1.Propofol with bone marrow mesenchymal stem cell transplantation improves the hind limb function and electrophysiological changes in rats with spinal cord injury

BACKGROUND:Bone marrow mesenchymal stem cels can be used to repair neurons, but have no ideal outcomes on nervous system injuries. OBJECTIVE:To investigate the effects of bone marrow mesenchymal stem cel transplantation combined with propofol on the hind limb function and electrophysiological changes of rats with spinal cord injury. METHODS:Eighty adult Wistar rats were selected to make animal models of spinal cord injury, and then randomized into four groups (n=20): bone marrow mesenchymal stem cel group, control group, combination group, propofol group. At 6 hours after modeling, rats in these four groups were injectedvia the tail vein with bone marrow mesenchymal stem cel suspension, cel culture medium, bone marrow mesenchymal stem cel suspension+propofol solution, and propofol solution using a 1 mL syringe, respectively. Rat motor function was assessed by Basso Beattie Bresnahan score, modified Tarlov score and inclined plane test before and at 1 day, 3 days, 1-4 weeks after modeling. Under fluorescence microscope, the survival and distribution of PKH-26-labeled bone marrow mesenchymal stem cels were observed at 4 weeks after modeling, and meanwhile, hematoxylin-eosin staining was used for pathological observation. Horseradish peroxidase tracer analysis was performed to analyze regeneration of nerve fibers, and motor and somatosensory evoked potentials were used to analyze the neurophysiological recovery of rats. RESULTS AND CONCLUSION:(1) The motor function of the rat hind limb recovered best in the combination group, better in the bone marrow mesenchymal stem cel group and propofol group, but worse in the control group. (2) There were a smal amount of nerve axon-like structures and smal syringomyelia in the bone marrow mesenchymal stem cel group and propofol group, but the combination group had more axon-like structures and no syringomyelia. In the control group, no axons but spinal cord defects and syringomyelia formed. (3) The amount of horseradish peroxidase-positive nerve fibers and the number of PKH-26 positive cels were ranked as folows: control group < propofol group and bone marrow mesenchymal stem cel group < combination group. There were significant differences between the groups (P < 0.05). (4) The latencies of motor and somatosensory evoked potentials were ranked as folows: control group> propofol group and bone marrow mesenchymal stem cel group > combination group, and there were significant differences between the groups (P < 0.05). (5) Amplitudes of motor and somatosensory evoked potentials were arranged as folows: control group < propofol group and bone marrow mesenchymal stem cel group < combination group, and the differences were statisticaly significant (P < 0.05). These findings indicate that both propofol and bone marrow mesenchymal stem cels can promote synaptic regeneration and improve the electrophysiological function and motor function of rats with spinal cord injury. Their combination has a better role than propofol and bone marrow mesenchymal stem cels used alone.
2.Study on predictive value of cerebral arteriosclerosis to cerebral infarction evaluated by transcranial doppler ultrasound

Objective To assess the predictive value of cerebral arteriosclerosis to acute cerebral infarction evaluated by transcranial doppler ultrasound .Methods 170 cases suspected cerebral arteriosclerosis consecutively enrolled were given transcranial doppler ultrasound and were followed up for 6 months,then we assessed the correla-tion between the levels of cerebral arteriosclerosis and acute cerebral infarction within 6 months.Results By Spearman rank correlation analysis ,there was positive correlation between the severity of cerebral arteriosclerosis de-tected by transcranial doppler ultrasound and acute cerebral infarction within 6 months,the correlation coefficient was 0.575(P<0.05).Conclusion Through the detection of transcranial doppler ultrasound ,the higher levels of cere-bral arteriosclerosis,the greater probability of acute cerebral infarction within 6 months.
3.Advance in Rehabilitation of Upper Limb Function in Hemiplegic Patients after Stroke (review)

The incidence of upper limb dysfunction of stroke patients was high, and the recovery was difficult, which seriously impact-ed their activities of daily living and quality of life. This article reviewed the new techniques and methods for hemiplegic patients after stroke. The main techniques and methods includeed bilateral isokine-matic training, mirror neuron system, gymnastics rods training, music therapy, motor imagery, upper limb rehabilitation robot, virtual reality, constraint induced movement therapy, functional electrostimulation, electromyographic biofeedback therapy, transcranial magnetic stimulation, neurophyisological therapy and so on.
4.Preparation of Monospecific Polyclonal Antibodies Against Glechoma hederacea Agglutinin

Glechoma hederocea agglutinin (Gleheda) is a novel glycosylated lectin isolated from the leaves of G. hederacea. Like other glycosylated proteins, the detection of Gleheda by immunological methods is often hampered by the cross-reactivity of the polyclonal antibodies with unrelated glycoproteins. Hence a protocol to purify monospecific polyclonal antibodies from a crude antiserum raised against Gleheda was developed. After selective ammonium sulfate precipitation and successive affinity chromatography on columns of Sepharose 4B with immobilized Gleheda and Robinia pseudoacacia agglutinin (RPA), respectively, ion-exchange chromatography on a column of Q Fast Flow was used for further purification. The specificity of the antibody fractions from each step was tested by double immunodiffusion assay and analyzed by Western blot. Results revealed that affinity chromatography of the immunoglobulin fraction on the immobilized Gleheda antigen yielded an antibody preparation that still cross-reacted with many proteins in leaf extracts. Depletion of nonspecific cross-reacting antibodies directed against the glycan part of the glycoprotein by affinity chromatography on immobilized RPA removed most but not all nonspecifically reacting antibodies. Only upon further purification by ion exchange chromatography an IgG fraction of monospecific antibodies that reacted exclusively with Gleheda could be obtained and accordingly was suitable for immunodetection studies. This antibody purification procedure promises simplicity and efficiency. In addition, this method does not require expensive facilities.

6.The effect of continuous quality improvement of nursing care on senile patient with gastroscopy

Objective:To explore the influence of continuous quality improvement of nursing care on the compliance of senile patient with gastroscopy.Methods: 100 senile patients with gastroscopy were selected, and they were divided into control and improvement groups ( each group included 50 cases ) depended on the different time of hospitalization. The control group was treated with routine nursing quality method, while the improvement group was treated with continuous quality improvement method. A series of indicators in the endoscopic examination, such as the success rate of one time intubation, the used time, yes or no cough and other adverse reactions and the patient's satisfaction for nursing services were compared between the two groups.Results:After 5 months of continuous quality improvement nursing, the success rate of one time intubation in the improvement group was significantly higher than that in the control group, and the checking time of improvement group was significantly less than that of control group, the differences were statistically significant(t=7.162,t=6.61; P<0.01). Besides, on check process, the incidence of adverse reactions in the improvement group was significantly lower than that in the control group and the satisfaction of nursing service was significantly higher than that of the control group, the differences were statistically significant(x2=23.077,x2=10.699,P<0.01).Conclusion:The application of continuous quality improvement nursing for senile patients during gastroscopy can improve the success rate of one time intubation, shorten the gastroscopy time, reduce the incidence of adverse reactions, enhance compliance of gastroscopy and satisfaction for nursing service, and its clinical effect is obvious.
7.Utility of parenteral nutrition in inflammatory bowel disease

Objective To analyze and discuss the utility of parenteral nutrition in inflammatory bowel disease (IBD).Methods In the light of the diagnostic criteria of IBD by the National Conference in 1993 as to the chronic noninfectious bowel disease,the inpatients of IBD in our department from Jan.1986 to Dec.1996 were analyzed,and the medical treatment of IBD and the role of parenteral nutrition evaluated.Results It was demonstrated that the annual average number of hospitalized patients had increased in the past 11 years,and the parenteral nutrition resulted in a better outcome,in addition to SASP or 5-ASA.Conclusion It was believed that following the principal treatment of IBD, nutritional support is also essential.

Objective To investigate the parameter of technical shaping process of Muxiang Shunqi Wan and select appropriate auxiliary materials to enhance the time limit of dissolution. Method Select parameter of technical shaping process by orthogonal test and choose the best dosage of disintegrant by well-proportioned design. Results The best preparation and technical shaping process of Muxiang Shunqi Wan is as follows:the drug powder is refined with CMS-Na 1.4%, CCNa 0.5%, 60 ℃ dry. Conclusion Pills made by this technical process appear round and smooth, the colour and luster are equal and conformable, the dissolving time limit is short and the quality is good, therefore, they are up to the GMP requirements.
9.Quality standard for Xianglian Zhixie Capsule

AIM:To establish the quality standard for Xianglian Zhixie Capsule(Radix Aucklandiae,Rhizoma Coptidis).METHODS:Radix Aucklandiae,Rhizoma Coptidis of Xianglian Zhixie Capsule were identified by TLC.Dehydro-costunolide,costunolide were determined by HPLC together.RESULTS:The characteristic identification by TLC was distinct and highly specific.The quantitative evaluation of dehydro-costunolide had the linear range of 0.217 4-2.174 ?g(r=0.999 9,n=6),the average recovery was 96.25%(RSD=1.37%,n=6).The quantitative evaluation of costunolide had the linear range of 0.199 2-1.992 ?g(r=0.999 3,n=6),the average recovery was 101.77%(RSD=1.23%,n=6).CONCLUSION:The method is reliable,accurate and specific.It can be used for quality control of Xianglian Zhixie Capsule.
10.Quality Standard for Qingxin Anshen Soft Capsules

Objective To establish the quality standard for Qingxin Anshen Soft Capsules. Methods Rhizoma Coptidis and Cortex Cinnamomi in Qingxin Anshen Soft Capsules were identified by TLC. Berberine hydrochloride and cinnamaldehyde were determined by HPLC. Results The characteristic identification by TLC was distinct and highly specific. The results of quantitative evaluation showed that berberine hydrochloride had the linear range of 0.6412 ?g~6.412 ?g(r=0.999 7,n=7),and the average recovery was 97.57 %(RSD=0.81,n=6);cinnamaldehyde had the linear range of 0.2148 ?g~2.148 ?g(r=0.999 8,n=7),and the average recovery was 97.70 %(RSD=0.88,n=6). Conclusion The method is reliable,accurate and specific. It can be used for the quality control of Qingxin Anshen Soft Capsules.
