1.Evolution and perfection of the HIV/AIDS designated hospital system in China

Objective:To summarize the development of the HIV/AIDS designated hospital system, analyze the main issues and explore solutions for this system. Methods:184 leaders/experts of three levels of healthcare facilities ( provinces, cities and counties) from 30 provinces engaged in AIDS prevention and control for over five years, and 2,432 people living with HIV/AIDS(PLWHA) from seven provinces were investigated through two different question-naires. Results:According to the staff responses, the issues of surgery and hospitalization are difficult and outstand-ing for PLWHA due to the current designated hospital system with scores of 6. 49 in severity. Of the ten largest prob-lems, ranked third is the need for improved AIDS prevention and control. 2 367 (97. 3%) PLWHAs had an aware-ness of the designated hospitals, 1,376 received treatment in the designated hospitals, and 85. 5% believed that the illness was effectively treated, while 9. 0% thought that the medical technology of the designated hospital was limited;18 . 7% of hospitalized HIV/AIDS patients or those who received surgery experienced prevarication by the non-des-ignated hospitals. Policy analysis shows that China’s designated hospital system for PLWHA has developed gradually with the characteristics of periodic and temporary changes. Conclusion:The designated hospital system for PLWHA in China has played an important role, but there are still several problems. The government should improve the existing system by strengthening the comprehensive medical service capacity of designated hospitals, improving the mechanism of consultation and referrals, coordinating the non-designated hospitals to provide technical support, reducing dis-crimination and fear from medical staff against HIV/AIDS, decreasing the risk of occupational exposure, and enhan-cing the communication among hospitals, CDCs and patients.
2.Comparative Studies on Antioxidant Activities of Green Tea and Four Tree-type Non-Camellia Teas

This article was aimed to compare the antioxidant activities of Green tea and four tree-type non-Camellia teas. The antioxidant activities of the water extract of Green tea, Eagle tea, Large-leaved Kuding tea, Shiya tea and Qingqianliu tea were evaluated with four different in vitro assays including 1,1-diphenyl-2-picryl-hydrazyl (DPPH), ferric-reducing antioxidant power (FRAP), ferrous ion-chelating (FIC) and β-carotene-linoleate bleaching assay. In addition, the polyphenol contents of different teas were estimated by the Folin-Ciocalteu assay. The results showed that Shiya tea had strong antioxidant activity; its FIC and FRAP abilities are better compared with Green tea. The Eagle tea and Large-leaved Kuding tea have better antioxidant activity. The FRAP activity of Large-leaved Kuding tea was better than green tea. The DPPH of Eagle tea and Large-leaved Kuding tea were slightly lower than Green tea, but higher than the butylated hydroxytoluene (BHT). The antioxidant ability of Qingqianliu tea was the lowest. It was concluded that Shiya tea, Eagle tea, Large-leaved Kuding tea had relatively high development value which re-quires in-depth research and promotion.
3.Effect of timeliness motivation theory-based continuing care on the self-care ability and healthy behavior in elderly patients with type 2 diabetes

Objective To explore the effect of timeliness motivation theory-based continuing care on the self-care ability and healthy behavior in elderly patients with type 2 diabetes. Methods A total of 202 elderly patients with type 2 diabetes in our hospital were selected,and were randomly divided into observation group and control group according to the random number table, with 101 cases in each group. The control group were given healthy related education during hospitalization , and post-discharge telephone follow-up every month, lasting 10~15min each time.The observation group were additionally given timeliness motivation theory-based continuing care by telephone follow-up each month , including emotional arousal , need motivation , examples stimulating and benefit motivation, lasting 6 months. Their self-care ability and healthy behavior in the two groups were compared. Result The self-care ability in the observation group was significantly significantly better than that of the control group (P<0.001), and had significantly much more healthy behaviors than the latter (P < 0.001). Conclusion Timeliness motivation theory-based continuing care can improve the self-care ability and healthy behavior in elderly patients with type 2 diabetes.
4.Main issues, causes and strategies for AIDS prevention and control in health care systems

Objective:To summarize the main issues, analyze the causes and offer a strategy for AIDS preven-tion and control for all types of healthcare facilities in the health care system. Methods:184 leaders and experts of tertiary-level healthcare facilities ( provinces, cities and counties) from 30 provinces who had been engaged in AIDS prevention and control for more than five years were studied through a questionnaire including the issues and causes in AIDS prevention and control. Results: According to the results of the respondents and experts’ demonstration, the main issues were identified, including a lack of human resources in the system of disease control facilities, a lack of relevant agencies involved in AIDS response, the difficulty for AIDS patients to receive surgery and follow-up, poor detection of HIV, shortcomings in medical insurance, and so on. Conclusion: Based on the analysis of issues and causes, the paper offers strategic solutions: Clearly define the responsibilities of various health institutions in AIDS prevention and control;promote the participation of primary health institutions in AIDS prevention and control; im-prove the medical service system for AIDS patients;improve AIDS medical security policies;reform the management system of antiretroviral therapy drugs;and strengthen technical support from disease control facilities.

The dried cane of Kadsura interior (Kadsurae Caulis) is recorded in Chinese Pharmacopoeia 2010 as Dian Jixueteng for the treatment of rheumatism,irregular menstruation,and deficiency of Qi and blood.In this paper,morphological characteristics,chemical constituents,and pharmacological activities in the cane of K.interior were summarized.Moreover,some suggestions about application and quality control of Kadsurae Caulis were proposed in order to provide a theoretical basis for further scientific use.
6.Review of oral anticancer chemotherapeutic agents regulation mode among community-dwelling cancer patients

7.Analysis of the effect of expanded non-invasive prenatal testing in prenatal screening population during pregnancy.

OBJECTIVE:
To explore the clinical effect of expanded non-invasive prenatal testing (NIPT-plus) for prenatal screening.
METHODS:
The screening result, prenatal diagnosis and pregnancy outcome of 3700 pregnant women who volunteered NIPT-plus screening at our center from September 2018 to December 2019 were reviewed.
RESULTS:
Among the 3700 pregnant women, 74(2.0%) were scored positive for clinically significant fetal chromosomal abnormalities and underwent NIPT-plus screening. Sixty three women with a high risk underwent invasive prenatal diagnosis, among whom 19 were diagnosed, which yielded a positive predictive value (PPVs) of 30.2% (19/63). Statistical analysis showed that NIPT-plus has higher PPVs for common aneuploidies and low-to-medium PPVs for sex chromosome aneuploidies and microdeletion/microduplication syndromes.
CONCLUSION
As a screening technique, NIPT-plus has broader applications compared with conventional techniques, and has reference value for clinicians and pregnant women during pregnancy.

8.Construction of self-management assessment indicators for cancer patients with oral chemotherapy at home

Objective? To construct the objective, scientific and practicable self-management assessment indicators for cancer patients with oral chemotherapy at home so as to provide a basis for medical staff to pertinently carry out self-management education and guarantee safety of patients with oral chemotherapy agents well. Methods? From July 2017 to June 2018, we preliminarily drew up the assessment indicators based on self-management theory by document analysis, semi-structured interviews and expert group discussion. And then, we determined the self-management assessment indicators for cancer patients with oral chemotherapy at home by two rounds of 18 experts enquiry with the method of Delphi. Results? Among two rounds of enquiry, the questionnaire recovery rate was 100% with high expert initiative. The expert authority coefficient and the coordination coefficient were 0.83 and 0.192 respectively (P< 0.05). Finally, a total of 5 primary indicators and 38 secondary indicators were determined. Conclusions? The self-management assessment indicators for cancer patients with oral chemotherapy at home are scientific and reliable which are convenient for medical staff to understand patients' self-management and to provide high quality of nursing for cancer patients with oral chemotherapy at home.
9.Is it necessary to choose NIPT-plus for pregnant women who opt for non-invasive prenatal testing? A study of 50 cases.

OBJECTIVE:
To explore whether it is necessary to choose NIPT-plus for the prenatal screening of pregnant women.
METHODS:
The results of NIPT and NIPT-plus sequencing data, fetal DNA concentration, prenatal diagnosis and pregnancy outcome of 50 pregnant women were compared.
RESULTS:
Compared with NIPT, NIPT-plus attained similar fetal DNA concentration and a 4.4-fold increase in sequencing data. NIPT was able to detect 4 cases of 21-trisomy, 2 cases of 18-trisomy, and 9 cases of sex chromosome aneuploidies (SCAs) signaled by NIPT-plus, but missed one 18-trisomy, and failed to detect rare chromosome aneuploidies (RCAs) and microdeletion/microduplication syndromes (MMS). The PPVs of NIPT-plus for 21-trisomy, 18-trisomy, SCAs, MMS and RCAs were 100%, 100%, 44.4%, 30.4% and 0%, respectively. And those of NIPT for 21-trisomy, 18-trisomy, and SCAs were 100%, 100%, and 44.4%, respectively.
CONCLUSION
It is necessary for pregnant women to select NIPT-plus to improve the detection rate of common trisomies, SCAs and disease-specific MMS, therefore reduce the occurrene of birth defect.

10.Results of non-invasive prenatal testing for 2473 women with twin pregnancy.

OBJECTIVE:
To assess the value of non-invasive prenatal testing (NIPT) for the detection of fetal chromosomal aneuploidies in women with twin pregnancy.
METHODS:
A total of 2473 women with twin pregnancy underwent the NIPT test to assess the risk for fetal chromosomal aneuploidies from January 2016 to September 2019. Those with a high risk by NIPT were confirmed by amniocentesis or chorionic villus sampling. All cases were followed up to evaluate the positive prediction value of NIPT for twin pregnancies.
RESULTS:
Among the 2473 women, the NIPT test has identified 31 cases (1.25%) with a high risk for fetal chromosomal aneuploidies, which included 5 cases of trisomy 21, 1 case of chromosome 21 deletion, 4 cases of trisomy 18, 7 cases of sex chromosome abnormality and 14 cases of microdeletion and microduplication. By invasive prenatal diagnosis or chromosomal karyotyping analysis of neonates, 5 cases of trisomy 21, 3 cases of trisomy 18, 1 case of sex chromosome abnormality, and 2 cases of microdeletion and microduplication were confirmed, which yielded a positive predictive value of 100%, 75%, 25% and 25%, respectively.
CONCLUSION
NIPT can be used for the screening of fetal chromosomal aneuploidies in women with twin pregnancy with high accuracy. The method is non-invasive, safe and effective for the screening of fetal chromosomal aneuploidies, in particular trisomy 21.
