1.Bifunctional law in acupuncture theory.

Taking the basic theory of Chinese medicine as the instruction, through the bidirectional features of clinical manifestations, the relative etiology and pathology, acupoints and treatments, the bifunctional law in acupuncture theory is discussed. And it is held that after the confirmation of bidirectional symptoms of some diseases or syndromes, acupoints which have special functions in the bifunctional theory of acupuncture with a narrow meaning can be taken as major acupoints. And at the same time, other proper acupoints and flexible treating methods can be selected under the instruction of the bifunctional law of the broad meaning, so as to get the best therapeutic effect.

2.Efficacy of radiofrequency ablation assisted spleen-preservation for splenic injury

Objective To investigate the efficacy of radiofrequency ablation assisted spleen-preservation for splenic injury.Methods The clinical data of 64 patients with splenic injury who were admitted to the Suining Central Hospital from August 2010 to May 2012 were retrospectively analyzed.Radiofrequency coagulation hemostasis devices were used for stanching bleeding directly.Results Of the 64 patients,35 received radiofrequency ablation assisted splenic bleeding stemosis,and 29 received radiofrequency ablation assisted partial splenectomy.The mean operation time and volume of intraoperative blood loss were (138 ± 48)minutes and 50-400 ml,respectively.Radiofrequency ablation assisted spleen-preservation was successfully carried out in 63 patients.One patient received reoperation (splenic resection) due to hemorrhage 24 hours after operation.Transient rise of temperature was detected in all the patients,and the temperature was back to normal after conventional management.The complication rate was 6.3% (4/63),l patient was complicated with anastomotic leakage and was cured by reoperation; 1 was complicated with left pleural effusion; 1 was complicated with pulmonary infection,and his temperature was back to normal 4 days later; 1 was complicated with effusion in the fossa of the spleen.Celiac drainage tube drainaged out 10-120 ml liquid in the first day after operation,5-25 ml on the day 2-3,and they were removed on the third day.There was no effusion around the spleens,and the texture of the spleen was even at 2 weeks after operation.The duration of hospital stay was (14 ±4)days (range,8-40 days).The results of B sonography showed that no effusion and necrotic lesions in the spleen,and immune examinations showed that the splenic function was normal.Conclusion Radiofrequency ablation assisted spleen-preservation for splenic injury is safe and easy to manipulate.
3.Advances in Studies on Surveillance after Resection of Colorectal Adenoma and Interval Cancer

Colorectal polyps are eminence lesions that protruded from mucosa into lumen,including adenomatous polyps and non-adenomatous polyps. Early detection and resection of adenoma has significance in prevention of colorectal cancer,and the surveillance afterwards is also crucial. Interval cancer is the colorectal cancer that develops between initial negative colonoscopy or all the polyps having cleared and the next colonoscopy. It is an important duty for endoscopists to reduce the incidence of interval cancer. This article reviewed the advances in studies on surveillance after resection of colorectal adenoma and the development of interval cancer.
4.Expressions of RTN4 and TG2 in Papillary Thyroid Cancer Tissues and Their Clinical Significance

Objective To investigate the expressions of RTN4 and TG2 in papillary thyroid cancer tissues and their clinical pathological significance.Methods A total of 40 samples of papillary thyroid cancer tissues and 20 samples of para‐carcinoma tissues were collected from the Department of Pathology ,Renmin Hospital of Wuhan University ,China.The expression levels of RTN4 and TG2 were detected by means of immunohistochemical staining and their relationship with clinicopathological charac‐teristics of papillary thyroid carcinoma was analyzed.SPSS 13.0 soft package was used for one‐way analysis of variance and SNK test of the average absorbance and the average positive area rate (α=0.05).Bivariate correlation analysis of the positive area rate of the expressions of RTN4 and TG2 was conducted (α=0.05).Results ①The expression levels of RTN4 and TG2 were much higher in the papillary thyroid carcinoma tissues than those in the para‐carcinoma tissues (P<0.05). ② The expres‐sion levels of RTN4 and TG2 were significantly higher in lymph node metastasis group than those in the group without lymph node metastasis (P<0.05).The positive expression rates of RTN4 and TG2 were much higher in clinical stage Ⅰand Ⅱ groups than those in clinical stage Ⅲ and Ⅳ groups (P<0.05).They were significantly increased in low differentiation group than in high or moderate differentiation group (P< 0.05).The expressions of RTN4 and TG2 were not statistically associated with tumor size ,age and sex (P>0.05). ③The positive correlation was found between the expressions of RTN4 and TG2 in papilla‐ry thyroid carcinoma tissues (r=0.587 ,P<0.05).Conclusion ① The expressions of RTN4 and TG2 were up‐regulated in pa‐pillary thyroid carcinoma tissues ,which is related with over‐proliferation and increase of apoptosis resistance of cancer cells. ②RTN4 and TG2 play an important role in the proliferation and metastasis of papillary thyroid carcinoma. ③ RTN4 and TG2 may play a synergistic role in the development of papillary thyroid carcinoma.

7.A study on the nonoperative treatment of adult blunt splenic injuries

Objective To increase curative rate of nonoperation treatment for adult blunt splenic trauma.Methods Data of 141 patients with blunt splenic trauma treated in our hospital from March 2010 to October 2014 were retrospectively analyzed in accordance with the spleen injury degree classification issued by the Spleen Surgery Group of Chinese Society of Surgery in 2000.According to the treatment within 6 hours of injury,these patients were divided into operative and nonoperative management groups.Results The proportion of non operation was 69.5% (98/141) and the success rate was 89.8% (88/98).43 cases were in operation group:34 cases in grade Ⅲ,9 cases with grade Ⅳ,3 cases died; 98 cases were in non operation group:including 32 cases of grade Ⅰ,50 cases of grade Ⅱ,15 cases of grade llⅢ,1 case with grade Ⅳ.10 cases failed non operation treatment including 1 case with grade Ⅰ complicated by pancreatic and intestinal injury causing peritonitis during the period of observation,1 case of grade Ⅱ complicated by contusion of the liver and secondary bleeding on day 3 after the trauma,7 cases of grade Ⅲ,were converted to surgery because of multiple organ injury and rebleeding secondary to pulmonary complications,and 1 case of grade Ⅳ with multiple organ injury and shock being immediately converted to surgery.In overall there were 2 deaths in nonsurgery group,1 case with grade Ⅲ and 1 case with grade Ⅳ.Condusions Nonoperative treatment of blunt splenic trauma is safe and feasible in grade up to]Ⅱ blund splenic trauma patients.
