1.Clinical application of impulse oscillation system determining bronchodilator reversibility test in the children of different ages

Objective To explotr clinical application of impulse oscillation system(IOS)determining bronchodilator reversibility test and the impact of age factor on positive values of bronchoddilator reversibility test in children.Methods Two hundred and fifteen patients during asthma attack were recruited randomly for this study.They were divided into three groups according to their age(4-7 year-old group,8-11 year-old group and 12-16 year-old group).Mensurated MEFV and IOS lung function.Carried on the bronchial dilation test with the results.Forced expiratory volume in first second(FEV1),the sensitivity and specificity of IOS parameters,total respiratory impedance(Zrs),respiratory resistance at 5 Hz(R5)and 20 Hz(R20),reactance at 5 Hz(X5)and resonant frequency(Fres)for diagnosing asthma at different improvement levels after inhalation of bronchedilator were calculated respectively through using FEV1 as a "sold standard"in the different age groups.Results After inhalation of bronchodilator,Fres,Zrs,R5,X5 and R20 of IOS were improved significantly.The improvement of X5 was the most obvious,however the decline of R20 was the lowest.The improving rate of Fres,Zrs,R5 had very significant difference in the three age groups(P<0.01).It displayed that the age factor was very important impact factor on the improving rate of Fres,Zrs,R5,however the improving rate of X5 was on difference in the three age groups(P>0.05).In the three age groups,there were significantly positive correlations between X5,R5,Zrs and FEV1(P<0.01).there wash't correlations between R20 and FEV1 in the 4-7 year-old group and the 12-16 year-old group(P>0.05).Furthermore,when an increase of FEV1 equaling to or greater than 15% was used as reference for positive bronchial reversibility,the improve rate of Fres,Zrs,R5,and X5 equaled to or greater than 13%,23%,17%,31%,respectively in 4-7 year-old group,18%,26%,25%,32%,respectively in 8-11 year-old group,25%,28%,28%,36%,respectively in 12-16 year-old group. Conclusions IOS can be used to assess bronchodilator reversibility test in children,and age factors should be paid attention.
2.Development of training curriculum in sexual health of breast cancer patients for oncology nurse

Objective To develop a curriculum of a training course for oncology nurses in regard to patient education in sexuality for breast cancer patients. Methods Delphi Method was used in the study. Two rounds of questionnaire investigation were conducted among 21 specialists in breast nursing care, sexology, and psychology. The curriculum was developed after revised based on the comments and suggestions brought up in the experts. Results The training curriculum in sexual health of the breast cancer patients for oncology nurse consisted of 5 sections, professional education, sexology, medicine, psychology and practice, which were divided into 45 teaching topics included in 13 courses. The comments of the specialists on the training curriculum were consistent. The coefficients of variation ranged from 0.07 to 0.20. The coordination coefficient was 0.344. Conclusions The current study developed an integral curriculum for oncology nurses in regard to patient education in sexuality for breast cancer patients, which will benefit oncology nurses for their nursing practice.
3.Efficacy and safety of early rapid infusion of icy normal saline in patients after cardiopulmonary resuscitation

Objective To assess the feasibility,safety,and effectiveness of early rapid icy normal saline infusion to attain mild hypothermia in cardiac arrest patients. Methods A single-center prospective randomized controlled trial was conducted. From March 2011 to October 2013,patients who had recovery of spontaneous circulation (ROSC)after cardiopulmonary resuscitation (CPR)in Beijing Daxing District People's Hospital were randomly divided into two groups. In icy normal saline group,patients received a rapid infusion of 1 000 mL of 4 ℃ normal saline intravenously to attain a mild hypothermia. In the control group,the patients were treated with ice bag on head,and axillary temperature was monitored. For all patients,rectal temperature was measured and recorded immediately and 1 hour later . The occurrence of pulmonary edema on initial chest X-ray at 6 hours ,occurrence of tremor within 48 hours,ventricular fibrillation recurring within 48 hours,and consciousness or death within 14 days were recorded. Results A total of 45 patients were enrolled,including 23 patients in icy normal saline group and 22 in control group. The patients in icy normal saline group had a rectal temperature descended from(36.7±0.9)℃to(34.9±0.7)℃1 hour later,while the patients in control group had a rectal temperature risen from(36.5±1.0)℃to(37.9±0.9)℃1 hour later. There was significant difference in rectal temperature between two groups (t=2.228,P=0.031). The number of patients who successfully awaken within 14 days in ice normal saline group was significantly larger than that in control group (13 cases vs. 7 cases,χ2=65.710,P=0.021). There was no statistical difference in the occurrence of acute pulmonary edema (4 cases vs . 6 cases),tremor (2 cases vs . 0 case),ventricular fibrillation recurrence (4 cases vs. 5 cases)and death within 14 days (11 cases vs. 12 cases,all P>0.05). Conclusions The study shows that early rapid i.v. infusion of 4℃normal saline is feasible,safe and effective for cerebral resuscitation.
4.Posttraumatic growth in cancer patients and its effect on patients' quality of life

Objective To investigate the relationship between the posttraumatic growth in cancer patients and patients' quality of life. Methods 123 cancer patients were measured with posttraumatic growth inventory (PTGI) and EORTC QLQ-C30. Means and standard deviations were used to describe the domain and the range change of posttraumatic growth. Correlation and multiple linear regression analysis were conducted to explore the relationship between the patients' posttraumatic growth and their quality of life. Results 4 of 5 factors in PTGI showed significant positive change, including connection with others [(28.17±7.97) scores], new possibilities [(16.15±6.06) scores], personal strength [(15.42±4.69) scores], and appreciation of life [(11.51±3.80) scores], meanwhile, the spiritual change [(5.07±2.67) scores] had no significant growth. The sum of PTGI was related positively to the role function of the patients. New possibilities (r=0.193), personal strength (r=0.290) and appreciation of life (r=0.284) were correlated positively with global health status. Multiple regression analysis indicated that personal strength (β=0.373, t=3.66) and appreciation of life (β=0.322, t=2.99) could predict positively to global health status, however, relating to others predicted negatively to global health status (β=-0.396, t=-3.63). Conclusion Posttraumatic growth has been found in cancer patients, including some predicting factors of patients' quality of life, such as personal strength, life appreciation and relating to others.
5.The current status and influencing factors of caring behaviors for oncology specialized nurses

Objective To investigate the current status and influencing factors of caring behaviors for oncology specialized nurses. Methods Using general information questionnaire and Caring Behaviors Inventory (CBI) to investigate oncology specialized nurses from the city of Beijing, Shanghai, Sichuan, Chongqing, Guangzhou and Tianjin. Results The total score of self-assessment CBI in oncology specialized nurses was relatively high (82.42±10.42), the scores of sub-scales from high to low in the order were assurance, respectful, knowledge and skill, and connectedness. Multiple stepwise regression analysis showed that degree of family care, whether the one-child and working years were influencing factors of caring behavior of oncology specialized nurses (t=11.995, 3.061, 2.608, P<0.01). Conclusions Nursing managers should put forward humanism-oriented theory into all parts of nursing work on the basis of scientific allocation of human resources, establish integrative care training model combing the school and hospital in order to improve caring behaviors of nurses.
6.Diagnosis of tracheobronchomalacia in childen with electron-bronchoscopy

Twenty five children with recurrent wheezing,intractable cough and recurrent respiratory infection were examined and treated by electron-bronchoscopy.Among them 9 cases were diagnosed as tracheobronchomalacia.simple endobronchial inflammation was found in 7 cases,foreign body in bronchus were found in 6 cases,bronchostenosis in 2 cases and pulmonary hemorrhage in 1 case.Tracheobronehomalacia is one of the leading causes of recurrent wheezing and intractable cough in children.Electron-bronchoscopie examination is important in diagnosis of tracheobronchomalacia in children.
7.Comparison of three neoadjuvant chemotherapy in treating cervical carcinoma

Objective To explore the application value of neoadjuvant chemotherapy (NACT) in treating cervical carcinoma. Methods Seventy-one cervical carcinoma patients who were divided into three groups:PF group (cisplatin and fluorouracil,23 cases) or CBP group (carboplatin,bleomycetin and cyclophosphamide, 24 cases) or TP group (paclitaxel and cisplatin, 24 cases); operation was made 14 - 21 days afterwards. The therapeutic effect, chemotherapy side-effect and the effect on operation and pathology for these three groups were analyzed and compared. Results The effective rate was 93.8%(30/32),88.6% (31/35),82.4%(28/34) in TP group,CBP group,PF group,respectively. However, the therapeutic effect had no statistics significance with age, preoperative tumor grade, pathologic type for all the three groups, the therapeutic effect had statistical significance with clinical stage. The rate which pathological examination showed no residual cancer was biggest in CBP group [CBP group was 28.6%(6/21), TP group was 4.3% (1/23), PF group was 11.8%(2/17), P < 0.05 ]. Conclusions Three NACT projects are safe and effective treatment for cervical cancer. However,each project has advantages. The NACT projects can be elected for different patient according to his illness and economy.
8.Analysis of electronic bronchoscopy results of 400 children with respiratory disease

Objective To investigate the value of electronic bronchoscopy in the diagnosis of children respiratory diseases. Methods The electronic bronchoscopy results in 400 children with respiratory disease were retrospectively analyzed. Results In 400 children, there were 246 cases with simple endobronchial inflammation, 67 cases with bronchial malacia, 38 cases with bronchial stenosis, 23 cases with bronchial foreign , 4 cases with tracheal bronchus, 4 cases with epiglottic cyst, 3 cases with bronchiectasis, 3 cases with endobronchial granulation, 3 cases with laryngomalacia, 2 cases with vascular ring compression of the trachea, 2 cases with laryngeal web, 1 case with subglottic cyst, 1 case with subglottic neoplasm, 1 case with tracheoesophageal fistula, 1 case with bronchial atresia (left) and 1 case with trachea cyst. All the children had no serious complications. Conclusions Electronic bronchoscopy can effectively improve the level of diagnosis and treatment of children' s respiratory system disease, and it is worth of clinical promotion.
9.Hepatotoxicity in lung cancer patients with hepatitis B virus infection

Due to high prevalence of lung cancer and hepatitis B virus infection in China, lung cancer patients complicated with HBV infection are not rare.The incidence of liver function impairment in lung cancer patients with HBV infection is much higher than those without HBV infection.Some special risk factors and mechanism of liver function impairment in lung cancer patients with HBV infection have been reported.This article reviews the incidence,mechanism and treatment of liver function impairment in those patients so as to provide information for prevention and management of this condition.
10.The effect of couple-centered psychological intervention on breast cancer patients——A Meta-analysis

Objective:To systematically evaluate the effect of couple-centered psychological intervention on breast cancer patients.Methods:Randomized controlled trials published in PubMed, Embase, Web of Science, Scopus, CINAHL, Cochrane Library, JBI, CNKI, Wanfang, Vip and CBM database on the effect of psychological intervention on breast cancer patients with husband and wife as the center were searched by computer. The retrieval time was from the establishment of the database to September 2020. Two reviewer independently conducted literature screening, data extraction and literature quality evaluation according to the inclusion and exclusion criteria, and Meta analysis was carried out using RevMan 5.3 software.Results:A total of 879 patients were included in 11 articles. Meta analysis results showed that the couple-centered psychological intervention not only improved the quality of life, but also improved the anxiety of breast cancer patients ( SMD=1.54, 95% CI 0.22-2.86, P=0.02) and depression ( SMD=1.22, 95% CI 0.12-2.33, P=0.03). Conclusions:After psychological intervention, the anxiety and depression of breast cancer patients were improved and the quality of life was also improved. Due to the inconsistency in the mode, frequency and evaluation index of psychological intervention, more large samples and high-quality studies are needed to further verify the effect of couple-centered psychological intervention on breast cancer patients and spouses centered on husband and wife.
