Posterior lip sclerectomies were performed in rabbits and cytosine arabinoside (Ara-C) was applied by topical instillation or subconjunctival injection. In both groups, the mean intraocular pressure (IOP) of the treated eyes was significantly lowered at postoperative week 1 and 2, but there was no significant difference between the mean IOP of the control eyes and that of the treated eyes at postoperative week 3 and 4. In both groups, at postoperative week 2, the sclerectomy sites of the control eyes were totally occluded by granulation tissue, but those of the treated eyes were partially replaced by granulation tissue. At postoperative week 4, the sclerectomy sites of the treated eyes were totally occluded by the granulation tissue ultimately in both groups. There were no differences in the mean IOP and the histologic finding of the treated eyes between the topical instillation group and the subconjunctival injection group. We concluded that either topical instillation or subconjunctival injection of Ara-C can delay wound healing at the surgical site after glaucoma filtering surgery in rabbits.

The safety and success rate of trabeculectomy using fornix-based conjunctival flaps were investigated. Patients with chronic angle-closure glaucoma(22 eyes), primary open-angle glaucoma(10 eyes), secondary glaucoma(4 eyes) and ocular hypertension(1 eye) had a trabeculectomy with a fornix-based conjunctival flap. We used the fluorophotometry to investigate the corneal epithelial barrier function after excimer laser photorefractive keratectomy (PRK). Twenty-five eyes of 21 subjects (13 women, 8 men) underwent PRK to correct rnyopia. Corneal epithelial healing time was measured and corneal epithelial permeability to sodiurn fluorescein was evaluated by fluorophotoinetry at I, 2, and 3 weeks after surgery. The corneal epithelial permeability increased significantly 1 week after surgery and returned to preoperative level 2 weeks after surgery. The permeability differences according to epithelial healing days and corrected diopters were not statistically significant(p>0. 05). These results suggest that PRK delays complete reconstruction of corneal epithelial barrier function. The corneal epithelium regained its functional barrier 2 weeks after PRK in patients, so, at least, during the first 2 weeks, care should be taken to miniinize further epithelial trauma from topical inedication or surgical manipulation.

The intraocular pressure (IOP) in 115 cases of extracapsular cataract extraction (ECCE) and ciliary sulcus fixation of posterior chamber lens implantation was measured more than 2 months postoperatively. These results were compared to the unoperated fellow eyes, as well as to a group of 14 patients who had ECCE and implantation of an anterior chamber lens. The posterior chamber lens implanted eyes demonstrated a significant IOP reduction after surgery compared to the unoperated fellow eyes and the anterior chamber lens implanted eyes. In the posterior chamber lens implanted eyes, there was a significant IOP reduction after surgery in all age groups and there was a significant IOP reduction in the eyes with a 2 to 6 months and a 7 to 12 months follow-up after surgery, but no IOP change in the eyes with more than 1 year follow-up after surgery. We concluded that in the case of ECCE and ciliary sulcus fixation of posterior chamber lens implantation, IOP was initially reduced after surgery, but approarhed the preoperative level over a period of time.

PURPOSE: To assess the proper differential diagnosis and treatment of scleritis with scleral melt following pterygium excision. METHODS: A retrospective study through a review of medical records of 5 patients diagnosed with surgically induced necrotizing scleritis (SINS) after pterygium excision and treated with systemic immunosuppressant and steroid. RESULTS: All the patients responded to the treatment and showed significant improvement. CONCLUSIONS: Immunosuppressive treatment without surgical intervention is effective in resolution of SINS after pterygium excision.

Polymorphonuclear leukocytes(PMNs) infiltrate to the wound site within a few hours after excimer laser photorefractive keratectomy(PRK). Oxygen free radicals produced by the PMNs may be injurious to the surrounding corneal tissues and also acts as an inflammatory mediator that deepen tissue damage. In this study we examined that the application of ascorbic acid would decrease corneal haze following excimer laser PRK. Excimer laser PRK was done in 9 rabbits (18 eyes). Ascorbic acid was injected subconjunctivally in one eye and BSS was injected in the opposite eye. The corneal epithelium re-surfaced by 2.6 +/- 0.5 days in the ascorbic acid treated group and by 2.7 +/- 0.7 days in the ESS injected group. Corneal hazes examined in postop 2wk, 4 wk, 6 wk, and 8 wk were 1.6 +/- 0.5, 1.4 +/- 0.5, 0.7 +/- 0.3, 0.5 +/- 0.2 in the ascorbic acid treated group and 2.1 +/- 0.6, 1.7 +/- 0.5, 1.1 +/- 0.3, 0.9 +/- 0.2 in the BSS injected group, respectively. These data suggest that ascorbic acid would decrease corneal haze after excimer laser PRK without significant complications.

I analyzed the incidence of fibrinous membrane formation in the eyes that received extracapsular cataract extraction and posterior chamber lens implantation. There was statistically significant difference between the incidence of fibrinous membrane formation in the eyes that received sub-Tenon's injection of Depo-Medrol(R) at the end of surgery(7.9%) and that in the eyes that did not receive it(34.8%)(p<0.025).
