long axial length group, the ACV showed a significantly negative correlation with age.There were no significant differences on ACV, ACA and ACD among mild long axial length group, moderate long axial length group and severe long axial length group (P>0.05), while the difference between other groups was significant (P<0.05).CONCLUSION: There has a correlation between the anterior chamber values (ACV, ACA, ACD) age and AXL for eyes which AXL were 22-27mm, while the correlation would disappear as the AXL became shorter or longer.There was a significant growth trend of anterior chamber values for eyes with AXL <27mm.

Because of the changed metabolic behaviors of cancer cells, tumor cells uptake a corresponding larger amount of glucose in physiological condition when compared with normal cells. And they were prone to metabolize glucose for generating energy in anaerobic glycolysis ways in order to grow quickly. Anaerobic glycolysis consumes more glucose than aerobic way when the same amount of energy is obtained, which also results in large demand of glucose in tumor cells. This review briefly describes therapy methods related to characteristic mentioned above, and summarizes the research progress of drugs, diagnostic reagents and carriers conjugated with glucose, glucose derivatives or other kinds of sugars for cancer targeting. Furthermore, typically relative research reports from 2012 till now were listed and analyzed.

Objective To explore the relationship between coping style and family cohesion and adaptation among postoperative patients with breast cancer undergoing chemotherapy.Methods A cross-section survey was conducted among 300 postoperative patients with breast cancer undergoing chemotherapy,and Medical Coping Modes Scale and Family Cohesion and Adaptation Scale were used for evaluating.Results The average score of confronce in breast cancer patients was significandy higher than that in domestic norm [ ( 18.53 ± 1.44) vs ( 19.39 ± 3.04 ) ],and the average scores of resignation and avoidance were significantly lower than that in domestic norm [ ( 14.67 ± 1.10) vs ( 8.47 ± 3.58 ),( 16.38 ± 1.63 ) vs ( 14.45 ± 2.97 ) ;t =4.66,29.36,10.54,respectively;P ＜0.01 ].The mean scores of ideal and unsatisfied family cohesion of patients were significantly higher than that in domestic norm,and the mean scores of ideal and unsatisfied family adaptation were significantly lower than that in domestic norm( P ＜ 0.01 ).A significant correlation was found and family cohesion,unsatisfied family cohesion,real family adaptation,and unsatisfied family adaptation were correlated with the coping style( r =-0.14 -0.25,P ＜0.05 ).Conclusions The coping style of postoperative patients with breast cancer undergoing chemotherapy is correlated with their family cohesion and adaptation,so it is important to take action in the intervention to patients' family and.improve the coping style for patients.

The method for analysis and determination the cleavage of soybean sterol, in which the soybean sterol was degraded and the products androst-1,4-diene-,17-dione (ADD) and androst-4-ene-3,17-dion (AD) were developed by Liquid Chromatography-mass Spectrometry. The HPLC conditions adopted were: a All- tima ODS-2 column (250 mm?4.6 mm, 5 ?m), a mobile phase consisted of menthanol-water (70:30), a flow rate of 1.0 mL/min, a room column temperature. and the detective wavelength was 244 nm.The ZMD Micromass electrospray ionization (ESI)-mass spectrometer was employed. In such conditions the corre- sponding HPLC chromatogram and MS spectrum were obtained. The method has a linear ranger of 0.01 mg/mL ~ 0.09 mg/mL, R2 =0.9999, the recoveries of ADD and AD were 102.6% and 105.90%, the RSD of ADD and AD were 3.02%, 3.5% and 3.08%, 3.24%. This method showed high sensitivity, accuracyand easy to perform. It is suitable to analysis the process cleavage of soybean sterol as well as quality control of product.

AIM: To compare the accuracy of intraocular lens(IOL)power calculations by using five formulas(Haigis, SRK-T, Hoffer Q, Holladay-1, SRK-Ⅱ)in eyes with long axial lengths in order to improve the accuracy of predicating IOL powers.METHODS: Fifty-one eyes of 51 cases of age-related cataract and with mild long axial(24.5mm27mm) were collected who`s optical biometry were performed by the Zeiss IOL Master500 before operation.They underwent regular phacoemulsification and posterior chamber IOL implantation.The actual postoperative refraction was measured with the methods of phoropter and subjective optometry 3mo after surgery.Then we compared the differences of the predicted and actual postoperative refraction of the five formulas in each group.RESULTS: In the mild axial lengths cases, the differences between SRK Ⅱ formula and the other four formulas were statistically significant (P<0.05), and the difference between Hoffer Q and SRK-T formula was statistically significant (P<0.05);there was no difference among the other formulas (P>0.05).In the moderate and severe long axial lengths cases, the differences between SRK Ⅱ formula and the other four formulas were statistically significant (P<0.05), and the difference between Hoffer Q and SRK-T formula, Hoffer Q and Haigis formula were statistically significant (P<0.05);there was no difference among the other formulas (P>0.05).The differences of all the five formulas between the two groups were statistically significant (P<0.05).CONCLUSION: In the mild axial lengths cases, Haigis, SRK-T, Hoffer Q, Holladay-1 performed well.In the moderate and severe long axial lengths cases, Haigis, SRK-T and Holladay-1 performed better than other formulas.The accuracy of all the five formulas decreases as the axial length getting longer.
