1.Performance evaluation of inpatients’medical service for major diseases in some Beijing municipal hospitals ;based on diagnosis-related groups

Objective To use diagnosis related group (DRGs ) for the first time in overall evaluation of inpatient service performance evaluation of major diagnostic category (MDC)for all the Beijing municipal hospitals,and recommend how to strengthen Beijing municipal hospitals system in diagnosis and treatment ability of main diseases and improve inpateint service performance.Methods BJ-DRGs burster software was used to analyze the first page information of the medical records of cases discharged from all the Beijing municipal hospitals between 2012 and 2014 to determine the weight of each DRG,and based on such weight the related indicators of such hospitals and central hospitals in 2012, 2013,2014 were compared and analyzed.Results Improvements were found in such indices as diagnosis and treatment difficulty of 50% MDC,time efficiency of 81.8% MDC,cost efficiency of 77.3% MDC, and general capacity of 54.5% MDC for all Beijing municipal hospitals.In addition,the municipal hospitals were found superior to the central hospitals in such indices as cost efficiency of 68.2% MDC, and time efficiency of 59.1% MDC.On the other hand however,they were found inferior to the central hospitals in such indices as diagnosis and treatment difficulty of 72.7% MDC,and the comprehensive ability index of the two systems were found equivalent.Another finding was that there was no obvious improvement of the coverage of disease types at major tertiary hospitals in Beijing for the past three years.Municipal hospitals of greater contribution of MDC weight were highly consistent with the hospitals assigned with national key projects of disciplinary developments. Conclusion The comprehensive evaluation results of inpatient service performance of main diseases at Beijing’s municipal hospitals based on DRGs system,showed that the Beijing’s hospital authority had played an important role in improving inpatient service performance especially in reducing the burden of patients,improving the service efficiency through increasing government investment,optimizing service organization and implementation of performance management.But it also suggested that measures such as collectivize construction and management should be taken to improve municipal hospitals’linical specialty ability, improve the MDC diagnosis and treatment difficulty,and resume their functions of tertiary hospitals.
2.Construction and application of cluster nursing measures for urinary retention after gynecological tumor radical operation

Objective:To develop cluster nursing measures for urinary retention after gynecological tumor radical operation, and explore the prevention methods of urinary retention after gynecological tumor radical operation.Methods:We established a gynecological rehabilitation nursing team, retrieved domestic and foreign literature, conducted a Meta-analysis on the key measures to effectively treat urinary retention, and designed and developed a set of cluster nursing measures for urinary retention after radical gynecological tumor surgery. A controlled trail was conducted in this study. Totally 54 patients treated in the gynecological ward of the First Affiliated Hospital of Shenzhen University from January 2016 to June 2017 were selected as the control group, while another 52 patients treated from July 2017 to August 2018 were selected as the experimental group. Patients in the control group received routine gynecological nursing, while patients in the experimental group received cluster nursing based on routine nursing. Data were collected using an observation record form, which included observations of patients' practice and limb mobility effects. The execution time and frequency of urinary interruption training, pelvic floor muscle function training, ankle pump exercise and thoraco-abdominal gymnastics, and the reasons for not using them, drinking test at extubation and intermittent clean self catheterization (CISC) . Data were processed and analyzed using SPSS 21.0.Results:One out of the 52 patients in the experimental group had urinary retention, accounting for an incidence rate of 1.92%, delayed extubation occurred in 2 cases, with a total complication rate of 5.77% (3/52) ; while 5 out of the 54 patients in the control group had urinary retention, accounting for an incidence rate of 9.26%, delayed extubation occurred in 6 cases, with a total complication rate of 20.37% (11/54) . There was a statistically significant difference in the incidence of complications between the two groups ( P<0.05) . Conclusions:The cluster nursing measures are a combination of multiple effective measures, and the clinical application results have shown that they can effective prevent the occurrence of urinary retention after gynecological tumor radical surgery.
3.Changes of hospitalization rates and in-hospital mortality for coronary heart disease in Beijing from 2007-2012.

OBJECTIVETo observe the changes of hospitalization rates and in-hospital mortality for coronary heart disease (CHD) in Beijing from 2007-2012.
METHODSPatients hospitalized for CHD in Beijing from 1 January 2007 to 31 December 2012 were identified from"The Cardiovascular Disease Surveillance System in Beijing". In total, 421 929 patients aged ≥25 years of permanent Beijing residents were admitted for CHD in Beijing during the 6 years. After excluding duplicate records and validation for the completeness and accuracy of the records, the hospitalization rates for CHD and in-hospital CHD mortality were analyzed. Trends in hospitalization rates and the in-hospital mortality for CHD were analyzed with Poisson regression models.
RESULTSThe age-standardized average hospitalization rate of CHD was 515.3 per 100 000 population in patients aged ≥25 years in Beijing. During the six years, an increasing trend was observed in the hospitalization rates for CHD after adjusting the age and gender (P<0.001). The age-standardized hospitalization rates of CHD increased by 43.0% in the past six years. The greatest increases of hospitalization rates were noted in both men and women between 45 to 54 years. The age-standardized in-hospital mortality decreased from 3.3% to 2.2% over the time (P<0.001), with a in-hospital mortality reduction for acute myocardial infarction from 11.3% to 8.5%.
CONCLUSIONSAn increasing trend in hospitalization rate was observed during 2007-2012 for Beijing residents aged ≥25 years, indicating an urgent need in CHD prevention in Beijing. The in-hospital mortality reduction during this period might reflect the improvement in the in-hospital treatment modalities of CHD.

4.Diagnostic Strategies and Clinical Practice of Bone Marrow Metastases from Ewing Sarcoma

Ewing sarcoma(EWS)is an invasive and primary bone tumor with a high incidence in children and adolescents.The presence and extent of metastases at the time of diagnosis remains the most important prognostic factor in determining a patient's prognosis.Up now,considerable ambiguity exists regarding the optimal modality for detecting bone marrow metastases.Bone marrow biopsy and/or aspiration(BMBA)is the gold standard for determining bone marrow metastases.This invasive and painful procedure may be amenable to being replaced by 18F-FDG PET/CT because of its high sensitivity in detecting EWS bone and extraosseous metastases.This review provides an overview of the current literature,concludes that there is no longer a systematic consensus on the implementation of BMAB criteria for the diagnosis of bone marrow metastases in EWS,and summarizes the current practical strategies and clinical practices for the diagnosis of EWS bone marrow metastases accordingly.
5.Clinical and laboratory diagnosis of indolent leukemic mantle cell lymphoma: report of one case and review of literature

Objective:To improve the understanding of indolent mantle cell lymphoma (MCL).Methods:The data of a patient with indolent leukemic MCL in the Second Affiliated Hospital of Nanjing Medical University in May 2013 were collected. The cell morphology was analyzed by using cell smear, the flow cytometry was used to make immunophenotype analysis, the karyotype analysis was performed by usig cytogenetic technique, and polymerase chain reaction (PCR) was used to make the immunoglobulin gene analysis. At the same time, lymph node pathology and immunohistochemistry were also analyzed. The related articles published were reviewed to sum up the characteristics and the treatment of indolent MCL.Results:The male patient aged 60 years was obviously asymptomatic accompanied with slow disease progression, leukemic manifestation and without lymphadenopathy. He received pathological biopsy because of located lymphadenopathy in 2008. Small cell morphology, Kappa light chain immunophenotype, t(11;14) translocation showed after the cytogenetic examination, clonal immune globulin gene rearrangement and low Ki-67 positive index were identified. In situ MCL was diagnosed by retrospective pathology.Conclusions:Indolent MCL is extremely rare. It is typically asymptomatic with none or minimal nodal involvement, indolent disease course, leukemic phase with mild lymphocytosis, Kappa light chain expression, simple karyotype, classical or small cell morphology of tumor cells and the positive index of Ki-67 <10%. In situ MCL can be seen in pathology examination. IgVH gene mutation positive and SOX11 negative expression are notable in indolent MCL. International prognostic index of MCL is probably not appropriate in the prognostic analysis of leukemic indolent MCL. It is emphasized that initial observation and having therapies only after the disease progression can be suited for indolent MCL.
6.Chimeric Antigen Receptor T Cells Therapy for the Treatment of B-lymphoblastic Hematologic Malignancy :A Rapid Health Technology Assessment

OBJECTIVE:To eval uate the effectiveness ,safety and economy of chimeric antigen receptor T cells (CAR-T) therapy for the treatment of B-lymphoblastic hematologic malignancy ,and to provide evidence-based reference for clinical decision. METHODS:Rapid health technology assessment (HTA)was adopted. PubMed ,Embase,Cochrane Library ,CNKI,Wanfang databases and foreign HTA official websites were systematically searched during the inception-Mar. 20th,2021. After inclusion , data extraction and quality evaluation of literatures according to the inclusion and exclusion criteria ,descriptive analysis was performed for the effectiveness ,safety and economy of CAR-T therapy for the treatment of B-lymphoblastic hematologic malignancy. RESULTS :A total of 2 HTA reports ,5 systematic reviews/Meta-analysis ,and 5 economics studies were included. In terms of effectiveness ,CAR-T therapy showed good efficacy in the treatment of B-lymphoblastic hematologic malignancy ;overall remission rate (ORR)of CAR-T therapy in the treatment of acute lymphoblastic leukemia was more than 63.5%,and the complete remission rate (CR)was 77.1%(95%CI:62.8%-87.1%);ORR of CAR-T therapy in the treatment of chronic lymphoblastic leukemia was 70.0%(95%CI:53.0%-80.0%),and the CR was 25.5%(95%CI:13.9%-42.1%);ORR of CAR-T therapy in the treatment of B-cell lymphoma was more than 44.4%. In terms of safety ,the incidence of cytokine release syndrome was more than 20% during the treatment of CAR-T therapy ,and 1/3 or more (9% believed in some studies )patients suffered from neurotoxicity ; the incidence of infection was 12.2%-33.3%,and the incidence of graft-versus-host disease was 23.4%(95%CI:8.6%-49.8%). In terms of economy ,most of the included studies believed that CAR-T therapy possessed economic advantages ,which were the results of evaluation in developed countries such as the United States and Japan. CONCLUSIONS :CAR-T,as a new product of treatment for hematological malignancy ,shows good effectiveness and low level of ADR ,which is basically controllable ;its economy needs to be further evaluated by relevant researches combined with domestic reality.
7.Role of stakeholders in Canadian oncology drug review and its enlightenment

OBJECTIVE To prov ide reference for improving the participation mechanism of stakeholders in the process of medical insurance negotiation for oncology drug in China. METHODS Based on the stakeholder theory ,combined with literature research,case analysis (taking the review of reimbursement of Bentuximab as an example )and other methods ,analysis and research were conducted on the Canadian oncology drug review process and the participation mechanism and role of stakeholders. The suggestions were put forward for our country. RESULTS & CONCLUSIONS Canadian oncology drug reimbursement review process was composed of four stages :the pre-submission planning stage ,the formal submission stage of application,the review stage,and the stage of forming reimbursement recommendations. As the role of stakeholders ,drug manufacturers ,patient representative advisory group , clinical review expert advisory groups and provincial advisory groups participated in the reimbursement review process of oncology drug by providing suggestions and feedback to CADTH. The participation of stakeholders had improved the transparency of the review of oncology drugs in Canada and made the reimbursement results of oncology drugs more scientific ,reasonable and accurate. In China ,it is recommended to define rights ,responsibilities and interests as well as the participation mechanism of stakeholders in the medical insurance negotiation process ,attach importance to the role of patients in the medical insurance negotiation process of oncology drug ,improve information disclosure and increase the transparency of the negotiation mechanism and process so as to increase the participation of stakeholders.
8.Semimonthly Table of Contents Comparison of the effect between drug donation model and national drug negotiated model on drug sales : taking trastuzumab as an example

OBJECTIVE To compare the effect between drug donation model and national drug negotiated model on drug sales,and to provide reference for pharmaceutical enterprises to optimize their business strategies . METHODS Based on medical insurance statistics and settlement data of city A in Jiangsu province from 2013 to 2018,trastuzumab was selected as the representative drug . The sales and utilization of trastuzumab were compared during the implementation of the two models . At the same time ,the changes of medication behavior of some patients were analyzed . RESULTS After the implementation of drug donation model ,sales volume and turnover of trastuzumab increase by 193.87% and 155.71% in that year compared with the previous year ,and then the growth trend became stable ;after including the national negotiated drug list ,the number of users of trastuzumab in the year increased by 87.68% compared with the previous year ,and the sales volume increased by 48.06%;turnover increased by 22.81% even though the unit price decreased significantly . Both models could improve drug sales ,and the results of multiple linear regression also proved that different models had a significant impact on the consumption sum of trastuzumab in patients. CONCLUSIONS Under the condition that the unit price level of drugs is approximately the same ,both drug donation model and national drug negotiated model can increase the sales volume and expand the coverage of drugs . The latter reduces the drug threshold of patients ,and plays a positive role in improving the drug accessibility of patients and the sales and development of enterprises.
9.Injury of Muscular but not Cutaneous Nerve Drives Acute Neuropathic Pain in Rats.

Acute pain is a common complication after injury of a peripheral nerve but the underlying mechanism is obscure. We established a model of acute neuropathic pain via pulling a pre-implanted suture loop to transect a peripheral nerve in awake rats. The tibial (both muscular and cutaneous), gastrocnemius-soleus (muscular only), and sural nerves (cutaneous only) were each transected. Transection of the tibial and gastrocnemius-soleus nerves, but not the sural nerve immediately evoked spontaneous pain and mechanical allodynia in the skin territories innervated by the adjacent intact nerves. Evans blue extravasation and cutaneous temperature of the intact skin territory were also significantly increased. In vivo electrophysiological recordings revealed that injury of a muscular nerve induced mechanical hypersensitivity and spontaneous activity in the nociceptive C-neurons in adjacent intact nerves. Our results indicate that injury of a muscular nerve, but not a cutaneous nerve, drives acute neuropathic pain.

Zinc levels are high in pancreatic β-cells, and zinc is involved in the synthesis, processing and secretion of insulin in these cells. However, precisely how cellular zinc homeostasis is regulated in pancreatic β-cells is poorly understood. By screening the expression of 14 Slc39a metal importer family member genes, we found that the zinc transporter Slc39a5 is significantly down-regulated in pancreatic β-cells in diabetic db/db mice, obese ob/ob mice and high-fat diet-fed mice. Moreover, β-cell-specific Slc39a5 knockout mice have impaired insulin secretion. In addition, Slc39a5-deficient pancreatic islets have reduced glucose tolerance accompanied by reduced expression of Pgc-1α and its downstream target gene Glut2. The down-regulation of Glut2 in Slc39a5-deficient islets was rescued using agonists of Sirt1, Pgc-1α and Ppar-γ. At the mechanistic level, we found that Slc39a5-mediated zinc influx induces Glut2 expression via Sirt1-mediated Pgc-1α activation. These findings suggest that Slc39a5 may serve as a possible therapeutic target for diabetes-related conditions.
