1.Lipoprotein glomenllopathy - a new type glomenllopathy with progressive sclerosis

To report a Chinese female adult suffering with lipoprotein glomerulopathy. Methods Light , electron microscopy and immunofluorescence were performed with renal tissue from biopsy. Results Mass proteinuria and nephrotic syndrome were seen as clinical features in this patient. Obvious expansion of glomerular capillary cavities, full of positive sudan 3 lipoprotein thrombi, was observed. Electron microscopy demonstrated cavities were full of various foaming lipid deposition in cluster and layer arrangement. Repeat renal biopsy after two months found expansive cavities decreased remarkably and lipoprotein thrombi were replaced by mesangial proliferation and sectional sclerosis gradually. Conclusion This case is diagnosed by pathology.
2.Study of gastrointestinal stromal tumors by light microscopy, electron microscopy and immunohistochemistry.

OBJECTIVETo investigate the morphological features, immunohistochemical speciality of the gastrointestinal stromal tumors (GISTs), and its histogenesis as well.
METHODSThe morphologic characteristics of GISTs were studied in 65 cases using light microscopy and 17 cases by electron microscopy. The expression of c-kit (CD117), CD34, vimentin, SMA, actin, desmin, S-100 and MBP were detected in all the cases with EnVision trade mark staining.
RESULTSAmong 65 cases of GISTs, 46 were spindle cell type, 6 epithelioid cell type and 13 mixture type, equivalent to 85.5% (65 of 76) of all of the mesenchymal tumors of gastrointestinal tract admitted in the same period. The epithelioid cell type tumors composed of mainly the epithelioid cells, predominantly short spindle, oval or round in pattern, with an overall eosinophilic cytoplasm by hematoxylin-eosin stain. Focal cytoplasmic vacuolization was often seen. Sometimes signet-ring like cells and cells with a clear cytoplasm were seen in the epithelioid stromal tumor. The tumor cells arranged in interlacing fascicles forming whorls or sometimes cell clusters. Electronic microscopy showed the presence of interdigitating cell processes, in some areas, synapse-like structure and numerous desmosome-like junctions as well as a few gap junctions and small round neurosecretory granules. There were also abundant intermediate filaments and thin filaments of actin-type with longitudinal condensations (dense bodies). All of the 65 stromal tumors were strongly positive for vimentin (100%), 61 out of 65 tumors positive for CD117 (c-kit) (93.8%) and 51 out of 65 positive for CD34 (78.5%). Some cases also expressed SMA, actin, S-100 and MBP.
CONCLUSIONSGISTs were the most frequent mesenchymal tumor seen in the gastrointestinal tract. Under light microscope, the morphology of stromal tumors looks sometimes like a leiomyoma and Schwannoma. The application of immunohistochemical markers (particularly CD117 and CD34) and ultrastructural study are considered necessary for the differential diagnosis. GISTs may originate from the pluripotential precursor cells like the interstitial cells of Cajal.

3.Intraoperative nursing cooperation for micro-surgical clipping of middle cerebral artery aneurysms

Objective To investigate the importance of nursing cooperation in the operation of middle cerebral artery aneurysm ( MCAA) microsurgical clipping. Methods We retrospectively analyzed 202 cases ( male 103 cases, female 99 cases) of MCAA patients who were underwent microsurgical clipping surgery ( from January 1997 to December 2013), and the importance of nursing cooperation in the process were analyzed. A total of 242 aneurysms were found during surgery of 212 cases underwent craniotomy clipping, in which 215 cases were MCAA, and 2 cases underwent autologous dura wrapping, except one unbroken MCAA case was embolism. Results All patients in this group underwent Yasargil′s pterional trans-sylvian approach, 212 MCAA surgery were successfully clipped. 174 cases were clipped without rupture (81. 3%), while rupture (18. 7%) occurred in 40 patients during surgery. All patients had been treated with controlled hypotension and intracranial hypotension, through timely blood transfusion, infusion and other response measures to ensure the successful completion of surgery. Conclusions It can help will the operation completed smoothly and improve surgical effects by personalized utilizing of controlled hypotension, brain tissue relaxation techniques and other measures in microsurgical clipping of MCAA, as well as good nursing cooperation during the process of aneurysms rupture.
4.Integration of autonomous maximal smile 3D image with digital 3D dental model and investigation of its accuracy

Objective This study aims to establish an approach to integrate autonomous maximal smile(AMS)3D facial image with digital 3D dental models to demonstrate the digital orthodontic set-up in the 3D facial context.Methods Using Geomagic Studio software,the AMS 3D facial image and pre-treatment dental model were manually and globally registered.Subsequently,the pre-treatment dental model was substituted with the predicted post-treatment dental model.The intraoral region of the AMS 3D facial image was removed,achieving a conjunctive display of the AMS 3D facial image and the post-treatment dental set-up.The distances between four groups of corresponding landmark pairs on the AMS 3D facial image and the pre-treatment dental set-up were calculated,and the accuracy of the registration operation was evaluated by paired t-test.Results The novel approach effectively facilitated the integration of AMS 3D facial images with the pre-treatment and predicted post-treatment 3D dental models.The average distances between the pairs of points were(1.19±0.55)mm and(1.55±0.59)mm for the two registrations,respectively.Notably,no statistically significant difference was observed be-tween the two measurements(P>0.05),indicating a high agreement(intraclass correlation coefficient=0.914).Conclu-sion This study established an approach to integrate AMS 3D facial images with digital 3D dental models.Through this approach,the digital orthodontic set-up design can be displayed in the context of a 3D facial image,which may help to improve the quality of outcome set-up in digital orthodontics,such as clear aligner therapy.
