1.Angiogenesic effects and molecular mechanisms of Shuanglong Pill on experimental myocardial infarction rats

ObjectiveTo study the effects and molecular mechanisms of Shuanglong Pill on angiogenesis of ischemic myocardium. Methods63 Wistar rats were ligated left anterior descending branch to be made into models of acute myocardial infarction(AMI).All rats were randomly divided into large-dosage group of Shuanglong Pill(LDG),small-dosage group of Shuanglong Pill(SDG),The control of acute MI(MIC).Other 11 rats were set as normal control(NC).Rats in every group had been treated half for 2 weeks and half for 4 weeks. New vessels' quantity in ischemic myocardium of acute MI rats were detected by immunohistochemistry double steps assay;the expressions of vessel endothelial growth factor(VEGF) and basic fibroblast growth factor(bFGF) on ischemic myocardium were detected by immunohistochemistry SP assay;the expressions of VEGF mRNA and bFGF mRNA on ischemic myocardium were detected by reverse transcription polymerase chain reaction(RT-PCR) assay.ResultsThere were more new capillaries on ischemic myocardium of LDG, SDG compared with that in MIC at every stage,moreover,there was significantly increase at the end of 4 weeks compared with that at 2 weeks.At the end of 2 weeks, the expression of VEGF in LDG were significantly elevated compared with that in MIC; and the expression of bFGF in LDG were significantly elevated compared with that in SDG and MIC.Whereas the expression of VEGF in LDG were significantly decreased at the end of 4 weeks compared with 2 weeks. There were significant differences of above comparison (P<0.05-0.01).At every stage the expression of VEGF mRNA in LDG, SDG were significantly elevated compared with that in MIC; but its expression was significantly decreased at the end of 4 weeks compared with that 2 weeks. The expression of bFGF mRNA was significantly elevated in LDC at the end of 2 weeks compared with that in other groups.ConclusionsShuanglong Pill could promote angiogenesis in ischemic myocardium,application of large dose of Shuanglong Pill could significantly upregulate the expression of VEGF, bFGF, VEGF mRNA and bFGF mRNA.
2.Longitudinal study on the correlation between symptoms and quality of life among nasopharyngeal carcinoma patients undergoing radiotherapy

Objective To describe the change of symptoms and quality of life among nasopharyngeal carcinoma patients who undergo radiotherapy, and analyze the correlation between them.Methods A total of 218 nasopharyngeal carcinoma patients who received radiotherapy were included by convenient sampling method and they were asked to finish the questionnaire of general information, Chinese Version of the M.D. Anderson Symptom Inventory(MDASI-C) and Quality of Life Instruments for Cancer Patients-Head and Neck (QLICP-HN) at five time points, which were beginning, intermediate (the volume of radiotherapy at 36-40 Gy), end of radiotherapy, three months and six months after radiotherapy.Results The severity of symptoms at five time points were (1.86±1.75),(3.20±1.75),(3.59±1.81),(0.65±0.59) and (0.52±0.50), presenting decreased gradually first, and then alleviated. The score of symptom severity at five time points were significantly different (F=182.86,P<0.05). Fatigue, appetite loss and dry mouth were the most sever and high-incidence symptoms during and within six months after radiotherapy. The quality scores of life were (65.39±10.90),(58.78±8.77),(57.82±9.20),(79.76±5.75) and( 76.79±4.78) at five time points, representing the same trend as the scores of symptom, and the difference were statistically significan(t F=129.75,P< 0.05). Spearman correlation analysissuggested that there were negative correlation between severity of symptoms and total score of quality of life (P < 0.05). Conclusions The symptoms of nasopharyngeal carcinoma patients exacerbates with dose of radiotherapy increase, and the quality of life is also decreased. Although most of symptoms disappear within six months after radiotherapy, fatigue, appetite loss and dry mouth persisted and influence patients' quality of life.Nurses should pay attention to patients' symptoms when they stay at hospital as well as discharged, and intervention is necessary to improve their quality of life.
3.Effects of improved catheter extubation method on urination

Objective:To compare the effects between improved catheter extubation method and the traditional one on urination pain, urinary retention, first urination time and first urination volume, and to evaluate the advantage of the improved method.Methods:144 patients with indwelling catheters after operation in our department were randomly divided into observation group and control group, with 72 cases in each group. The control group returned to the ward after surgery and began to clamp the urinary tube to train the bladder function, the catheter was removed by traditional method. Observation group: urination reflex was evaluated before extubation, according to more than 250 ml urine in the urine bag. Pumping the saline of the gas bag and injecting back 0.5ml to keep the wall of the bag smooth, eventually the catheter was excreted when urinating. Urethral pain, urinary retention, first micturition time and first micturition volume of two groups were analyzed.Results:Urethral pain, urinary retention, first micturition time in observation group and control group were 1.47±1.48, (20.44±12.98) min, 95.8% (69/72) and 3.11±1.98, (28.03±27.00) min, 83.3% (60/72), respectively, and the difference between the two groups was statistically significant ( t value was -5.644, -2.148, χ2 value was 6.628, all P<0.05). The first micturition volume in observation group and control group were (258.6±41.57) ml and (248.14±48.82) ml, respectively, and there was no significant difference between the two groups ( t value was 1.377, P>0.05). Conclusion:The improved catheter extubation method could significantly reduce the urethral pain, shorten the time of the first urination, and improve the success rate of self-urination, which of clinical promotion.
4.Clinical characteristics and prognosis analysis of acute myeloid leukemia patients with PTPN11 gene mutation

Objective To investigate the clinical characteristics and prognosis of acute myeloid leukemia(AML)patients with PTPN11 gene mutation.Methods Total 115 adult AML patients who underwent initial diagnosis,treatment,and second-generation sequencing(NGS)detecting at hospital were recruited in this study.Clinical da-ta included disease characteristics,treatment efficacy,long-term prognosis,immune cell subpopulations,and leu-kemia stem cells were collected to analyze the clinical characteristics and prognosis of AML patients with PTPN11 gene mutation.Results PTPN11 gene mutation rate in newly diagnosed adult AML was 9.57%,and the mutation site mainly occurred in exon 3 region with all mutation type being point mutation.Compared with PTPN11 wild-type group,PTPN11 gene mutation group had a higher early mortality rate(18.18%vs 4.00%,P=0.048),a lower complete response rate(33.33%vs 67.71%,P=0.039),a higher recurrence rate(83.33%vs 42.31%,P=0.043),a shorter median overall survival time(9 months vs 20 months,P=0.026),a lower proportion of ef-fector T cells[(1.39±0.12)%vs(3.56±0.46)%,P=0.038],and a higher proportion of leukemia stem cells[(13.82±3.66)%vs(3.87±1.40)%,P=0.021].Conclusion PTPN11 gene mutation is a poor prognostic marker for AML.Those patients have a high early mortality rate,low complete remission rate,high recurrence rate,short median overall survival time,a low proportion of effector T cells,and a high proportion of leukemia stem cells.
5.Clinical efficacy of precise electro-acupuncture stimulation in the pterygopalatine fossa for the treatment of chronic sinusitis and imaging evaluation

Objective We aimed to explore the clinical efficacy of precise electro-acupuncture stimulation in the pterygopalatine fossa for the treatment of chronic sinusitis.Methods In total,40 patients with chronic sinusitis who visited the Xiyuan Hospital,China Academy of Chinese Medical Sciences between January 2022 and January 2023 were selected.The subjects were randomly divided into two groups:the treatment group(receiving precise electro-acupuncture in the pterygopalatine fossa)and the control group(receiving standard western medical treatment),with 20 cases in each groups.The treatments were administered twice a week for a period of 12 weeks.The effectiveness of the treatments was evaluated through computed tomography(CT)of the sinus changes and the Lund-Mackay scoring system.Results In cases of total opacity and partial opacity,there were significant differences between the two patient groups before and after treatment(P<0.05).Intra-group comparison revealed statistically significant improvements in both groups before and after treatment(P<0.05).The treatment group exhibited significant improvement in various sinus conditions(P<0.05),while in the control group,a significant improvement was only observed in the left maxillary sinus(P<0.05).Inter-group comparison also demonstrated that the treatment group's improvements were superior to those of the control group across multiple indicators(P<0.05).Conclusion The precise electro-acupuncture stimulation technique in the pterygopalatine fossa demonstrated significant therapeutic effects in the treatment of chronic sinusitis,with superior outcomes with respect to radiological changes compared to standard western medicine treatment.
6.Longitudinal study on the changes of symptom clusters among nasopharyngeal carcinoma patients undergoing radiation therapy

Objective To investigate the dynamic changes of symptom clusters among nasopharyngeal carcinoma patients receiving radiotherapy and provide evidence for intervention clinically.Methods The data were collected at three time points that were before radiotherapy,during the radiotherapy and after radiotherapy, by surveying 273 first visit of nasopharyngeal carcinoma patients from May 201 4 to January 201 5 with a self-designed general information questionnaire and the Chinese version of the M.D.Anderson symptom inventory and observing the changes of symptom clusters in our hospital.One-way analysis of variance and the exploratory factor analysis were applied to observe the incidence of symptoms and the changes of symptom clusters during radiotherapy.Results The statistics revealed that the incidence and intensity of all the symptoms at 3 time points were significantly increased(P <0.05),except the symptom of amnesia.There were 3 symptom clusters among 3 time points,named as gastrointestinal symptom cluster,sickness symptom cluster and somatic symptom cluster,but the composition and the variance were different at different time point.Gastrointestinal symptom cluster was made of nausea and vomiting,adding poor appetite only before radiotherapy.The gastrointestinal symptom cluster was most prominent before radiotherapy.Sickness symptom cluster consisted of drowsiness, numbness symptom before radiotherapy, while it consisted of shortness of breath, numbness, difficulty remembering symptoms in other time.Somatic symptom cluster mainly consisted of pain and fatigue,dry mouth symptom joining it during and after radiotherapy.Conclusions With the treatment of radiotherapy,the incidence and intensity of symptoms are significantly enhanced,and the constitution of symptom clusters is different at different stage.Therefore,it is important for clinical nurses to strengthen dynamic assessment of all symptoms,monitor the changes and composition of symptom clusters,thus establish corresponding management measures to improve patients′quality of life.
7.Construction of management index system for rational drug use of key monitoring drugs

OBJECTIVE To establish management index system for rational drug use of key monitoring drugs, and provide reference for the management of key monitoring drugs in the hospitals. METHODS First, the management index system for rational drug use of key monitoring drugs was drafted by collecting the evidence from related medical literature. Next, using a modified Delphi method, twenty experienced experts from the fields of pharmacy, medical practice, healthcare insurance, and finance were selected to participate in two rounds of questionnaire consultations. Based on the expert enthusiasm coefficient, authority coefficient, degree of opinion concentration, and degree of coordination, the final indicators were determined to establish a management index system for rational drug use of key monitored drugs in medical institutions. RESULTS The expert enthusiasm coefficients reached 100% in both rounds of consultation. In first-level, second-level and third-level indicators, the authority coefficients of experts were 0.89, 0.86 and 0.87, and coordination coefficients of the experts in importance score were 0.300 (P< 0.05), 0.125 (P<0.05) and 0.139 (P<0.05), respectively. The average score for the importance of all indicators reached over 3.5, in which the full score ratio ranged from 35% to 100%. Except that the variation coefficient of a third-level indicator “number of specifications purchased for key monitored drugs” was 0.26, the variation coefficients of rest indicators were less than or equal to 0.25. Based on the results of expert consultation, final version of the management index system established in this study, including two first-level indicators (drug procurement and use, and rational drug use), five second-level indicators (such as the accessibility, cost-effectiveness) and twenty third-level indicators (such as the number of specifications purchased for key monitored drugs, the increase in the cost of key monitored drugs). CONCLUSIONS The management index system established in this study possesses high reliability and strong operability, and may provide a reference for the management of key monitoring drugs in the hospitals.
