1.Interventive effect of Angongniuhuang injection on inflammatory response after brain injury in rats

Objective To observe the influence of Angongniuhuang injection on the level of in- terleukin-1?(IL-1?),intercellular adhesion molecule-1 (ICAM-1),serum protein S100B and neuron specific enolase (NSE) after brain injury to explore its protective effect on the injured brain tissues. Methods Brain contusion model was made in rats by Feeney's method.Then,the levels of IL-1?and ICAM-1 in the brain tissues and the levels of serum protein S100B and NSE in serum were measured by ELISA method at different time points.Results The level of IL-1?and ICAM-1 in brain tissues and that of S100B protein and NSE in serum in treatment group were lower than that in control group 6-48 hours after injury (P<0.05).Conclusion Angongniuhuang injection can alleviate inflammatory re- sponse after brain injury and protect effectively brain tissues.
2.Effect of Transcutaneous Electrical Nerve Stimulation with Different Electrode Placement on Phantom Limb Pain

Objective To study the effect electrode placement of transcutaneous electrical nerve stimulation(TENS) on phantom limb pain in the postoperative phase of amputation.Methods 60 cases with phantom limb pain after amputation were equal divided into the treatment group and the control group.The electrodes were placed in the contralateral side similar as phantom limb in the treatment group,but were placed at the stumps in the control group.Results After two course of treatment,93.3% of the treatment group improved,and 76.7% very improved,which were better than those of the control group(P<0.05).Conclusion Electrode placement of TENS effects the different result of treating phantom limb pain.
3.Application of artificial humeral head prosthesis in the treatment of proximal humerus bone tumor

Objective To evaluate the clinical effects of the artificial humeral head prosthesis in the treatment of proximal humerus bone tumors. Methods Fifteen patients with proximal humerus bone tumor had excision of bone tumor and application of artificial humeral head prosthesis were analyzed retrospectively. Among them,8 patients had osteosarcoma,4 patients had giant cell tumor of bone,2 patients had chondrosarcoma, 1 patient had fibrosarcoma. Results All cases were followed up for average (39 ± 5 ) months after the surgery.The duration time of operation were( 110 ± 10) minutes in average. The bleeding volume during operation were (600 ± 41 ) ml in average. One patient with osteosarcoma died of recurrence and metastasis 20 months after the surgery. The functions of the other 14 patients were assessed by Enneking system and the average score was 23.3 ± 3.7. The overall rate of excellent or good results was 84.6% ( 11/15 ). Conclusion Limb salvage surgery with the artificial humeral head prosthesis is an efficacious treatment for proximal humerus bone tumors,which can improve short-term clinical outcome.
4.Standardization of Malignancies report in General Hospital promoted by combination of training and management

Objective To explore appropriate management of cancer report for general hospital and analyze the effect.Methods Based on the requirements of cancer report for general hospital that formulated by the health administrative departments,we set the rule and process of malignancy report.We clear the responsibilities of reporting and the management,strengthen the training of malignancies report,make regular supervision and evaluation,and gradually use information live system to monitor malignancies report.Results In 2006-2010,the number of reports of malignancy in our general hospital has a upward trend.The false negative rate and delaying rate decreased year by year.The pass rate of the report card increased year by year,and the difference was statistically significant (<0.05).With the increase of the rate of training for malignancies report,rate of timely report increased significantly.Conclusion General hospitals should use their own conditions to the development of scientific and operational report management systems and processes for malignancies.Attention should be paid to the combine of training and management and the gradual standardizing of the malignancies reporting.
5.Clinical analysis of 48 cases with pulmonary lymphangioleiomyomatosis:a report of three cases and review of domestic literatures published in the past ten years

Objective To improve the understanding and diagnosis of pulmonary lymphangioleiomyomatosis (PLAM) by the comprehensive review of domestic literatures in the past ten years.Methods Three new cases with PLAM were reported and integraed with other 45 cases reported domestically in the past ten years for analysis of their clinical features.Results The newly reported three cases of PLAM were all women at child-bearing age, with initial symptom of dyspnea after activity. Two of them complicated with extra-pulmonary PLAM. All the three cases were free of chylous effusion. Forty-seven of 48 cases with PLAM were pathologically diagnozed, with ages of onset of 5~69 (mean?s of 34?10) years. Their clinical manifestations were mainly respiratory, including dyspnoea (95.8%), haemoptysis (52.1%), pneumothorax (45.8%), chylous effusion (33.3%),cough (31.3%) and chest pain (12.5%). Abnormal manifestations in abdomen, including renal mass, retroperitoneal mass and retroperitoneal lymphadenopathy, were detected in 16 cases. Thirty-nine cases had their high-resolution CT (HRCT) examined and appearance of multiple cysts distributed throughout the bilateral lung fields could be discerned in 38 of them. Obstructive ventilation disturbance could be observed in 23 of 30 cases with the data or conclusions on pulmonary function tests, and mixed ventilation disturbance in seven cases. Respiratory failure was complicated in 17 of 28 cases with the data of arterial blood gas analyses.Conclusions HRCT had confirmative value for diagnosis of PLAM. In practice, HRCT, as well as other routine abdominal and pelvic imaging examinations, should be performed in time for child-bearing-age women with progressive dyspnoea, haemoptysis, or spontaneous pneumothorax, to detect if they complicate with PLAM.
6.Investigation of cerebral iron deposition in aged patients with ischemic cerebrovascular disease using susceptibility-weighted imaging

Objective To investigate iron deposition level in patients with ischemic cerebrovascular disease and its correlation with cerebral small vessel disease (CSVD) imaging markers.Methods A total of 74 patients with first-ever transient ischemic attack (TIA) (median age:69 years old;30 males,44 females) and 77 patients with positive ischemic stroke history (median age:72 years old;43 males,34 females) were studied retrospectively.On phase image of susceptibility weighted imaging (SWI),regions of interest (ROIs) were manually drawn at the bilateral head of the caudate nucleus (CN),lenticular nucleus (LN),thalamus (TH),frontal white matter (FWM),and occipital white matter (OWM).The correlation between iron deposition level and the clinical and imaging variables was also investigated.Results Iron deposition level at LN was significantly higher in patients with previous stroke history (P < 0.05).It was linearly correlated with the presence and number of cerebral microbleed (CMB) but not with white matter hyperintensity (WMH) and lacunar infarct.Conclusions Ischemia may cause iron deposition elevation at basal ganglia,and LN may be more susceptible to ischemia-induced alteration.CMB may be an independent risk factor for higher iron deposition.
7.Therapeutic Observation of Triple Needling plus Warm Needling for Chronic Pelvic Inflammation

Objective To observe the clinical efficacy of triple needling plus warm needling in treating chronic pelvic inflammation due to cold and damp stagnation.Method Sixty patients with chronic pelvic inflammation due to cold and damp stagnation were randomized into a treatment group and a control group, 30 in each group. The control group was intervened by ordinary acupuncture, while the treatment group was by triple needling and warm needling in addition to the ordinary acupuncture method. The symptom and sign scores were evaluated before and after intervention, and the clinical efficacies were compared. Result The symptom and sign scores were changed significantly after intervention in both groups (P<0.01,P<0.05). After intervention, the symptom and sign score in the treatment group was significantly different from that in the control group (P<0.05). The total effective rate was 93.3% in the treatment group versus 80.0% in the control group, and the difference was statistically significant (P<0.05).Conclusion Triple needling plus warm needling is an effective method in treating chronic pelvic inflammation.
8.Immunobiologic study of two different isolated DC subsets from the donons' peripheral blood induced by G-CSF

Objective To separate the CD1c+ MDC and CD304 PDC subsets and analyze their immunobiologic properties.Methods MDC or PDC were separated by immunomagnetic beads and then induced by TNF-α or CpG2006 OND.The phenotype of different DC subsets and allogeneic T-cell-stimulating capacity were detected,respectively.Results The purlty of separated DC was about ninety-five percent.MDC expressed HLA-DR,CD11c,CD4 and CD33,PDC expressed HLA-DR,CD4 and CD45RA,and the percentage of CD40,CD80 were up-regulation after induced to mature.The results of mixed lymphocyte reaction(MLR):MDC had strong potential to stimulate allogeneic T lymphocytes proliferation,while PDC had the weak stimslate potential.The supematant IFN-r in all groups was increased.which the groups of MDC and mMDC were more obviously.The supematant IL-10 was increased obviously in groups of PDC and mPDC.The expression of IFN-r in CD4+ cells were increased obviously after stimulated by MDC or mMDC.PDC and mPDC had the effect of inducing of CIM+ CD25high regulatory T lymphocyte in MLR.The expressing of Foxp3 mRNA had no evident difference in all groups.Conclusion Two different DC subsets in the donors peripheral blood which mobilized with G-CSF were in the state of immature and could be induced to mature in the suitable condition.although expressed high constitutive levels of HLA,DR.No matter how MDC was mature or not,they had strong potential to stimulate allogeneic T lymphocytes proliferation.MDC enhanced the production of IFN-r among T lymphocytes,and the increase of IFN-r was probably the result of TH1 polarization.Unlike MDC,PDC was less efficient present antigens and had poor T-cell-stimulating capacity,probably because of their lesser ability to phagocytose,process and present antigens to the MHC molecules.Although PDC could promote the production of IFN-r,it seemed that did not produced by TH 1.PDC could not promote TH2 cells to product IL-4,and the polarization of TH2 probably reflect the production of IL-10.PDC could induce the generation of CD4+ CD25high regulatory T cells.
9.Effects of postconditioning with α7 nicotinic acetylcholine receptor agonist and ischemia on myocardial ischemia-reperfusion injury in rats

Objective To investigate the effects of postconditioning with α7 nicotinic acetylcholine receptor (α7nAChR) agonist and ischemia on myocardial ischemia-reperfusion (IR) injury in rats. Methods Fifty adult male SD rats weighing 290-320 g were randomly divided into 5 groups ( n = 10 each): Ⅰ sham operation group, Ⅱ IR group, Ⅲ ischemic postconditioning group, Ⅳ α7nAChR agonist postconditioning group and Ⅴpostconditioning with α7nAChR agonist and ischemia group. Myocardial I/R was induced by ligation of anterior descending branch of left coronary artery for 30 min followed by 1 80 min of reperfusion. In group] the anterior descending branch was only exposed but not ligated. In group Ⅲ the hearts were subjected to 3 episodes of 10 second ischemia at 10 second intervals at the end of 30 min ischemia before 180 min reperfusion, Intraperitoneal PNU282987 2.4 mg/kg was injected at the end of 30 min ischemia before 180 min reperfusion in group Ⅳ and Ⅴ .Blood samples were taken from right internal jugular vein at 180 min of reperfusion. Then the rats were killed and hearts removed to determine the concentrations of serum cardiac troponin-I (cTnI), TNF-α and high-mobility group box 1 (HMGB1) by ELISA. The infarction size was measured by Evans blue and triphenyltetrazolium chloride staining. Results The infarction size was significantly larger in the other groups and concentrations of serum cTrI, TNF-α and HMGB1 were significantly higher in group Ⅱ than in group Ⅰ ( P < 0.05). The infarction size was significantly smaller and concentrations of serum cTnI, TNF-α and HMGBI were significantly lower in group Ⅲ, Ⅴ than in group Ⅱ (P < 0.05). The infarction size was significantly smaller in group Ⅴ and concentrations of serum cTnI, TNF-α and HMGB1 were significantly lower in group Ⅳ and Ⅴ than in group Ⅲ (P <0.05). The infarction size was significantly smaller and concentrations of serum cTnI, TNF-α and HMGB1 were significantly lower in group Ⅴ than in group Ⅳ ( P < 0.05 ). Conclusion Postconditioning with α7nAChR agonist and ischemia can reduce myocardial I/R injury and the efficacy is better than that of α7nAChR agonist postconditioning or ischemic postconditioning alone.
10.Levels of programmed death-1 and programmed death ligand-1 in the peripheral blood of patients with oral squamous cell carcinoma and its clinical implications.

OBJECTIVETo detect the expression levels of programmed death-1 (PD-1) and programmed death ligand-1 (PD-L1) in the peripheral blood of patients with oral squamous cell carcinoma (OSCC) and to discuss their biological and clinical significance.
METHODSPD-1/PD-L1 expression on the surface of T-lymphocytes and the counts of T-lymphocyte subpopulations of peripheral blood in 82 patients with OSCC (OSCC group) and 25 healthy controls (control group) were examined via flow cytometry. The expression levels of soluble PD-1 (sPD-1) and soluble PD-L1 (sPD-L1) in the serum were observed through enzyme-link immunology method. The data were tested and analyzed with SPSS 17.0 software.
RESULTSThe percentage of CD8+ T cells in the OSCC group was significantly higher than that in the control group (P<0.05), whereas the percentages of CD3+ and CD4+ T cells as well as CD4+/CD8+ ratio were significantly lower than those in the control group (P<0.05). The positive rates of PD-1 and PD-L1 in CD3+ and CD4+ T cells in OSCC peripheral blood were remarkably higher than those in the control group (P<0.01). Difference was not observed between the expression levels of sPD-1 in the serum of OSCC group and those in the control group (P>0.05), but the average of sPD-L1 was remarkably higher than that in the control group (P<0.05). sPD-L1 expression was related to clinical stage, tumor cell differentiation, and lymph node status (P<0.05) but not related to sex, age, tumor location, and tumor size.
CONCLUSIONT-lymphocyte subpopulations in the peripheral blood of patients with OSCC developed immunosuppression with different degrees. PD-1 and PD-L1 expression levels on the surface of CD3+ and CD4+ T cells significantly increased. Abnormal increase in sPD-L1 expression may be associated with OSCC development.
