OBJECTIVETo explore the perioperative cardiovascular dysfunction and its relevance to age in patients with silent coronary heart disease (or silent myocardial ischemia), and explore the clinical treatment and recovery of perioperative arrhythmias.

METHODSOne hundred and eighty cases were selected from selective surgery patients with silent myocardial ischemia (SMI). Among the cases, 130 patients older than 51 years old were divided into 51 - 60 year-old group, 61- 70 year-old group and 71 - 80 year-old group. Control group was set up by other 50 patients younger than 51 years old. Electrocardiogram data of 24 h before the operation, 24 h after the operation and 48 h after the operation were continuously monitored by dynamic electrocardiogram (DCG). The electrocardiogram data of ST shifting, arrhythmia incidences of different type and at different time were analyzed by professional doctors. At the same time, the treatment and recovery of perioperative arrhythmia were recorded.

RESULTSAs the age increase, the magnitude and duration of ST shifting appeared upward trend compared to the control group (P < 0.05, P < 0.01). The incidence of ST elevation in 71 - 80 year-old group was higher than the control group (P < 0.05). The ST depression duration in 61 - 70 and 71 - 80 year-old group and ST elevation magnitude in 71 - 80 year-old group were higher than 51 - 60 year-old group (P < 0.05). Compared to the control group, the incidence of accelerated idioventricular rhythm (AIR) in 61 - 70 year-old group and the incidence of sinus bradycardia (SB), ventricular premature beat (VPB), ventricular tachycardia (VT) in 71 - 80 year-old group were higher (P < 0.05, P < 0.01). Compared to the 51 - 60 year-old group, the incidence of atrial fibrillation (AF) in 61 - 70 year-old group and the incidence of VP, VT, AF in 71 - 80 year-old group were higher (P < 0.05, P < 0.01). The arrhythmia incidences in 24 h after operation were higher than 48 h after operation and 24 h before operation (P < 0.01). As the age increase, the recovery incidence by removing inducement was decreased, but the recovery incidences by drug and electric-shock treatment were increased (P < 0.05).

CONCLUSIONOld SMI patients have high levels of perioperative myocardial ischemia and arrhythmia, and 24 h after operation is the period of high incidence.

Objective To investigate the role of intercellular adhesion molecule-1 (ICAM-1) in the up-regulation of peripheral blood somatic stem cells after acute myocardial infarction in rats. Methods The models of acute myocardial infarction were established in 16 rata by ligation of left anterior descending branch of left coronary artery through chest incision, and the animal were divided into control group(n=8) and experiment group (n=8). The hearts of another 2 rats were obtained for normal myocardial tissue sections as controls. Monoclonal antibody of ICAM-1 was infused from the caudal vein in experimental group, and no invervention was conducted for control group. Blood samples were obtained from caudal vein on the first, third, seventh and fourteenth day after operation in these two groups. Serum concentration of ICAM-1 was measured by ELISA, positive rate of CD34 cells in peripheral blood was detected by flow cytometry, and the parameters of concentration of ICAM-1 and positive rate of CD34 cells at each time point were compared between groups. Results The concentration of ICAM-1 in peripheral blood of experiment group reached the lowest of (59.01±2.22) pg/mL on the seventh day. The concentrations of ICAM-1 in peripheral blood of experiment group were lower than those in control group, and there were significant differences between these two groups on the seventh and fourteenth day(P < 0.01). The positive rate of CD34 cells in peripheral blood of experiment group reached the highest of (12±2.11)% on the seventh day. The positive rates of CD34 cells in peripheral blood of experiment group were higher than those in control group, and there were significant differences between these two groups on the third, seventh and fourteenth day(P<0.05 or P<0.01). Conclusion ICAM-1 can inhibit the up-regulation of peripheral blood somatic stem cells after acute myocardial infarction in rats.

Objective To study effects of fluid resuscitation on thoracoabdominal injury combined with hemorrhagic traumatic shock.Method A total of 98 patients,who were treated in Affiliated Zhongshan Hospital of Dalian University from November 2004 to December 2006,were retrospectively analyzed.The patients were diagnozed according to Surgery(fifth edition).Patients were divided into delayed fluid resuscitation group(n= 51)and immediate fluid resuscitation group(n=47).Patients in delayed fluid resuscitation group were given with balanced salt solution for the body to maintain basic requirements.Patients in immediate fluid resuscitation group were rapidly administered with a lot of isotonic crystaUoid and(or)colloid solution after admission. Objective To study the characteristics of Genetic typing and the antibiotic susceptibility testing of strains from Pseudomonas aeruginosa keratitis patients.Design Experimental study,Participants 23 eyes of 23 patients of Pseudomonas aeruginosa keratitis.Methods The genomic was extracted and amplified with PCR.The PCR products were purified and sequenced.The results were registered in MIST web Antibiotic susceptibility testing were performed in theses strains,Main Outcome Measures Sequence types and antibiotic susceptibility.Results The isolates were resolved into 20 STs.Two lineages were identified.MIC test showed that strains were more susceptible to aminoglycosides,The activity of quinolones and cephalosporin were higher than that of aminoglycosides.Conclusion MIST can determine homology of the strains from Pseudomonas aeruginosa keratitis by clustering results. There is no finding about relationship between Genetic typing and drug resistance.

Objective To investigate protein expression of telomerase reverse transcriptase(TERT),telomerase-associated protein 1(TP1) and telomeric repeat binding factor 2(TRF2) in human fetal cadiacmyocytes.Methods Using immunohistochemical method,sections of human fetal hearts were stained with antibodies against TERT,TP1 and TRF2.Results Expression of TERT was gradually decreased with development in human fetal cadiacmyocytes nuclear.Expression of TP1 and TRF2 was gradually increased with development in human fetal cadiacmyocytes cytoplasm.Conclusion Down-regulation of TERT and up-regulation of TP1 and TRF2 may promote the permanent withdrawal of cardiomyocytes from the cell cycle and enter terminal differentiation and myocardium hypertrophy.

OBJECTIVETo study clinical effects of short-segment fixation and injured vertebra bone grafting through injured pedicle for the treatment of thoracolumbar burst fractures under MAST Quadrant retractor via a paraspinal muscle approach.

METHODSThe data of 42 patients with thoracolumbar burst fractures treated from June 2009 to September 2012 were reviewed. There were 19 males and 23 females, with an average age of (55.2±11.9) years old. The mean injury time was (5.8±4.3) days. Fracture segments included T10 in 3 cases, T11 in 6 cases, T12 in 13 cases, L1 in 9 cases, L2 in 7 cases, and L3 in 4 cases. According to Denis classification, 9 patients were type A, 21 patients were type B, 5 patiens were type C, 5 patients were type D, and 2 patients were type E. All the patients were treated with short-segment pedicle screw-rod system fixation under MAST Quadrant via the paraspinal muscle approach. The operative time, blood loss, complications and the height of vertebra, kyphosis Cobb angle, VAS scores, JOA scores were measured before and after treatment.

RESULTSAfter treatment, the vertebral height and kyphosis Cobb angle were restored. Compared with preoperative results, postoperative vertebral height and kyphosis Cobb angle, VAS scores and JOA scores were all improved. But there was no statistically significance in vertebral height, kyphosis Cobb angle between postoperative at 1 week and 1 year.

CONCLUSIONInternal fixation combined with injured vertebra bone grafting through the injured pedicle for the treatment of thoracolumbar burst fractures via the paraspinal intermuscular approach under MAST Quadrant is a safe, minimally invasive, effective and satisfactory method.
