1.The application of combined detection of autoantibodies in primary Sj(o)gren's syndrome

Objective To compare the specificity and sensitivity of antibodies against SSA and SSB,anti-M3 receptor polypeptide antibodies,anti-α-fodrin(IgG)antibodies in primary SjSgren's syndrome (pSS).Methods One hundred and ten pSS patients(mean age was 49.2±14.8.mean disease duration was 5.6±4.6),80 systemic lupus erythmatosis(SLE)patients(mean age was 25.5 4-4.6,mean disease duration was 2.5±1.2)and 80 rheumatoid arthritis(RA)patients(mean age was 44.6±3.5.mean disease duration was 4.2±1.1)were studied.Enzyme-linked immunosorbent assay was used to measure these antibodies.Results The seropositive rates of anti-SSA,anti-SSB antibodies,anti-M3 receptor polypeptide antibodies and anti-α-fodrin(IgG)antibodies were 45.5%,30.9%,78.2%and 77.3%,respectivelv in pSS.They were much higher than those in RA and SLE patients(P<0.05).Specificities of SSA、SSB antibodies,anti-M3 receptor antibodies and anti-α-fodrin IgG were 83.8%,97.7%.92.0%and 90.O% respectively.With the combination of these antibodies in the diagnosis of pSS.the Sellsitivity can be increased at least to 88.2%and the specificity was not decreased significantly.Conclusion Combination of these antibodies can significantly improve the sensitivity of these antibodies in the diagnosis of pSS.Anti-SSA and SSB antibodies,anti-M3 receptor antibodies and anti-α-fodrin(IgG)antibodies are specific antibodies for the diagnosis of pSS.
2.RELATION BETWEEN CX43 EXPRESSION AND APOPTOSIS AND PROLIFERATION IN DEVELOPMENT OF STRESS ULCER

To determine the effect of CX43 expression on gastric mucosa apoptosis and proliferation under stress conditions, a water immersion-restraint stress(WRS) model was performed with SD rats. Apoptotic cells were quantitated in gastric mucosa by terminal deoxynucleatidyl transferase-mediated dUTP nick and labelling (TUNEL) techniques. Cell proliferation was detected by immunohistochemical staining for proliferating cell nuclear antigen (PCNA). The expression of CX43 mRNA was detected by in situ hybridization. The results revealed that in control rats not exposed to WRS, apoptotic cells were seen only occasionally at the surface epithelium, The PCNA cells occurred in the proliferation zone of the gastric glands. The CX43 mRNA predomination occurred in the zone of the gastric glands. In contrast, the apoptotic quantities rose significantly 2h to 5h after the termination of WRS and reach a peak gradually, while PCNA proteins and the CX43 mRNA decreased significantly. At 8~12h after WRS the apoptotic cells gradually declined, while the PCNA cells increased to reach a peak. The CX43 mRNA almost returned to normal level and then increased to reach a peak. At 24h after WRS, the apoptotic cells remained significantly higher than normal level while the expression of PCNA proteins and CX43 mRNA returned to the values observed in the intact rats.These results suggested the expression of CX43 might associate with cellular differentiation, proliferation and the stabilization of glandular zone and affect the apoptosis of the surface epithelium.
3.Effect of several anti-ulcer drugs on the apoptosis and proliferation of gastric mucosa cells in development of stress ulcer in rat

Objective To investigate the effect of antiulcer agents Omeprazole, Misoprostol and Talcid on the gastric mucosa lesion by analyzing the gastric mucosa cell apoptosis and proliferation and CX43 transcriptional expression. Methods Mucosa lesion were evaluated by UI, 32 mice were randomly divided in equal numble into Omprazole (O), Misoprostol (M), Talcid (T), Comparison (C). Apoptosis cells in gastric mucosa were quantitated by terminal deoxynucleatidyl transferase mediared dUTP nick and labelling (TUNEL) techniques, while the expression of PCNA proteins were detected by immunohistochemical staining, CX43 mRNA was detected by In situ hyberdization. at 2h after WRS. Results We found that in comparison with control group, the group of pretreatment with Omeprazole, Misoprostol and Talcid showed a significant reduction in damaged mucosa and epithelial cell apoptosis and increase in the expression of PCNA proteins, and the effect of Omeprazole and Misoprostol was better than Talcid. All three antiulcer agents increased CX43 mRNA expression, the effect of Misoprostol and Talcid was better than Omeprazole. Conclusions Omeprazole, Misoprostol and Talcid can attenuate the multiple gastric mucosa lesion induced by WRS. It may be the ultimate way to prevent and cure stress ulcer by exerting direct cytoprotective effect to improve the cell ability and inhibit stress injury.
4.Influence of Inhaled Ambroxol Hydrochloride and Budesonide Suspension on Partial Pressure of Oxygen and Partial Pressure of Carbon Dioxide of Premature

Objective To explore the effect of inhaled ambroxol hydrochloride(mucosolvan) and budesonide suspension(pulmicort respules) on the prevention and treatment of neonatal respiratory distress syndrome(NRDS).Methods Ninty-two preterm infants without asphyxia and mother suffered from illness,were selected from neonatal intensive care unit(NICU) in Jiangyin hospital from Oct.2003 to Mar.2006.Their gestational age ranged from 29 to 33 weeks and they were all cured within 2 hours after birth.They were divided randomly into treatment group(group A) and control group(group B) at equal number.All of them were given oxygen treatment,aminophylline,VitK1,nutrition support treatment and kept in infant incubator.Every 8 hours,inhaled ambroxol hydrochloride(10 mg/kg) and budesonide suspension(0.25 mg) driven by oxygen were given to group A.Meanwhile,only ambroxol hydrochloride(10 mg/kg) were given to group B.After 3 days,the data of p(O2) and p(CO2) of premature were detected at d1,2,3 in 2 groups.Result At d2,d3,the arterial blood P(O2) in group A were(8.01?0.62),(9.25?0.76)kPa respectively;At d2,d3,arterial blood P(O2) value in group B were respectively(7.63?0.59)kPa,(8.75?0.63)kPa.By that statistics handles two set of preemie d2,d3 arterial blood P(O2)value,difference had notable.Group A preemie d2,d3 arterial blood P(CO2) value were respectively:(4.55?0.58),(4.20?0.51)kPa;group B preemie d2,d3 arterial blood P(CO2) value were(4.87?0.67)kPa,(4.44?0.59)kPa respectively.By that statistics handles two set of preemie d2,d3 arterial blood P(CO2) value,difference had notable.Conclusion Inhaled ambroxol hydrochloride and budesonide suspension treatment has be well effect in NRDS.
5.Glycosyl isomerization based on the biosynthesis of natural-product sugar from microorganism.

Glycosylation, one of the most common and important reactions in biological systems, results in diverse functions and is often found in biologically active small-molecule natural products produced by microorganisms. Furthermore, sugar moieties are generally critical for their activities. Alternating the sugar structures thus provides the potentials for enhancing the biological activities of natural products, which evokes researchers to study the sugar biosynthetic machinery and its application in the modification of sugar moieties with an aim of generating unnaturally glycosylated natural product drugs with better activities. This review will briefly outline current studies on sugar biosynthesis and glycosyltransferase, with a few selected experiments designed to alter natural-product sugar structures.
6.Survival and differentiation of human embryonic neural stem cells in rats with brain injury

BACKGROUND:Finding of neural stem cells(NSCs)brings new hope for repairing central nervous system(CNS)injury.However,the influence of internal environment after brain injury on the survival and differentiation of NSCs is a complexand variable process.OBJECTIVE:To observe the survival and differentiation of human embryonic NSCs following implantation into rats with fluid percussion brain injury.DESIGN:Open experiment.SETTING:Department of Neurosurgery,Guangdong Provincial Hospital of Traditional Chinese Medicine;Beijing Institute of Neurosurgery.MATERIALS:This experiment was carried out In the Laboratory of Neural Stem Cells,Beijing Institute of Neurosurgery from September 2002 to March 2003.Twenty-four female SD rats,aged 7 weeks,with body mass of(250±10)g,were provided by the Experimental Animal lnstitute, Chinese Academy of Medical Sciences[License Nd. SCXK(Jing)2002-2003]. Cerebrum of 8-week aborted fetus was obtained (Informed consents were obtained from parturients and their relatives). Fetal survival was monitored by B ultrasonic wave during abortion. BrdU monoclonal antibody(Sigma Company),rabbit anti-nidogen polyclonal antibody(Chemicon Company),mouse anti-microtubule-associated protein 2 (MAP-2)monoclonal antibody(Neomarkers Company),rabbit anti-gliaI fibrillary acidic protein(GFAP)polyclonal antibody (Biogenex Company).METHODS:① CerebraI cortex cells of 8-week aborted human fetus was harvested and cultured in vitro for obtaining human embryronic NSCs.②Rat models of hydraulic impact injury were developed.Bone window of motor sensory area of cerebral cortex was set at 2.5mm posterior to bregma which was zero point and 3.0 mm lateral to midline.Hydraulic impact injury parameters were set as impact pressure 0.3 MPa. impact time 25 ms and impact time once. ③BrdU-labeled human embryronic NSCs were implanted into injured area al 24 hours after injury.After 1 and 4 weeks.rats were sacrificed.Adjacent sections were doubly stained bv BrdU/MAP-2 and BrdU/GFAP.MAIN OUTCOME MEASURES:①Survival and immigration of implanted human embryonic NSCs.②Differentiation of implanted human embryonic NSCs.RESULTS:①BrdU-positive cells were oval and brown.At 1 and 4 weeks after implantation,BrdU-positive cells survived and migrated,and they migrated more widely at 4 weeks after implantation. ②At 1 week after implantation,more BrdU-positive cells were found in the subcortical granular layer and subcortex,and BrdU/MAP-2-positive cells were more than BrdU/GFAP-positive cells;At 4 weeks after implantation,BrdU-positive cells were significantly reduced,and found in choroid plexus and blood capillary,and BrdU/GFAP-positive cells were more than BrdU/MAP-2-positive cells.CONCLUSION:Implanted human embryonic NSCs can survive in the region of brain injury.gradually differentiate into astrocytes during rehabilitation and are easily digested by endothelial phagocytes.It indicates that immunological rejaction possibly influences the survival of human embryonic NSCs.
7.Factor Analysis of Nurses Observation Scale for Inpatinent Evalu ation

Objective: To improve clinical utility of the Nurses Observation Sca le for Inpatient Evaluation (NOSIE-30) in China. Methods: Based on the original scale, which only provides names and ranked scores for items, we developed clear definitions for each item and anchoring descriptions for score rating. 164 firs t-episode schizophrenic patients were evaluated with this scale every two weeks after admission. A total of 994 evaluations were completed. The collected data w ere randomly divided into two independent data sets. The number and content of t he subscales were revised based on principal component factor analysis and item analysis of the first data set. The revised scale was then evaluated and compare d with the original 7-subscale version of the instrument using the second data s et. Results: Factor analysis found that the factor structu re of the revised scal e was markedly different from that of the original instrument. Item analysis res ulted in elimination of 30 original items and redistribution of the remaining 26 items into 5 subscales. Comparison of the revised scale with the original instr ument revealed better psychometric properties for the former than the latter.Conclusion: The subscale scores of the original 7-subscale v ersion of NOSIE-30 did not reflect independent symptom clusters in Chinese schizophrenic inpatients. T o improve the usefulness of the scale in China, the present study recommended a better revised scale with re-definition of items into 5 subscales.
8."Digital anatomical position of the ""point"" in cervical vertebra fixed-point rotatory technique"

BACKGROUND: Although the fixed-point cervical vertebra rotating reduction has a notable treatment effect, yet it has not been deeply studied in the medical field. Some clinical surgeons feel difficult to make an accurate control on the power and rotatory joint position while operating, even results in iatrogenic injury. OBJECTIVE: To discuss the action mechanism of rotatory technique based on the central rotatory point of cervical vertebra fixed-point rotatory technique. METHODS: Samples were scanned through a 64-row spiral CT working platform at 1-mm layer distance. The picture's profilogram data were extracted from the image processing functional module in PHILIPS MEDICAL SYSTEMS, and then the three-dimensional structure of the upper cervical vertebra was reconstructed and displayed. Taking the axis spinous process peak (point A), odontoid process vertical axes (point B), and the midpoint (point C) of their link as the rotating axes (the rotating central point in simulation), spherical system on each point was set up. The intersection angle of the links between the axis' spinous process peak and the lower jaw, and between the odontoid process vertical axes and the lower jaw before and after rotation were all measured. RESULTS AND CONCLUSION: While applying fixed-point rotation of the cervical spine, the rotatory centre is the vertical axle center of the odontoid process, rather than the handy axis spinous process peak. The rotatory angle of the axle centre is larger than the observation angle. A new concept of fixed-axis rotation should be accepted and its principle should be comprehended in order to appropriately apply the cervical rotatory technique.
9.The therapeutic effects of radiofrequency thermocoagulation denervation mediated by digital subtraction angiography in lumbar facet joint syndrome

Objective To observe the therapeutic effects and assess the feasibility of radiofrequency thermo- coagulation denervation(TRD)mediated by digital subtraction angiography(DSA)for patients with lumbar facet joint syndrome(LFJS).Methods Eighty LFJS patients were randomly divided into two groups:a denervated group(groupⅠ)and a control group (groupⅡ).The lumbar facet joints of the patients in groupⅠwere treated with TRD,while those in groupⅡhad their lumbar facet joint cavities injected with the mixture of 1 mg betamethasone and 1 ml lidocaine.All patients were assessed using the Visual Analog Scale(VAS)before treatment and 30 rain,1 d, 1 w,1 m and 6 m after.Schober values were measured at 1 w,1 m and 6 m after treatment.The therapeutic effects and side effects were assessed at 6 m after treatment.Results VAS scores in groupsⅠandⅡat 30 min,1 d,1 w and 1 m after treatment were remarkably lower than before treatment.In addition,those in groupⅠ6m after treatment were also significantly lower(P<0.01).Compared with those in the groupⅡ,VAS scores in groupⅠwere signifi- cantly decreased at 1 m and 6 m after treatment(P<0.05 and P<0.01,respectively).The schober values in groupⅠat 1 m and 6m were obviously increased in comparison with those in groupⅡ,while the therapeutic effects in groupⅠat 6m after treatment were better than those in groupⅡ(P<0.01).There were no side effects,such as injuries to the nerve roots or dysfunctional dermal sensation,in either group.Conclusion TRD,mediated by DSA,has ther- apeutic effects on LFJS.Such treatment is safe,convenient,and gives long-term pain relief.
10.The value of anti-M3 receptor polypeptide antibody in the diagnosis of sj(o)gren's syndrome

Objective To establish the enzyme-linked immunosorbent assay (ELISA)method in detecting the anti-M3RP antibody in Sj(o)gren's syndrome (SS) patients.and to explore the significance of this autoantibody in the diagnosis of SS.Methods The synthesized M3 receptor polypeptide was used as antigen in EUSA to detect the anti-M3RP antibody in sera of patients with SS.other CTDS and healthy controls.and the association between the clinical features of SS and anti-M3RP antibody was analyzed.Results Antibodies against M3RP were detected in 84.6%patients with pSS and 81-3% with sSS.8.8%with other CTD and 1%in heahhy controls.The positive rates of antibodies in pSS and sSS were higher than those in other CTDs and healthy controls.The presence of anti-M3RP antibodies had no significant correlation with clinical manifesta-tions and internal organ involvement.Furthermore.the positive rates of anti-M3RP antibodies in anti-α-fodrin.SSA,SSB,and ANA antibodies negative SS patients were 85%.89-3%,88.9%and 95.2%,respectively.body is a complementary parameter in the diagnosis of antibody-negative SS.
