2.Significance of the expression of matrix metalloproteinases-7 in benign and malignant thyroid tissues

Objective To investigate the expression of matrix metalloproteinases-7 (MMP-7) in benign and malignant thyroid tissues and its clinical significance. Methods The expression of MMP-7 in 50 thyroid cancers, 45 adenoma and 20 adjacent noncancerous portion tissues were studied by microwave-LSAB immunohistochemical method. Results The positive rate of MMP-7 in thyroid cancer was 64. 0%, which was significantly higher than 37. 0% in adenoma and 25.0% in adjacent noncancerous portion(X_2=8. 72,6. 52, P < 0.01 ). No correlation was observed between the expression of MMP-7 and histological grading of thyroid cancer. The expression of MMP-7 in lymph node metastasis and Grade Ⅲ~Ⅳ were both higher than that in negative cases and in Grade Ⅰ~Ⅱ ( P < 0. 05 ). The percentage of recurrence and death in MMP-7 positive cases were notably higher than that in negative cases( P <0. 05). Conclusion The expression of MMP-7 can be regarded as a parameter for evaluating the degree of malignancy, biological behavior and prognosis of thyroid cancer.
3.Protecting myocardium by inhibiting the activation of nuclear factor kappa B in open heart surgery

Objective To investigate the relationship of the activation of nuclear factor kappa B(NF-kB) with myocardial neutrophil infiltration and injury in human open heart surgery,and to observe the inhibiting effect on the activation of NF-kB and protecting effect on myocardium of the coenzyme Q10,a scavenger of oxygen free radicals.Methods Forty-seven adult patients undergoing open heart surgery were randomly divided into two groups,the control and the treatment.CoenzymeQ10 tablets were given to the treatment group 5 days before operation.Biopsy of right atrium for myocardial pathology,activated NF-kB detection and ultrastructure observation were done prior to cardiopulmonary bypass,45 minutes of ischemia and 45 minutes of reperfusion.The dynamic indexes,vasomotor drug dosage,and outcomes were observed postoperatively.Results Upon 45 minutes of ischemia and 45 minutes of reperfusion,in control group there were neutrophil accumulation and adhesion of vascular endothelium,ultrastructural damages,and positive expression of NF-kB both in nuclei and cytoplasm,in myocardium.In treatment group,there were only mild neutrophil infiltration and ultrastructural damages,and weak positive expression of NF-kB both in nuclei and cytoplasm.However,the dynamic indexes,vasomotor drug dosage,and outcomes of two groups were not significantly different.Conclusion NF-kB plays an important role in pathophysiological process of myocardial ischemia and reperfusion in open heart surgery.CoenzymeQ10 has obvious inhibiting effect on activation of NF-kB and protecting effect on myocardium.
4.Significance of expression of SLeX and p16 in papillary thyroid carcinoma and their relationships

Objective To investigate the correlation of expression of SLeX and p16 gene protein with the potential of invasion and metastasis in papillary thyroid carcinoma(PTC). Methods The expression of SLeX and p16 gene protein in 69 cases of PTC were studied by microwave-labelled strept-avidin biotin immunohistochemical method. Results The positive rates of SLeX and p16 in PTC were 69.6% and 58.0% respectively. The posi-tive rate of SLeX expression in the patients with tumor invasion and lymphatic metastasis was significantly higher than that without tumor invasion and lymphatic metastasis (P<0.05). But the positive rate of p16 expression in the patients with tumor invasion and lymphatic metastasis was significantly lower than that without tumor invasion and lymphatic metastasis (P <0. 05). There was a negative correlation between SLeX and p16 expression in PTC (P <0. 05). Conclusions Expression of SLeX and p16 protein were closely correlated with the invasive and metastasic potential in PTC. SLeX and p16 could be considered as prognostic indications for PTC.
5.Stability Study on 3 Pieces of Chinese Medicinal Formula after Decoction

OBJECTIVE:To preliminarily study the stability of 3 pieces of Chinese medicinal formula(CMF)after decoction, and provide reference for guaranteeing storage quality of decocted liquid and improving safety of drug use. METHODS:3 represen-tative formulas of Gegen Huangqin Huanglian decoction(A formula),Wuling powder(B formula)and Didang decoction(C for-mula)from Shanghan Zabing Lun were selected,the decocted liquid were stored under ambient temperature(25 ℃)and refrigerat-ed temperature (4 ℃) after decocting by automatic boiling-machine and packing. The microorganism,precipitation,pH and con-tents of total flavonoids,alkaloid,polysaccharide,total protein after 1,7,14,21,28 d were detected. RESULTS:Compared with the first day,contents of total flavonoids,polysaccharide in formula A at ambient temperature group were significantly in-creased on the 28th(P<0.05),content of polysaccharide in refrigerated temperature group was significantly increased(P<0.05). Content of polysaccharide in formula B at ambient temperature group was significantly decreased(P<0.05). The pH and content of total flavonoids in formula C at ambient temperature group and refrigerated temperature group were significantly increased (P<0.05 or P<0.01). Other indexes showed no obvious changes during the trial period. CONCLUSIONS:Under ambient temperature and refrigerated temperature,liquid ingredients of above decocted CMF will change when storing for 4 weeks. It indicates that the storage time of decocted CMF should not be more than 3 weeks.
6.Effect of electroacupuncture on the rat model of type 2 diabetes mellitus combined with renal hypertension

Objective To observe the therapeutic effect of acupuncture therapy for the rat model of type 2 diabetes mellitus (T2DM) combined with renal hypertension and try to explore its mechanism. Methods We randomly select 10 Wistar rats as the blank group and 40 rats were used to make the model groups, which were divided into simple diabetes group, simple renal hypertension group, the compound group with electroacupuncture and the compound group without elec?troacupuncture, with 10 rats in each group. After a high fat and sugar diet for 4 weeks, the Wistar rats were given strepto?zotocin i. p. injection to establish models of type 2 diabetes mellitus. Renal hypertension was developed by the“2K1C” im?proved method to make unilateral renal artery ligation?induced renal artery stenosis. Then, electroacupuncture treatment was performed on the rats for 2 weeks except the compound group without electroacupuncture. The changes of values of BP, FBG, Cr, BUN, glycated hemoglobin, renin and Ang II were recorded and analyzed. Results The values of BP, FBG, Cr, BUN, glycated hemoglobin, renin and Ang II in the compound group with electroacupuncture showed a significant re?duction compared with the compound group without electroacupuncture after 2 weeks (P<0?01), but there was no obvious changes in the values of Cr and BUN(P>0?05). Conclusions The blood glucose and blood pressure in the rat model of compound group can be reduced to a normal level with continuous electroacupuncture at bilateral acupoints Zusanli, and it can also be kept at a stable level after single electroacupuncture for 2-3 days. The acupuncture therapy is more suitable for early clinical treatment and can be used in basic research with advantages of economic, safe, no side effect and so on.
7.Preparation of genistein-loaded chitosan microspheres.

OBJECTIVETo optimize the preparation of genistein chitosan microspheres with central composite design (CCD).
METHODThe chitosan microspheres were prepared by the O/W/O multiple emulsion method. Second-order polynomial and linear equations were fitted to the data, and the resulting equations were used to produce response surface graphs and the best experiment conditions.
RESULTThe theoretical drug content was 13%-15%, the concentration of organic phase was 30%-40% and the concentration of oil phase was 68%-72%.
CONCLUSIONThe best experiment conditions can be obtained by central composite design and response surface methodology. The observed values agree well with model predicted values.

9.Effect of Drug Pair of Rhizoma Polygoni Cuspidati and Ramulus Cinnamomi on TLR4/MyD88 Signal Transduction Pathway of Rats with Acute Gouty Arthritis

Objective To observe the effects of the drug pair of Rhizoma Polygoni Cuspidati ( Huzhang) and Ramulus Cinnamomi ( Guizhi) on the Toll-like receptor 4 mediated myeloid differentiation factor 88 ( TLRs/MyD88) signaling pathway of rats with acute gouty arthritis induced by monosodium sodium urate (MSU) , so as to explore its therapeutic mechanism. Methods Forty-eight male SD rats were divided into normal group, modele group, blank plasmid group, positive plasmid group, Huzhang- Guizhi herb-pair (7 g/kg) group, and Huzhang-Guizhi herb-pair ( 7 g/kg) siRNA group, 8 rats in each group. The normal group, plasmid groups and model group were given physiological saline, and the left groups were given the corresponding drug by intragastric administration for 10 continuous days ( once daily ) . On the seventh day of intragastric gavage, acute gouty arthritis were induced by injection of MSU into the rat ankle joint, and normal group was injected with the samevolume of normal saline. Positive plasmid group and Huzhang-Guizhi herb-pair siRNA group were injected with the constructed siRNA-TLR4 plasmid targeting TLR4 gene ( TLR4-siRNA) to inhibit the in-vivo TLR4 gene expression. Pathological changes of the synovial tissues were detected, the contents of peripheral blood tumor necrosis factor alpha ( TNF-α) and interleukin 1 beta ( IL-1β) were detected by double antibody sandwich method, and the mRNA and protein expression levels of TLR4, MyD88, TNF receptor-associated factor 6 ( TRAF-6) in peripheral blood mononuclear cells of rats were detected by real-time fluorescence quantitative polymerase chain reaction ( PCR) and Western blot methods. The nuclear factor kappa B ( NF-κB) p65 immunoactivity was assayed by immunohistochemistry. Results Compared with the normal group, the model group had obvious hyperplasia of synovial cells and the inflammatory cell infiltration ( dominated by lymphcytes and monocytes) , and had amount of cellulose adhesive on the synovial membrane surface. Compared to the model group, positive plasmid group, Huzhang- Guizhi herb-pair group and Huzhang-Guizhi herb-pair siRNA group could obviously relieve the inflammatory cell infiltration, and improve synovial cell proliferation reaction. Compared to the normal group, serum levels of TNF-α and IL-1β, and the expression levels of TLR4, MyD88, TRAF-6 mRNA and protein in the peripheral blood mononuclear cells as well as the synovial NF-κB p65 ex pression in the model group were significantly increased ( P<0.01). Compared to the model group, positive plasmid group, Huzhang-Guizhi herb-pair group and Huzhang- Guizhi herb-pair siRNA group showed significant decrease in the levels of TNF-α, IL-1β, TLR4 MyD88, TRAF-6 and NF-κB p65 ( P<0.05 or P<0.01) . Conclusion Huzhang-Guizhi herb-pair can regulate the cytokines of the synovial membrane tissue in acute gouty arthritis rats, which may be related with its effect on inhibiting abnormal activation of TLR4-MyD88-NF-κB pathway in synovial tissue.
10.The problems and solving countermeasures in application of nursing vertical management in five hospital in Beijing

Objective To explore the problems and solving countermeasures in application of nursing vertical management,and provide references for nursing post management reform.Methods Through purposive sampling method,nursing administers,clinical nurses,heads of clinical departments and performance directors from five hospitals were interviewed by semi-structured interview method.Data were analyzed using 7-step Colaizzi's phenomenological analysis method.Results Four themes were formed about the problems in application of nursing vertical management,including:affecting department management and doctor-nurse collaboration,insufficient scientificity and fairness of performance evaluation,significant issues regarding post setting of senior nurses,competence and sense of belonging of temporarily deployed nurses.Four themes were formed about the solving countermeasures in application of nursing vertical management,including:improving participation of clinical departments in nursing management,making performance system to public and asking advices of nurses,providing career development for senior nurses,and conducting standardized training and establishing reserve nurses tank.Conclusion The main problems in application of nursing vertical management consisted of affecting department management and doctor-nurse collaboration,insufficient scientificity and fairness of performance evaluation,post setting of senior nurses,and competence and sense of belonging of temporarily deployed nurses.We suggested to better apply nursing vertical management by improving participation of clinical department in nursing management,making performance system to public and asking advices of nurses,providing career development for senior nurses,and conducting standardized training and establishing reserve nurses tank.
