Objective To explore the clinical efficacy and safety of intraoperative radiotherapy (IORT) for the patients with unresectable locally advanced pancreatic cancer. Methods Cochrane library, PubMed, EMbase, CBM, CNKI, VIP database and WANFANG database were used to retrieve, in addition to manual retrieval important literature references and conference papers. The randomised controlled trials, cohort study, controlled clinical trials of the IORT in unresectable locally advanced pancreatic cancer were searched. According to the inclusion criteria and exclusion criteria, after the extraction of literature and data, RevMan 5.3 software was used for meta-analysis. Results The total of 1 401 articles were retrieved, 7 met the inclusion criteria, including 1 for randomized controlled trials, 4 for cohort study, 2 for case-control study. 313 cases in the IORT group, 362 cases in the control group, and meta-analysis showed that IORT group was better than control group in overall survival (OS) (Z=4.15, P<0.000 1, RR=2, 95%CI 1.05-2.94). Because lack of complete data in local control rate, pain remission rate, complications, descriptive analysis was done, rather than meta-analysis. Conclusions IORT may improve the OS of patients with unresectable locally advanced pancreatic cancer and the local control rate, and effectively alleviate pain with favorale safety but it still needs high quality randomized controlled trials to confirm.

Aim To develop a rapid and inexpensive method for purification of human albumin, a method of immunomagnetic microspheres (IMMS) based on enzyme-linked immunosorbent assay (ELISA)for the purification of human albumin from human serum. Methods Polystyrene magnetic microspheres with carboxyl groups as carriers were prepared, and then the carboxyl groups on the surface of the microspheres were activated by ethylcarbodiimide (EDC). Finally rabbit anti-human serum albumin (HSA) antibodies were covalently bound to it and the complex can specifically capture HSA. After the procedure of capturing HSA, through taking rabbit anti-human albumin protein antibodies as a capture antibody, and goat anti-human albumin protein antibodies as a detection antibody, an ELISA on IMMS was developed, which can determine the recovery yield of HSA from the human serum. Results The result of the experiment was that the recovery of human albumin with IMMS was (86 ± 4) % , and IMMS were reused for two other purifying cycles, the results of which were (69.0 ± 0.6) % and (40.8 ± 0.8) % , and the purity of the product was about 90%. Conclusion The results above prove that the immunomagnetic purifiying strategy was shown to be efficient and offers an new thought for a large scale production of highpurity HSA.

OBJECTIVE:To establish a method for the determination of dissolution of Xiaocaihu pill, and compare the differ-ence of preparation from different manufacturers. METHODS:Using 0.1 mol/L HCl as dissolution medium, rotating basket method was used to determine the dissolution of preparations. HPLC was adopted to determine the content of baicalin:column was TSKgel ODS C18 with mobile phase of methanol-water-phosphoric acid (65∶35∶0.7,V/V/V) at a flow rate of 1 ml/min,detection wave-length was 280 nm,column temperature was 30 ℃,and injection volume was 5 μl. RESULTS:The linear range of baicalin was 0.488-124.8 mg/L (r=0.999 9);RSDs of precision, stability and reproducibility tests were lower than 2.0%;recovery was 100.14%-104.78%(RSD=1.58%,n=9). The average t50(50% dissolution time)of baicalin was 85.81 min. CONCLUSIONS:The method is simple with good precision, stability and reproducibility, and can be used for the dissolution determination of Xiaocaihu pill. Xiaocaihu pill from different manufacturers shows great differences, both preparation formulation and clinical use should attach importance to the dissolution of solid preparations.

The intracellular trafficking and subcellular distribution of exogenous gene is very important for gene delivery. A successful gene vehicle should overcome various barriers including endosomal membrane barriers to delivery gene to the target organelle. Traditional nonviral vehicle is unable to avoid endosomal pathway efficiently, so the efficiency of gene delivery is low and the application of gene drugs is limited. In order to achieve efficient nonviral gene delivery, a lot of researches based on endosomal escape have been carried out and some agents with the function of endsomal escape have been found. These agents facilitate the endsomal escape via various mechanisms, such as fusion into the lipid bilayer of endosomes, pore formation in the endosomal membrane, proton sponge effect and photochemical methods to rupture the endosomal membrane. In this review, various reported strategies for endsomal escape are described according to the escape mechanisms, and their applications in intracellular gene delivery are also discussed.

Objective To compare and evaluate the usefulness of diagnostic tests of IFA with HEp-2 cell substrate and ELISA coated with purified nuclear antigens for ANA in SLE.Methods Sera derived from 226 SLE cases and 183 healthy controls were tested for ANA and all parameters were compared such as sensi- tivity, specificity, accuracy, positive predictive value, negative predictive value, positive likelihood ratio,nega- tive likelihood ratio, result consistency, rank correlation coefficient and kappa of ANA detected by IFA and ELISA.Accuracy was evaluated by ROC for two methods. All 36 samples with different results from two meth- ods were detected for ENA.The correlation of titer to A ratio of different patterns as studied. Results The sensitivity of IFA and ELISA was 91.15% and 92.04% respectively for SLE patients, specificity was 96.17% and 92.90%,accuracy was 93.40% and 92.42%,positive predictive value was 96.71% and 94.12%,negative predictive value was 89.80% and 90.43%,no significant difference was found between the two methods (P>0.05).No significant difference was found in accuracy of both methods by ROC (P=0.409).Good agreement was found between two methods with rank correlation coefficient (R=0.823) and kappa (k=0.825).All of 36 samples with different ANA results from two methods were detected for ENA.In 14 cases with IFA positive and ELISA negative, the titer of one case was up to 1:1000 and the pattern was Golgi by IFA,the titers of the rest were about cutoff level and the pattern were granular and nucleolus mostly. In 22 cases with IFA nega- tive and ELISA positive,11 cases of them had the A ratio ranged from 2.67~30.5.Positive rate of ENA was 14.29% in 14 cases with IFA positive and ELISA negative,68.18% in 22 eases with IFA negative and ELISA positive and the difference was significant (P<0.01).Poor correlation of titer to A ratio for granular pattern samples (R=0.083),but good correlation for homogeneous pattern was found (R=0.595).Conclusion IFA as the recommended detecting method for ANA is intuitive and can provide more information by different pattem than ELISA but it needs fluorescence microscope and experienced technician. While ELISA is very simple and the concentration of ANA can be evaluated by A ratio value.ELISA can be a substitute method for ANA be- cause both IFA and ELISA have high sensitivity, specificity, accuracy, agreement rate, kappa and rank correla- tion coefficient. In addition,ELISA is more accessable for screen test because of low rate of false negative re- sult.Result of ELISA is more accurate if new and uncommon antigens are coated such as Golgi and nucleolus. The new work flow in which ELISA is used to screen out the positive ANA samples and IFA is used then to detect the nuclear pattern of ANA can save time, cost, and in turn improve work efficiency.
