Medical Journal of Chinese People's Liberation Army 2001;0(09):-
To evaluate percutaneous trans hepatic puncture and intraluminal bleomycin injection for hepatic hemangioma, 20 patients accepted the therapy. With local anesthesia, a PTC needle was punctured into the lumen of hepatic hemangioma, and the bleomycin was injected. One time a week, and three times made a course of treatment. The total curative ratio was 100%. Fever and distension of the abdomen occurred in 25% of the patients. It is concluded that the method is a mini traumatic and effective therapy for hepatic hemangioma.

Problem-based learning is a student-centered teaching mode, which is based on creating question circumstances. In our clinical teaching,we put up questions based on real cases and inspire students'enthusiasm, activity and creativity. The results show the PBL can elevate students' clinical ability and also can promote teachers'capability.

Objective To investigate the effects of ketamine combined with electroconvulsive shock (ECS) on inflammation and amyloid-beta peptide in hippocampus of depressive rats. Methods Chronic unpredictable mild stress (CUMS) was used to generate animal models of depression.Forty-eight adult male Sprague-Dawley rats were randomly divided into 4 groups (n=12):depression model group (group D),electroconvulsive shock group (group DE),ketamine combined with electroconvulsive shock group (group DKE),and ketamine group (group DK).Rats in group D received sham ECS treatment;rats in group DE received ECS treatment;rats in group DKE were given intraper-itoneal injection of ketamine (100 mg/kg) and then received ECS treatment;rats in group DK were given intraperitoneal injection of ketamine (100 mg/kg) and then received sham ECS treatment.Morris water maze was used to assess the memory abilities of rats. The expression levels of IL-1β and TNF-α were measured by real-time PCR.Enzyme-linked immunosorbent assays were used to detect the levels of soluble Aβ.Results Before the administration of ECS or ketamine treatment, there was no significant difference in the escape latencies and space exploration time between the 4 groups (P>0.05).After the ECS and ketamine treatment, rats of group DKE exhibited a shorter escape latencies and a longer space exploration time, and the expression of IL-1β and TNF-α mRNA were down-regulated while the concentration of Aβ1-40 and Aβ1-42 were increased compared with group DE with significant difference (P<0.05).Conclusion Ketamine can alleviate ECS-induced learning and memory impairments in depressive rats. This cognition-protecting effect of ketamine may be attributed to its suppression of ECS-induced neuroinflammation and decrease of the levels of soluble Aβ in the hippocampus of depressive rats.

Objective To provide an accessorial reference platform in purchasing medical equipment for the City. Methods The Medical Equipment Selection System(MESS) was designed and developed by combining the medical equipment selection model and selection way with delphi7 based on Windows XP, then a selection open-platform of human-computer interaction was formed. Results The tested index data in which five types of semi -automatic biochemistry analyzer, including SBA-830, SBA-610, CA-958E, AVE-852, Microlab 300, these can prove the system running well and have strong objective and scientific. Conclusion The platform has good stability and sensitivity, which can provide an accessorial decision-making platform for purchasing medical equipment.

Objective To summary the experience of AngioJet mechanical thrombectomy for the treatment of acute deep venous thrombosis (DVT) of lower extremity. Methods Clinical data of 28 patients suffering from acute DVT of lower extremities treated by AngioJet mechanical thrombectomy were analyzed retrospectively from October 2013 to February 2015.Venous recanalization was graded by a thrombus score based on pre-and post-treatment venography.Follow-up was performed by Doppler ultrasound and clinical evaluation. Results Twelve patients were planted temporary vena cava filters. The average length of hospitalization was (3.9 ± 1.1) days. The average operation time was (1.2 ± 0.4) hours. The dosage of urokinase was (320 ± 120) × 104 U and heparin was (46 ± 10)mg during surgery. The average reduction of hemoglobin was (7.5 ± 2.6) g/L.Two patients (Grade Ⅰ thrombolysis) were converted to catheter-directed thrombolysis (CDT).There were 10 (35.7%) cases achieving grade Ⅱ and 16 (57.1%) cases achieving grade Ⅲ,respectively.Technique success rate were 92.9%.Minor bleeding events occurred in 2 patients.Nine (32.1%) patients had stents implanted. One year follow-up found patent deep venous in 26 (92.9%) patients and stent patency rate was 88.9% (1/9).Two (7.1%) cases developed mild postthrombotic syndrome (PTS).Conclusion AngioJet is a safe and effective tool to treat acute DVT accelerating thrombolytic therapy, and reducing bleeding complications.

Objective To investigate the value of ultrasound integrated backscatter (IBS) in quantitative analysis of in-traluminal thrombus in abdominal aortic aneurysm (AAA).Methods IBS of intraluminal thrombus in 29 patients were measured, including All (average image intensity) and SDI (standard deviation of image intensity).Meanwhile, different echoes and characteristics of IBS in thrombus were compared and pathologic analysis was performed. Results ①Individual All value of homogenous thrombus differed remarkably,SDI value was low. There was no significant difference about All in homogeneous group and adjacent cavity group (P>0.05).All and SDI value in adjacent wall group were higher than those in other groups (P<0.01).②According to pathologic analysis, cellulose contents were abundant in adjacent wall group, but fewer in adjacent cavity group and homogeneous group. Conclusion IBS might be regarded as a sensitive and specific method and a quantitative basis for estimating and predicting the rupture risk of AAA.

Objective:To study the changes of platelet and blood coagulating function during endovascular graft exclusion(EVGE), providing reference for reasonable use of heparin and platelet. Methods:Using sonoclot analysis (SCT), 20 patients accepted EVGE were measured for ACT, clot rate, platelet function and hematocrit (HCT) and platelet count (PLT) after anesthesia induction(T 1), heparination(0.3 0.5 mg/kg)(T 2) and EVGE(T 3), respectively. The reasons for variability were analyzed. Results:ACT, clot rate and blood platelet function were normal at T 1. At T 2 ACT was prolonged [(289? 61.1) s,] clot rate and platelet function were decreased ( P
