1.Effect of Naoxintong Capsule on the Myocardial Perfusion and Platelet Activation Function in Patients with Acute Myocardial Infarction after PCI

OBJECTIVE:To explore the effect of Buchang naoxintong capsule on the myocardial perfusion and platelet activation function in patients with acute myocardial infarction (AMI) after PCI. METHODS:100 AMI patients were randomly divided into control group and observation group by parallel random sampling,50 cases in each group. Control group was given conventional PCI, preoperatively chewing 300 mg aspirin+300 mg clopidogrel,postoperatively taking 100 mg/d aspirin+75 mg/d clopidogrel. Observation group was additionally treated 2-4 with Buchang naoxintong capsules(after bracket release),3 times a day,the treatment course for both groups was 7 d,observation period was 1 month. Infarct-related artery myocardial perfusion grade(TMPG)in 2 groups,platelet activation function indexes [CD62P,CD63 and monocyte platelet aggregation (MPA) positive expression rates] before and after treatment,the incidences of adverse cardiac events (MACE) and complications were observed and compared 15 min after PCI. Meanwhile,the incidence of adverse reactions was recorded. RESULTS:After treatment,the cases of TMPG perfusion grade 3 in observation group were significantly higher than control group,the incidence of MACE was significantly lower than control group, with statistical significance(P<0.05);there was no significant difference in the incidences of complications and adverse reactions in 2 groups(P>0.05). Before treatment,there was no obvious difference in CD62P,CD63,MPA positive expression rates in 2 groups;after treatment,CD62P,CD63,MPA positive expression rates in 2 group were significantly lower than before,and observation group was lower than control group,with significant differences(P<0.05). CONCLUSIONS:In PCI treatment of AMI patients,Buchang naoxintong capsule can rapidly improve platelet activation functions,TMPG perfusion,reduce the incidence of MACE,with good safety and no increase of complications.

Objective:To explore improving impact of Sdosodine Hydrochlorida(SBHL) on immune fuction in mice treated with cyclophosphamide(CTX).Methods:BALB/c mice treated with CTX were administrated with SBHL(40 mg/kg) for 7 d.The weights of spleens and thymus of the mice,specific antibody formation of splenocytes,serum agglutinin titer and hemolysin CH50 in mice immunized with SRBC were assayed.The proliferation of mouse splenic lymphocytes actived with ConA were measured.The peritoneal macrophage phagocytosis in mice were analysized.Results:After treatment with SBHL,the indexes of thymus and spleen,and the proliferation of mouse splenic lymphocytes activated with ConA were increased markedly in a dose-dependent manner.The peritoneal macrophage phagocytosis was also improved.Only at the dose of 40 mg/kg were the weights of spleens of mice,specific antibody formation by splenocytes,serum agglutinin titer,hemolysin CH50 of the mice immunized with SRBC increased significantly.Conclusion:SBHL has a remarkable improving effect on the immune functions inhibited by CTX in mice,which may be another important antitumor mechanism of SBHL.

5.Research advances in pharmacological prevention of pancreatitis after endoscopic retrograde cholangiopancreatography

Endoscopic retrograde cholangiopancreatography (ERCP)has become an important method for the diagnosis and treatment of cholangio -pancreatic duct diseases.Post -ERCP pancreatitis (PEP)is the most common complication and its incidence has increased up to 40% in recent years.The prevention of PEP has always been a hot topic in clinical research.This article reviews the research advances in the pharmacological prevention of PEP and points out that controlled clinical trials with a large sample size are still needed to investigate the effect of preventive drugs,and further studies should focus on optimal medication time,route of administration,and dose.In addition,de-mographic features of populations from different countries and different age groups should be taken into consideration.
6.Correlation between fatigue and cognitive impairment and depression in patients with lacunar stroke: a retrospective case series study

Objective To investigate the correlation between fatigue and cognitive impairment and depression after lacunar stroke.Methods A total of 103 patients with lacunar infarction admitted from September 2009 to November 2010 were enrolled.Post-stroke fatigue was evaluated with the Fatigue Scale (FS-14) and the Fatigue Severity Scale (FSS-9); cognitive function was evaluated with the mini-mental state examination (MMSE) and the Montreal cognitive assessment (MoCA); and depression was evaluated with the self-rating depression scale (SDS) and the Hamilton Depression Rating Scale (HAMD).Results There were 34 patients (33.01%) (FSS-9) and 45 patients (43.69%) (FS-14) had fatigue.Of the 34 fatigue patients determined with FSS-9 scores,20 had cognitive impairment,26 had depression,and 16 had both cognitive impairment and depression.The Pearson correlation analysis showed that the FS-14 scores were siguificantly negatively correlated with the scores of MMSE (r =-0.307,P =0.002) and MoCA (r =-0.457,P=0.000),and significantly positively correlated with the scores of SDS (r =0.368,P =0.000) and HAMD (r =0.526,P =0.000); the FSS-9 scores were significantly negatively correlated with the scores of MMSE (r =-0.292,P=0.003) and MoCA (r=-0.340,P=0.000),and significantly positively correlated with the scores of SDS (r =0.403,P =0.000) and HAMD (r =0.564,P =0.000).Conclusions The incidence of fatigue,cognitive impairment and depression was higher.There was a certain correlation between fatigue and cognitive impairment and depression.
7.Therapeutic Observation of Electroacupuncture plus Ginger-partitioned Moxibustion for Sudden Hearing Loss

Objective To observe the clinical efficacy of electroacupuncture plus ginger-partitioned moxibustion in treating sudden hearing loss. Method Sixty patients with sudden hearing loss were randomized into a treatment group and a control group, 30 in each group. The treatment group was intervened by electroacupuncture plus ginger-partitioned moxibustion, while the control group was by electroacupuncture alone. The audiometry was conducted before and after intervention, and the therapeutic efficacy was also evaluated.Result Respectively after the first and second treatment course, the threshold of audibility was significantly changed in both groups (P<0.05); there were significant differences in comparing the threshold of audibility between the two groups (P<0.05). The total effective rate was 80.0% and 90.0% respectively after the first and second treatment course in the treatment group, versus 53.3% and 63.3% in the control group, and the differences were statistically significant (P<0.05).Conclusion Electroacupuncture plus ginger-partitioned moxibustion is an effective approach in treating sudden hearing loss.
8.Analysis of Clinical Features and Drug Sensitivity of 67 Newborn Infants with Klebsiella Pneumoniae

Objective To explore the clinical characteristics and drug sensitivity of newborn pneumonia of klebsiella pneumoniae,and investigate the positive rate of extended-spectrum ? lactamases(ESBLs)in klebsiella pneumoniae.Method Sixty-seven cases of sputum samples newborn pneumonia of klebsiella pneumoniae from Jan.2003 to Dec.2005 were collected.Bacteria identification and drug sensitivity test were performed.At the same time,ESBLs were detected.Results The clinical characteristics of newborn pneumonia of klebsiella pneumoniae were non special,and the positive rate of ESBLs in klebsiella pneumoniae was 17.91%.The klebsiella pneumoniae was 100%sensitivie to carbopenems,but resistant to commonly used ?-antibiotics such as penicillin and cephalosporin.ESBLs negative strains had higher drug sensitivity than ESBLs strains.Conclusions Partial klebsiella pneumoniae are ESBLs strains and insensitive to commonly used antibiotics.In order to decrease ESBLs strains,the antibiotics treatment shall be used appropriately and the selection of antibiotics shall be guided by the results of drug sensitivity tests.
9.Change of Brain Stem Auditory Evoked Potential in Newborn Infant with Hypoxic-Ischemic Encephalopathy after Hyperbaric Oxygen Therapy

Objective To investigate the therapeutic role of hyperbaric oxygen(HBO) on neonatal hearing impairment induced by hypoxic-ischemic encephalopathy(HIE),by observing the brain stem auditory evoked potential(BAEP) changes of newborn infants with HIE.Methods One hundred and six cases of HIE in newborn infants(in which 82 cases were mild and 24 cases were severe) treated with HBO and conventional therapy were chosen as HBO treatment group,while other 42 cases of HIE in newborn infants(in which 38 cases were mild and 4 cases were severe) treated with conventional therapy only were served as control group.BAEP was measured at the 3rd to 7th day on patients before treatment and 42 days after birth with or without HBO therapy respectively,and the differences of BAEP changes were compared between the both groups.Results 1.In control group,the rate of abnormality was 47.4% and 31.6%,respectively before and after therapy in mild HIE cases(P=0.240),and 75.0%,50.0% in the moderate-severe HIE cases(P=0.462).2.In HBO group,the abnormal rate was 45.1% and 25.6%,respectively in mild HIE cases before and after therapy,while 75.0% and 41.7% in moderate-severe HIE cases(Pa=0.014,0.039).3.There was obvious difference between control group and HBO group in mild HIE cases(P=0.013),while there was no significant change between the 2 groups in moderate-severe HIE cases(P=0.717).Conclusions Treatment with HBO combined with conventional therapy may improve the abnormal rate of BAEP in patients with HIE according to the present test,especially in mild HIE cases,thus earlier HBO therapy may be used to mitigate hearing impairment and improve prognosis induced by the disease.
10.Diagnosis and Treatment of Delayed Radiation-induced Brain Injury

Objective To study the diagnosis and treatment of delayed radiation-induced brain injury. Methods 21 patients with delayed radiation-induced brain injury were diagnosed by CT, MRI and MRS, and were treated with dehydration, corticosteroid, neuroprective drugs and hyperbaric oxygen or surgery. Results The symptoms of 66.7% patients were obviously improved, 19.0% patients partly recovered and the symptoms of 14.3% patients were not obviously improved. Conclusion The early diagnosis and reasonable treatment could decrease delayed radiation-induced brain injury to minimal extent. The operative treatment should be performed in the patients who had operative indicators. And it was important to prevent the occurrence of delayed radiation-induced brain injury.
