1.CT staging of esophageal carcinoma in 304 patients treated chiefly by radiotherapy

Objective To evaluate the feasibility of CT staging for esophageal cancer. Methods A retrospective analysis of 304 patients treated,from Jan. 1996 to Dec. 1998 chiefly with radiotherapy(126 conventional radiotherapy,55-65 Gy/27-35 fx/5.5-7.0 w;178 late-course accelerated hyperfractionated radiotherapy ,55-60 Gy/33-36 fx/ 4.5 -5.5 w)complete with CT data before treatment was done. The long survival was compared with pre-therapy CT findings and CT staging. ResultsThe survival rate of stage T1+T2 was very significantly different from those lesion with stage T3,T4(? 2=12.90,P0.05).The survival rates of patients positive for lymph nodes or distant metastasis were lower. Conclusion CT staging is quite optimal for non-operable esophageal cancer in clinical staging,as it is conducive to predict the prognosis.
2.Risk Assessment on Decorated Indoor Environment and Human Health

Objective In order to know the harm of indoor environment pollutants to the human health. Methods Formaldehyde, benzene, dimethylbenzene, ? radiation and radon were selected as the factors of indoor environment pollution assessment, considering about the inhale and exposure path of indoor air, assessment model which from the production of inland and overseas was used to evaluate the health risk during Mar.-Oct.2005. Results The average risk of the indoor air pollution in Jiangxi province was 1.93?10-4/a, radiation (? radiation, radon) was 1.32?10-4/a, accounted for 68%. Formaldehyde was 4.64?10-5/a, accounted for of 24%. Benzene was 1.41?10-5/a, accounted for 7%. Dimethylbenzene was 1.32?10-9/a, which could be ignored. Conclusion The health risk of indoor environmental air pollutants in each city of Jiangxi province is higher than the acceptable lever(10-5/a).
3.Study on amotile bacteria of positive blood culture in new-born:the analysis of plasmid and restriction enzyme and determination of outer membrane protein

Objective To search for the reasons of high positive rate of amotile bacteria and the diagnosis of septicemia in new-born Methods The blood was drawn from the different site of the new-born with septicemia and carricd out blood culture. The drug sensitivity test had been done by the method of paper stripdiffusion. The plasmids of bacteria were extracted rapidly by medified Birnboim method and the plasmid analyss was carried out. The plasmids's DNA of 35 epidemic strain was cut off by both restriction enzyme of Hind Ⅲ and EcoR Ⅰ. The outer membrane protein (OMP) was determined by SDS-polyacrylamide gel electrophoresis.Results There are 51 patients with positive blood culture amotile bacterium,of them, pollution; 35 cases (68.6%), septicemia: only 16 cases (31.4%),54.8% (57/104) strains bacteria have drug resistance to more of 12 drugs. 87.3% (165/189) strains bacteria have plasmids. They are cut off as 6 DNA fragments (1.9,2,4,5, 8.5 and 18Kb) by Hind Ⅲ restrietion enzyme. and as 5 DNA fragments (2,2.6,3.2, 6.3 and 22 Kb) by EcoR Ⅰrestrietion enzyme, it is showed that they come from a same clone. The epidemic strain include 10 slips OMP, but non-epidemic strain have 11 slip OMP, increase a 25Kd belt. The amotile bacteria with above-mentioned plasmid spectrum, restriction enzyme spectrum and OMP spectrum are only seen in the air, therapeutic dish and syringe needle.Conclusion The pollution is an important reason of amotile bactorium high positiye rate in new-born.Diagnosing septicemia should depend on bacteria culture, plasmid analysis restriction enzyme analysis of plasmid DNA, oMP determination and combining medical history and clinical manifestation.

5.Effect of modified Wuhuang oil on fibroblast growth and proliferation

BACKGROUND: Wuhuang oil has a bacteriostatic action to treat burn wound and promote traumatic healing, but the action on inhibition of scars formation remains poorly understood.OBJECTIVE: To investigate the effects of modified Wuhuang oil at different concentrations and administration times on the growth and proliferation of human fibroblasts in vitro.DESIGN, TIME AND SETTING: Comparison observation regarding cytology in vitro was performed at the Burns Institute in the First Affiliated Hospital of Nanchang University between April 2006 and January 2007.MATERIALS: Prepuce specimens were harvested from patients who underwent circumcision in Department of urinary surgery, at First Affiliated Hospital of Nanchang University and Jiangxi Provincial Children Hospital. All patients aged 2-12 years old, and informed consents were obtained from their relatives. Wuhuang oil and modified Wuhuang oil (water-solubility) were offered by Department of Pharmaceutical Preparation in the First Affiliated Hospital of Nanchang University, China. METHODS: Human fibroblasts cultured in vitro were divided into 2 groups at random, experiment and control. Experiment group was treated with 300 g/L Wuhuang oil, while control group with 300 g/L modified Wuhuang oil. Serum-free culture fluid was used to prepare 6 concentrations of oil solution: 0 (blank control), 100, 150, 200, 250, 300 g/L.MAIN OUTCOME MEASURES: MTT assay was used to determine the growth and proliferation of human fibroblasts at 2, 3, 4, 5,6 days; inhibition rate of cell growth was observed at 2, 4, 6, 8, 10 days.RESULTS: Modified Wuhuang oil (0-300 g/L) concentration positively correlated with inhibition of human fibroblast proliferation;the inhibition was not related to culture time. Modified Wuhuang oil (300 g/L) had the greatest inhibition rate of human fibroblasts at 8-10 days, there were significant differences between experiment group and control group (P < 0.01).CONCLUSION: Modified Wuhuang oil has an effective inhibition on the proliferation of human fibroblasts in vitro, and shows a dose-dependent tendency. Compared with Wuhuang oil, 300 g/L modified Wuhuang oil is superior to suppress the growth of human fibroblasts.

7.Identification of two clinical isolates of Mycobacterium intracellulare

To identify the species of Mycobacterium clinical isolates by molecular biology techniques,six clinical isolates which were preliminarily recognized as Mycobacterium tuberculosis by the TCH and PNB culture methods were selected in this study.PCR was applied to amplify the oxyR-ahpC interval resistant-zone(intergenic region) and the length of PCR product in four strains was the same as that of H37Rv,while the length in the other two strains was different from that of H37Rv but same as Mycobacterium intracellulare 95002.With sequencing and on-line homology comparison with H37Rv,U18263 and U71061,the DNA sequence of oxyR-ahpC intergenic region displayed a 99% homology with Mycobacterium intracellulare U71061 and an 84% homology with Mycobacterium tuberculosis H37Rv.In addition,the results of hsp65 PCR-restriction fragment length polymorphism analysis and multi-locus PCR amplification in the two strains were identical with those of Mycobacterium intracellulare 95002.These two clinical isolates which were preliminarily recognized as Mycobacterium tuberculosis by PNB and TCH culture methods were finally identified as Mycobacterium intracellulare.Results predicted that the application associated with various techniques of molecular biology would provide a faster,easier and more correct way for the species identification of Mycobacterium.
8.MR-DTI Study on Wallerian Degeneration of the Optic Radiation after Occipital Lobe Chronic Infarction

Objective To evaluate the possibility of diffusion tensor imaging(DTI)in detecting and quantifying the Wallerian degeneration of optic radiation in occipital lobe chronic infarction.Methods 20 patients with unilateral occipital chronic infarction were undergone DTI.Quantitative fractional anisotropy(FA)and mean diffusivity(MD)were obtained from the ipsilateral optic radiation of the occipital lobe infarction and compared with that of contralateral region using the independent samples t-test.Results On the three dimensional color-coding tensor fractional anisotropic map,the regions of occipital lobe chronic infarction were markedly low signal intensity,the FA value and MD value were 0.274?0.062 and(1.226?0.372)?10-3mm2/s,while in the contralateral region of optic radiation,FA and MD values were 0.495?0.035 and(0.775?0.070)?10-3mm2/s respectively,there was significant difference in FA and MD values in comparing both side(P〈0.01).Conclusion DTI can detect and quantify the Wallerian degeneration in optic radiation after occipital lobe chronic infarction.
9.Analysis of related factors of tonsil postoperative pain.

OBJECTIVE:
To explore the related factors of tonsil postoperative pain.
METHOD:
After founding databank a statistic analysis was performed on 90 cases with tonsillectomy who admitted in our hospital. Chi-square test were used to investigate the related factors for postoperative pain in those patients.
RESULT:
After Chi-square test, the important effect factors in the postoperative pain were gender, age, surgical methods, preemptive analgesia, physical analgesia and preoperative anxiety level.
CONCLUSION
The pain after tonsillectomy is influenced by not only surgery itself, but also by preemptive analgesia, preoperative anxiety level and so on.

10.Effects of sleeping body posture on sleeping structure and respiratory events in patients with OSAHS.

OBJECTIVE:
To observe the effects of sleeping body posture on sleeping structure and respiratory events in patients with OSAHS.
METHOD:
We assessed the sleeping body position, the sleeping structure, position specific AHI and the Epworth Sleepiness Scale (ESS) in a total of 80 patients with Positional OSAHS. The patients were grouped according to AHI: mild OSAHS (5 < or = AHI <15), moderate (15 < or = AHI < 30) and severe (AHI > or = 30). The polysomnography data and clinical characteristics were compared between each group.
RESULT:
The severe OSAHS group, when compared with the mild and the moderate ones, had a significant different in REM%, NREM%, NREM LSaO2, RDI, S-AHI and L-AHI due to posture (P < 0.05). The severe and the mild OSAHS groups had significant different in MSaO2, LSaO2 due to posture (P < 0.05). The moderate and the mild OSAHS group had significant different in LSaO2, REM LSaO2 and RDI (P < 0.05). In all mild, moderate and severe groups, the LT% were higher than ST%, but the difference was not significant (P > 0.05). For mild-to-moderate groups, there was no correlation between the ESS and the AHI for any position different (P > 0.05). For severe group, the ESS was significantly correlated with L-AHI (r = 0.551; P < 0.01); the REM L-AHI and NREM L-AHI was also significantly correlated with ESS of severe group (r were 0.516 and 0.528, P < 0.01).
CONCLUSION
The L-AHI, NREM L-AHI, REM L-AHI and NREM LSaO2 were considered to monitor the stability of OS-AHS, while REM LSaO2 were consider to clarify the severity of OSAHS.
