1.Studies on predictors in patients with cryptococcal meningitis

Objective To investigate the factors that might influence the effectiveness of treatment for cryptococcal meningitis. Methods Retrospective analysis was conducted for 11 patients with cryptococcal meningitis admitted to Peking Union Medical College (PUMC) Hospital during 1980-1995. Results Seven of 11 patients with cryptococcal meningitis were successfully cured with combination of amphotericin B and flucytosine for 14 to 31 weeks (five months in average), paraventriculostomy was performed in five of them with hydrocephalus complicated with deteriorated mental status and one of them was administered with additional fluconazole for 15 days during its early stage of therapy. Four of the 11 patients died from cryptococcosis during the course of antifungal therapy. The duration of illness varied from three weeks to four and a half months (two months in average) . And four of them had underlying illness with steroids / cytotoxic therapy and another one had AIDS. Factors associated with unfavorable outcomes included more rapidly progress of the illness (2/11), delay in diagnosis and antifungal therapy, severe impairment of host defense mechanisms against infection, underlying illness involving central nervous system, and significantly abnormal cerebrospinal fluid for a sustained period. Conclusions Cryptococcal meningitis could hardly be diagnosed earlier. Combination of antifungal therapy with paraventriculoatomy at adequate time, if necessary, was effective in treatment for it.
2.Effect of compatibility between Caulis Aristolochiae Manshuriensis and Cortex Moutan on aristolochic acid Ⅰ

Objective To investigate the thermostability of aristolochic acid Ⅰand the effect of the compatibility on aristolochic acid Ⅰ in Caulis Aristolochiae Manshuriensis decoction and to explore the detoxification mechanism of compatibility for aristolochic acid Ⅰ.Methods Analyzing the contents of aristolochic acid Ⅰ by HPLC in the single decoction of Caulis Aristolochiae Manshuriensis,the concoction of Caulis Aristolochiae Manshuriensis with Cortex Moutan,the residues of decocted Caulis Aristolochiae Manshuriensis and the residues of concocted Caulis Aristolochiae Manshuriensis with Cortex Moutan,respectively.Results Aristolochic acid Ⅰ decreased after heating in pure water,a new peak was found in HPLC spectra and supposed to be the derivate of aristolochic acid Ⅰ,which was also found in the decoction of Caulis Aristolochiae Manshuriensis.The content of aristolochic acid Ⅰ in the concoction of Cortex Moutan with Caulis Aristolochiae Manshuriensis is lower than that in the single Caulis Aristolochiae Manshuriensis decoction.Furthermore,the quantity of aristolochic acid Ⅰ in the residues of Caulis Aristolochiae Manshuriensis after concoction is lower than that in the residues of single Caulis Aristolochiae Manshuriensis decoction.Conclusion Aristolochic acid Ⅰ is unstable in decoction and a part of it was changed into another compound.The stripping of aristolochic acid Ⅰfrom Caulis Aristolochiae Manshuriensis is not inhibited when Cortex Moutan concocted with Caulis Aristolochiae Manshuriensis.It is the chemical reaction of aristolochic acid Ⅰ,which could decrease the toxicity in the decoction or concoction of Caulis Aristolochiae Manshuriensis.
3.Study on relations among psychiatric nurses' psychological capital and job burnout

Objective To investigate the current situation of psychological capital and job burnout of psychiatric nurses and explore the relationship between psychological capital and job burnout.Methods There were 142 psychiatric nurses collected in the study by convenience sampling.The Psychiatric Capital Questionnaire (PCQ) and Nurse Burnout Scale (NBS) were used to measure the level of psychiatric capital and job burnout of psychiatric nurses.The Pearson correlation analysis was used to explore the relationship between psychological capital and job burnout.Results The psychological capital of psychiatric nurses had a negative correlation with job burnout; the higher of the nurses' psychological capital,the lower their sense of job burnout,vice versa.Conclusions In order to improve nursing quality,nursing managers can reduce the job burnout by improving the psychological capital of psychiatric nurses.
4.Lung cancer and chronic obstructive pulmonary diseases

Lung cancer and chronic obstructive pulmonary disease (COPD) are two high-mortality diseases in the world.Numerous epidemiological studies have demonstrated that presence of COPD increases the risk of lung cancer.Habitual cigarette smoking frequently develops lung cancer as well as COPD,However the links between the two diseases should be more than smoking alone.The underlying mechanisms may include genetic predisposition,inflammation and cell injury,oxidative and noxious stress,extracellular matrix and proteinases,some of which might represent the targets for chemoprevention or chemotherapy.
5.Effect of 5-Aminosalicylic Acid on Cell Proliferation,Apoptosis and Cell Cycle in Human Colon Cancer Cell Line HT-29

Background:Ulcerative colitis-associated colorectal cancer(UcCRC)is the most serious complication of ulcerative colitis(UC). 5-Aminosalicylic acid(5-ASA)could reduce the risk of UC progressing to UcCRC. Aims:To investigate the effect of 5-ASA on cell proliferation,apoptosis and cell cycle in human colon cancer cell line HT-29 for verifying the inhibitory effect of 5-ASA on UcCRC. Methods:HT-29 cells were treated with different concentrations(0,10,20,30, 40 mmol/ L)of 5-ASA. Cell proliferation was measured by CCK-8 assay. Cell apoptosis was determined by TUNEL method. Cell cycle was analyzed by flow cytometry. Expressions of cell mitotic regulators AuroraB and BubR1 were determined by immunohistochemistry. Results:Proliferation inhibition rates and apoptosis rates of HT-29 cells in 5-ASA 10,20,30,40 mmol/ L groups were increased with increase in concentration of 5-ASA(P ﹤ 0. 05). Proportions of cells in phase G0 / G1 in 5-ASA 30,40 mmol/ L groups were significantly lower than those in 5-ASA 0,10,20 mmol/ L groups (P ﹤ 0. 05);proportions of cells in phase S in 5-ASA 0,10,20,30 mmol/ L groups were significantly lower than that in 5-ASA 40 mmol/ L group(P ﹤ 0. 05);while proportions of cells in phase G2 / M in 5-ASA 30,40 mmol/ L groups were significantly higher than those in 5-ASA 0,10,20 mmol/ L groups(P ﹤ 0. 05). Mean optical density(MOD)values of AuroraB and BubR1 were decreased with increase in concentration of 5-ASA(P ﹤ 0. 05). Concentration of 5-ASA was positively correlated with proliferation inhibition rate and apoptosis rate of HT-29 cells(P ﹤ 0. 05),but was negatively correlated with MOD values of AuroraB and BubR1( P ﹤ 0. 05). MOD values of AuroraB and BubR1 were negatively correlated with proliferation inhibition rate and apoptosis rate of HT-29 cells as well as proportion of cells in phase G2/ M (P ﹤0. 05),but were positively correlated with proportion of cells in phase G0/ G1(P ﹤0. 05). MOD value of AuroraB was positively correlated with MOD value of BubR1(P ﹤ 0. 05). Conclusions:5-ASA can inhibit proliferation and induce apoptosis in HT-29 cells,the mechanism may be related to inhibiting expressions of AuroraB and BubR1 and the resulted effect on cell cycle.

Acute liver failure (ALF) in children is severe syndrome with varied etiology and a rapid deterioration of liver function which results in coagulopathy abnormality and encephalopathy.ALF in children often carries a high morbidity and mortality.Laboratory tests is helpful for diagnosis and severity assessment of ALF in children and is useful for predicting survival or need for liver transplantation.The objective of this article is to introduce the values of laboratory tests in clinical diagnosis,prognosis and timing for transplantation of ALF in children.

8.Study on abnormal myocardial motion of left ventricle by quantitative tissue velocity imaging

Objective To observe the velocity of left ventricular wall motion features of normal subjects and patients with abnormal regional wall movement of left ventricle with the quantitative tissue velocity imaging(QTVI),and to know whether variances exit between the velocity of left ventricular myocardial motion of the normal subjects and the abnormal subjects with coronary artery disease(CAD) and myocardial ischemia. The practical value of QTVI on myocardium movement of left ventricle was evaluated. Methods The segmental left ventricular wall motion velocity was measured on 27 normal subjects and 21 patients with CAD and myocardial ischemia,including three waves,peak velocity of systolic S wave,diastolic E wave,A wave, and time velocity integrate of S wave(S TVI) was used as measurement parameters.The parameters were analysed between the two groups. Results The velocity and S TVI were gradually decreased from base to apex in the same wall in the control group.There were some abnormal segmental movements in the abnormal group. S wave ,E wave and S TVI of diseased segments in the abnormal group were much lower than those of control group ( P 0.05 ). Conclusions QTVI can quantitatively measure the velocity in abnormal movement segment of ventrical wall of patients with CAD and myocardial ischemia.S wave,E wave and S TVI are better parameters of assessing systolic function of left ventricle.QTVI has clinical application in regional myocardial motion function evaluation.
9.Post-ERCP hemorrhage in patients with choledocholithiasis: risk factors and prevention

Objective To investigate the risk factors and prevention of the ERCP-related upper gastrointestinal bleeding in patients with choledocholithiasis.Methods The clinical data of 678 patients who received ERCP in Yijishan Hospital of Wannan Medical College between January 2014 and December 2014 were reviewed.Univariate and multivariate analyses were performed to define the independent risk factor(s) for delayed hemorrhage following ERCP.Results A total of 383 patients,in whom 157 were males and 226 were females,the age ranged from 19 to 88 years,were recruited to the study.ERCP-related upper gastrointestinal bleeding occurred in 41 patients(10.7%),intraoperative bleeding occurred in 27(7.0%) and postoperative hemorrhage in 14 (3.7%).Twenty-eight cases were mild bleeding,9 moderate (4 caused by laceration of the cardiac mucosa) and 4 were severe bleeding.All patients recovered and were discharged,and no death occurred.No single case required surgical conversion.Univariate analysis revealed that cholangitis,medical history of aspirin/clopidogrel,giant diverticulum and the major papilla inside the diverticulum were risk factors for delayed hemorrhage following ERCP (P < 0.05).Multivariate analysis indicated that independent risk factors were associated with cholangitis (OR =4.125,95% CI:1.306-13.031,P < 0.05),medical history of aspirin/clopidogrel (OR =10.220,95% CI:2.997-34.853,P < 0.01) and major papilla occurred inside the diverticulum(OR =14.064,95% CI:1.888-104.762,P < 0.05).Conclusion Cholangitis,medical history of aspirin/clopidogrel and major papilla at the diverticulum may increase the risks for upper gastrointestinal bleeding in ERCP in patients with choledocholithiasis.However,the hemorrhage should be actively managed,especially in endoscopy,once the bleeding occurs.
10.The targeting expression of IL-2 gene driven by alpha fetoprotein promoter and hepatitis B virus envelope in hepatic carcinoma cell

Objective To observe if IL-2 gene can express in hepatic carcinoma cells by double targeting of hepatitis B virus envelope (HBVE) and alpha fetoprotein (AFP) promoter.Methods HepG2 cells,L02 cells,and HepG2.2.15 cells were cultured in vitro.HBVE was obtained by PEG8000 concentration assay,and the acquired HBVE was used to pack recombintional gene.Human AFP promoter-IL-2 recombinational gene was obtained by PCR.Then HepG2 cells and L02 cells were transfected by transient transfection and the expression of IL-2 was detected by RT-PCR and Western blot.Results IL-2 was detected in HepG2 cells by RT-PCR and Western blot but not in L02 cells.Conclusion By using HBVE as a gene transporter,human AFP promoter-IL-2 recombinational gene can express in hepatic carcinoma cells,thereby it can increase the safety of exogenous gene transfection of hepatic carcinoma cells.
