2.The therapeutic value of endoscopic band ligation in gastric stromal tumors

Objective To evaluate the therapeutic value of endoscopic band ligation in gastric stromal tumors.Methods Twenty-nine patients with small gastric stromal tumors(diameter<12mm)underwent endoscopic band ligation,and were followed up with routine endoscopy and EUS.Results Twentyeight patients recovered completely,with the surface healing up 4-6 weeks(average 4.8 weeks)thereafter.The neoplasm did not slough off in one patient,so he underwent a second ligation.Hemorrhage occurred in one patient three days after ligation,but was successfully managed with hemoclips.Other patients showed no complications.All the patients were followed up for 36 to 51 months,and there was one case of recurrence.Conclusion Endoscopic band ligation is an effective and safe treatment for small GISTs of less than 12 mm.

The Illustrations of Moxibustion, a remained book in Dunhuang Caves, is now available the earliest monograph of moxibustion methods since having writing in our country. In this paper, the points selected and therapeutic methods in The Illustrations of Moxibustion are used for treatment of typical cases of cerebral palsy, rheumatoid arthritis, diarrhea due to gastroenteritis, impotence, premature, diabetes, asthma and so on to explore characteristics of using points, find out the law, test and verify the actual effect and find guiding values for modern clinic, indicating that The Illustrations of Moxibustion can be applied for treatment and rehabilitation of many kinds of refractory diseases with unique therapeutic effects and practical significance.

5.Observation of epithelial herpes simplex keratitis treated by combination of sparse wind clearing heat method and western medicine

AIM: To observe the clinical curative effect of sparse wind clearing heat method combined with western medicine treatment on epithelial herpes simplex keratitis ( HSK) .
METHODS: A total of 75 cases ( 75 eyes ) of patients with epithelial HSK were divided into the treatment group 38 cases ( 38 eyes ) and the control group 37 cases ( 37 eyes) . The patients of the control group were treated with ganciclovir drop and symptomatic treatment. On the basis of the above treatment plan, the patients of the treatment group were treated with sparse wind clearing heat decoction. Clinical curative effect was observed and analyzed.
RESULTS:In the treatment group, curative rate was 79% and effective rate was 95%. In the control group, curative rate was 54% and effective rate was 78%. The differences had significant difference between the two groups (P<0. 05). The corneal lesion healing index in the treatment group was better than that in the control group (P<0. 05). At 6mo follow-up the recurrence rate was 10%in the treatment group, and the control group with 40%, and there was significant difference between the two groups ( P < 0. 05 ). The average healing time of the treatment group was 14. 74±8. 58d, and the control group was 19. 68 ± 8. 71d. There was significant difference between the two groups (P<0. 05).
CONCLUSION: Sparse wind clearing heat method combined with western medicine treatment on epithelial HSK has preferably curative effect.
6.Feasibility of combining anterior and posterior fontanelle acoustic windows for median sagittal plane in fetal cranial ultrasound scanning

Objective To explore the feasibility of combining anterior and posterior fontanelle acoustic windows for fetal median sagittal plane cranial scanning in prenatal ultrasound.Methods From July 5 to August 25 in 2012,200 pregnant women in Huai'an First Hospital at 22 to 24 gestational weeks accepted transabdominal uhrasonography,with anterior fontanelle,longitudinal suture and posterior fontanelle as acoustic windows to obtain the median sagittal plane.In the mean time,ten aborted or induced fetuses (with malformations or maternal complications) underwent cranial ultrasound with the same acoustic windows.The successful rates of obtaining median sagittal plane were calculated.Data were analyzed by Chi-square test.Results (1) Among the ten aborted or induced fetuses,all median sagittal planes were successfully obtained through anterior,posterior fontanelle,or longitudinal suture alone.(2) Among the 200 cases of prenatal uhrasonography,the successful rate of obtaining median sagittal plane through posterior fontanelle alone was 76.0%(152/200),which was higher than that through anterior fontanelle alone [59.5% (119/200),x2=12.5,P<0.01].The successful rate was 95.5% (191/200) when combining the anterior and posterior fontanelle,which was higher than that through anterior fontanelle or posterior fontanelle alone (x2 =74.3 and 31.1,both P<0.01).The successful rate of obtaining median sagittal plane through posterior fontanelle was 67.3% (65/102) in cephalic presentation,which was also higher than that through anterior fontanelle [37.3% (38/102),x2 =14.3,P<0.01].The successful rate was 88.8% (87/98) through posterior fontanelle in non-cephalic presentation,which was similar to that through anterior fontanelle [82.7%(81/98),x2 =1.5,P>0.05].When both anterior and posterior fontanelle applied,the successful rate in cephalic presentation was 9.8% (10/102),lower than that in non-cephalic presentation fetuses [71.4% (70/98),x2 =79.1,P<0.01].Conclusions The successful rate in obtaining median sagittal plane through posterior fontanelle is higher than that through anterior fontanelle,and might be increased when both anterior and posterior fontanelle were used.

8.Imaging diagnostic features of adrenal injury

Objective To discuss the imaging diagnostic features of adrenal injury. Methods The imaging features of the 29 patients of adrenal bruise and hernatoma (20 male and 9 females, average age 37) were retrospectively analyzed. The clinical appearances were all flank and hack pain, local sensitive to percus-sion and associated injury appearance. Among the 29 cases, 25 cases(86%) had adrenal injuries on right side, 2 cases(7%) on left side, and 2 cases(7%) on both sides, and no apparent abnormality was found in the relevant endocrine examination after injury. CT (n=29), MRI (n=5) and ultrasonography (n=6) were checked. CT follow-up were taken in 23 eases. MRI (n=1) and ultrasonography (n=l) were followed as well. Results The first-time exam coincidences of CT, MRI and sonography were 28/29 (97%), 5/5 (100%) and 3/6 (50%) respectively. One case of simple right-side adrenal hematoma 3 weeks after injury wasn't clearly diagnosed by CT, which was later diagnosed by MRI. The CT features of adrenal bruise were local or diffuse intumescence and focus high-density hemorrhage shadow. The CT appearances of acute stage adrenal hematoma were round-like high-density shadow without enhancement and the diameters were 1-3 cm. MRI appearances of 5 cases of subacute and chronic phase hematoma were typical high signal of T1WI, T2WI and DWI and toroid low signal around T2WI. Hematorna was not be enhanced when CT or MRI en-hancement scanning, and formed characteristic "nut-like" image feature with toroid high-density or high sig-nal enhanced shadow forming around. Uhrasonography appearances of 3 cases of hematoma were abnormal shadow of the adrenal gland. Conclusions CT is the prior imaging method for adrenal bruise and hemato-ma. MRI has the characteristic appearance for the few cases which are difficult to be identified by CT and ul-trasonography. Characteristic "nut-like" image feature is helpful for the diagnosis and differential diagnosis.
9.Expressions and clinical significance of putative hepatic stem cell biomarkers associated with tumour angiogenesis

Objective To investigate the expression of liver cancer stem cells(LCSCs) biomarkers and tumour angiogenesis in patients with hepatocellular carcinoma (HCC) and its clinical significance.Methods Immunohistochemistry was used to detect the expression of CD90,CD133 and CD34 in tumor tissues from 61 patients with HCC undergoing curative hepatectomy.Results The positive rates of CD90and CD133 in HCC tissues were 29.5% (18/61)and 31.1% (19/61),the positive rates of CD90 and/or CD133 in HCC were 45.9% (28/61).The positive rate of LCSCs biomarkers in patients with vascular invasion,tumor differentiation Ⅲ/Ⅳ,non-complete encapsulation,and TNM stage Ⅲ/Ⅳ was significantly higher than those with non-vascular invasion,tumor differentiation Ⅰ / Ⅱ,with complete encapsulation,and TNM stage Ⅰ/Ⅱ.There were a significant positive correlation between LCSCs biomarkers expression and MVD(r =0.5,P < 0.05).As compared with negative LCSCs expression and MVD low group,patients with a positive LCSCs biomarkers expression and MVD high in tumours had significantly lower overall survival (OS) and poor relapse-free survival (RFS).Conclusions High expression levels of LCSCs biomarkers are related to markers of angiogenesis microvessel density (MVD).By combining two of those we can classify paitients and anticipate the prognosis after surgery.
10.Enhancing glutamate decarboxylase activity by site-directed mutagenesis: an insight from Ramachandran plot.

Glutamate decarboxylase (GAD) can catalyze the decarboxylation of glutamate into γ-aminobutyrate (GABA) and is the only enzyme of GABA biosynthesis. Improving GAD activity and thermostability will be helpful for the highly efficient biosynthesis of GABA. According to the Ramachandran plot information of GAD 1407 three-dimensional structure from Lactobacillus brevis CGMCC No. 1306, we identified the unstable site K413 as the mutation target, constructed the mutant GAD by site-directed mutagenesis and measured the thermostability and activity of the wide type and mutant GAD. Mutant K413A led to a remarkably slower inactivation rate, and its half-life at 50 °C reached 105 min which was 2.1-fold higher than the wild type GAD1407. Moreover, mutant K413I exhibited 1.6-fold higher activity in comparison with the wide type GAD1407, although it had little improvement in thermostability of GAD. Ramachandran plot can be considered as a potential approach to increase GAD thermostability and activity.
