1.Study on the Composite Graft with an Acellular Xeno-dermis and Cultured Keratinocyte Sheets

Objective To study a graft method of an acellular dermis composited with cultured human keratinocyte sheets,and investigate the outcomes of the composite graft on full-thickness wounds.Methods Using porcine skins,we prepared the acellular dermis and cultured human keratinocyte sheets in vitro.Athymic null mice were divided into the experimental group and the control group,the acellular xeno-dermis was implanted into the subcutaneous of the dorsal skin in experimental group,one week after the implantation the cultured keratinocyte sheets were grafted on the full-thickness defective area of the dorsal skin in the experimental group,in control group only implanted human keratinocyte sheet on the defective area wound,healing status was regular assessed,and biopsies for histological analysis and immunohistochemistry test were performed in post-operation.Results Compared with the control group,the appearance of wound coverage in the experimental group was better.Moreover,the histologic analysis revealed that fully differentiated epidermis,organized proliferated collagen fibers and significant reconstruction of epidermis-dermis junction were seen in the experimental group which lack of the acute immuno-rejection response.Conclusion As a kind of dermal substitutes,the acellular xeno-dermis can be compositely transplanted with cultured human keratinocyte sheet and can improve the quality of wound healing.
2.Magnifying endoscopy combined with FICE imaging in the diagnosis of early gastric cancer

ObjectiveTo evaluate magnifying endoscopy combined with flexible spectral imaging color enhancement(FICE)for diagnosis of early gastric cancer.MethodsThe suspicious small depressive lesions(SDL) was further studied with magnifying endoscopy or magnifying endoscopy combined with FICE from April 2009 to March 2011 in our hospital.The endoscopic diagnostic criteria of early gastric cancer was made according to the structure of the pit,the microvascular and the demarcation line.ResultsIn 74 SDL,17 were pathologically diagnosed as cancer,and 56 were noncancer.The sensitivity,specificity and accuracy of magnifying endoscopy were 41.18%,94.74%and 82.43%,respectively,which were 86.67%,96.49% and 91.89%,respectively for magnifying endoscopy combined with FICE.The diagnostic sensitivity of magnifying endoscopy combined with FICE was significantly higher than that of magnifying endoscopy (P <0.05 ).ConclusionMagnifying endoscopy combined with FICE can reveal the structure of the pit,microvascular and demarcation line more clearly.The endoscopic diagnostic criteria is a more comprehensive description of the mucosal micro-morphological features of early gastric cancer.
3.Current survey of readiness for hospital discharge of patients who had undergone spinal operation

Objective To investigate the readiness for hospital discharge of patients who had undergone spinal operation at West China Hospital, Sichuan University, to guide the medical care and health education. Methods A total of 164 patients after spinal operation who have readying to discharge were selected and investigated by General Information Questionnaire, Activity of Daily Life Scale and Readiness for Hospital Discharge Scale. Apply the Excel for data inputting, SPSS19.0 for statistical analysis and choose rate, x±s and so on for statistical description according to the types of data. Readiness for Hospital Discharge Scale was (142.38±34.23 points and the average score of all the articles was (7.12±1.72) points. Conclusions The readiness for hospital discharge score of patients who had undergone a spinal operation at West China Hospital, Sichuan University is low and their readiness for hospital discharge is inadequate. Medical staff can improve the patients′readiness for hospital discharge by strengthening health guidance in order to promote the recovery of patients.
4.Recent advances in the study of accelerated blood clearance phenomenon of PEGylated liposomes

It is generally believed that liposomes modified with polyethylene glycol (PEG) have no or lower immunogenicity. However, based on many recent literatures, when the PEGylated liposomes were repeatedly applied to the same animal, the immune responses occurred. The first injection of PEGylated liposomes resulted in a reduction in the circulation time and an increase in hepatic and splenic accumulation of the second dose of PEGylated liposomes in a time-interval, which was called "accelerated blood clearance (ABC)" phenomenon. Such immunogenicity of PEGylated liposomes presents a barrier in the research of liposomal formulations and their use in the clinics. This review focused on the definition, the method of verification, the development of the reason for ABC phenomenon, influencing factors of ABC phenomenon, and discussed if other PEGylated nanocarriers also induce ABC phenomenon.
5.Experimental training of medical genetics based on typical diseases

6.Modified spiral ileal orthotopic neobladder: Experience with 32 cases of invasive bladder cancer

Objective To assess the outcomes of modified spiral ileal orthotopic neobladder.Methods From January 1998 to January 2008, 32 patients (all male) underwent radical cystectomy and spiral ileal orthotopic substitution for muscle invasive bladder cancer. A segment of 40 to 45 cm ileal loop was isolated, detubularized, and reconfigured in spiral shape to form a pouch. Bilateral ureters were reimplanted by inserting the 1 cm distal segment into the pouch to form a Split-Cuff nipple.The bottom of the pouch was opened and anastomosed with the urethra (4 stitches). Results There were no perioperative deaths. The mean operative time was 281.2±48.7 min. Blood loss was 545.4±181.9 ml. Twenty cases required a blood transfusion, the mean volume of intraoprative blood transfusion was 430.8±235.9 ml. The average hospital stays were 26.8±9. 7 days. Rate of daytime continence was 93. 7% (30/32) while nighttime continence was 81.3% (26/32). Urodynamic studies were carried out in 23 cases 6 months after surgery. Neobladder capacity at first desire to urinate was 315.0± 33.4 ml(270-420 ml). The maximum neobladder capacity was 490. 3±39.7 ml(350-600 ml).The maximum flow rate (Qmax) was 16.5 ± 5.9 ml/s. Full resting pressure was 22. 5 ± 11.8 cm H2O. Peak voiding pressure was 78.3 ± 14.7 cm H2O. After mean 58. 4 months' follow up (range 22 to 132), 4 cases died of metastasis of bladder cancer. Conclusion Modified spiral ileal orthotopic neobladder is a reasonable option for treating invasive bladder cancer.
7.Distribution of drug resistance Acinetobacter Baumannii in surgical ICU and its prevention

Drug resistance of Acinetobacter baumannii ( Ab) in surgical ICU from January 2008 to December 2009 was investigated retrospectively. Total 114 clinical strains of Ab were isolated from surgical ICU and 92 strains were from respiratory tract (80.7% ). The prevalence rate of multiresistant Ab in 2009 was higher than that in 2008 (75. 7% vs 50. 0% , x2 = 7. 703, P = 0. 006). The results indicate that to monitor and control drug resistant of Ab constantly is important for the prevention of nosocomial infection.

Thyroid hormone insensitivity syndrome is an inherited syndrome characterized by high serum concentrations of thyroid hormone,accompanied by normal or slightly high serum TSH concentration due to the reduced responsiveness of target tissues to thyroid hormone.Thyroid hormone insensitivity syndromes are related to gene mutations of thyroid hormone receptors,thyroid hormone cell transport defect,and thyroid hormone metabolism defect.This article reviews the progress of pathogenesis,diagnosis,and therapeutics of thyroid hormone insensitivity syndrome.
9.Determination of Content of Nutmeg Lignan and Dehydrodiisoeugenol in Nutmeg by HPLC

Objective To develope a high performance liquid chromatograph ( HPLC ) method for simultaneous determination of nutmeg lignan and dehydrodiisoeugenol before and after processing of nutmeg. Methods The column was Diamonsil C18(250 mmí 4. 6 mm, 5 μm). The mobile phase was methanol-water in a gradient elution mode. The UV detection wave length was 274 nm. The column temperature was 25℃ . The flow rate was 1. 0 mL·min-1 . Results Nutmeg lignan and dehydrodiisoeugenol had a good linear correlation in ranges of 10. 24-61. 44 μg·mL-1(r=0. 999 6) and 3. 0-18. 0 μg·mL-1 (r=0.999 8), respectively. The average recovery rates were 97. 94% (RSD=2. 17%) and 97. 11% (RSD=2. 17%). Conclusion The method is simple, accurate, reproducible, and can be used for the simultaneous determination of nutmeg lignan and dehydrodiisoeugenol before and after the processing of nutmeg.
