1.Impacts of Tone on Auditory Perceptual Result of Hypernasality

Objective To investigate how the tones impact the perceptual result of hypernasality for designing subjective evaluation material for hypernasality. Methods 20 normal children and 20 children with cleft palate were asked to read the material with different tones.The degree of hypernasality and nasality was recorded. Results There was no significant difference in the degree of perceived hypernasality and nasality rate among all the children in all the tones (P>0.05). Conclusion The impact of tones on perceived hypernasality can be ignored when designing the subjective evaluation material for hypernasality.
2.Acoustic Characteristics Study on Tone/i/with ContinuousIy VariabIe Pitch and Loundness Made by NormaI AduIts

Objective By analyzing the graph features of fundamental frequency at different rates of/i/with continuously variable pitch and loundness made by normal adults,we could explore that the rate and gender have some impact on the graph to lay the foundation of regulating the clinical maximum counting ability.Methods Nor-mal adults of 51 (25 males,26 females were included).After each deep breathing,the subjects respectively took sustained speech/i/ with continuously variable pitch and lounolness twice at different rates of fast,medium and slow speed.Some parameters were measured byDoctor Speechat different rates of the tone/i/:Total time(T), Mean time standard deviation (MTSD),Mean wave crest (MWC),Mean wave trough (MWT),Mean frequency range (MFR),Mean frequency range standard deviation (MFRSD),comparing the speed having some effect on those above parameters with gender.ResuIts ①With the increasing rate of speech,the tone/i/of T (P<0.001), MFR (P<0.01)and MTSD (P<0.001)were significantly decreased,MWT significantly increased (P<0.001), While the MWC and MFRSD did not change significantly (P>0.05 ).②The tone /i/ of T (P>0.05 )were not effected by gender,female's MWT (P<0.001),MWC (P<0.001),MFR (P<0.001),MFRSD (P<0.01)were obviously higher than male's,while female's MTSD were significantly larger than male's (P<0.01).ConcIusion We should consider the impact of rate of speech and gender in clinical tests and values of MCA.The maximum counting capacity can be respectively tested at three different rates of speech for each subject,but only are the tone/i/of fun-damental frequency graphs with MWT,MWC,MFR,and MFRSD in normal range with corresponding speed and gender,can we get the value or can the value be valid.
3.A comparative study of the speech respiratory characteristics of 7-15 years old ordinary children and children with spastic cerebral palsy

Objective To explore the speech respiratory characteristics of 7-15 years old children with spastic cerebral palsy and the differences between ordinary children and children with cerebral palsy.Methods The maximum phonation time (MPT) and maximum counting ability (MCA) of 90 ordinary children and 58 children with cerebral palsy aged 7-15 years were measured and a comparison between the 2 groups were conducted.Results (1) Age has significant effects on MPT and MCA of the ordinary children (P <0.05).The MPT and MCA of the ordinary children at the age of 7 ~9 years and 10 ~ 12 years are lower than the ones at the age of 13 ~ 15 years.Age has almost no effects on MPT and MCA of children with cerebral palsy(P >0.05) ; (2)Sex has no significant effects on MPT and MCA of ordinary children and children with cerebral palsy (P >0.05) ; (3) The MPT and MCA of children with cerebral palsy are significantly lower than those of ordinary children (P <0.05).Conclusions The MPT and MCA of ordinary children are increased with the growth of age,especially after the age of 13 years,but the children with spastic cerebral palsy do not show that kind of trend and demonstrate a state of retardation of speech respiratory function.Compared with ordinary children,the children with spastic cerebral palsy are easier to suffer from insufficient respiratory support and poor respiratory-phonatory coordination.
4.Security evaluation of bupivacaine, ropivacaine combined with fentanyl in postoperative continuous epidural analgesia.

OBJECTIVETo investigate the effects, side-effects and security of bupivacaine, ropivacaine combined with fentanyl in postoperative continuous epidural analgesia.
METHODSA total of 1 600 postoperative continuous epidural analgesia patients receiving different agents in SICU were divided into two groups: 0.1% bupivacaine +5 microg/ml fentanyl group (group B, n = 920) and 0.2% ropivacaine +2 microg/ml fentanyl group (group R, n = 680). The effects (visual analog-scale score and content to analgesia), side effects were analyzed retrospectively in the two groups.
RESULTSCompared with group B, patients in group R had higher analgesia contentment (P < 0.05), but no difference in visual analog-scale score was found in the two groups. The incidences of urinary retention, nausea and vomiting, skin itching in group B were significantly higher than those in group R (P < 0.05). In each group, patients over sixty had higher ratio of hypotension than those under sixty (P < 0.05); The female patients had a higher incidence of nausea and vomiting than male patients (P < 0.05); The incidence of debility and numbness of lower limbs in patients with lumbar segments epidural analgesia was higher than those with thoracic analgesia (P < 0.05).
CONCLUSIONS0.1% bupivacaine +5 microg/ml fentanyl and 0.2% ropivacaine +2 microg/ml fentanyl can provide adequate pain relief in postoperative continuous epidural analgesia, and 0.2% ropivacaine +2 microg/ml fentanyl comes with less side effects. The incidence of complication is related with analgesics, age, gender and the position of epidural puncture.

5.The Correlation between Plasma N-terminal Probrain Natriuretic Peptide (NT-proBNP) and the Severity of Coronary Artery Disease

Objective To investigate the correlation between plasma NT-proBNP and the severity of coronary artery disease. Methods One hundred and eighty five subjects were divided into three groups:83 patients in acute coronary syndromes group, 62 patients in stable angina pectoris group and 40 healthy subjects in normal group. The plasma NT-proBNP concentrations were measured by enzyme-linked immunosorbent assay (ELISA) . The plasma NT-proBNP levels of acute coronary syndrome patients and stable angina patients were analyzed. Furthermore, the coronary syndrome patients were divided into three groups according to the result of angiography:single vessel group, dual-vessel group and multiple-vessel group. The relationship of plasma NT-proBNP levels with the severity of coronary artery disease was analyzed. Results (1) In the group of acute coronary syndromes, the plasma NT-proBNP concentrations were obviously higher than the stable angina pectoris group and normal group (P<0.01) . (2) In the groups of coronary heart disease, the plasma NT-proBNP concentrations of the multiple-vesel group were obviously higher than the single vessel group and the dual-vessel group (P<0.01) . The plasma NT-proBNP concentrations of dual-vessel group were obviously higher than the single vessel group (P<0.01) . (3) The plasma NT-proBNP levels and the severity of coronary lesions were positively correlated (rs=-0.796, P<0.01) . Conclusion The plasma NT-proBNP levels is closely related to the severity of coronary artery disease.
6.Recombinant Mycobacterium smegmatis expressing taco mRNA specific 10-23 deoxyribozyme mediate inhibition of taco expression in macrophage

Objective To construct a recombinant bacterial vaccine which can express specific 10-23 deoxyribozyme(DZ) in macrophage, identify the intracellular production of specific 10-23DZ and detect the activity of this recombinant bacterial vaccine on inhibiting the expression of TACO gene in macrophage.Methods The pSDE02 was obtained by inserting the replicon of Mycobacterium into pSDE01, a plasmid which can express 10-23DZ in eukaryotic cells. The expression sequence of DZ1, a 10-23DZ targeting the TACO mRNA of macrophage designed in our previous study was synthesized and inserted into pSDE02. The resulted plasmid was named pDZM01. pDZM01 was then transferred into Mycobacterium smegmatis by electroperation. The recombinant M. smegmatis, named rMs-DZ1 was screened on low-salt LB medium containing Zeocin and identified by Colony PCR. The targeted delivery property of recombinant M. smegmatis was observed by Ziehl-Heelson stain and GFP expression observation via fluorescence microscope. rMs-DZ1 was used to infect RAW264.7 cells and the expression of DZ1 in macrophage was identified by dot-blot assay. At 24 h and 48 h after infection, total RNA and proteins were extracted and the TACO mRNA and protein expression level was assayed by RT-PCR and western-blot respectively. Results Restrictive analysis and sequencing data showed that the Mycobacterium-eukaryotic cell shuttle plasmid pSDE02 and pDZM01 was successfully constructed. rMs-DZ1 was confirmed by colony PCR. When engulfed by macrophage, rMs-DZ1 would express DZ1 in RAW264.7 cells and inhibit the expression of taco gene. When compared to uninfected macrophage, rMs-DZ1 significantly reduced the taco mRNA by 67.90% and 57.14% and down-regulated the expression of TACO protein by 53.85% and 68.92% at 24 h and 48 h respectively. Conclusion A recombinant M. smegmatis vaccine was successfully constructed which could generate specific 10-23DZ in macrophage and inhibit the expression of target gene of interest. To our knowledge, this is the first bacterial vector which can express intracellularly 10-23DRz in targeted manner. This study may further prompt the feasibility of using 10-23 DNAzyme to achieve effective and targeted gene silence.
7.Immuno-modulatory effect of somatostatin combined with traditional Chinese medicine on severe acute pancreatitis at early stage: a randomized control trial

OBJECTIVE: To evaluate the immuno-modulatory effect of short course administration of somatostatin (stilamin) continuously at early stage in patients with severe acute pancreatitis (SAP). METHODS: Thirty-nine patients with SAP (22 men, 17 women; the middle age was 49 years)were randomly allocated into control group (20 patients treated with non-surgical integrated traditional Chinese and Western medicine) and treatment group (19 patients treated with somatostatin administered intravenously at a dosage of 250 mug/h for consecutive 72 hours as well as the treatment for the control group). Laboratory parameters, including the expressions of CD(3), CD(4) and CD(8) in lymphocytes (tested by flow cytometry) and C reactive protein (CRP), and indexes of therapeutic effect, including the occurrence of organic dysfunction, local complication and mortality between the two groups were compared. Another group of 30 healthy volunteers (19 men, 11 women; the middle age was 47 years) were recruited for testing the normal levels of CD(3), CD(4) and CD(8). RESULTS: (1) The levels of CD(3), CD(4) and CD(4)/CD(8) in lymphocytes before treatment in both groups were significantly lower than those in the healthy subjects (P<0.05), but there were no statistical differences between the two groups. At the 4th day, CD(3), CD(4) and CD(4)/CD(8) increased significantly in the treatment group (P<0.05) while no changes in the control group; the levels of CD(4) and CD(4)/CD(8) in the treatment group were also higher than those in the control group (P<0.05). (2) The CRP levels of the 2 groups showed no statistical difference before and 4 days after the treatment, but the CRP level in the treatment group was significantly lower than that in the control group at the 7th day (P<0.05). WBC count, serum levels of amylase, lipase, lactate dehydrogenase, aspartate aminotransferase, as well as the score of APACHE II in the treatment group recovered more quickly than those in the control group (P<0.05). (3) The occurrences of organic dysfunctions, local complications and mortality in both groups were not statistically different. CONCLUSION: The short course administration of somatostatin continuously at early stage can reduce the inflammatory response, up-regulate the cell immune function and improve the conditions of the patients with SAP, but its effect on mortality and morbidity needs further study.
8.Stable replication and antigen expression of hepatitis B virus in HepG2 cells

Objective To establish the HepG2 cell lines which can stably express and replicate hepatitis 13 virus (HBV).Methods One point two X unit length of HBV genome was cloned intn SalⅠsite of the eukaryotic expression vector pREP10 to construct the recombinant plasmid pREP-HBV. Human hepatoblastoma cell HepG2 was transfected with pREP-HBV by Lipofectamine 2000 and seh,cted by bygromycin at the concentration of 250?g/mL.HBsAg and HBeAg were monitored by enzyme linked immunosorbent assay (ELISA)kits.H13V particles presemed in supernatant were ex- amincd by electronic microscopy.DNA isolated from intracellular HBV core particles was analyzed by Southbern blot using HBV-specific probe.Results The recombinant vector pREP HBV containing 1.2?unit length of HBV DNA was constructed successfully.After transfection of pREP-HBV to HepG2 cells and consistently cultured in hygromycin selective medium.5 drug-resistant cell lines, RHBV1-RHBV5.were established,and all of them could stably express HBsAgand HBeAg.South ern blot analysis revealed that HBV could replicate in all cell lines,as confirmed by the presence of replicateintermediatc DNA in intracellular HBV core particles.Clustered 42 nm Dane particles as well as 22-26 nm spherical H13sAg particles in condensed cuhure supernatant were visualized by elec tronic microsopic analysis.Conclusion HepG2 ceil lines in which HBV can replicate and express specific antigens are successfully established.Up to now,the cells have been passaged every three days for 50 times.
9.Change of Capillary Pericapillary Cells in Rats with Myocardial Infarction and Effect of Supplementing Qi and Activating Blood Circulation Herbs

Objective To observe the change of capillary pericapillary cells in rats with myocardial infarction and the influence of supplementing qi and activating blood circulation herbs, and explore its mechanism of improving myocardial perfusion. Methods The rat model was established by ligaturing the left anterior descending coronary artery. On the base of ECG evaluation, successfully modeled rats were randomly divided into the model group, group treated with supplementing qi and activating blood circulation Chinese medicine (activating blood and supplementing qi group), group treated with Perindopril (Perindopril group), group treated with Tongxinluo Capsules (Tongxinluo group). The sham-operation group was taken as the control. There were totally 5 groups. The model group and the sham-operation group were treated with normal saline. The changes of myocardial capillary density (MCD) and number of pericapillary cells on the 7th, 28th day after medicinal administration were observed. Results On the 7th and 28th day, the MCD decreased significantly and the number of capillary pericapillary cells increased significantly in the model group compared with the sham-operation group (P<0.01). The MCD increased significantly in the activating blood and supplementing qi group, Perindopril group and Tongxinluo group compared with the model group (P<0.01). The number of pericapillary cells decreased significantly in the activating blood and supplementing qi group, Perindopril group and Tongxinluo group compared with the model group (P<0.01). Conclusion The supplementing qi and activating blood circulation herbs can improve regional myocardial blood supply by decreasing the number of pericytes and promoting regeneration of capillary.
10.Effect of Comprehensive Rehabilitation on Shoulder Subluxation of Stroke Patients with Hemiplegia

ObjectiveTo observe the effect of comprehensive rehabilitation on shoulder subluxation of patients with stroke hemiplegia.MethodsComprehensive rehabilitation was practiced on 20 hemiplegic post-stroke patients with shoulder subluxation, including putting injured limb at correct position, keeping the range of passive motion, resistance exercise of scapula muscle group, and shoulder sting. The therapeutic effect was evaluated after 6 weeks treatment.ResultsThere was a significant difference in the shoulder subluxation and movement function of the upper extremities between pre-therapy and after therapy ( P<0.05~0.01).ConclusionComprehensive rehabilitation has obvious therapeutic effect on shoulder subluxation and motor dysfunction of patients with stroke hemiplegia.
