1.Dynamic expressions of synapsin Ⅰ and phosphorylated synapsin Ⅰ in hippocampus of vascular dementia rats

AIM:To observe the dynamic changes of synapsin I expression and its phosphorylation in hippocampus in vascular dementia(VD)rats.METHODS:Eighty SD rats were randomly divided into sham-operated group(n=40)and VD model group(n=40),and the latter were established by repeatedly clipping the common carotid arteries with an intraperitoneal injection of sodium nitroprusside solution in anesthetized condition.The synaptic ultrastructural changes in hippocampal CA1 region and the expression levels of synapsinⅠ and phosphorylated synapsinⅠin hippocampus were observed by TEM and immunohistochemical staining method respectively in both sham-operated group and VD model group at 15 d,1 month,2 months and 4 months time points.RESULTS:No obviously pathological changes to CA1 area synapse were found in SO group.In model group rats,synaptic circa membrane ambiguity and fusion,synaptic circa membrane structure decreased the postsynaptic density,reduced synaptic vesicles and vesicle cluster.Above pathological changes became gradually severe along with the time prolongation after model-making operation.Compared with sham-operated group,the expression of synapsin I significantly reduced in CA1 region(P0.05)in model group was observed.The number of p-synapsin I positive neurons in DG and hippocampal CA1 region was less in model group than that in sham-operated group(P0.05).CONCLUSION:The damaged synaptic structure and depressed expression of synapsin I and its phosphorylation in presynaptic parts of hippocampus induced by repeatedly cerebral ischemia/reperfusion may contribute to the synaptic transmission disorders,especially the presynaptic disorder which may be one of the important pathogenesis of the initiation and development in vascular dementia rats.
2.Climical perception for 100 cases valvular heart disease operated at the basic level hospital

Objective To summarize the experience of surgical treatment for 100 patients with valvular heart disease.Methods 100 valve patients were operated on from June 2002 to July 2006.The heart function(NYHA) was class Ⅱ in 24,class Ⅲ in 54,class Ⅳ in 22 including 7 patients with sever cardiac failure.74 patients had rheumatic disease,19 congenital disease,2 cases with SBE.7 aortic vavle degenerative disease including 1 case with ascending aortic dissection aneurysm,61 atrial fibrillation,10 left atrium thrombus and 3 coronary heart disease.Results There were low output syndrome in 14,including 2 death,4 reoperation for postoperative bleeding and 1 gasembolism in brain stem.1 case needed reoperation for peri-prosthetic leak half a year after discharge.In the follow-up period,91 cases of these patients were in NYHA class Ⅰ,4 cases were class Ⅱ,5 cases were recovering.Conclusions Result of vavle replacement surgical treatment is satisfactory.Pay attention to preparation before operation,accurate protection for myocardium and correct repair.Tricuspid repair should be alse considered actively.Eliminating gas in left heart before re-beat is important.
4.Antiviral efficacy of adefovir ester on fatty liver of HBeAg positive patients with chronic hepatitis B

Objective To investigate the impact of hepatic steatosis on the outcome of adefovir therapy in chronic hepatitis B patients with HBeAg positive. Methods The medical records of 154 patients who were diagnosed with CHB positive for hepatitis B e antigen (HBeAg) and received adefovir therapy for 24 weeks between January, 2011 and December, 2013 were analyzed retrospectively. Baseline status and virologic response, biochemical response and serology response between patient with (n = 68) and without (n = 86) steatosis who were proven by ultrasound. Both groups were given oral adefovir dipivoxil 10 mg/d for 24 weeks , recovery rate in ALT/AST, negative conversion rate of HBV DNA and HBeAg were recorded and compared. Results Recovery rate in ALT/AST in observation group was 67.7%, which is lower than control group (83.7%), and the difference is significant(P < 0.05). Negative conversion rate of HBV DNA and HBeAg in control group is 81.4%and 83.7%, which is significantly higher than observation group (whose values are 58.8% and 44.1%, P < 0.05). Conclusion Hepatic steatosis has significant impact on the outcome of adefovir treatment.
5.Impact of fatty liver on antiviral effect in chronic hepatitis B

Objective To investigate the impact of fatty liver on antiviral effect in the patients with chronic hepatitis B (CHB) .Methods A total of 204 patients with hepatitis Be antigen (HBeAg ) positive CHB in the outpatient department of our hospital from January 2011 to April 2014 were selected as the research subjects and divided into the two groups according to wheth‐er complicating fatty liver :the simple CHB group(control group ,122 cases) and CHB complicating nonalcoholic fatty liver disease (NAFID) group(observation group ,82 cases) .The two groups all received the 24‐week antiviral therapy of adefovir (10 mg/d) . The differences in the normalization rate of ALT/AST (biochemical response) ,HBeAg negative conversion rate (serological re‐sponse) and HBV DNA negative conversion rate(virological response) were compared between the two groups .Then the cases ob‐taining the virological response after treatment in the two groups were compared again .Among the cases obtaining virological re‐sponse,thedifferencesinthenormalizationrateofALT/AST(biochemicalresponse)andHBeAgnegativeconversionrate(serologi‐cal response) were compared between the simple CHB group and CHB complicating NAFID group .Results The normalization rate of ALT/AST after 24‐week treatment in the CHB complicating NAFID group was 58 .5% ,which was significantly lower than 72 .1% in the simple CHB group ,the difference was statistically significant ;the negative conversion rate of HBV DNA and HBeAg after 24‐week treatment in the simple CHB group was 67 .2% and 52 .5% ,which were significantly higher than 51 .2% and 37 .8%respectively ,the differences were statistically significant ;among the cases obtaining the virological response ,the normalization rate of ALT/AST in the simple CHB group was 96 .3% ,which was significantly higher than 66 .7% in the CHB complicating NAFID group ,the difference was statistically significant .But among the cases obtaining the virological response ,the negative conversion rate of HBeAg was 78 .0% in the simple CHB group and 73 .8% in the CHB complicating NAFID group ,the simple CHB group was slightly higher than the CHB complicating NAFID group without statistical difference .Conclusion NAFLD has impact on the antiviral effect of adefovir treatment in CHB patients with positive HBeAg ,whereas complicating NAFLD is an important reason for reaching the virological response and serological response ,but without reaching the biochemical response in the antiviral treatment of CHB complicating NAFID .
6.Argatroban in treatment of patients with transient ischemic attack

Eighty-three patients with transient ischemic attack (TIA) were randomly divided into study group (n =42) and control group (n =41) from July,2011 to Mar,2015.Patients in both groups were given aspirin 100 mg/d ; in addition,patients in study group received intravenous infusion of argatroban 10 mg,q12h,for 7 days and those in control group received subcutaneous injection of low molecular weight heparin (LMWH) 2 050 AXa IU,b.i.d for 7 days.The cases with cure,significantly effectiveness and effectiveness in study group and control group were 10,14,16 and 6,12 16,respectively.Compared with the control group,argatroban shows superiority in treatment of TIA with higher safety and lower side effect.
7.A METHODOLOGICAL STUDY FOR THE EVALUATION OF THE VITAMIN C STATUS IN HUMAN BODY BY THE RATIO OF REDUCED VITAMIN C TO CREATININE IN A RANDOM URINE SAMPLE

One hundred and ten college students, 66 males and 44 females, were studied for the evaluation of vitamin C status by use of the ratio of reduced vitamin C to creatinine in a random urine sample. During the experimental period, the dietary source of creatinine was strictly abstained. After voiding in the morning, the volunteers were weighed and their urine were collected at the end of 1st, 2nd, 4th, 8th, 12th and 24th hr respectively. The contents of reduced vitamin C and creatinine in each collected urine sample were analysed, and then the total amounts in 24 hrs were calculated.The experimental results showed:1. The distribution of the creatinine excretion in 24 hrs was nearly constant.2. The percentage of reduced vitamin C excretion per hour to the total 24 hrs excretion was similar in subjects of different vitamin C status and was lowest in 12 hrs of night urine. A curvilinear regression was given for the percentage of reduced vitamin C excretion in relation to the time.3. The ratio of reduced vitamin C (as mg) to creatinine (as gm) and the ratio of reduced vitamin C to creatinine (as mg) per body weight (as kg) were calculated from their contents in 12 hrs urine in those who exc- reted reduced vitamin C in urine less than 80 mg in 24 hrs. The relationship between these two ratios and the excretions of reduced vitamin C in 24 hrs urine was studied and four equations of curvilinear regressions were given.4. According to these equations, the correlated values of vitamin C/ crcatinine and vitamin C/creatinine/body weight in random urine samples in respect of four different grades of vitamin C excretion of 24 hrs urine were recommended.It was shown to be a new simplified method for evaluating the nutritional status of vitamin C in human body. The practical aspect of its application was discussed.
8.Relationship between interleukin-17 gene polymorphisms and outcomes of hepatitis B virus infection in He-bei Han population

Objective To explore the relationship between the polymophisms of interleukin?17 (IL?17)?197A/G and the outcomes of hepatitis B virus (HBV) infections. Methods The polymorphism of genotypes and al?leles of IL?17?197A/G in 222 chronic HBV infected patients and 88 HBV infected ones as controls. The contains of serum HBV DNA were detected using PCR combined with DNA amplification in vitro, serum IL?17 expression was detected with ELISA and genotype and allele frequency of IL?17?197A/G locus was detected by Beckman SNP kit. Results There was a significant difference in the serum IL?17 expression (F=158.1, r2=0.61, P<0.000 1) among all the groups. For IL?17?197A/G, there were significant differences for the comparison between the two groups, and the frequency of AA genotype was 47.72%in liver cirrhosis group. Moreover, the highest frequency of GG was in HBV clear?ance group (74.81%);and the lowest group was in chronic HBV liver cirrohosis group (19.05%). Conclusion IL?17?197A/G AA genotype and allele might be associated with susceptibility of HBV infection in Han population in Hei?bei Province.
9.Endothelial dysfunction in young pre-hypertension and its influencing factors

Objective To investigate the endothelial dysfunction in pre-hypertension and its influencing factors.Methods A total of 373 youth were divided as the subjects into hypertension group (HBP group),prehypertension group (PHT group) and normal blood pressure group (NBP group).Endothelial function was assessed based on carotid intima-media thickness (IMT),brachial artery flow-mediated dilation (FMD) and brachial-ankle pulse wave velocity (baPWV).Results IMT and baPWV in PHT group were higher than those in NBP group (P<0.05),but did not reach the significant difference when compared with HBP group (P>0.05).Compared with HBP,the levels of FMD in PHT group significantly increased (P< 0.05);however,no difference was observed in comparison with NBP group (P>0.05).In the early stage of hypertension,diastolic BP (β=-0.120,P<0.05) and body mass index (β=-0.115,P<0.05) were negatively correlated with FMD;diastolic BP (β=0.146,P<0.05),2-hour glucose (β=0.147,P<0.05),high-density lipoprotein cholestrol (β=0.150,P<0.05),and waist-hip ratio (β=0.126,P<0.05) showed a positive correlation with IMT.baPWV was correlated with systolicBP (β=0.358,P<0.01),waist circumference (β=0.254,P<0.05),fasting glucose (β=0.155,P<0.05),postprandial 2 h blood glucose (β =0.152,P <0.05),uric acid (β =0.206,P < 0.05),and C-reactive protein (β=0.099,P<0.05).Corclusion Our study shows that endothelial dysfunction may exist in the prehypertensive young,and several cardiovascular risks contribute to its development in the early stage of hypertension.
10.Clinical and electrophysiological diversity of the mesial temporal lobe epilepsy confirmed by stereotactic electroencephalogram

Objective To study the diversities of imaging, symptoms, electrophysiology and clinical value of the stereoelectroencephalography(SEEG) in patients with mesial temporal lobe epilepsy.Methods Eight patients with intractable epilepsy in Epilepsy Center of Yuquan Hospital of Tsinghua University who underwent mesial temporal lobectomy were recruited in this study, and their epileptic foci could not be accurately positioned.Therefore stereotactic brain electrodes were implanted, and their usual attack originated from mesial temporal lobe structure were confirmed.There was no seizure in the one year follow-up.Results Symptoms of the eight patients behaved differently, and the onset of the seizures in scalp electroencephalograph or SEEG showed diversities.Epileptic discharges were found originated from the mesial temporal lobe after implanting electrodes: in the early stage of discharges, four cases had the conduction to insular lobe structure;two cases had the conduction to contralateral mesial temporal lobe;one case had the conduction to retrosplenial cortex;one case had the conduction to parietal lobe;one case had the conduction to frontal lobe and rapid generalization (one case had the conduction to insular lobe and contralateral mesial temporal lobe meanwhile).Conclusions There is difference in clinic, imaging and electrophysiology of the patients with mesial temporal lobe epilepsy The non-specificity can be explained by the evolution of the intracranial electroencephalography, which can help us know its network conduction pattern Insular lobe is the most common conduction approach of mesial temporal lobe epilepsy in early stage SEEG can be used as a microinvasive, accurate preoperative localization method, which can help us to locate accurately and understand the discharges and conduction mode.
