1.Discussion on the Suitable Concentration of Potassium Permanganate Collyria in the Clinic

OBJECTIVE:To provide recommendation for the clinic on the optimal application concentration of potassium permanganate collyria.METHODS:3different concentrations of potassium permanganate collyria were prepared,the local ir-ritation of2of which on eyes of115conjunctivitis cases receiving conjunctival sac irrigation and the effects of the3different concentrations of potassium permanganate collyria on the comfort degree,corneal epithelium and bacterial colonies before and after eye irrigation of18pairs of oculus normalis receiving conjunctival sac irrigation were observed.RESULTS:Of the total115conjunctivitis cases,98(about85%)complained to have severe irritation;8cases(about7%)felt unbearable because of the strong irritation.Degree of comfort decreased with the elevation of concentration of collyria,while failed to have any effect on corneal epithelium and the numbers of bacterial colonies before and after irrigation.CONCLUSION:The recommended concentration of100ml∶1.5mg of potassium permanganate collyria for the irrigation of conjunctival sac in the clinic.

4.Intervention Effect of Evidence-based Nursing on Health Behavior of Elderly Patients with Hypertension in the Community

Objective To explore the intervention effect of evidence-based nursing on the health behavior of elderly hypertensive patients in the community.Methods 187 elderly hypertensive patients who were hospitalized in December 2015 and met the inclusive criteria were divided into observation group (n=93) and control group (n=94).The observation group received evidence-based nursing according to the evidence-based medical system evaluation while the control group was given regular nursing both for 6 months.The clinical efficacy of two nursing modes was observed and the differences between pre-intervention and pose-intervention and groups were recorded and analyzed with a self-designed questionnaire, drug compliance questionnaire and health behavior scale.Results In the observation group, the blood pressure was effectively controlled or reduced, treatment compliance was significantly increased and body mass index (BMI) was within the ideal range 6 months later.Scores of unhealthy behaviors such as those in living habits and mental stress were lower than those in the control group, which showed significant difference (P<0.05).There was no significant difference in activity participation between the two groups (P>0.05).Conclusion Evidence-based nursing intervention can effectively control the hypertension, improve the treatment compliance of the community elderly hypertension patients and help lead them to a more positive and healthier life style.
5.Construction and application of clinical nursing quality evaluation index system in patients of ophthalmology

Objective To investigate the scientific and objective quality management methods of ophthalmic nursing to improve the quality of ophthalmic clinical nursing. Methods According to the implemention details of review standards of tertiary special eye hospital (2011) and the management specification of clinical nursing of Guangdong province, combining with the characteristics of ophthalmic nursing to formulate the 21 indicators. The indicators had been applied in 12 wards to evaluate the nursing quality since 2013 each quarter. Using the Plato analysis method to analyze the data dynamically since 2013, then controlling and improving the nursing quality in view of the analysis result. The clinical nursing quality in the first half of 2013 was compared with the second half of 2013. Results The ophthalmic nursing indicators of 12 wards in the second half of 2013 were higher than the first half of 2013. Significant statistical difference was found in the implementation rate of periocular clean, cut fingernails preoperative, infusion system, identification, send medicine to the mouth, patients at high risk of falling drop of bed evaluation, wrist band awareness, patients′ satisfaction between two groups (χ2=4.115-12.308, P < 0.05 or 0.01). Conclusions Establishing and implementing the nursing quality evaluation indicators system of ophthalmology can evaluate the clinical nursing quality objectively and promote the continuous improvement of clinical nursing quality.
6.Prevention of liver dysfunction during anti-tuberculosis treatment in chronic hepatitis B virus infected patients complicated with tuberculosis

Objective To evaluate the preventive effect of entecavir on liver injury in chronic HBV infected patients complicated with tuberculosis receiving anti-tuberculosis treatment.Methods A total of 102 chronic HBV infected patients complicated with tuberculosis were collected from Guangzhou Eighth People' s Hospital and Guangzhou Chest Hospital during January 2011 and May 2012.Patients were divided into three groups:group A (n =33) received entecavir plus anti-tuberculosis treatment,group B (n =29) received lamivudine plus anti-tuberculosis treatment,and group C (n =40) received anti-tuberculosis treatment only.Liver injury,termination of treatment,liver function and HBV DNA load before and after treatment were observed.SPSS 13.0 was used for statistial analysis.Results Two cases (6.1%) in group A,6 cases (20.6%) in group B and 22 cases (55.0%) in group C had liver injury,and the difference among three groups was of statistical difference (x2 =22.126,P < 0.01),but the difference between group A and group B was not significant (x2 =3.024,P>0.05).One case (3.0%) in group A,3 cases (10.3%) in group B and 15 cases (37.5%) in group C terminated the treatment,and the difference among three groups was of statistical significance (x2 =16.008,P < 0.01),but the difference between group A and group B was not significant (x2 =1.410,P >0.05).ALT and AST in group A and group B were not of significant differences before and after anti-tuberculosis treatment,but those in group C were significantly higher (Z =18.306,16.821,P < 0.01).There was no significant difference in HBV DNA load among three groups before the treatment (Z =0.460,P > 0.05),while HBV DNA loads in group A and group B significantly decreased during the treatment,and the difference among three groups after the treatment was significant (Z =23.213,P <0.01).In addition,lower HBV DNA load was observed in group A compared with group B after one month anti-tuberculosis treatment (Z =8.109,P < 0.01).Conclusion Early use of entecavir can effectively prevent liver injury during anti-tuberculosis treatment,ensuring anti-tuberculosis treatment and anti-HBV treatment carried out as planned.
7.Identification of a HPGD mutation in three families affected with primary hypertrophic osteoarthropathy.

OBJECTIVETo detect mutation of HPGD gene among three pedigrees affected with primary hypertrophic osteoarthropathy (PHO) by DNA sequencing and high-resolution melting (HRM) analysis.
METHODSGenomic DNA was extracted from peripheral blood samples collected from the pedigrees. PCR and direct sequencing were carried out to identify potential mutations of the HPGD gene. Amplicons containing the mutation spot were generated by nested PCR. The products were then subjected to HRM analysis using the HR-1 instrument. Direct sequencing was carried out in family members and healthy individuals to confirm the result of HRM analysis.
RESULTSA homozygous mutation c.310_311delCT was detected in 2 affected probands, while a heterozygous mutation c.310_311delCT was detected in the third proband. HRM analysis of the fragments encompassing HPGD exon 3 showed 3 curve patterns representing three different genotypes, i.e., the wild type, the c.310_311delCT homozygote, and the c.310_311delCT heterozygote. Result of DNA sequencing was consistent with that of the HRM analysis and phenotype of the subjects.
CONCLUSIONThe c.310_311delCT mutation may be the most prevalent mutation among Chinese population. HRM analysis has provided an optimized method for genetic testing of HPGD mutation for its simplicity, rapid turnover and high sensitivity.

8.Effects of mechanical vibration on micoRNA-214-3p and serum interleukin-1β in ovariectomized rats with a fracture

Objective:To observe the effect of mechanical vibration on the expression of microRNA-214-3p and serum interleukin-1β (IL-1β) in the broken ends of fractured bones in ovariectomized rats.Methods:Thirty 3-month-old female Wistar rats were randomly divided into a control group, a model group, and a vibration group, each of 10. An operation of ovariotomy was performed in those of the model and vibration groups to establish osteoporosis model. Five months later a model of mid femur fracture was made with animals in all the groups. Five days after their fracture, the vibration group received 20 minutes of whole-body vibration treatment at 35Hz for 5 days a week, while the control group and the model group received natural rearing without any additional intervention At 2 and 6 weeks after the operation, the Lane-Sandhu X-ray scoring system was used to evaluate the quality of fracture healing, and reverse-transcription polymerase chain reactions and enzyme-linked immunosorbent assays were used to detect miR-214-3p in the fractured bones and the serum levels of IL-1β.Results:At 2 weeks and 6 weeks after the operation the average growth score of the broken ends in the model group was significantly lower than that of the control group, while that of the vibration group was significantly higher than the model group′s average. Compared with the control group at the same time point, the average miR-214-3p content of the model and vibration groups was significantly higher 2 and 6 weeks after the surgery. Compared with the model group, the average level of miR-214-3p of the fractured ends of the vibration group was significantly lower at 6 weeks. Two and six weeks after the surgery, the average IL-1β of the model group was significantly higher than the control group′s average, and that of the vibration group was significantly lower.Conclusion:Mechanical vibration can promote osteoporotic fracture healing by inhibiting the expression of miR-214-3p and reducing the level of IL-1β at the broken ends of fractured bones.
9.Application of peritoneal dialysis routine examination in reducing peritonitis associated with peritoneal dialysis

Objective:To explore the application of peritoneal dialysis routine examination in reducing the incidence of peritonitis associated with peritoneal dialysis.Methods:From July 2018 to June 2019, 191 patients with peritoneal dialysis who were followed up regularly in the nephrology department were selected as the study subjects.Using convenient sampling method, outpatient follow-up on Tuesday were selected as control group(95 cases) and fixed outpatient follow-up on Thursday were selected as observation group(96 cases). Routine follow-up was performed in the control group, and routine examination of peritonitis was performed in the observation group during the follow-up.Intervention was given immediately when the problems were found to the naked eye and the examination results were abnormal.The incidence of peritonitis in the two groups was compared.Results:Peritonitis occurred in 24 cases in the control group(25.26%), and 9 cases in the observation group(9.38%). The incidence of peritonitis in the observation group was lower than that in the control group, and the difference was statistically significant(χ 2=5.972, P<0.05). Conclusion:The occurrence of peritoneal dialysis-related peritonitis is related to a variety of factors.Paying attention to the routine examination of peritonitis and observing the color of peritonitis, strengthening the aseptic fluid exchange operation, and following up the patients can effectively reduce the occurrence of peritonitis and improve the quality of dialysis.
10.Role of gut microbiota in the pathogenesis and treatment of chronic kidney disease

Chronic kidney disease (CKD) is a serious health problem worldwide, whereas there is still no efficient cure. The gut microbiota plays a crucial role in maintaining human health and disease resistance, and multiple studies have confirmed that the gut microbiota is closely related to the occurrence and development of CKD. Starting from the "gut-kidney axis" theory, this article provides a systematic review of the changes in gut microbiota composition and function in patients with CKD, such as a decrease in the abundance of butyrate-producing bacteria Roseburia and Faecalibacterium prausnitzii. Besides that, the article explores the mechanisms by which the gut microbiota affects CKD progression, such as inflammation and immunity, and also describes the application methods of using the gut microbiota as a therapeutic target for CKD, such as fecal microbiota transplantation, microecologics, and dietary therapy, in order to provide microbial- based targets for the clinical diagnosis and treatment of CKD.
