1.Comparation of video-assisted thoracoscopic surgery and open-chest operation on surgery trauma

Objective By means of control test,to compare the surgical trauma and perioperative recovery between video-assisted thoracoscopic surgery(VATS) and conventional thoracotony for lobectomy.Methods Eightysix non-small cell lung cancer patients who had undergone operations from April 2010 to April 2011 were chosen.According to operative mcthods,the patients were divided into two groups:video-assisted thoracoscopic surgery group(VATS) and open chest group(OT).The protein and hormone level of serum Cortisol(Cor),growth hormone (GH),triiodothyronine (T3),tetraiodothyronine (T4),C-reactive protein (CRP) were selected as indexes to investigate the surgical trauma on non-small cell lung cancer patients.The duration of operation,quantity of lymph node dissection,chest tube duration,postoperative hospital stay,postoperative complications were chosen as indexes to measure operation result and perioperative recovery.Results Under the condition that patients undergone the same operation level(lobectomy + Systemic lymph node dissection),there was no ststistical significance in preoperative level of serum Cor,GH,T3,T4,CRP.The postoperative concentrations of serum Cor,GH,T3,T4,CRP on the 1 d,2 d,3 d and 7 d were lower in VATS group,and there were obvious statistical significance(P <0.01).And there were obvious statistical significance in duration of operation (P < 0.01),postoperative hospital stay(P < 0.01),quantity of lymph node dissection (P =0.477).There were no obvious statistical significance in chest tube duration (P =0.508),postoperative complications (P =0.787).Conclusions VATS lobectomy is able to achieve a same result for radical treatment outcone and safety to routine open chest lobectomy in patients.Compared witt routine open chest lobectomy,VATS lobectomy has less surgical trauma,much rapid recovery,it should be recommended for patients with non-small cell lung cancer.
2.Value of combined detection of serum BNP and TnI in patients with acute heart failure

Objective To observe and explore the value of combined detection of serum BNP and TnI in patients with acute heart failure(AHF). Methods 60 AHF patients from February 2015 to April 2016 were selected as observation group, and divided into Level III group(n=32) and Level IV group(n=28) according to NYHA classification;60 healthy patients at the same period were selected as control group.BNP and TnI levels were detected by chemiluminescence in each group, and were compared.Results BNP and TnI of observation group were(2624.4 ±378.6) ng/mL and(0.87 ± 0.22) ng/mL, while in control group were(72.4 ±20.2) ng/mL and(0.13 ±0.02) ng/mL, the differences were significant(P<0.05);BNP and TnI in level III group before treatment were significantly lower than grade IV group(P<0.05), after treatment, the level of BNP and III in group IV and group TnI were significantly lower(P<0.05), and the levels of BNP and TnI were significantly lower than those in III group after treatment(P<0.05). Conclusion BNP combine with TnI levels monitoring play an important role in AHF diagnosis and prognosis evaluation, can help to guide the treatment of AHF.
3.Clinical value of cerebrospinal fluid ADA,IL-23 joint detection in the diagnosis of tuberculous meningitis ;related diseases

Objective To evaluate the clinical value of cerebrospinal fluid ADA,IL-23 joint detection in the diagnosis of tuberculous meningitis related diseases.Methods 253 cases with tuberculous meningitis related diseases were selected as the research subjects.According to the diagnosis,they were divided into tuberculous meningitis group (meningitis group,138 cases ),tuberculous meningitis complicated with hydrocephalus group (hydrocephalus group,35 cases)and control group(80 cases).All patients after admission received lumbar puncture, the part of cerebrospinal fluid specimens were inspected,the cerebrospinal fluid ADA,IL -23 joint test was conducted.Meningitis group and hydrocephalus group were given anti -TB drugs (INH,RFP and PZA +sm ) combined with chemotherapy.3 months after treatment,the meningitis group and hydrocephalus group received lumbar puncture cerebrospinal fluid ADA,IL -23 joint test again.The levels of ADA,IL -23 in cerebrospinal fluid were compared.Results Before treatment,cerebrospinal fluid ADA,IL-23 levels in the meningitis group were (12.64 ± 5.54)u/L and (48.38 ±10.78)pg/mL,those in the hydrocephalus group were (15.81 ±6.92)u/L and (77.21 ± 13.42 mm)pg/mL,which in the control group were (3.21 ±2.20)u/L and (9.05 ±3.89)pg/mL,ADA,IL-23 levels in meningitis group and hydrocephalus group before treatment were significantly higher than the control group (F=117.24,724.97,P<0.001).Spearma analysis showed that each group of cerebrospinal fluid ADA and IL-23 had no correlation.After treatment,the cerebrospinal fluid ADA,IL-23 levels in the meningitis group were (3.79 ± 3.13)u/L and (13.46 ±6.62)pg/mL,which in the hydrocephalus group were (6.42 ±4.35)u/L and (25.42 ± 8.54)pg/mL,the meningitis group before and after treatment had statistically significant differences in cerebrospinal fluid ADA,IL-23 (t=16.34,32.43,all P<0.001);hydrocephalus group before and after treatment had statistically significant differences in cerebrospinal fluid ADA,IL-23 (t=6.80,19.26,all P<0.001 ).Conclusion Cerebro-spinal fluid ADA,IL-23 joint detection in the early diagnosis of tuberculous meningitis related diseases and clinical observation has high clinical value.
4.PAF inhibits protein kinase C(PKC) activation in T cells

Abstract Objective: To investigate the regulatory effect of PAF on early events in signal transduction during T-cell activation. Methods:T-cell activation was achieved by stimulation of human T cells with CD3 mAb or PMA/ionomycin in the presence or absence of PAF. T cell Pro-liferatilon was determined by 3 H-thymidine incorporation, IL-2 production was measured by MTT method, IL-2R(CD25)expression was evaluatedby flow cytometry and,PKC activity was assayed by the method described by Hauschildt. Results:PAF inhibited CD3 mAb-induced T-cell pro-liferation, IL-2 production and CD25 expression. AIthough it inhibited T-cell proliferation and IL-2 production induced by PMA/ionomycin, PAFfailed to inhibit IL-2 production induced by PMA/ionomycin.The translocation of PKC was also inhibited by PAF if T cells were activated byCD3mAb. Conclusion: PAF inhibits T-cell activation via its inhibitory effect on PKC activation. Its differential effect on IL-2 production suggeststhat PAF regulatory more early events in signal transduction such as the activation of phospholipase C ?-1.
5.A clinical analysis of 86 maxillofacial fractures in children

An analysis of the characteristics of the facial fractures in children was performed in this paper by investigating the cases of the facial fractures in children.In the study,86 cases of facial fractures in children under 12 years from January 2000 to June 2008 were reviewed,who were treated by the Fourth Military Medical University. The information included the age,gender,time distribution of jury,type of fracture, nerve injury, and associated systemic injuries. The youngest age was 13 months, the ratio of male to female was 2.19∶ 1. The majority of fractures were occurred in summer. The most cases were caused by the traffic accidents, and the mandible fractures were higher than others. The number of conservative treatment were higher than operation,78% cases of condyle fractures were treated conservative, the operation of maxilla and zygomatic arch fracture were less than 50%. The males were higher than the females according to the facial fracture in children.Traffic accident was the main risk factor of condylar fracture. The mandibular fractures were occurred frequently and treated normally by conservative method.
6.Regulation of transcription of rRNA synthesis by Apak under hypoxic conditions

Objective To explore the effect and mechanism of Kruppel-associated box(KRAB) type zinc-finger protein Apak on the transcription of ribosomal RNA(rRNA) under hypoxic conditions.Methods Real-time quantitative PCR(qRT-PCR) was used to detect the level of rRNA and the effect of Apak on the transcription of rRNA.The expression and phosphorylation level of proteins were examined by Western blotting.Immunofluorescence assay was used to observe the location of Apak.Results Under hypoxia (0.3%O2),the transcription level of rRNA increased first and then decreased in Apak wild type cells.In Apak knock-out cells,the transcription level of rRNA kept decreasing under hypoxic conditions.The repression of Apak on rRNA synthesis and the nucleolus location of Apak disappeared,and the expression and phosphorylation level of Apak were down-regulated under hypoxic conditions for 3 h.Conclusion Under hypoxic conditions,the disapperance of Apak repression on rRNA transcription could lead to a temporary increase in rRNA.
7.Clinical observation of artificial bone rod combined with decalcified bone matrix for the treatment of osteonecrosis of the femoral head

Objective To evaluate the clinical outcome associated with the core decompression in combination with the nano-hydroxyapatite/collagen composite rod combined with decalcified bone matrix in a consecutive series of patients with osteonecrosis of femoral head,especially the prevention of collapse of femoral head and its predisposing factors.Methods From August,2012 to May,2013,46 pationts (50 hips) who had undergone core decompression in combination with nano-hydroxyapatite/collagen composite rod insertion in corporated with decalcified bone matrix in our hospital were involved in this study.Postoperative care consisted of prophylactic intravenous antibiotic and anticoagulation therapy.Patients were instructed to be non-weight-bearing for 3 weeks,to partial weight-bear for the next 3 weeks,and to weight bear as tolerated thereafter.All patients were evaluated both clinically and radiographically.The primary clinical outcome of this study was functional improvement assessed with the Harris hip score.Serial radiograms of the pelvis were taken at 1,3,6,12 months post-operatively to analyze the process of osteonecrosis.Results All patients followed up for 12 months,no one suffer complications.The mean Harris score pre-operation was 65.6 ± 10.6,post-operation score was 87.5 ± 15.3,with a mean improvement of 21.8 ± 13.2 (P < 0.05).According to Harris hip score system,excellent for 30 hips,good for 14 hips,fair for 2 hip and poor for 4 hips.Refer to the Kaplan-Meier survivorship curve,the success rate at 12 months post-operatively was 92%.Radiological changes coincided with clinical changes.Conclusion Core decompressionin combination with nano-hydroxyapatite/ collagen composite rod insertion in corporated with decalcified bone matrix provided a minimally invasive surgical treatment option to treat early stage osteonecrotic hips(stage Ⅰ and Ⅱ) and to prevent femoral heads from collapsing,with clinical outcomes and success rates priorto other commonly used surgical procedures.
8.Clinical Significance of IL-22 Expression and Percentage of IL-22-producing Th22 Cells in Primary Sj?gren′s Syndrome

Objective to measure the percentage of th22 cells and evaluate the levels of plasma interlukin-22(IL-22)in human peripheral blood, so as to determine their clinical significance in primary Sj?gren′s syndrome(pSS). Methods Patients with pSS were divided into three subgroups based on severity of labial gland involvement:mild,moderate and severe. Healthy people served as controls. the percentage of th22 cells and the lev-els of IL-22 in human peripheral blood from pSS patients and healthy controls were measured and compared. Results Compared to healthy con-trols,pSS patients had significantly higher percentage of th22 cells and higher plasma IL-22 levels(P < 0.05). Among pSS patients,the more seri-ous illness they had,the higher percentage of th22 cell and levels of IL-22 were observed. Severe patients had higher percentage of th22 cells and IL-22 levels than moderate patients(P < 0.05),and moderate patients had higher percentage of th22 cells and IL-22 levels than mild patients(P <0.05). Conclusion Increased peripheral IL-22-secreting th22 cells are detected in primary Sj?gren′s syndrome,which have a close association with disease severity. these data suggest that th22 and its released cytokine IL-22 may be considered as potential valuable biomarkers for severity of pSS,which may provide a novel therapeutic target for treatment.
9.Observations on the Therapeutic Effect of Electroacupuncture on Mild and Moderate Female Stress Incontinence

Objective To investigate the clinical efficacy of electroacupuncture in treating mild and moderate female stress incontinence.Methods One hundred and eighty female patients with mild or moderate stress incontinence were randomly allocated to treatment and control groups, 90 cases each. The treatment group received electroacupuncture and the control group took midodrine hydrochloride tablets. The International Consultation on Incontinence Questionnaire Urinary Incontinence Short Form (ICIQ-UI SF) score was recorded and 1-hour pad test leakage was measured in the two groups before and after treatment. The clinical therapeutic effects were compared between the two groups.Results There were statistically significant pre-/post-treatment differences in the ICIQ-UI SF score and pad test leakage in both groups (P<0.01). There were statistically significant post-treatment differences in the ICIQ-UI SF score and pad test leakage between the treatment and control groups (P<0.01). The total efficacy rate was 86.7% in the treatment group and 68.9% in the control group; there was a statistically significant difference between the two groups (P<0.05).Conclusions Electroacupuncture is an effective way to treat mild and moderate female stress incontinence. It can improve urinary continence and reduce urine leakage in the patients.
10.Effect of magnesium content on microstructure and properties of powder metallurgy Ti-Mg alloy

BACKGROUND: High-strength titanium alloy can be prepared bY using powder metallurgy technique, but there are still some problems such as containing toxic elements or high elastic modulus in prepared alloys.OBJECTIVE: A new Ti-Mg alloy with non-toxic and elastic modulus which is close to human bones and strengths satisfy the requirements for human body implants was successfully prepared by means of powder metallurgy technology, and the effects of magnesium content on the microstructure and mechanical properties of Ti-Mg alloy were systematically studied.METHODS: The Mg powder and 17 powder in different mass ratios were prepared by powder metallurgy process to get a new integrated medical Ti-Mg alloy with excellent comprehensive properties. The porosity, fracture surface morphology and phase constituents of samples were observed by optical microscopy, scanning electron microscopy and X-ray diffraction, and the mechanical properties including bending strength, impact toughness and hardness of the alloys were measured,RESULTS AND CONCLUSION: Magnesium mass increasing firstly decreased and then increased admissible strain. When magnesium mass was 10%, admissible strain was 0.97%, which was highly closed to 0.67% of cortical bone. The porosity decreased from 18.3% to 3.8%, the holes became more tact and distributes more uniform, with the increased of mass ratio of magnesium. And with the increased of mass ratio of magnesium, the hardness, bending strength and elastic modulus of alloys with similar pattern of change, there were little changed when the mass fraction of magnesium less than 10%, they decreased significantly and then tended to stable with the continued to increased of magnesium mass fraction. While the admissible strain firstly decreased and then increased, with the increased of the mass fraction of magnesium. When the mass fraction of magnesium was 10%, the hardness, bending strength, impact toughness satisfied the mechanical requirements for human body implants, and the bending strength and admissible strain were close to human bones, which indicated well biomechanical compatibility.
