Objective To compare the clinical curative effect between mini laparoscopic cholecystectomy (MLC) and conventional laparoscopic cholecystectomy (LC).Methods Using random number table method to divide the patients into conventional laparoscopic cholecystectomy group (30 cases) and mini laparoscopic cholecystectomy group (27 cases).Compared the operation time,bleeding volume,VAS score,postoperative hospital stay,complications.Results No conversion to laparotomy and serious complications,but each group has 6 cases from the three holes increase to four holes,and 4 cases of MLC group were converted to traditional LC.MLC group and LC group,average operation time (57.5 ± 17) min and (50 ± 15) min (P ＞ 0.05);the average postoperative hospital stay was (2.5 ± 1.4) d and (3.1 ± 1.5) d (P ＞ 0.05);postoperative pain degree of MLC group of 6 h and 12 hrespectively (4.8±2.6) and (3.5±1.1),LC group were (5.5 ±2.7) and (5.0±2.1) points (P＜ 0.05).Conclusion Postoperative pain of MLC is lighter than LC,in appropriate cases,is a safe and effective treatment method.

A mild rapid method for the determination of Pb in grain was established by diluted acid extraction and direct sampling detection with graphite furnace atomic absorption spectrophotometer(GF-AAS). Some factors, such as the grain size, nitric acid concentration, extraction time, solid, liquid proportion, and the instrument conditions were optimized. The results indicated that, at room temperature, the extraction rate of Pb in grain by diluted nitric acid was between 92. 14% and 94. 75%, and the average recoveries of Pb in grain extract reached 91. 65%-94. 58%, the precision of method was less than 5%, and the method detection limits and quantification limits were 0. 64 μg/L and 2. 14 μg/L, respectively, meanwhile, the extraction time was shorten to no more than 30 min. Compared with the results pretreated by microwave digestion and determined by GF-AAS, the detection values obtained by diluted acid extraction had no significance difference (p<0. 1).

Objective To evaluate the effectiveness and safety of total pelvic floor reconstruction treating female pelvic organ prolapse with prolift system .Methods The effectiveness and complications 24 cases genital pro-lapse according to POP Q stages who underwent total pelvic floor reconstruction surgery with prolift system were retro -spectively reviewed.Results The mean operative time was (97.5 ±23.3) min.The average blood loss (68.5 ± 28.1)mL.The mean postoperative hospital′s day was (8.9 ±2.3)days.The symptoms of all patients with peivic floor prolapse disappeared .2 cases sufferd pain and mesh erosion ,1 case had buttock abscess during follow-up.All recov-ered after treatment .Conclusion The short-term curative of total pelvic floor reconstruction surgery with prolift sys-tem is good.These cases have some postoperative complication .Such operation mode should be selected comprehen-sively.

[Abstrict] Objective To explore the effect of systemic family dynamics,egma minnen bardndom on the psychological traits,treatment efficacy of the first onset schizophrenia.Methods 53 schizophrenia patients with first onset were assessed by Self-rating Scale of Systemic Family Dynamics (SSFD),Egma Minnen Bardndom Uppforstran (EMBU),Neurotism Scale (Nf),Psychoticism (PSY),Paranoia Scale (Pa),Psychopathic Deviate Scale (Pd),Tolerance Scale(To) of MMPI-2,and lie Scale (L) was used to control assessment effectiveness.40 normal control cases were assessed by SSFD and EMBU.The scale scores between study group and control groupwere compared,and correlation and regression of study group were analyzed.Results Scores of family atmosphere,individuation,system logic of SSFD were higher in study group than those in control group (t=4.04,3.295,2.44,all P＜0.05),scores of emotional warmth and understand of EMBU in father and mother were lower in study group than those in control group (t=2.925,3.45,all P＜0.01),scores of refused and deny and punishment and stern of EMBU in father and mother were higher in study group than those in control group((t=4.715,4.971,4.331,4.067,all P＜0.01)).And correlation analysis showed that refused and deny of EMBU of father and mother,punishment and stern of EMBU of mother and PSY had significantly positive correlation(r=0.3378,0.4677,0.2895,all P＜0.05),refused and deny and punishment and stern of EMBU of father and mother and Pa had significantly negative correlation (r=-0.2913,-0.3254,-0.5003,-0.3242,all P＜0.05),punishment and stern and excessive interference of EMBU of father and Pd had significant positive correlation (r=0.3417,0.2958,all P＜0.05),PSY of MMPI-2 and treatment effect had significant negative correlation (r=-0.3194,P＜0.05).Regression analysis showed that psychoticism to forecast the poor treatment effect the rate was 10.2％,father and mother the refused and deny to forecast psychoticism high mark the rate respectively were 11.41％ and 21.87％,mother the severe punishment to forecast the psychoticism high mark the rate was 8.13％,Conclusion Father and mother the refused and deny,mother severe punishment have effects on psychological traits of the first onset schizophrenia,psychoticism of psychological traits has an affect on treatment efficacy,the psychoticism high mark presages the poor treatment efficacy.

Objective To evaluate the effect of irbesartan and nifedipine controlled-release tablets on morning blood pressure surge in patients with essential hypertension. Methods Ninety patients with essential hypertension were randomly divided into irbesartan group ( group A, n = 45) and nifedipine group (group B, n =45). Irbesartan tablets 150 mg/d and nifedipine controlled-release tablets 30 mg/d were respectively given in group A and group B for 4 weeks. The levels of diastolic blood pressure (DBP), systolic blood pressure (SBP), mean arterial pressure ( MAP ) and morning blood pressure surge ( MBPS ) before and after treatment were measured by ambulatory blood pressure monitoring. Results The DBP, SBP, MAP and MBPS levels of two groups after treatment for 4 weeks were all decreased (P < 0.01). All the indexes of group B decreased more significant compared to group A (P < 0.05 ). Conclusion To control blood pressure and prevent morning blood pressure surge, nifedipine controlled-release tablets are better than irbesartan.

Objective To study the effect of continuously compressive pressure(CCP) and human periodontal ligament cells(HPDLCs) on the differentiation of osteoclast-like cells(OLC) induced from umbilical cord blood cells in vitro,and to investigate the role of continuously compressive pressure and human periodontal ligament cells in alveolar bone rebuilding during orthordontic tooth movement.Methods Mononucleared cells of umbilical cord blood(HCMNCs) were separated by density gradient centrifugation,HPDLCs were isolated from human periodontal ligament by explanting enzymatic digestion with trypsin and collagenase.We also established transwell co-culture system with HCMNCs in the lower layer and HPDLCs in the upper layer.Group A: HCMNCs and HPDLCs were co-cultured with 150 kPa CCP for 1.5 hours on the model.Group B: only HCMNCs were cultured with the same CCP as Group A.Groups A'and B' were the respective control group of Groups A and B with no CCP exerted.The cell appearance was observed under the phase contrast microscope,and its identification was performed by histochemical analysis of tartrater-resistant acid phosphatase(TRAP).The capacity of bone resorption of the cell was assessed by lacunae forming ability on bone slice.Results HCMNCs in Group A began to fuse on the 2nd day,More positive multinucleated cells could be seen with TRAP staining and cortical bone pit formation on the 3rd day.Only a few multinucleated cells formed in the other groups,with no cortical bone pit formation.Conclusion HCMNCs can fuse into multinucleated OLC under CCP with the induction of HPDLCs.

Objective To find a better method to reconstruct total ear in one stage. Methods In the procedure, a compound framework was formed, in which Medpor acted as ear framework and homogenous rib cartilage as helix-framework after being sculptured, then they were vamped together. The formed framework was covered with adjacent skin flap and homogenous skin graft after it had been covered with superficial temporal fascia flap. Results From January 2001 to Augest 2003, 8 cases of congenital microtia or antotia were treated in this way, and all of them obtained satisfactory appearance in auricle. Conclusion The reconstructed ear has a delicate structure and good appearance, and looks and feels as a real one by this new procedure.

Objective In order to determine the effect of colchicine(Col) on the changes of serum activity of tumour necrosis factor (TNF) induced by lipopolysaccharide(LPS) in rabbits.Methods Twelve New Zealand white rabbits were equally divided into 2 groups, namely group A and group B. Objective To investigate the possible role of protein kinase(PK) activity in the induction of AP-1 by ox-LDL in mesangial cells and to elucidate the upstream signal pathways involved in the ox-LDL-induced AP-1 binding. Methods Rat mesangial cells were randomly divided into the normal cells, ox-LDL-treated cells and PK inhibitor + ox-LDL-treated cells. Treatments with the PKC inhibitor bisindolylmaleimide I, the PKA inhibitor H89, the PTK inhibitor genistein (GEN), the MEKi inhibitor PD98059, or the p38 MAPK inhibitor SB203580 were applied prior to a 24-hour incubation of ox-LDL in mesangial cells. The phosphorylation of MAPK families was detected by Weatern blot analysis. AP-1 binding was determined by gel shift assay. Results Ox-LDL stimulated the phosphorylation of JNKi/SAPK and p38 MAPK( P 0. 05) . Bisindolylmaleimide I at 50, 100, 200 nmol/L appreciably reduced the ox-LDL-induced AP-1 binding. H89 at 0. 5, 5 umol/L significantly inhibited AP-1 binding by ox-LDL. GEN at 25, 50 umol/L did not reduce the AP-1 binding by ox-LDL, but when GEN rose up to 100 umol/L, the ox-LDL-induced AP-1 binding significantly decreased in mesangial cells. However, SB203580 and PD98059 did not reduce the ox-LDL-induced AP-1 binding in the present study. Conclusion Multiple protein kinases may involve in the stimulation of AP-1 by ox-LDL in mesangial cells.
