Objective:To investigate the clinical manifestation and determine the distribution of pathogens and their characteristics of drug susceptibility to bloodstream infections (BSIs), and provide evidence for clinical anti-infection treatments after renal transplantation.
Methods:Totally 81 episodes of BSIs occurred in 71 patients between July 2003 and June 2013. We retrospectively analyzed the pathogens and their drug susceptibility characteristics with BD microbiological assay system. We also collected the clinical and laboratory data of the patients . Results:The main pathogens were gram negative bacteria (67.90%), followed by gram positive bacteria (28.40%) and fungi (3.70%). The most common gram negative bacillus was Escherichia coli. While for gram positive bacteria, the main bacillus was coagulase-negative staphylococci. The gram negative bacteria were relatively sensitive to aminoglycosides and carbapenem. The gram positive bacteria were sensitive to glycopeptides and oxazolidone.
Conclusion:The clinical manifestations included high body temperature, onset in the early period after kidney transplantation and high mortality. Though gram positive coccus plays an important role, most infections are caused by gram negative bacteria in BSIs after the renal transplantation. The antibiotic resistant rate for gram negative bacteria is very high as well as gram positive bacteria.

ObjectiveTo explore the relationship of circadian distribution of acute myocardial infarction with AMI location and ST segment changes in elderly patients.MethodsThe time of infarction, its anatomic location, changes of ST segment, and coronary angiography were studied in 909 elderly patients with acute myocardial infarction (AMI) ( 412 with anterior AMI and 423 with inferior AMI) admitted to our coronary care units from January 1996 to January 2006.ResultsThe onset of inferior myocardial infarction were more frequent between midnight and 6AM than other periods of the day (n=138/423,32.6% of all inferior myocardial infarction patients, P<0.01). The onset of anterior myocardial infarction were more frequent between 6AM and noon than other periods of the day (n=156/412, 37.9% of all anterior myocardial infarction patients, P<0.01). Coronary angiography was performed in 789 patients (86.8%, 516/909).118 cases of them with inferior infarction occured between midnight and 6AM, including 85.6% of them were due to right coronary artery occlusion and 14.0%(17/118) of them were due to left coronary artery occlusion (P<0.01).275 cases of them with inferior infarction oecured between 6AM and midnight, including 52.2% (149/275) of them were due to right coronary artery occlusion and 45.8% of them were due to left coronary artery occlusion (P>0. 05). The onset of inferior myocardial infarction between 6AM and noon was the most frenquent in patients with ST segment elevation (44.0%, 263/644), while the onset of inferior myocardial infarction between midnight and 6 AM was the most frenquent in patients with non-ST segment elevation (36.6%,96/265). ConclusionsThe frequency of AMI at night is higher in elderly patients with ST segment elevation than in elderly patients with non-ST segment elevation.AMI at night is usually due to right coronary artery occlusion, which suggests that a protective role of sleep may be limited to left coronary artery -related events and AMI of non-ST segment.

AIM:To explore the involvement of oxidative stress and apoptosis in the pathogenesis of diabetic cataract induced by Streptozotocin ( STZ) and the interventions of puerarin in order to supply references for clinical treatment.
METHODS:Male SD rats were divided into four groups randomly, control group, diabetic group, apocynin group and puerarin group. The diabetic group were replicated by single injection of STZ (65mg/kg, ip). The expression of p22, p47, p67, Bax/Bcl2, Caspase 3 and P53 proteins were detected by Western Blotting.
RESULTS:The diabetic rats were replicated successfully and the expression of Bcl2 was downregulated while the expression of p22, p47, p67, Bax, Caspase 3 and P53 were upregulated in diabetic group with a significant statistical differences when compared with control group (P<0. 05). Apocynin and prerarin can reverse the abnormal expression of the aforementioned proteins dramatically (P<0. 05).
CONCLUSION: NADPH oxidase mediated oxidative stress and P53, Bax/Bcl2 mediated apoptosis are involved in the pathogenesis of diabetic cataract and puerarin can alleviate cataract greatly by inhibiting the aforementioned signal pathway.

OBJECTIVETo study the correlation between EEG characteristics of medial prefrontal cortex (mPFC) and drug-seeking behavior of rats with morphine dependent place preference under shuttling condition.
METHODSForty rats were randomly divided into four groups (n = 10): morphine PL group, NS PL group, morphine IL group and NS IL group. After embeding the electrode in prelimbic (PL) or infralimbic (IL) cortex of each group by brain stereotaxic operation, the model of morphine dependent conditioned place preference (CPP) in rats was established. The differences of EEG wave percentage in mPFC were telemetered and analyzed when rats shuttled before and after the model.
RESULTSAfter the model, the withdrawal symptoms were evident in morphine PL and IL group, and the activity time and distance in white box were increased obviously. Compared with control group, after the model, the EEG in morphine PL group showed that: when the rats shuttled to white box, 8 wave decreased obviously, P wave increased obviously. When the rats shuttled to black box, brain waves showed opposite changes. The EEG in morphine IL group showed that: when the rats shuttled to white box, a wave increased obviously, P and a wave decreased obviously. When the rats shuttled to black box, the brain wave had no significant differences compared with control group.
CONCLUSIONThe EEG changes are different in PL and IL cortex of morphine CPP rats under shuttling condition, and the EEG changes are also different when rats shuttling to white or black box. There is possibly different mechanism, when different drug-seeking environmental cues caused EEG changes in different regions of mPFC.
