3.Role of regulatory B cells in children with newly diagnosed immune thrombocytopenia

Objective To investigate the role of regulatory B cells (Breg)in children with newly diagnosed immune thrombocytopenia (ITP).Methods A total of 35 newly diagnosed ITP children admitted to the Pediatric De-partment,the First Affiliated Hospital of Zhengzhou University from January to December 201 4 were recruited in this study,and another 20 gender -and age -matched healthy children from the Department of Medical Examination Center of the same Hospital were recruited as controls during the same period.Peripheral blood samples (3 mL from each chil-dren)were collected from all the newly diagnosed ITP children and the normal controls.Breg cells were tested by Flow Cytometry,and the expression levels of interleukin -1 0 (IL -1 0)and transforming growth factor -β1 (TGF -β1 ) mRNA were measured by real time fluorescence quantitative polymerase chain reaction.Meanwhile,the correlation be-tween Breg cells and the expression levels of IL -1 0,TGF -β1 mRNA were analyzed by Pearson correlation.Results The percentages of Breg cells in the peripheral blood of the newly diagnosed ITP children [(2.37 ±0.67)%]were sig-nificantly lower than those of the normal controls [(4.92 ±1 .32)%],and there was a significant difference (t =-7.47,P =0.000);the expression levels of IL -1 0 mRNA in the newly diagnosed ITP children(0.202 ±0.059) were significantly decreased compared with those of the normal controls(0.41 5 ±0.21 2),and there was a significant difference(t =-5.1 75,P =0.000);while the expression levels of TGF -β1 mRNA in the newly diagnosed ITP chil-dren(1 .587 ±0.823)were significantly increased than those in the normal controls(0.61 9 ±0.322),and there was a significant difference(t =4.081 ,P =0.001 ).There was a significant positive correlation between Breg cells and the ex-pression levels of IL -1 0 mRNA(rs =0.828,P <0.05),but no correlation between Breg cells and expression level of TGF -β1 mRNA was found (rs =0.527,P =0.1 1 7).Conclusions The decrease expressions of Breg cells can be found in the newly diagnosed ITP children,and the abnormal expression of Breg cells may play a key role in the immu-nological pathogenesis of the newly diagnosed ITP children.
4.The effects of the neotype hand-arm sling on shoulder subluxation and transferring movement in hemiplegic stroke patients

Objective To explore the effects of the neotype hand-arm sling on shoulder subluxation and transferring movement in hemiplegic stroke patients.Methods Thirty hemiplegic stroke survivors with shoulder subluxation and poor elbow flexor muscle tone (<2 on the Modified Ashworth Scale).were selected and randomly divided into three groups.Group 1 was trained in static upright standing,plus 10 min of sit-to-stand training and 20 min of walking training with no support for the hemiplegic,subluxed shoulder.Group 2 did the same training using a bagtype shoulder sling,and group 3 used the neotype hand-arm sling.Index finger palpation was used to evaluate the degree of shoulder subluxation,and the Five Times Sit-to-Stand (FTSST) and Timed Up-and-Go (TUG) times for the three groups were compared.Results In the static standing training,both the bag-type sling and the neotype hand-arm sling could effectively reduce shoulder subluxation.However,after 10 min of sit-to-stand training the neotype hand-arm sling was found to be significantly superior to the bag-type shoulder sling in improving shoulder subluxation,with the latter having no effect on shoulder subluxation.In the 20 rin of walking training,the neotype hand-arm sling provided significantly more effective support than the bag-type shoulder sling,although both could support the shoulders.Those using the neotype sling had shorter FTSST times,though there were no significant differences in the average TUG times.Conclusions The neotype slings can more effectively counter shoulder subluxation in upright stance,especially during sit-to-stand maneuvers and walking.It might have positive effects on transferring ability.
5.Meta-analysis of laparoscopic versus open partial nephrectomy

Objective To provide Meta-analysis evidence of laparoscopic partial nephrectomy (LPN) vs open partial nephrectomy (OPN) in assisting clinical decision making.Methods By searching CHKD,PUBMED,Wanfang and VIP database self-built library up to June 30,2012,both Chinese and English literatures of LPN and OPN efficacy in controlled study were included with strict exclusion criteria by two independent screenings of the literature.Data extraction and quality assessment were done by using the RevMan 5.1 META analysis software.Results A total of four English and six Chinese literature were included in this Meta-analysis.There were 1636 cases of partial nephrectomies.Of these patients,794 cases were treated with LPN,842 cases were treated with OPN.Meta analysis results showed that:in terms of operative time (SMD =0.10,95% CI-O.40-0.59,P =0.70),surgical complication (OR =1.03,95% CI0.73-1.44,P =0.88),positive surgical margin (OR =1.64,95 % CI0.83-3.23,P =0.16),warm ischemia time (SMD =1.07,95% CI-0.02-2.16,P =0.05),postoperative tumor recurrence (OR =0.58,95% CI 0.26-1.30,P =0.18),there was no significant difference.But in terms of intraoperative blood loss (SMD=-1.08,95%CI-1.57--0.59,P<0.01),postoperative hospital stay (SMD=-0.81,95%CI-0.97--0.65,P <0.01),the differences were significant in favor of LPN.Conclusion Comparing with OPN,LPN has advantages in intraoperative blood loss and post-operative hospital stay,no obvious advantages in operative time,surgical complications,positive surgical margin,warm ischemia time and tumor recurrence.

7.The myocardial protection of coen zyme compel x in patients with VR

Objective To evaluate effection of the myocardial protective in treatment of coenzyme complex to the VR patients .Methods Eighty patients who need to VR were divided into control group ( n=40) and coenzyme complex group ( n =40) with double -blind.The automatic recovery rate of heart beats after cardiac resuscitation , the changes of postoperative arrhythmia rate , postoperative serum CK-MB, cTnI in 12h in two groups were recorded .Results The automatic recovery rate of heart beats was higher in coenzyme complex group than that in control group , (85%vs 67%, P <0.05).The changes of postopera-tive arrhythmia rate (26%vs 38%), serum CK-MB, cTnI at clamp off of aorta, end of operation,6hour of postoperation and 12 hour of postoperation were lower in coenzyme complex group than those in control group respectively ( all P <0.05 ) .Conclusion Coenzyme complex shows positive protection of myocardial in the VR patients.
8.A clinical study of Yangxueqingnaokeli in preventive treament of migraine

Objective To investigate the therapeutic efficacy and adverse-events of Yangxueqingnaokel i (main composition includes Chinese angelica, ligusticum wallichii, Paconia lac tiflora, Asarum sieboldii, etc) in patients with migraine. Methods One hundred and twelve patients with migraine with an average age of 38.1?9.8 years( ranging from 19~75) were investigated in a multicenter, doub le-blind, randomized, controlled and perspective study. Results (1) The total number an d total duration of migraine attacks were markedly decreased in therapeutic group of Yangxueqingnaokeli than those in placebo group, 3.1?2.5 and 4.2?2.7 ( Z=2.1 6, P=0.03); 31.0?48.0 and 51.2?73.8(Z=2.86, P=0.004), respe ctively. 9.Effects of bioactive extracts from Fructus Jujubae in attenuating the inflammation induced by Radix Kansui

Aim: To evaluate the effects of bioactive extracts from Fructus Jujubae in attenuating the inflammation induced by Radix Kansui. Methods: Exoteric splenic lymphocyte ( SPL) of mouse and peritoneal macrophage ( PMψ) of rat were used as cell model to evaluate the anti-inflammatory effects of fractions from Fructus Jujubae. MTT method was used to assay SPL proliferation, and Griess method was used in NO release of PMψ. Firstly, Radix Kansui fraction was applied to induce inflammatory effects of the cells, and then, the anti-inflammatory effects of five extracts of Fructus Jujubae were investigated to determine the active fractions in Fructus Jujubae, which probably contribute to the attenuation of Radix Kansui. The effects of Fructus Jujubae extract in interfering the gastrointestinal acute damages caused by the extract of Radix Kansui were observed according to the pathological sections of mice. Results: The extract RK-3 ( diterpenes) exhibited the most significant pro-inflammatory effect. The bioactive extracts B (micromolecule saccharides and amino acids), D(flavonoid glycosides), and E (triterpenes) of Fructus Jujubae could attenuate the action of RK-3. The effective extract of Fructus Jujubae protected stomach and duodenum of mouse from acute damages caused by the extract of Radix Kansui. Conclusion: Fraction RK-3 (diterpenes) of Radix Kansui had pro-inflammatory effects, and some extracts of Fructus Jujubae could attenuate this effect of Radix Kansui. The results helped to clarify that Fructus Jujubae attenuates the toxic effect of Radix Kansui in certain cases, and the bioactive extracts of Fructus Jujubae discovered in the experiments offer the basis for further research to the active components of Fructus Jujubae.
10.Research on in vivo kinematics characteristics of the spine in adolescent idiopathic scoliosis patients based on 3D image volume fusion technique

Objective To characterize the spinal motion of 6 degrees of freedom (6DOF) under the fulcrum bending in Lenke type 1 adolescent idiopathic scoliosis (AIS) patients using 3D image volume fusion technique.Methods Ten volunteers with Lenke type 1 AIS were examined by the Gemstone Spectral Imaging to obtain their T1-S1 vertebral morphological data in the supine position and fulcrum bending position.With the computer software,images of 2 different positions were fused with the 3D image to obtain in vivo kinematics data of each vertebral body in the target area.Results In the apical vertebrae (AV) region,the main range of motion in the fulcrum bending position ranged from-2.57°± 0.95° to-1.37°± 0.65°,showing significant differences between segments (P < 0.05);along the vertical axis the coupling rotation ranged from-2.71° ± 0.48° to-1.27° ± 0.49°;along the coronal axis,the coupling flexion and extension ranged from 0.19° ± 0.85° to 1.51° ± 0.42°.In the upper end vertebrae (UEV) area,the main ROM ranged from-2.20° ± 1.13° to-1.91° ± 0.76°,showing no significant differences between the 3 segments (P > 0.05);along the vertical axis,coupling rotation ranged from 0.18°±0.47° to 1.35°±0.52°;along the coronal axis,coupling flexion and extension ranged from 1.62° ± 0.41° to 2.07° ± 0.37°.In the lower end vertebrae (LEV) region,the main movement ranged from-0.79°± 0.63° to 0.73°± 1.09°,showing significant differences between the 3 segments (P < 0.05);along the vertical axis,the coupling rotation ranged from-1.62°± 1.05° to-0.60°± 0.60°;along the coronal axis,the coupling flexion extension ranged from -1.41° ± 0.87° to-0.75° ± 0.66°.Conclusionts In the fulcrum bending position,Lenke type 1 AIS shows a unique motion and coupling mechanism in the spine.The coupling rotational movement in the UEV region is at the same side as the lateral flexion movement.The coupling rotational motion in the AV and LEV regions is opposite the lateral flexion movement.
