1.A Case of Herpes Simplex Virus Esophagitis in a Renal Transplant Child.

Herpes simplex esophagitis can occur in those with normal immune function, but is more often seen in those who are immunocompromised. In one series, 5 percent of post-kidney transplant recipients had herpes esophagitis. We experienced a case of herpes simplex esophagitis, following renal transplantation in a 9 year old male. He complained of epigastric pain, nausea and blood-tinged vomiting. Endoscopic examination showed volcano ulcer, mucosal friability and multiple confluent ulcers covered by whitish exudates on elevated margin in the middle and lower esophagus. Microscopic findings revealed multinucleated giant cells, margination of chromatin, intense nonspecific inflammation and strong positive for herpes simplex virus immunohistochemical staining. Esophageal lesions and symptoms improved after acyclovir therapy.

2.VP7 Genotypes of Human Rotavirus from Hospitalized Children with Severe Diarrhea by Reverse Transcription - Polymerase Chain Reaction.

Human rotavirus has now been established as the leading cause of gastroenteritis in young children worldwide. At least fourteen serotypes of group A rotavirus have been identified on the basis of antibody responses to major neutralizing glycoprotein, VP7 (G type for glycoprotein), present in the outer capsid of the virus. Serotype 1, 2, 3 and 4 are the most highly prevalent in human. In Korea, rotavirus is also the principal cause of severe nonbacterial diarrhea requiring hospitalization in infants and young children, which is commonly detected by EIA method. The epidemiology of rotavirus infection has been monitored by only serologic methods without electropherotyping in Korea. This study shows seasonal and age related variations .of rotavirus infection in Korea according to the genotype using the reverse transcription polymerase chain reaction (RT-PCR). Fecal specimens were obtained from 39 children hospitalized with acute watery diarrhea and gastroenteritis in Ewha Womans University MokDong Hospital in Seoul from Jan. to Dec. of 1996. All four (1, 2, 3, 4) major G serotypes were identified by amplification of segment of the gene for VP7 using RT-PCR. Rotavirus Gl 749 bp, G2 653 bp, G3 374 bp and G4 583bp were shown on 2.9 or 3.3% NuSieve agar gel. Results were as follows: 1) Rotavirus was detected at 53.8% (21/39) by EIA and 89.7% (35/39) by RT-PCR. 2) Serotype Gl, G2, G3, G4 when detected by RT-PCR accounted for 80.0% (28/35), 14.3% (5/35), 2.9% (1/35) and 2.9% (1/35), respectively. 3) Thirty five strains of rotavirus were detected at the frequency of 17.1% (6/35) in Oct., 20.0% (7/35) in Nov. and 20.0% (7/35) in Dec. 4) As for the age range, children affected by rotavirus were mostly under 1 years.

3.Detection of Astrovirus Infection from Hospitalized Young Children Feces by Reverse Transcription - Polymerase Chain Reaction.

Astrovirus is frequently associated with diarrhea in children. It can not be readily isolated by cell culture, and an electronmicroscope is usually used for detection of this agent. Recently in 1995 a combined method of reverse transcription-polymerase chain reaction (RT-PCR) was designed for easier detection of astrovirus, which is based on the conserved sequence in 3'-end of genomes of the 7 known serotypes of human astrovirus. As of yet there has not been any report of astrovirus data in Korea using the RT-PCR methods. The purpose of this study was to detect astrovirus incidence, severity of symptoms, seasonal variation and coinfection rate with rotavirus in Korean children inpatients with diarrhea. Fecal specimens from 61 young children hospitalized with gasteroenteritis Korea from Jan. 1996 through Mar. 1997. They were examined for astroviurs infection by RT-PCR method. Results are as follows: 1. Astrovirus was detected at 9.8% (6/61) from fecal specimens of children with severe diarrhea by EIA using monoclonal antibody coated plates. 2. Astorvirus was detected at 29.5% (18/61) from fecal specimens of children with severe diarrhea by RT-PCR. 3. The age of the 18 children affected by astrovirus ranged from 2 monthes to 7 years with mean of 3.0 years. 4. Mean hospital stay of the 1S children was 6.1 days. 5. Five (27.8%) astrovirus RT-PCR positive strains were confirmed in November and in December, respectively out of 18 specimens in total. 6. Astrovirus coinfection with rotavirus type G1 was confirmed in 15/16 specimens (93.8%), and with type G2 was in 1/16 specimens (6.3%).

4.Open reduction and internal fixation of mandibular fracture in an 11-month-old infant: a case report.

Mandibular fractures in infants are rare. This case report describes management of a mandibular fracture in an 11-month-old infant using a microplate and screws with open reduction. The surgical treatment was successful. Because the bone fragments were displaced and only the primary incisors had erupted, conservative treatment, such as an acrylic splint and circummandibular wiring, was not recommended. Nine weeks after surgery, the microplate was removed. The results showed complete clinical and radiological bone healing with normal eruption of deciduous teeth.

5.Detection by PCR of Adenovirus and Human Herpes Virus 6 in Peripheral Blood Monocyte from Young Children who were Hospitalized with Lower Respiratory Tract Infection.

There are reports that the second most causative viral agent which causes lower respiratory tract infection (LRTI) in young children is adenovirus (ADV). Human herpes virus 6 (HHV-6) is also reported as a rare agent of LRTI in young children. But there is no report of simultaneous detection of ADV and HHV-6 in LRTI using the same peripheral blood monocyte (PBM) by nested-polymerase chain reaction (PCR) or PCR. Firstly, we detected ADV antigen (Ag) and HHV-6 Ag in serum by each monoclonal antibody with enzyme immunoassay (EIA). Secondly we tested two viruses in peripheral blood monocyte by nested-PCR or PCR. Twenty nine cases of young hospitalized children with LRTI (mean age 11.3 months, mean hospitalization period 5.7 days) had bronchiolitis or viral pneumonia and were confirmed by X-ray findings. Positivity of ADV Ag in serum by EIA was 75% (21/28) and positivity of HHV-6 Ag in serum by EIA was 10.7% (3/28). ADV in PBM by nested-PCR positivity was 89.7% (26/29) and HHV-6 in PBM by PCR positivity was 42.9% (12/28). ADV and HHV-6 dual infection in PBM by PCR was 11/29 (37.9%). Young children with dual infection were hospitalized (mean 6.3 days) with severe bronchiolitis.

6.A Case of Thyroid Papillary Cancer Derived from Diffuse Goiter in a Patients with Acromegaly

Patients with acromegaly have a reduced life expectancy rnainly due to cardiovascular, respiratory or cerebrovascular diseas-. Malignancy also seems to occur with greater than the expected incidence. In particular, the published retrospective or prospective studies have suggested a strong association of colonic neoplasia with acromegaly. But, there were a few reports of thyroid cancer in acrornegaly. We report a case of thyroid papillary cancer derived from diffuse goiter in acromegaly, sugge- sting the possible carcinogenic role of growth hormone.

7.Luteolin reduces fluid hypersecretion by inhibiting TMEM16A in interleukin-4 treated Calu-3 airway epithelial cells

Rhinorrhea in allergic rhinitis (AR) is characterized by the secretion of electrolytes in the nasal discharge. The secretion of Cl– and HCO3 – is mainly regulated by cystic fibrosis transmembrane conductance regulator (CFTR) or via the calciumactivated Cl– channel anoctamin-1 (ANO1) in nasal gland serous cells. Interleukin-4 (IL-4), which is crucial in the development of allergic inflammation, increases the expression and activity of ANO1 by stimulating histamine receptors. In this study, we investigated ANO1 as a potential therapeutic target for rhinorrhea in AR using an ANO1 inhibitor derived from a natural herb. Ethanolic extracts (30%) of Spirodela polyrhiza (SPEtOH) and its five major flavonoids constituents were prepared. To elucidate whether the activity of human ANO1 (hANO1) was modulated by SPEtOH and its chemical constituents, a patch clamp experiment was performed in hANO1-HEK293T cells. Luteolin, one of the major chemical constituents in SPEtOH, significantly inhibited hANO1 activity in hANO1-HEK293T cells. Further, SPEtOH and luteolin specifically inhibited the calcium-activated chloride current, but not CFTR current in human airway epithelial Calu-3 cells. Calu-3 cells were cultured to confluency on transwell inserts in the presence of IL-4 to measure the electrolyte transport by Ussing chamber. Luteolin also significantly inhibited the ATP-induced increase in electrolyte transport, which was increased in IL-4 sensitized Calu-3 cells. Our findings indicate that SPEtOH and luteolin may be suitable candidates for the prevention and treatment of allergic rhinitis. SPEtOH- and luteolin-mediated ANO1 regulation provides a basis for the development of novel approaches for the treatment of allergic rhinitis-induced rhinorrhea.

10.Discrepancy of Measured Renal Length between Ultrasonography and Dimercaptosuccinic Acid (DMSA) Scintigraphy.

PURPOSE: An inter-observer variability exists in the measurement of the renal length during ultrasonography, but dimercaptosuccinic acid (DMSA) scintigraphy allows greater objectivity. Thus, the aim of this study was to evaluate the discrepancy of the renal length measured using the two methods. MATERIALS AND METHODS: Between January 2003 and June 2005, 249 consecutive patients underwent DMSA scintigraphy and ultrasonography. Due to the exclusion of kidneys where a photon defect was observed during DMSA scintigraphy, the number of kidney enrolled in this study was 439. The renal length measurements in any plane were longest using ultrasonography. The renal lengths were measured using DMSA scintigraphy by assessing the maximum pixel length, which was then converted to centimeters. RESULTS: The mean renal lengths measured by DMSA scitigraphy and ultrasonography were 8.06+/-1.49 and 6.80+/-1.38cm, respectively. The mean difference in the renal length measured between the two methods was 1.26+/-0.84cm. The sonographic renal length was significantly shorter than that found via scintigraphy (Student's t-test, p<0.001). CONCLUSION: Our results were similar to those of previously reported studies, where the scintigraphic renal length exceeded that obtained sonographically. Due to the possibility of underestimating the renal length when using ultrasonography, clinicians must be cautious when following up pediatric patients using ultrasonography.
