2.Difference of Physical Symptoms, PWI and JCQ according to Sasang Constitutions for Industrial Workers.

PURPOSE: This study was conducted to analyze industrial workers' stresses from physical symptoms, PWI, JCQ and tasks in classification of Sasang constitutions. METHOD: Data were collected from industrial workers in J Province from May 2003 to June 2003. Questionnaires were distributed and collected on the day of their physical examination. Collected data were analysed through chi2-test. RESULT: Health-related characteristics showed that most of smokers and drinkers were Taeumins and this result was statistically significant. Physical symptoms related to constitutions indicated that GI and neurologic symptoms are significantly more frequent in Soeumin. The percentage of subjects with moderate risk was high in order of Soyangin, Taeumin and Soeumin while the percentage of subjects with high risk was high in order of Soeumin, Taeumin and Soyangin. CONCLUSION: Through this study, Lee Jae Ma's theory was be confirmed compatible with disease characteristics. However, research with more various subjects and variables needs to be made.

3.Clinical Evaluation on Diaphyseal Fractures of the Both Forearm Bones in Adults

Fractures of both forearm bones present specific therapeutic preoblems encountered infrequently in the fractures of other shafts of long bones. Although some of these fractures can be treated conservatively, majority of fractures are treated by open reduction and internal fixation for accurate anatomic reduction and rigid fixation. From January 1979 to December 1986, seventy two cases of both forearm bone fractures had been treated by four different methods at the Department of Orthopaedic Surgery, College of Medicine, Ewha Womans University and results were obtained as follows : 1. The most common fractures site was middle third of radius and ulna. 2. Comminuted fx was most common in radius and oblique fracture was most frequent. 3. Eleven cases were treated conservatively and 61 cases were treated by open reduction. Among them, 2 cases were fixed with intramedullary nailing, 36 cases by plating and 23 cases by combined method. 4. The time required for bony union was shorter in group of open reduction and plating than in groups of the other treatment. 5. The functional result was satisfactory in 53 cases(74%) and was best in group of open reduction and plating. 6. Among 5 cases of nonunion, 3 cases were developed in open fractures.

4.Clear Cell Sarcoma of Flexor Tendon in Wrist: A Case Report

While benign tumors of tendons and aponeuroses are observed commonly and have been defined adequately, malignant tumors originating from these tissues are rare. Among those reported most had been classified as synovial sarcoma or fibrosarcoma until Ezinger in 1965 described a new type that he called “clear cell sarcoma” of tendon and aponeurosis. Recently, Ultrastructural and histochemical analysis has revealed that the tumor cells contain melanin in the form of melanosomes in varying stages of development and currently, the neoplasm is considered a primary malignant melanoma of the soft tissue. A csse of clear cell sarcoma of flexor tendon in the wrist is reported, with an emphasis on its electron microscopic description.

5.Treatment of Fracture of the Femoral Shaft with the Brooker

The intramedullary nailing is the effective device now available for patient with appropriate fracture of the femoral shaft in adulthood. But problems with shortening and rotation of the femur persist, especially in comminuted fracture of the femoral shaft. So, the fixation with proximal and distal screw was performed. The Brooker-Wills nail achieves distal fixation easily with a transverse fixator deployed through slot in the nail. 24 patients with 25 femoral shaft fractures who treated at Dept. of Orthopaedic Surgery, Ewha Womans University Hospital from April, 1987 to April, 1988 were clinically analysed and the results were as follow. l. Among 25 cases, 22 cases were male and 3 cases were female. Average ages were 35.3 years and 3rd decade was more prominent. 2. 25 cases were classified according to the Winquist-Hansen classification. The Grade II was most common in 20 cases(80%). 3. 18 cases were treated by closed method and 7 cases were treated by open method. The closed method was required for 1.8 hours and open method was required for 2.2 hours. 4. The average time of union of Grade II was most shortened(11 weeks) and Grade IV was most delayed(16 weeks). 5. The complications were non-union in 1 case, mild angulation in 3 cases and limitation of motion of knee joint in 2 cases. So the accurate anatomical reduction and nailing to just above the epiphyseal scar in the metaphysis will be decreased of any complications.

6.A Case of Hereditary Multiple Osteochondromatosis

Multiple osteochondromatosis is a hereditary disorder affecting the endochondral skeleton during period of growth. We have studied a family which have multiple osteochondromatosis clinically and radiologically. In this family, eight of ten members could traceable, have the lesions of multiple osteochondromatosis. The most common site of the lesions was in femur, and the next common site was in tibia, especially, near the knee joint.

7.Two Cases of Rhabdomyosarcoma

Rhabdomyosarcoma is highly malignant soft tissue tumor which arise within skeletal muscles of the body. It makes up about 10 to 15 percent of sarcomas of the soft somatic tissues. Approximately two third of them originate in the muscles of the extremties, usually the legs. We have experienced two cases of rhabdomyosarcoma, the one arised within left forearm skeletal muscles and the other, within right calf muscles. The former was pleomorphic type, the latter was embryonal type in histological study.

8.Treatment of Ossification of Posterior Longitudinal Ligament in Cervical Spine with Anterior Fusion and Anterior Decompression: Report of 3 Cases

Since ossification of posterior longitudinal ligament in cervical spine (O.P.L.L.) was presented at first by Tsukimoto in 1960, many cases have been reported in Japan. But very few cases were reported outside of Japan, including Korea. It is yet unknown what mechanism is the cause and why it is prevalent among the Japanese. And so, there are various theories on the cause. With progression, this lesion cas cause cervical myelopathy and radiculopathy. The treatment of O.P.L.L. is divided into conservative and operative treatment, and the latter is composed of anterior fusion,anterior decompression and posterior decompression. We experienced 3 cases of O.P.L.L. of which two cases were treated by anterior fusion and one tred by anterior decompression with partial removel of ossification mass from March, 1985 to May, 1986. On the follow-up, results were excellent to good. We report our experience in treatment of O.P.L.L. and recommand to be concerned about the lesion.

9.A Clinical study on Non-union of the Femoral Shaft Fractures in Adults

Femoral shaft fractures are usually accompanied with extensive soft tissue injury due to high energy force and are difficult to manage for functional stress with weight bearing. According to Boyd(1965), femoral shaft is the first of non-union among the long bone shafts. And because of inadequate internal fixation, poor immobilization and infection, non-union of femoral shaft is still of interest to all the orthopaedic surgeons. During past 10 years, 19 cases of non-union of the femoral shaft were treated at dept. of Orthopaedic Surgery, Ewha Womans University hospital and following results were obtained. l. Of 19 cases, male between 20-40th age group was predominent. Prevalent type and site was mid. 1/3 comminuted fractures(p > 0.05). 2. Initial treatment was 10 cases of intramedullary nailing; 8 of plate fixation; 1 of screw fixation. Bone graft was added to 10 cases. 3. The probable causes of non-union were complex; 11 of inadequate interal fixation, 4 of poor external immobilization and 4 of infection. 4. 4 cases of infection were managed with control of infection and fixation. And 11 cases were fixed with plate and the other 4, with intramedullary nailing. All were supplemented with autogenous bone graft. 5. Postoperative immobilization was done with 15 cases of hip spica cast and 4 of long leg cast for average 9.3 weeks. 6. All were united at average 8.9 months. Limitation of motion of knee was remained in 3 cases in spite of physiotherapy. Severe osteoporosis was noted in 3 cases and leg length shortening with limping was found in 1 case.

10.The Effect of Aging on the Left Ventricular Mass.

The fundamental alterations in the cardiovascular system that occur consequent to aging are of great pratical importnace to clinicians. Senescent cardic muscle has many features of prolonged tension development, impaired relaxation and diminished response to receptor-mediated inotropic interactions. We estimated left ventricular mass by 2-D echo area-length method using a high quality planimeter. Age-related increments in left ventricular mass demostrated, but left ventricular enddiastolic cavity volumes were unaffected by age. The increase in left ventricular mass observed with aging is typical of pressuer-overload hypertrophy and its stimulus may be increased afterload from senescent changes.
