Objective To explore the clinical significance of the inferior displacement sign on the initial anterioposterior (AP) view X-ray for fractures of humeral greater tuberosity.Methods This study retrospectively analyzed the imaging and clinical data of 24 patients with isolated fracture of humeral greater tuberosity who had sought medical treatment during the period from January 2008 to June 2015.They were 10 males and 14 females,with an average age of 54 years (from 19 to 68 years).Laterality:8 left sides and 16 right sides.The AP view X-ray films of the 24 patients present the inferior displacement sign defined as the inferior cortical margin of the greater tuberosity overlapping the proximal humeral shaft on the initial AP view.The patients received further CT examination to determine the displacement direction and whether the humeral neck fracture was complicated.Results A slight displacement of humeral anatomic neck fracture was shown on the initial AP view X-ray in 10 patients.Further CT examination revealed that humeral anatomic neck fracture was confirmed in 23 patients,the greater tuberosity was displaced posteriorly in 22 patients and posteroinferiorly in 2 patients.Radiographic measurements showed:the mean posterior displacement was 10.5 ±4.5 mm;the displacement between the humeral head and the humeral shaft was 2.5 ± 1.5 mm;the neck shaft angle was 147.2° ± 9.2°;the distance between the humeral head and greater tuberosity was 10.8 ± 3.4 mm;the overlap between the inferior edge of greater tuberosity and the upper edge of lateral humeral shaft was 12.5±5.9 mm.Pearson correlation analysis showed no correlation between the overlap and the head-tuberosity distance (P ＞ 0.05),but a significant correlation between the overlap and the neck-shaft angle (P ＜ 0.05).Conclusions The inferior displacement sign on the initial AP view X-ray is actually the overlap of posterior displaced greater tuberosity fracture shown on the X-ray.The posterior displaced greater tuberosity fracture complicated with slight displaced anatomic neck fracture is a presentation of the valgus impacted fracture of the proximal humerus.The inferior displacement sign in the fracture of humeral greater tuberosity can highly suggest the presence of occult or slight displacement of the humeral anatomic neck fracture.

Objective To analyze clopidogrel combined with aspirin therapy of acute cerebral infarction the safety and efficacy of micro brain hemorrhage patients.Methods Select from May 2015 and May 2015 months in the hospital for diagnosis and treatment of 76 cases of acute cerebral infarction combined micro cerebral hemorrhage patient study,observation group in the conventional treatment at the same time giving oral aspirin and clopidogrel,the control group in the conventional treatment at the same time,only to give oral aspirin.To observe the nerve function defect National institutes of Health stroke scale score(National Institute of Health stroke scale,NIHSS)and daily life activities ability pap index,determination of patients' blood plasma viscosity,whole blood low shear viscosity,AA induction and the platelet maximum aggregation rate induced by ADP(MARAA,MARADP),observation records during the treatment patients with adverse reactions,were analyzed.Results Compared with before treatment,the observation group and control group after treatment for 2 weeks,NIHSS score,score and blood related indexes were significantly improved,statistically significant difference(P< 0.05).Compared with control group,the observation group after treatment the NIHSS score,pap score and blood related indexes were significantly improved,total effective rate increased significantly,the differences were statistically significant(P< 0.05).Two groups of adverse drug reactions occurred situation is no statistical difference.Conclusion Clopidogrel in combination with aspirin can effective treatment in patients with acute cerebral infarction combined micro brain hemorrhage,and security.

Objective:To compare the diagnosis value of color echocardiography and high resolution CT for early left ventricular dysfunction.Methods:72 cases of left ventricular dysfunction patients in our hospital were chosen, and all of patients were detected with color echocardiography and high resolution CT inspection. Each patient was followed-up for 18 months and then to compare the diagnosis values of the two kind methods.Results: After 18 months follow-up review, the diagnostic accuracy of color echocardiography and high resolution CT were 95.8% (69/72) and 97.2% (70/72), respectively. Both of two methods were higher, and the difference was not statistically significant (x2=0.207,P=0.649). All sorts of cardiac function detection result, such as LVSV, LVEF, LVEDV and LVESV, were similar between color echocardiography and high-resolution CT, and the difference was not statistically significant (t=-0.775,t=1.387,t=-0.624,t=-1.209;P>0.05). Conclusion: Both of the diagnostic accuracy of color echocardiography and high-resolution CT for early left ventricular dysfunction were higher. All of them are worthy generalization and applying in clinical diagnosis for early heart failure.

Objective:To examine the effect of cyclosporine A(CsA) on the differentiation of Th17 cells from splenocytes after stimulation with anti-CD3 mAb+IL-23.Methods:Total splenocytes were cultured with anti-CD3 mAb alone or anti-CD3 mAb plus IL-23 in the presence or absence of different concentrations of CsA.After stimulation,production of IL-17 and IFN-? was assayed by ELISA.The subsets of IL-17-producting T cells were examined at single-cell level by flow cytometry.Results:The splenocytes stimulated with anti-CD3 mAb alone could induce low level of IL-17 production and addition of IL-23 could promote high level of IL-17 in a dose-dependent manner.CsA inhibited the differentiantion of Th17 cells from splenocytes stimulated with anti-CD3 mAb+IL-23 in a dose-dependent manner.Kinetic studies indicated that CsA also inhibited the differentiantion of Th17 cells from activated T cells stimulated by anti-CD3 mAb+IL-23 for 48 hours.In addition,CsA inhibited production of IFN-? and IL-2.The analysis of subsets of T cells demonstrated that CD4+T cells but not CD8+T cells produced IL-17.Conclusion:CsA can inhibit Th17 cell differentiation.This observation shed a new light on the potential mechanisms for the immunosuppressive and anti-inflammatory functions of CsA and provideds a theoretical and experimental foundation for clinical application.

Objective: To study the efficiency and prognosis of preoperative transcatheter arterial chemoembolization(TACE)treatment in patients with hepatocellular carcinoma(HCC)after hepatectomy. Methods: The clinical pathological data of 479 patients with resectable HCC from 1998 to 2007 were analyzed retrospectively. Fifty patients had preoperative TACE, 379 cases had not and others were lost of follow up or dead. Clinical pathological data, tumor-free survival rate and overall survival rate after hepatectomy were analyzed. Results: There was a significantly difference in the age and the tumor grade between the groups, other clinical pathological factors were no difference. The preoperative TACE group was younger than non-TACE group (?2=6.228, P=0.012), And had lower tu-mor grade, more tumor necrosis (?2=53.874, P=0.000). The disease tumor-free survival rate and overall survival rate were no difference between the groups (?2=0.736, P=0.391;?2=0.000, P=0. 995). Conclusions: For the unresectable HCC patients by one-stage operation, the preoperative TACE treatment has no adverse effect. However, tumor-free survival rate and overall survival rate are similar to non-TACE group.

It was found that HDL-c and HDL-c /TC were increased, but(VLDL + LDL)-c and TC were decreased. LPL activety was positively correlated with the increase of HDL-c and the decrease of ( VLDL+LDL )-c.
