1.Effects of electroacupuncture on uterine prostaglandin F2α, cyclooxygenase 2 and nuclear factor κB in rats with primary dysmenorrhea

Objective: To observe the effects of electroacupuncture (EA) on uterine prostaglandin F2α (PGF2α), cyclooxygenase 2 (COX-2) and nuclear factor κB (NF-κB) in rats with primary dysmenorrhea (PD) and to discuss the possible mechanism in EA intervening PD. Methods: Forty Sprague-Dawley female rats were randomly divided into a blank group, a model group, an EA group and an ibuprofen group, with 10 rats in each group. The PD model was established using estradiol benzoate combined with oxytocin in the model group, EA group and ibuprofen group. At the same time of modeling, rats in the EA group were given EA at Guanyuan (CV 4) and Sanyinjiao (SP 6) once a day for 20 min each time for 10 consecutive days. Ibuprofen was intragastrically administered once a day for 10 consecutive days in the ibuprofen group. The same amount of normal saline was intragastrically administered once a day for 10 consecutive days in the blank group and model group. The number of writhing of rats in each group within 30 min was compared on the 11th day just after the interventions. The uterine homogenate supernatant was separated and the PGF2α level was detected by enzyme-linked immunosorbent assay. Western blot was applied for the detection of the expression levels of COX-2, phospho-NF-κB p65 and NF-κB p65 proteins in uterine tissues. Results: Compared with the blank group, the number of writhing in the model group increased significantly (P<0.01), and the expression levels of PGF2α, COX-2, phospho-NF-κB p65 and NF-κB p65 proteins in uterine tissues were significantly increased (all P<0.01). Compared with the model group, the number of writhing in the EA group and ibuprofen group were significantly reduced (both P<0.01), and the expression levels of PGF2α and COX-2 protein in uterine tissues were significantly reduced (both P<0.01). Compared with the model group, the phospho-NF-κB p65 level in uterine tissues in the EA group was significantly reduced (P<0.01). Compared with the ibuprofen group, the phospho-NF-κB p65 level in the EA group was significantly reduced (P<0.01). Conclusion: The mechanism of EA for PD rats may be related to inhibiting the phosphorylation of NF-κB and reducing the levels of COX-2 and PGF2α in uterine tissues.
2.Survey of GPI-PLD Gene Polymorphisms in Healthy Persons and Patients With Systemic Lupus Erythematosus

Objective To analyze the polymorphisms of glycosylphosphatidylinositol-specific phospholipase D (GPI-PLD) gene Exon1 and Exon25,GPI-PLD activity of leucocyte in peripheral blood and their relationship in systemic lupus erythematosus patients of Han nationality in Hunan Chinese.Methods The polymorphisms were analyzed by PCR-SSCP and sequencing. GPI-PLD levels were determined by using glycosylphosphatidylinositol (GPI) anchored placental alkaline phosphatase (PLAP) as substrate and triton-X114 partioning.Results There were fourteen variations in the coverage of GPI-PLD gene Exon 1 of both 109 healthy persons' and 62 systemic lupus erythematosus patients',including seven variations at transcriptional start site upstream and seven variations at Exon1. The codons of variation were 14TTG→AAG (Leu14Lys),17CTC→GTC (Leu17Val),20AGA→AAA (Arg20Lys),21GGT→GGG (Gly21Gly),30GTA→ATA (Val30Ile),which leaded to four missense mutations and one synonymous mutation. In addition, the frequency of the variation at transcriptional start site upstream -24 C→T was significantly different between healthy control and systemic lupus erythematosus patients. At meanwhile, there was a variation in Exon25, the +60496 G→A, between two groups. The total various frequency, which was determined by SSCP, of healthy person was 33 03 percent and systemic lupus erythematosus patient was 25 81 percent. On the basis of the percentage of GPI-anchored PLAP conversion, the leucocyte GPI-PLD activities of total 62 systemic lupus erythematosus patients were measured. As compared with 109 healthy persons as control group, the leukocyte GPI-PLD activities of systemic lupus erythematosus patients were significantly raised, but GPI-PLD activities were not correlated with polymorphism sites significantly.Conclusions The leukocyte GPI-PLD gene in peripheral blood, which belongs to healthy persons and systemic lupus erythematosus patients of Han mationality in Hunan Chinses,is polymorphism.The frequency of transcriptional start site upstream -24 is significantly different between healthy control and systemic lupus erythematosus patients.The leukocyte GPI-PLD activeties of patients' also significantly increase.
3.Comparison of tensile properties of palmar or dorsal placement of stitches in repairing the tendons across the joints

Objective This study was designed to evaluate the biomechanical properties in tendons across the joints when Palmar or dorsal stitches were used. Methods Twenty porcine tendons were transected and then repaired in zone Ⅱand divided into two groups,using modified Kessler by placing the stitches in volar or dorsal part of tendons.The repaired tendons were placed on a joint model with flexion of 60?. An instron tensile testing machine was used to investigate 2 mm gap formation force and ultimate tensile strength. Results 2 mm gap formation force and ultimate tensile strength were both higher when dorsal placement of stitches was used. Conclusions Dorsal placement of stitches was recommended for repairing the tendons across the joints because it can offer greater tensile strength.
4.Ralationship between hepatocellular apoptosis and glycogen contents during cold preservation-reperfusion of liver and its mechanism

Objective To study relationship between hepatocellular apoptosis and glycogen contents during cold preservation-reperfusion of liver and its mechanism. Methods Rat liver models with different contents of glycogen were established and the rats were divided into 4 groups (A, B, C and D). During cold reservation-reperfusion of liver, the changes in hepatocellular apoptosis and tissue SOD, GSH and MDA were observed in the groups. Results After cold preservation for 9 h in each group, 60?min reperfusion could result in obvious apoptosis in livers. There was difference in the numbers of apoptotic hepatocytes among all the groups. At the same time, there also were significant difference in the levels of SOD, GSH and MDA in liver tissue among the four groups.Conclusion Intracellular abundant glycogen may significantly depress hepatocellular apoptosis during hepatic cold preservation-reperfusion by decressing oxygen free radicals.
5.An comparative study of single-level anterior decompression and fusion with and without cervical plating

Objective To discuss the necessity of cervical spinal plating after single-level anterior decompression and fusion on mono-segmental cervical disc herniation and cervical spondylosis without obvious kyphotic deformity and segmental lability. Methods The ranges of motion of the two groups with and without plating were tested under the load of 2.0 Nm on cervical spine specimens taken from 16 fresh cadavers. Finite element method was used to calculate the strength of bony callus and the average stress of the parts adjacent to the fused segments at various time intervals. 45 cases after single-level anterior decompression and fusion with and without cervical plating were followed-up at least 3 years, and 33 ones without cervical plating, more than 10 years. Objective To investigate the effect of intracellular glycogen on mitochondrial respiratory function of the ischemic reperfusion hepatocytes and its values.Methods During liver ischemia-reperfusion in the 21 rabbits,the tissue ATP content,hepatic enzymological change,respiratory control ratio(RCR) and P/O ratio of hepatocellular mitonchondria were observed in liver glycogen content significantly different three groups.Results Rabbit liver tissue with the higher content of glycogen has shown the higher mitonchondrial ATP content,also showed the higher RCR and P/O ratio of hepatocellular mitonchondria and the slighter injury of liver function.Conclusion Intracellular abundant glycogen may significantly improve respiratory functon of mitochondria,which can play an important role in keeping the hepatocellular function well during liver ischemia-reperfusion.
7.Analysis of mortality rate and death causes for diabetic inpatients in West China Hospital during 1996-2004

Objective To understand the variation of the motality and death causes for diabetic inpatients in West China Hospital(WCH) during 1996~2004. Methods First,the amount of diabetic inpatients(14670) and died persons (9597)were got each year by computer search system of our hospital from 1996 to 2004,and the case code of died diabetic subjects (842)were obtained.Then,those medical records were investigated and reviewed in details to get useful information.The motality,death causes and its constituent ratio of dead diabetic inpatients were analysed. Data input and analysis were conducted using the tool Epidate3.0,packages SPSS13 and SAS8.0. Results There were totally 14760 inpatients and 842 deaths who were suffered from DM during 1996-2004.The mortality rate was 5.74% and the standardized mortality rate was 5.37%.Tumor was the first cause of deaths (22.3%),followed by infection(15%),fatal cardiovascular events(14.4%),cerebrovascular events(12.3%).Diabetic nephropathy(DN)(6.3%) took the 6th place just after COPD(8%).There were 9269 deaths in the hospital in total(≥15 years old) during 1996-2004 while DM patients contributed 9.08% to them.The proportion of diabetic patients among the total deaths kept increasing every year(χ2=71.9070,P<0.01). Conclusions The mortality proportion of diabetic inpatients among the total deaths in West China Hospital from 1996 to 2004 is increasing every year.It needs to pay more attention to the main death cause of tumor in diabetic patients
8.Clinical effects of subcutaneous tunnel cholecystocholangiostomy in the treatment of hepatocholangiolithiasis and hepatic portal bile duct stenosis

Objective To summarize the clinical effects of cholecystocholangiostomy (STHG) in the treatment of hepatocholangiolithiasis and hepatic portal bile duct stenosis. Methods The early complications, clinical effects, and functions of gallbladder in a total of 135 patients, admitted since 1994 were for STHG were retrospectively analyzed. Results No early complications were observed after the operation. The contraction and concenration function of the gallblader remained basically normal. The color of gallbladder mucosa was normal,and no edema or erosion was found under choledochoscopy. Conclusions This operation not only maintained the normal physiological functions of the gallbladder and Oddi sphincter, but also the physiological flow of the bile, farthermere it prevented reflux of intestinal juice and reflax chdangitis. Because the anastomotic stoma was large, concentrated bile within the gallbladder could freely get access to the intrahepatic biliary tract, the concentration of bile acid in the intrahepatic bile duct was markedly elevated, so that the solubility of bilirubin crystal, was increased and recurrence of calculus formation eould be retarded. There fore, STHG should be considered to be an optimal operation in dealing with hepatocholangiolithiasis and biliary stenosis.
9.Study on preventive health care service in Traditional Chinese Medicine and policies under the concept of disease prevention theory

The concept of “disease prevention theory” contains a wealth of preventive medicine thought. Pre-ventive health care service in Traditional Chinese Medicine( TCM) is an important measure for enhancing health qual-ity and improving the quality of living. This contributes to the building of a harmonious society and suits the changing medical model. Despite a series of achievements, there are also some problems in preventative health care service. Based on an analysis of influencing factors on policy implementation, the problems are discussed in the paper. The study provides some suggestions on preventive health care services, including the perfection of operations and man-agement mechanisms, the improvement of awareness on preventive health care service, the strengthening of the talent team, an increase in funding, and an evaluation of policy implementation.
10.Modified spiral ileal orthotopic neobladder: Experience with 32 cases of invasive bladder cancer

Objective To assess the outcomes of modified spiral ileal orthotopic neobladder.Methods From January 1998 to January 2008, 32 patients (all male) underwent radical cystectomy and spiral ileal orthotopic substitution for muscle invasive bladder cancer. A segment of 40 to 45 cm ileal loop was isolated, detubularized, and reconfigured in spiral shape to form a pouch. Bilateral ureters were reimplanted by inserting the 1 cm distal segment into the pouch to form a Split-Cuff nipple.The bottom of the pouch was opened and anastomosed with the urethra (4 stitches). Results There were no perioperative deaths. The mean operative time was 281.2±48.7 min. Blood loss was 545.4±181.9 ml. Twenty cases required a blood transfusion, the mean volume of intraoprative blood transfusion was 430.8±235.9 ml. The average hospital stays were 26.8±9. 7 days. Rate of daytime continence was 93. 7% (30/32) while nighttime continence was 81.3% (26/32). Urodynamic studies were carried out in 23 cases 6 months after surgery. Neobladder capacity at first desire to urinate was 315.0± 33.4 ml(270-420 ml). The maximum neobladder capacity was 490. 3±39.7 ml(350-600 ml).The maximum flow rate (Qmax) was 16.5 ± 5.9 ml/s. Full resting pressure was 22. 5 ± 11.8 cm H2O. Peak voiding pressure was 78.3 ± 14.7 cm H2O. After mean 58. 4 months' follow up (range 22 to 132), 4 cases died of metastasis of bladder cancer. Conclusion Modified spiral ileal orthotopic neobladder is a reasonable option for treating invasive bladder cancer.
