Objective To investigate the functional states of stroke patients using the International Classification of Functioning, Disability and Health (ICF) core sets for stroke. Methods Fifty in-patients with stroke participated in this study. Twenty-five patients less than one month after a stroke were assigned to the acute group, and another 25 patients with more than one month since their stroke were assigned to the sub-acute and chronic group. All the patients were assessed using the Glasgow coma scale, Rankin grading and the Barthel index. The ICF core set for stroke assessment was also used with both groups. The data was analyzed by using a Chi squared frequency test. Results Rankin grades and Barthel indices were significantly different between the two groups, and the functional states in the acute group were better than those in the sub-acute and chronic group. Some of the items, such as consciousness state, functions of structures adjoining the eyes, hearing, substitutive sound, accessory respiration, temperature regulation and protection function of the skin had no significant difference between the groups. The number of patients suffering from disorders of joint motion, joint stability, muscle endurance and control of voluntary movement was significantly greater in the sub-acute and chronic group than in the acute group. Both groups were influenced to different degrees by most of the environmental factors. Conclusion The ICF core sets correctly indicated joint motion, joint stability, muscle endurance and control of voluntary movement difficulties of stroke patients.

Recent studies have demonstrated that osteopontin (OPN) is a kind of agent that is sensitive to mechanical stress.OBJECTIVE: To study the role of OPN in mediating mechanical stress effect on bone, and to investigate the relationship of OPN and bone rebuilding, action mechanism of OPN and cell surface receptor and the relationship of OPN and osteoblast and osteoclast, in order to reveal the molecule mechanism of residual ridge resorption mainly caused by stress, and also to be a basic theory for clinical molecule therapy or gene therapy.RETRIEVE STRATEGY: PubMed was searched from January 1985 to June 2007 for English literatures with the searching words of "OPN, mechanical stress and bone resorption" and China Journal Full-text Database was also searched from January 1995 to June 2007 for Chinese literatures with the searching words of "OFN, mechanical stress and bone resorption". Fifty-two literatures were retrieved. The data were primarily selected. Inclusive criteria: being relative to role of OPN in mediating mechanical stress effect on bone. Exclusive criteria: repetitive studies.LITERATURE EVALUATION: Among the 52 literatures, including 6 Chinese and 46 English, 5 were rejected for objective independent from this study, 13 for repetitive contents, and so 34 literatures were involved for further analysis on the role of OPN in mediating mechanical stress effect on bone, 6 being review and the others being clinical or basic studies.DATA SYNTHESIS: ①After tooth extraction, residual ridge resorption is a complicate process. OPN, as a main protein, participates in the bone resorption and bone rebuilding. ⑤These OPN can be seen as a chemotatic factor to guide prosoma osteoclast to migrate to the surface of the bone, and mediate it to combine with the integrin on the surface of osteoclast so to promote the resorption of bone. So, OPN is a necessary factor in mechanical stress induced bone loss. ⑥OPN is a signal of transmission between osteoblast and osteoclast. ⑦OPN can be seen as a long acting feedback inhibition signal to inhibit bone mineralization.CONCLUSION: OPN is a kind of agent that is sensitive to mechanical stress. It plays an important role in mediating mechanical stress effect on bone.

Objective To investigate the relationship of the activation of nuclear factor kappa B(NF-kB) with myocardial neutrophil infiltration and injury in human open heart surgery,and to observe the inhibiting effect on the activation of NF-kB and protecting effect on myocardium of the coenzyme Q10,a scavenger of oxygen free radicals.Methods Forty-seven adult patients undergoing open heart surgery were randomly divided into two groups,the control and the treatment.CoenzymeQ10 tablets were given to the treatment group 5 days before operation.Biopsy of right atrium for myocardial pathology,activated NF-kB detection and ultrastructure observation were done prior to cardiopulmonary bypass,45 minutes of ischemia and 45 minutes of reperfusion.The dynamic indexes,vasomotor drug dosage,and outcomes were observed postoperatively.Results Upon 45 minutes of ischemia and 45 minutes of reperfusion,in control group there were neutrophil accumulation and adhesion of vascular endothelium,ultrastructural damages,and positive expression of NF-kB both in nuclei and cytoplasm,in myocardium.In treatment group,there were only mild neutrophil infiltration and ultrastructural damages,and weak positive expression of NF-kB both in nuclei and cytoplasm.However,the dynamic indexes,vasomotor drug dosage,and outcomes of two groups were not significantly different.Conclusion NF-kB plays an important role in pathophysiological process of myocardial ischemia and reperfusion in open heart surgery.CoenzymeQ10 has obvious inhibiting effect on activation of NF-kB and protecting effect on myocardium.

Objective To summarize the surgical skills of the pancreaticoduodenectomy via mesenteric approach for the treatment of duodenal gastrointestinal stromal tumors (GISTs).Methods The clinical data of 1 patient with huge duodenal GIST combined with gastrointestinal hemorrhage and liver metastasis who was admitted to the Second Affiliated Hospital of Harbin Medical University in June 2014 were retrospectively analyzed.The patient had continuous bleeding before the operation.The results of computed tomography (CT) showed that there was a solid tumor below the liver and in front of the right kidney,with the size of 12.2 cm × 8.1 cm,inferior vena cava was squeezed by the tumor,a solid tumor was detected in the right lobe of liver,and the colon was suspiciously invaded by the tumor.The patient received pancreaticoduodenectomy,right hemicolectomy and right lobectomy of liver metastases.The patient was followed up by out patient examination and telephone interview up to August 2014.Results The operation time,volume of blood loss and volume of red blood cell (RBC) transfusion were 420 minutes,800 mL and 2 U,respectively,with a full recovery of patients.The duodenal GIST with metastasis tumor and necrosis located at the colon,pancreatis,liver and renal hilum was confirmed by pathological diagnosis.The life quality of the patient was good by follow-up at postoperative month 3.Conclusion In the pancreaticoduodenectomy via mesenteric approach,surgeons perform vigorously the method of mesenteric approach at the first stage of operation,it could maximally reduce the risk of operation and the volume of blood loss.

Aim To investigate the effects of an angiotensin Ⅱ receptor blocker,candesartan, on aorta oxidative stress-LOX-1 pathway in salt-loaded stroke-prone spontaneously hypertensive rats (SHRSP).Methods 12-week-old salt-loaded SHRSP were treated with candesartan(1.0 mg?kg-1?d-1)or a diuretic, trichlormethiazide(TCM,1.6 mg?kg-1?d-1) or no treatment(n=6) in each for 2 weeks. Age-matched salt-loaded WKY rats were served as control(n=6).Systolic blood pressure(SBP)was measured weekly throughout the 2-week period by means of the tail-cuff method.Thoracic aortas were extracted and 24 h urine was collected.NAD(P)H oxidase subunits(p22 phox, p47 phox and gp91 phox)mRNA expression in aorta were assayed by real-time PCR. LOX-1 and type Ⅳ collogen mRNA expression were examined by RT-PCR. gp91 phox and LOX-1 protein expression in aorta were assayed by immunohistochemistry.Urinary albumin excretion was examined by ELISA.Results At the end of the 2nd week, SBP was significantly higher in salt-loaded SHRSP than that in salt-loaded WKY rats. Treatment with candesartan and TCM significantly decreased SBP in salt-loaded SHRSP at similar levels.NAD(P)H oxidase subunits (p47 phox and gp91 phox)and LOX-1 mRNA expression in aorta were markedly higher in salt-loaded SHRSP than those in salt-loaded WKY rats.Candesartan and TCM had the effect of reducing the systolic blood pressure at similar levels. OBJECTIVEEffects of ginsenoside Rb1, Rg1 and Re on neurotrophic factor signal transduction pathway using liposome-mediated transfection of eukaryotic cells approach.

METHODThe injury model was established by treating SH-SY5Y cells with 0.6 mmol x L(-1) of corticosterone (CORT) by 24 h. SH-SY5Y cell were pretreated with CORT for 30 min followed by co-treated with 120,60 and 20 micromol x L(-1) of Rb1, 120, 80 and 40 micromol x L(-1) of Rg1 and 120, 80 and 40 micromol x L(-1) of Re for 24 h. Cells viability was determined by Cell Counting Kit (CCK) assay. CREB expressing Luciferase reporter gene was constructed and transfected with plasmid containing hRaf, hcAMP, hAkt, hCaMK gene into human embryonic kidney (HEK293) cells using liposornal transfection reagent lipofection 2000. The expression of CREB before and after it addion of Rb1, Rg1 and Re was examined by Luc assay system and Western blotting.

RESULTCompared with normal control group, CORT significantly decreased the viability of SH-SY5Y cells to 67.21% (P < 0.01). CCK results show that Rb1 (60 micromol x L(-1)), Rg1 (80 micromol x L(-1)) and Re (80 micromol x L(-1)) on SH-SY5Y cells have significant protective effect (P < 0.01). Lucassay and Western blotting results show that the gene and protein levels of CREB increased significantly through the pathway of Raf and Akt with Rb1 and Rg1 (P < 0.01), Re can increase significantly the gene and protein levels of CREB through the pathway of Raf and CaMK II.

CONCLUSIONRb1, Rg1 and Re protects SH-SY5Y cells from CORT-induced damage and the neuroprotective mechanism may be associated with the Raf-CREB, Akt-CREB and CaMK II -CREB pathways.
