1.Effect of patella resurfacing versus nonresurfacing in total knee arthroplasty on anterior knee pain

[Objective]To compare retrospectively the clinical outcome in total knee arthroplasty with and without patellar resurfacing.[Method]From October 1994 to October 2004,125 patients(148 knees)with osteoarthritis were enrolled in the clinical trial of total knee arthroplasty,patients received resurfacing(84 knees)or retention patellar(64 knees).Evaluation was done from preoperatively to final followed-up postoperatively.The patients were scored using American HSS Score for knee and the Feller Score for patalla.The data were analyzed using SPSS software.[Result]In resurfacing patella group,the preoperative and final follow-up HSS scores of patients were(39.6?9.8)and(90.9?8.2)points respectively,scores of patailar were(14.4?6.4)and(25.2?4.8),scores of anterior knee pain were(4.6?3.9)and(10.6?4.1).In nonresurfacing patella group,the preoperative and final follow-up HSS scores of patients were(38.8?9.8)and(90.2?8.9)points respectively,scores of patallar were(14.2?6.2)and(25.1?4.8),scores of anterior knee pain were(4.8?3.8)and(10.3?4.1).There were significant differences in both groups preoperatively and postoperatively in anterior knee pain score,but no significant difference between the two groups in anterior knee pain score.[Conclusion]Whether the patellar replaced or not,the most importance for avoiding anterior knee pain postoperatively is the reasonable selection of patient and correct operation.
2.Therapeutic effect of Qiliqiangxin capsule combined on refractory heart failure

Objective To observe the therapeutic effect of Qiliqiangxin capsule combined on refractory heart failure.Methods 60 patients with refractory heart failure were randomly recruited into a control group and a therapeutic group. The control group received the conventional treatment, and the therapeutic group took Qiliqiangxin capsule on the base of the conventional treatment (3 times a day, 4 granules/each time) combined with 120 mg cAMP meglumine. Treatment in both group lasted 14 days. Results: After the treatment, the therapeutic group showed improvement in the symptoms of heart failure. Comparing with the control group, the difference was significant (P<0.05). Conclusion The therapeutic effect of Qiliqiangxin capsule combined on refractory heart failure is satisfied and can improve the quality obviously.
3.Anthropometric characteristics of growth and pubertal development in adolescent idiopathic scoliosis

Objective To investigate the pubertal growth patterns and anthropometric characteristics of the patients with adolescent idiopathic scoliosis(AIS) by anthropometry measurements. Methods The anthropometric data of 256 AIS girls were collected. The mean age of diagnosis was 14.1 years old(from 12 to 16 years old). The children with more than 10 degree of the scoliosis angle at coronal level were diagnozed as AIS. The control group consisted of 462 age-matched health girls. Four variables were measured: height, sitting height, arm span and menstrual states. Height and sitting height were adjusted using Bjure formula to make up the height loss from spinal deformity. The above variables were analyzed between AIS group and the controls. Results There were no differences in the composition of age between two groups. There were no significant differences in height,sitting height and arm span between the 256 cases of AIS girls and the 462 controls. However, the corrected height and sitting height were significantly higher in AIS than those of the controls. The menstrual onset time was earlier in AIS than that in controls. There were no statistical differences in corrected height, corrected sitting height and arm span between AIS girls and the controls at the age group of 12 years old. On the contrary, the AIS girls showed shorter uncorrected height than the controls did at this age. However, the scoliotic girls were found in the corrected height, the corrected sitting height and the arm span significantly taller than the controls between ages of 13 and 15 years. There were significant differences in corrected height and corrected sitting height between AIS girls and the controls at the age of 16 years old, while no difference were found in the variables of height, sitting height and arm span. Conclusion There is an abnormal growth pattern in AIS during pubertal growth spurt, which may be related to the pathogenesis of AIS.
4.Health security measures of food and drinking water during military training on the plateau

Objective To investigate the health security requirements on food and drinking water during military training on the plateau,and explore ways to improve military health security support capability.Methods Seven army units were randomly chosen.By means of on-site check and sample detection,both internal and external environmental factor changes and health and safety conditions of food and drinking water were surveyed on the plateau.Internal and external standard methods were used to test the stability of portable instruments.Results There was considerable difference in diurnal temperature and relative humidity between indoors and outdoors.In two field units,raw materials of foods were purchased in a centralized manner and suppliers were fixed,they were purchased independently in the othev five units. Facilities of food storage and transportation were lacking in all the seven units.In one of the fifty-three food samples, residues of pesticides exceeded the national standard.Among the three water samples,the level of turbidity,ammonia nitrogen and nitrate nitrogen was lower than the standard limits,but free residual chlorine and total chlorine were not detected.The plateau environment had no significant effect on spectrophotometers,but had significant influence on enzyme-linked immune detectors.Conclusion There are potential health hazards to training troops on the plateau that arise from foods and drinking water.Improvement of management and equipment adaptability is needed to further enhance the field health security efficacy.
5.Arthroscopic release for frozen shoulder: Report of 25 cases

Objective To evaluate the efficacy of arthroscopic capsular release of frozen shoulder.Methods Twenty-five patients with frozen shoulder were treated arthroscopically from October 2001 to October 2003 in this hospital.The operation was performed under general anesthesia.The arthoscope was inserted through a posterior approach and the radiofrequency electrotome was inserted via an anterior approach.The synovitis of the biceps tendon and the rotator interval was thermo-coagulated.The superior glenohumeral ligament,the middle glenohumeral ligament,and the intra-capsular part of subscapularis tendon were cut to release the capsule.The combination manipulation was performed to release the residual contracture.The range of motion of the shoulder and the American Shoulder & Elbow Surgeons(ASES) scores were recorded at 3 and 6 postoperative months,respectively.Results The operative time was 75~95 min(mean,85 min).No intraoperative complications were found.As compared with preoperative conditions,the motion of inward rotation revealed no significant improvement at 1 week after operation(?~2=8.558,P=0.073) and other shoulder motions were significantly improved at different postoperative stages(P0.05).Conclusions Arthroscopic release for the treatment of frozen shoulder may obviously shorten the period of the disease and obtain good curative effects.

8.Low frequency-based non-uniform sampling strategy to improve Chinese recognition in cochlear implant.

To enhance speech recognition, as well as Mandarin tone recognition in noice, we proposed a speech coding strategy called zero-crossing of fine structure in low frequency (LFFS) for cochlear implant based on low frequency non-uniform sampling (LFFS for short). In the range of frequency perceived boundary of human ear, we used zero-crossing time of the fine structure to generate the stimulus pulse sequences based on the frequency selection rule. Acoustic simulation results showed that although on quiet background the performance of LFFS was similar to continuous interleaved sampling (CIS), on the noise background the performance of LFFS in Chinese tones, words and sentences were significantly better than CIS. In addition to this, we also got better Mandarin recognition factors distribution by using the improved index distribution model. LFFS contains more tonal information which was able to effectively improve Mandarin recognition of the cochlear implant.

9.Effect of rofecoxib on expressions of VEGF and inducible NOS in human gastric carcinoma tissues established in nude mice by orthotopic transplantation

Objective To evaluate the inhibitory effect of rofecoxib in human gastric carcinoma tissues established in nude mice by orthotopic transplantation through the expressions of vascular endothelial growth factor (VEGF) and inducible nitric oxide synthase (iNOS). Methods After the models of human gastric carcinoma in nude mice by orthotopic transplantation were established, rofecoxib was administered intragastrically. The expressions of VEGF and iNOS were evaluated in the local tumors by Envision immunohistochemical method. 10.Clinical analysis of prognosis and recurrence of borderline epithelial ovarian tumors in aged women

Objective To evaluate the effect of clinic-pathological variables on prognosis and recurrence of the aged women suffered from borderline epithelial ovarian tumors. Methods The clinical and follow-up data of 102 cases were analyzed retrospectively. Univariate and multivariate logistic regression analysis were used to assess independent prognosis and recurrence factors.Results The postoperative accessory treatment, peritoneal implantation and lymph nodes metastasis could influence the 5-year survival rate(χ2=19.311, 11.054, 18. 559, all P<0.01). The surgery procedure, peritoneal implantation, lymph nodes metastasis and microinvasion could influence the recurrence rate(χ2=23.606, 23.583, 25.774, 23.583, all P<0.01). The logistic regression model showed that the postoperative accessory treatment and peritoneal implantation were the two independent prognosis factors (OR=0.060, 35.319, all P<0.01). The surgery procedure,peritoneal implantation and microinvasion were the three independent factors affecting recurrence(OR =0.001, P<0.05;OR=60.952, P<0.05;OR=88.874, P<0.01). Conclusions The early standard treatment on borderline epithelial ovarian tumors of the aged women have an excellent prognosis, the risk of recurrence remains in some patients. We emphasize that these patients with highrisk factors should be main surveillance population.
