Objective To explore the relationship between the levels of plasminogen activator inhibitor-1(PAI-1) and type 2 diabetes mellitus (T2DM) with vascular complication. Methods The plasma concentrations of PAI-1 were determined by ELISA in 34 control subjects (Control group) and 134 diabetic patients (T2DM group),which were further subdivided into 4 subgroups based on body mass index (BMI) and with or without macro-vascular complication. Results The PAI-1 levels in four diabetic subgroups were significantly increased compared with those of the control. Objective To explore the causes of readmission after prostatectomy for BPH and to discuss the treatment approaches. Methods A total of 106 cases who were eligible for the study were reviewed.47 cases had residual and recurrent adenoma;of these cases,42 underwent TURP and 5 open prostatectomy again.32 cases developed urethral stricture;of these cases 27 underwent transurethral cold knife incision plus electric resection of scar and 3 urethral orifice reconstruction and 2 suprapubic cystostomy.11 cases who had experienced bleeding after TURP underwent transurethral bladder washing and coagulating.7 cases had neuropathic bladder dysfunction;of these cases 2 underwent transurethral bladder neck circle muscle resection,3 intermittent self-catheterization and 2 suprapubic cystostomy.6 cases had prostatic cancer;of these cases 1 underwent radical prostatectomy,3 palliative TURP plus orchiectomy and 2 orchiectomy.2 cases had incontinence after TURP;of the 2 cases 1 used external collecting device and 1 was transfered to other hospital for surgery.1 case had abdominal incision hernia and then underwent hernia repair. Results Among 106 cases, 97 cases underwent operations and of them 84 (86.6%) cases achieved successful results.The follow-up time was 6 months to 15 years with a mean of 5 years and 8 months.9 cases with urethral stricture required regular urethral dilation after surgery.4 cases who had had unsuccessful surgery underwent suprapubic cystostomy.Other 9 cases did not undergo operations due to poor health and other reasons. Conclusions Correct preoperative diagnosis, rational surgical type and adequate intraoperative and postoperative management are key to preventing readmission. Transurethral surgery is most favorable treatment choice for readmitted patients after prostatectomy for BPH.

Objective To evaluate the survival status and efficacy,prognosis and safety of several combined chemotherapies in previously untreated patients with follicular lymphoma (FL).Methods Clinical data of 62 previously untreated FL patients were analyzed retrospectively,in order to analyse survival status and to compare the efficacy and safety of different combined chemotherapies.Results The percent of FL patients achieved complete response (CR) after initial therapy was 80.0 ％ (44/55),while achieved for more than 5 years accounted for 23.6 ％ (13/55).Ten-year overall survival (OS) rate was 77.8 ％.The relapse rate was 34.5 ％,and the OS rate in patients with recurrence was significantly lower than in non-recurrence patients (93.8 ％ vs 61.6 ％,P =0.012).The disease-free survival (DFS) and progression-free survival (PFS)rates in FC/FMD (fludarabine,cyclophosphamide/fludarabine,mitoxantrone,dexamethasone) group comparing with CHOP (cyclophosphamide,epirubicin,vincristine,prednisone)-like group were significantly higher (80.0 ％ vs 21.1％ and 80.0 ％ vs 29.2 ％,P ＜ 0.05),but the differences had no statistical significance when adding rituximab (88.9 ％ vs 72.7 ％ and 90.0 ％ vs 73.5 ％,P ＞ 0.05).The OS,DFS and PFS rates in rituximab group were higher than those in the group without it (96.4 ％,79.2 ％ and 79.2 ％ vs 82.9 ％,39.3 ％ and 44.5 ％,P ＜ 0.05).The DFS and PFS rates in fludarabine group comparing with the group without it were significantly higher (86.2 ％ vs 49.4 ％,87.1％ vs 52.7 ％,P ＜ 0.05),but the differences between OS rates had no statistical significance (92.9 ％ vs 89.1％,P ＞ 0.05).The major adverse effects were hematologic toxicity,infection,nausea/vomiting and abnormal liver function.The hematologic toxicity of chemotherapies including fludarabine was stronger,but well-tolerated.Conclusion With initial combined chemotherapies,the CR rate of FL could receive a higher level.It is considered that chemotherapy might cure a part of FL patients thoroughly.Rituximab could increase the OS time significantly.FC/FMD regimen could increase the PFS time,comparing with CHOP-like regimen,while the difference has no statistical significance when adding rituximab.Systemic chemotherapies are well tolerated.

Two patients were admitted to the hospital for 2-month history of pruritic eruptions on the forehead and 2-week history of pruritie eruptions on the leg, respectively. Both patients had a history of pet contact. Topical application of glucocorticoids did not work well. Dermatological examination revealed a patch measuring 5 cm ×6 cm on the forehead of one patient and a patch measuring 2 cm × 3 cm on the leg of the other patient. Both patches were surrounded by red papules and scaling. Microscopic examination of skin scales revealed hyphae and chain-like spores, and culture of skin scales grew Microsporum gyeum. Both the isolates of Microsporum gyeum were sensitive to ketoconazole, miconazole, bifonazole, terbinafine, and voriconazole. Both patients were healed after treatment with oral terbinafine and topical ciclopirox olamine.

This article was aimed to study pretreatment method of styrene type macroporous resin and its application in the industrial production. The organic residues and heavy metals of styrene type macroporous resin were detected respectively by gas chromatography and atomic absorption spectrophotometry before and after pretreatment to optimize the pretreatment process. The results showed that contents of organic residues and heavy metals of styrene type macroporous resin with the preferred method of pretreatment are in line with relevant regulations. It was concluded that this pretreatment method was simple and feasible, which is suitable to be applied in the industrial production.

Objective To evaluate the effect of using serf-designed facial support in prone position for patients after vitrectomy and silicone oil tamponade. Methods 100 patients with vitrectomy and sili-cone oil tamponade were randomly divided into the experimental group and the control group with 50 pa-tients in each group. The self-designed facial support in prone position was used in the experimental group, while the routing nursing measure was used in the control group. The ocular adverse effect and re-lapse rate were compared between the two groups. Results The incidence rate of corneal edema, in-creased intraocular pressure and retinal redetachment in the experimental group was higher than those in the control group, the time of keeping position in the experimental group was longer than that in the con-trol group. Conclusions Self-designed facial support in prone position can decrease discomfort and pain caused by compulsive position, reduce the incidence of complications, improve retinal attachment and increase the successful rate for the patients with vitrectomy and silicone oil tamponade.

Objective To study the elongation factor 1α(EF-1α) gene functions in prostate cancer cell line DU145 in the aspects of cell proliferation and clone formation by using the RNA interference technique. Methods DU145 cell lines were divided into control group, transfection control group transfected with scramble siRNA and experimental group transfected with EF-1α siRNA. After transfecting EF-1α siRNA into DU145 cell line, the down-regulation of EF-la expression in DU145 cell line was confirmed by Western blotting and immunofluorescence staining. Then, the cell proliferation and clone formation assays were carried on in these 3 groups of DU145 cells. Results Compared with controls, the specific down-regulation of EF-1α expression was achieved in experimental group only. Compared with control group, after the down-regualtion of EF-1α in DU145 cell line, the cell proliferation rate decreased from day 4 to day 7 after transfection by 45. 9%, 53. 5% , 35. 3% and 38. 1% , respectively(P<0. 05). The clone formation number in experimental group decreased by 67.0% (P<0. 01). Conclusions The down-regulation of EF-1α has a negative impact on prostate cancer cell proliferation and clone formation. EF-1α might be an appropiate targeting gene in prostate cancer targeting therapy.

Objective To study the effect of down-rdgulation of EF-1 alpha gene in prostate cancer cell line DU-145 on cancer cell migration and invasion by using RNA interference technique. Methods The prostate cancer cell line DU-145 was divided into three groups: the control group (untransfected with siRNA), randomly control group (randomly transfected with siRNA) and experimental group (transfected with EF-1 alpha siRNA). Localization of EF-1 alpha and its relationship with F-actin in cytoplasm were analyzed by immunofluorescence technique. Cancer cell migration and invasion capability of DU145 cells were studied by transwell technique in these three groups. Results EF-1 alpha expression in DU145 cell line was down-regulated by using RNA interference technique. EF-1 alpha was localized in cytoplasm and co-located with F-actin. The down-regualtion of EF-1 alpha did not change the F-actin distribution in cytoplasm. The cell migration and invasion study showed that after seeding 20×104 DU145 cells into the upper chamber of transwall for 12 hours, the cells collected in the lower chambers were (10.6±1.0)×104 in control group, (11.2±0.8)×104 in randomly control group and (3.9±0.6)×104 in experimental group. Compared with controls, the cancer cell migration and invasion capability was significantly inhibited to only 37.1% (t= 13.9, P<0.05) after the specific down-regulation of EF-1 alpha expression in DU145 cells. Conclusions The down-regulation of EF-1 alpha expression has negative impacts on prostate cancer cell migration and invasion. EF-1 alpha plays important roles in prostate cancer local invasion.

OBJECTIVE To understand the distribution of common pathogens and their drug resistance trend and to provide the basis of the correct selection of antibiotics for clinic.METHODS Retrospective analysis was taken for blood culture specimens during 2 years in our hospital.And statistical analysis was done.All of 4028 cases of blood culture specimens were detected on automatic BacT/Alert3D rapid blood culture system,strains isolated were taken to VITEK-2 automatic microbiological analysis/sensitivity system for identification and drug susceptibility testing.Drug susceptibility results were analyzed using WHONET 5.3 software.RESULTS Totally 435 pathogen were isolated from 4028 cases of blood culture sample and the positive rate was 10.8%.195 Gram-negative bacteria strains accounted for 44.8%,mainly Escherichia coli,Klebsiella pneumoniae,Burkholderia cepacia,Acinetobacter baumannii,and Pseudomonas aeruginosa.Gram-positive bacteria accounted for 49.9%;mainly Staphylococcus epidermidis,S.haemolyticus and S.aureus dominated.Fungi were 23 strains(5.3%),mainly Candida albicans.Among them,extended-spectrum ?-lactamases(ESBLs) produced by E.coli and K.pneumoniae strains were 43.9% and 48.8%,respectively.Meticillin-resistant S.aureus and coagulase-negative staphylococci were 35.3% and 72.9%,respectively.But vancomycin-resistant S.aureus was not found.CONCLUSIONS The bacteria identification detected in blood culture class is quite complicated,and the drug resistance is high.Laboratories should increase the detection rate of bacterial culture and provide drug monitoring results for the clinics on time based on CLSI norms.Clinicians should use the antibiotics reasonably based on the drug susceptibility results in order to reduce nosocomial infections and the emergency of multiple drug-resistant strains.

Objective To evaluate the performance of VITEK 2-Compact GN13 methods for testing imipenem susceptibility of Klebsiella pneumoniae and assess the reliability of its Advanced Expert System (AES).Methods A retrospective study was conducted with a total of 157K. pneumoniae strains, which were isolated from blood and intra-abdominal infections in the First Affiliated Hospital of Nanchang University from 2014 to 2015. Thein vitro minimum inhibitory concentration (MIC) values of imipenem were determined by disc diffusion, VITEK 2-Compact GN13 and broth microdilution methods, respectively. Categorical agreement (CA) rates of disc diffusion and VITEK 2-Compact GN13 methods were determined using broth microdilution as reference method. The genes encoding ESBLs and carbapenemase were screened by PCR and sequencing analysis. The phenotypic confirmatory tests such as modified Hodge test, PCR and DNA sequencing were used to confirm the resistance mechanism and evaluate the reliability of AES in interpreting the imipenem susceptibility of K. pneumoniae.Results Among the 157 isolates, 64 and 8 were identified as resistant and intermediate strains by broth microdilution method, respectively; 52 and 10 were tested as resistant and intermediate strains by disc diffusion method, respectively; 54 and 13 were determined as resistant and intermediate strains by VITEK 2-Compact GN13 method, respectively, while 70 and 3 were judged as resistant and intermediate strains by VITEK 2-Compact GN13 method plus AES validation. The CA of disc diffusion and VITEK 2-Compact GN13 methods compared with broth microdilution method were all higher than 90 %. However, the major error (ME) rate was 3.8 % and very major error (VME) rates were all 0.6 % in imipenem susceptibility testing by VITEK 2-Compact GN13 and disc diffusion. The imipenem susceptibility of 16 strains were modified by the AES, which eliminated 0.6 % VME, but increased major error by 1.3 % and minor error by 1.9 %. Phenotypic confirmatory tests showed that 75 % (12/16) of these strains were validated as producers of both ESBLs and carbapenemase, which was consistent with the result of AES validation. PCR and DNA sequencing analysis proved that 62.5 % (10/16) of these strains produce IMP-4/KPC-2 /NDM-1 and ESBLs.Conclusions Both disc diffusion and VITEK 2-Compact GN13 methods can be used for testing the imipenem susceptibility of K. pneumoniae isolates with reliable and accurate results. Attention should be paid to the possibility of ME and VME when testing imipenem susceptibility. The VME can be avoided by the AES mechanism. However, AES intervention will increase ME and minor error, which may be associated with decreased expression of carbapenemase.
