1.Non-operative and operative treatments of senile comminuted distal radial fractures:a comparative research of their clinical results

Objective To compare the clinical results of operation and manual reduction and plaster immobilization in management of senile comminuted distal radial fractures.Methods Retrospective analysis of the clinical data was done for the 178 senile patients with comminuted distal radial fracture who were treated from January 2000 to September 2004 in our department.102 cases were treated with manual reduction and plaster immobilization,46 cases with external fixator and limited internal fixation,and 30 cases with T-plate internal fixation.Results Follow-up duration averaged 27 months(13 to 52 months).According to Dienst criteria,the excellent rate of functional outcome was 93.5%for external fixator combined with limited internal fixation,78.4%for manual reduction and plaster immobilization and 80.0%for T-plate internal fixation.The differences were statistically significant(P
2.Comparison between radial head resection and open reduction and internal fixation in clinical treatment of Mason type- Ⅲ comminuted fractures of radial head

Objective To compare the clinical results of radial head resection with those of open reduction and internal fixation in treatment of Mason type- Ⅲ comminuted fracture of radial head. Methods 58 cases of Mason type- Ⅲ radial head fracture were treated in our department from January, 1983 to July, 2005. 28 cases had resection of the fractured redial head, while the other 30 cases had open reduction and internal fixation. The outcomes were assessed on the basis of pain, motion, strength, radiographic findings, and elbow functional rating score. Results The resection group had a mean follow- up of 5.5 years (2 to 6), while the internal fixation group had 10 years (3 to 13). The average pain VAS (visual analog scale) score was 18.7 (11 to 25) for the resection group and 23.5 (17 to 25) for the fixation group with a P value of 0.0023. The elbow extension averaged - 12.4? in the resection group and - 5.8? in the fixation group (P0.05) , but the decrease of elbow extension and forearm rotation differed statistically between the 2 groups (P
3.Treatment of acute osteochondral fractures of the talus using autologous osteochondral transplantation

[Objective]To study the clinical results of treatment of local osteochondral fracture lesions on the talus dome associated with ankle fracture using autologous osteochondral transplantation.[Method]There were 18 patients with osteochondral fracture defects of the talus dome associated with ankle fracture, male 10, female 8, with the average age 33.5 years (18~49 years). Transplantation procedure was osteochondral cylinders from non-weight bearing surface of the ipsilateral knee and implant the donor graft into the recipient holes of talus cartilaginous defects with press-fit technique. The outcomes were assessed on the basis of ankle functional rating score, radiographic findings and MRI findings.[Result]The patients were followed up for 17 to 40 months (mean, 28 months). The ankle functional rating score was measured by the Baird and Jackson functional rating score system. The averaged scores was 96.3?3.5,excellent in 14 cases, good in 3 cases and fair in 1 case. The rate of being excellent and good was 94.4%.The rate of posttraumatic arthritis was 5.6% (1/18) and osteochondritis dissecans was 5.6% (1/18).[Conclusion]Autogenous osteochondral transplantation using ipsilateral knee osteochondral grafts is an effective surgical procedure on treating acute local osteochondral fracture lesions of the talus associated with ankle fracture.

5.Efficacy and Safety of Naloxone Combined with Conventional Therapy in the Patients with Pulmonary En-cephalopathy

Objective: To observe and analyze the efficacy and safety of naloxone in the treatment of patients with pulmonary encephalopathy. Methods:50 patients with pulmonary encephalopathy were randomly divided into the observation group and the con-trol group. All the patients were carried out continuous oxygen with non-invasive ventilator. The control group was treated with the con-ventional treatment, and the observation group was given naloxone on the basis of conventinal treatment. The treatment course was 7 days. The clinical efficacy, clinical indicators and adverse events of the two groups were observed. Results:The total effective rate of the observation group reached 92. 00%, which was higher than that of the control group without statistical significance (72. 00%, P>0. 05). The levels of pH, PaO2, PaCO2, angiotensinⅡand IL-8 were significantly improved in both groups after the treatment (P<0. 05), and the improvement in the observation group was much better than that in the control group (P<0. 05). The incidence of ad-verse reactions of the two groups after the treatment showed no significant difference (P>0. 05). Conclusion: Early application of naloxone in the treatment of pulmonary encephalopathy can improve the clinical indices significantly with low incidence of adverse reac-tions and high safety, which is worthy of promoted use in clinics.
6.A Clinical & Electrocardiographic Analysis of 35 Cases with Complete Atrio-ventricular Block(CAVB)

The clinical and electrocardiographic data of 35 cases with CAVB were analyzed. They could be divided into two groups: chronic (28 cases) and transcient(7 cases). The primary diseases of the patients in the chronic group were coronary heart disease(CHD) in 11 cases, a condition of unknown cause in 12, and miscellaneous diseases in 5.The clinical and ECG manifestations of the 12 cases with unknown cause were suggestive that the disorder was most likely to be primary degeneration of conduction bundles(PDCB). PDCB differed from CHD by the following:(1) In PDCB, there was neither history of chest pain nor factors vulnerable to CHD being found. No ischemic changes cound be found on the ECG of PDCB cases.(2) The duration of CAVB was longer in PDCB cases (mostly over 3 years) than in CHD cases (all under 2 years).(3) The onset of CAVB occurred at a younger age in PDCB(an average of 50.6 years) than in CHD(an average of 66.3 years).The difference between the two was significant(P
7.The messenger management on asthma control and pulmonary function in young and middle-aged asthma patients

Objective To investigate instant messenger management on moderate asthma control in young and middle-aged patients.Methods Sixty young and middle-aged asthma patients enrolled were divided into 2 groups by random number table,traditional management group (group A) and messenger management group (group B),and received the asthma treatment with traditional management or messenger management for 1 year.The compliance,control condition,pulmonary function and Saint George respiratory questionnaire(SGRQ) were compared between two groups.Results After one year management,there were 20 patients in proceeding of the standard treatment,15 total control,10 partial control and 5 uncontrolled in group A,while there were 26 patients in proceeding of the standard treatment,21 total control,7 partial control and 2 uncontrolled in group B.Compared with group A,the treatment adherence(x2 =6.331,P < 0.05),satisfaction degree (t =6.051,P < 0.01),asthma control rate (x2 =6.490,P < 0.05),efficacy satisfaction (t =6.051,P < 0.01) were all increased in group B.Forced expiratory volume in 1 second was elevated more significantly in group B than that in group A [(0.132 ± 0.175) L vs.(0.326 ± 0.125) L] (t =5.099,P< 0.01).SGRQ scores were decreased more obviously in group B than that in group A (t =4.316,P < 0.01).Conclusions The asthma control,respiratory and pulmonary function can be improved by messenger management in young and middle-aged patients.So promoting the messenger management model actively is advantageous to improve the level of prevention and treatment of asthma in China.
8.miR-134 regulates cisplatin resistance of human lung adenocarcinoma cells

AIM:To investigate the roles of microRNA-134 (miR-134) in the cisplatin resistance of lung ade-nocarcinoma cells.METHODS:miRNA microarray was applied to compare the miRNA expression profile between A549/CDDP and A549 cells.Real-time PCR was used to confirm the expression of miR-134.miR-134 mimics and inhibitors were transfected into A549/CDDP and A549 cells, respectively.MTT assay was used to detect the sensitivity of lung cancer cells to cisplatin.Western blot was applied to test whether miR-134 regulated forkhead box protein M1 ( FOXM1 ) and multi-drug-associated protein 1 ( MRP1 ) expression.RESULTS: Based on the data of miRNA microarray, 13 miRNAs were found to be differentially expressed in A549/CDDP cells compared with A549 cells, among which miR-134 was the most significantly down-regulated one.Compared with control group, A549/CDDP cells transfected with miR-134 mimics showed greatly enhanced sensitivity to cisplatin as indicated by IC50 values (P<0.01).In contrast, suppression of the miR-134 level in the A549 cells resulted in a decreased sensitivity to cisplatin (P<0.01).FOXM1 siRNA down-regulated the pro-tein levels of FOXM1.A549/CDDP cells transfected with si-FOXM1 showed enhanced sensitivity to cisplatin (P<0.01). In addition, the result of Western blot showed that miR-134 repressed MRP1 protein expression.CONCLUSION: miR-134 effectively increases the sensitivity of lung adenocarcinoma cells to cisplatin, and this effect of miR-134 may be partly due to its regulation of FOXM1 and MRP1 expression.
9.Studies on the Non-isothermal Decomposition Kinetics of Vitamin B6

AIM The purpose is to study the non-isothermal decomposition process and mechanics of vitamin B6. METHOD The TG technique was used to observe between 30~700℃. RESULTS The decomposition of vitamin B6 was performed by two stages. Vitamin B6 loses HCl at the first stage together with losing H2O. The kinetic equation obtained was dα/dt=A*e-E/RT*1/2(1-α)3; activation energy obtained was 325.27 kJ/mol; and preexponential factor A obtained was 7.22×1032/s as well. CONCLUSION Vitamin B6 is rather thermal stable, and it loses HCl together with losing H2O at temperature range of 173℃~271℃.

Objective To study the role of soluble intercellular adhesion molecule-1 ( sICAM-1 ) in the pathogenesis of adults bronchial asthma and analyze the relationship between the level of sICAM-1 and the severity of bronchial asthma.Methods Serum levels of sICAM-1 in 134 cases with different periods of bronchial asthma patients and healthy volunteers were measured by ELISA and pulmonary functions of acute asthma patients were determined by pulmonary function analyzer. Results Serum sICAM-1 levels in 63 cases of acute asthma patients were increased significantly when compared with those in remission asthma patients and healthy volunteers and its value was increased significantly with the exacerbation of asthma;A negatively correlation between FEV1% of pulmonary function and serum sICAM-1 level in patients with acute asthma was found in this study. Conclusion sICAM-1 was one of the important adhesion molecules in the pathogenesis of bronchial asthma. It could be used as one indicator of disease severity and guide the drug application in clinic.
