1.Analysis of the Protection Knowledge about One's Own Rights and the Perception in Practice for Patients

Objective:Investigate the awareness of their own rights and the safeguarding of the rights in practice,raise the patients' sense of safeguarding of the rights and help them defend the legal rights.Methods:A survey was taken among patients by applying the questionnaire.Results:There are 5 items about the patients' knowledge about their own rights and 3 items about defending their rights in practice,in which more than 85 percent of 111 patients chose the answer "always or almost".From this we can see that the awareness and safeguarding of their rights are not good enough.Conclusion: The government is appealed to break up the existence of the monopoly mold in medical treatment and to take measures to protect the rights of patients.The medical personnel should renew their concepts,pay more attention to publicizing and defending patients' rights,and help patients realize their rights.
2.The expression analysis of EGF and EGFR in esophageal restenosis tissue after stenting

Objective To study the expression of EGF and EGFR in restenosis tissue of animal e-sophagus after several times of stenting in esophagus, and their relation with esophageal restenosis . Methods Animal models of esophageal stenosis after stenting were made from 16 adult healthy dogs, and allocated into 4 groups to take restenosis tissue as experimental samples at week 1,2,4 and 8. In addition, other 6 normal dogs served as control. To study the expression of EGF and EGFR at protein and mRNA level in these samples by immunohistochemistry ( SABC) and RT-PCR. Results At week 1 and 2, EGF and EGFR clearly expressed, the expressing level increased greatly as compared with the normal controls, the expression of EGF and EGFR existed almost in all the tissues. At week 4 the expression lessened greatly, and finally at week 8 disappeared. The positive cells for EGF were mainly macrophages, some vascular endothelial cells and smooth muscle cells, the positive cells for EGFR were mainly macrophages, lymphocyte and Fb. Conclusion After esophageal stenting, lymphocyte, macrophage and Fb in the local tissues were closely related to restenosis, and served as the important regulatory cells in the pathogenesis of restenosis.
3.RESEARCH ON THE INJURY OF "Z" STENT ON ESOPHAGUS IN EXPERIMENTAL DOGS

To observe the changes in mucosal blood flow and permeability after esophageal "Z"stenting, 12 adult healthy dogs were divided into two groups, and an esophageal stent was placed in each dog. Mucosal blood flow was measured with Dopplar blood flowmeter. Two hours after stenting PEG400 was instilled into the esophagus continuously under a constant pressure for two hours in the first group. The amount of PEG400 in the stented tissue was measured with gas liquid chromatography.In the second group the same experiment was carried out 24 hours after stenting. The mucosal structure was examined with electron microscope after stenting. It was found that the mucosal blood flow decreased evidently, and the amount of PEG400 was much greater in tissue after being stented as compared with that in normal esophagus. Electron microscopy revealed that the number of desmosome of epithelium decreased, and it was disrupted significantly after stenting. The interval epithelial cells were enlarged, basement membrane was damaged. Stenting might lead to evident decrease of esophageal mucosal blood flow, which significantly impaired the junction of epithelial cells and mucosal structure, and the permeability for PEG400 was increased significantly. All these findings demonstrate that stenting might impair the esophageal mucosa.
4.Study on the changes of collagen content in the esophageal tissue after stenting

Objective To study the changes of collagen content in the esophageal tissue after stenting. Methods To select healthy adult dogs as experimental subjects.Esophageal stent was installed by the method of “autogenous fascia lata femoris transplantation and fixation",the dogs were killed at the end of 1,2,4 and 8 weeks,and the esophageal tissues with stent were taken out and studied by gross observation,light microscopy and electromicroscopy. The bulk density and distribution of collagen fiber were studied by special staining and computer technology.The content of P I CP and PⅢNP in restenosis tissue were measured by radio immunoassay(RIA).The contents of hydroxyproline and total amino acid(AA) in restenosis tissue were measured by amino acid analyser. Results At week 1 and 2 the inflammatory reaction occurred evidently in stenting esophagus with granulation and fibrosis,in some sites the esophageal tissue began to proliferate towards the lumen where the fibroblasts were in the state of active proliferation and secretion .The contents of hydroxyproline and total amino acid were significantly higher in the esophageal tissue within 1 and 2 weeks of stenting than those in normal esophagus.At week 4 and 8 esophageal lumen narrowed conspicuously,with a lot of fibrotic tissue and a little of inflammatory cells.The staining density of collagen elevated gradually within 4 weeks,there after the content of collagen was stabilized.The change of P I CP and PⅢNP accorded with that of collagen staining.The content of AA at week 4 increased significantly compared with that at week 2,its level was similar to that between 4 and 8 weeks. Conclusions Re-stenosis mainly expressed as fibrosis. At week 4,8 the fibrosis stabilized gradually with the lessening of inflammatory reaction. The content of collagen in re-stenosis tissue elevated within 4 weeks,and stablilized after week 4,it was in accordance with the pathological changes of re-stenosis.
5.Identif ication of Herba Sedi collected in the different habitats and periods by FT-IR

Objective: To explore the characteristics of IR spectra of the crude drug Herba Sedi collected in different habitats and seasons.Methods: The infrared spectra of all the samples of the whole plants were obtained by FT-IR spectroscopy,the sample powder tablets were used,and identifi cation features were determined through comparative study.Results: The harvest seasons of the samples are remarkly related to the positions(cm-1),intensity or ratios of some major absorbing peaks of the infrared spectra from the 15 samples of several different months and 4 habitats in Hubei Province,China.As for peak positions,the peak values(1625?7)cm-1 of all the samples in spring are less than 1623cm-1,but in autumn(April to October) are generally more than 1628 cm-1.And peak values(1057?16)cm-1 of the samples of spring are more than 1050 cm-1,but less than 1050 cm-1 for the samples of summer and autumn.Moreover,the habitats of samples are also related to IR spectra to some certain extent.Conclusion: The harvest seasons of Herba Sedi from Hubei Province can be preliminarily ascertained according to the IR spectra features.IR technique should be paid attention to in research and identifi cation of the harvest periods of crude plant drugs.
6.Establishment and characterization of a cell line derived from human ovarian mucinous cystadenocarcinoma

Objective To establish a cell line of human ovarian cancer, and study its characterization. Methods The cell line was established by the cultivation of subsides walls, and kept by freezing. The morphology was observed by microscope and electromicroscope. The authors studied its growth and propagation, the agglutination test of phytohemagglutinin (PHA), the chromosome analysis, heterotransplanting, immuno histochemistry staining, the analysis of hormone, the pollution examination and the test of sensitivity to virus etc. Results A new human ovarian carcinoma cell line, designated ovarian mucinous cystadenocarcinoma 685 (OMC685), was established from mucinous cystadenocarcinoma. This cell line had subcultured to 91 generations, and some had been frozen for 8 years and revived, still grew well. This cell line possessed the feature of glandular epithelium cancer cell. The cells grew exuberantly, and the agglutinating test of PHA was positive. Karyotype was subtriploid with distortion. Heterotransplantations,alcian blue periobic acid schiff (AbPAS),mucicarmine,alcian blue stainings, estradiol (E 2) and progesterone were all positive. Without being polluted, it was sensitive to poliovirus Ⅰ, adenovirus 7 and measles virus. Conclusions OMC685 is a distinct human ovarian tumous cell line.
8.Healing effect of porcine acellular dermal matrix combined with split-thickness autologous skin composites on full-thickness cutaneous wounds in SD rats

Objective To prepare a new type of micropore porcine acellular dermis matrix with the aid of laser (LPADM),and to validate the healing effect of the LPADM through the phrase Ⅰ composite transplanting on the back of the full- thickness skin defects in SD rats.Methods In vitro,the allogeneic fibroblasts were separately cultured with the LPADM (LPADM group) or the non-pore PADM (non-pore LPADM group),while fibroblasts cultured by pure medium were used as control.After culture of 1 day,3 days and 5 days,the contents of IL-10,IL-6,TGF-β1,LN,VEGF expressed by fibroblasts were determined by double-antibody sandwich ELISA method.In vivo,the phrase Ⅰ transplantations of LPADM graft with split-thickness autologous skin were carried on the backs of the full-thickness cutaneous defects of SD rats.The healing condition was observed and analyzed by histological tests.Results The differences of the absorbance value between the LPADMgroup,PADM group and control group in each day were not statistically significant (F=0.050-1.763,P>0.05).The transplantation of LPADM graft with split-thickness autologous skin graft resulted in high rate of surviving without signs of rejection 3 weeks later.After 1-month of transplantation,the regenerated skin was well enough to be lifted without any serious scars.Conclusions The phrase Ⅰ transplantation of LPADM graft with split-thickness autologous skin graft can accelerate the healing process of full-thickness skin wounds with high biological safety.
9.Quality control testing and evaluation for digital mammography

Objective To carry out the quality control testing and evaluation for three digital mammography systems.Methods The performance of three digital mammography systems was assessed by applying methods recommended in the European guidelines for quality assurance in breast cancer screening and diagnosis and Chinese specification for testing of quality control in X-ray mammography.The performance of X-ray generator of three digital mammography systems were tested and evaluated.CDMAM 3.4 phantom with four different thickness(30,40,50,60 mm) were exposured in DR,PCM,and CR system,respectively.The average glandular dose (AGD) value was measured and image quality figure (IQF) analysis was performed in each thickness.Results The X-ray machine performance of DR and CR was in accordance with existing standard,however the standard was inappropriate to evaluate part of X-ray machine performance of PCM system.The AGDs for system DR were 1.20,1.42,1.75 and 2.20 mGy for 30,40,50 and 60 mm PMMA thickness,respectively.The respective AGDs for system PCM and CR were 0.82,1.19,1.33,1.70 mGy and 0.59,0.88,1.47,2.19 mGy.For the same phantom thickness sequence,the IQFs were 21.36,21.57,27.25 and 30.58 for system DR,28.02,29.10,35.90,and 41.24 for system PCM,whereas they were 39.78,39.30,43.85 and 48.08 for system CR.Conclusions The AGDs of all three systems were in accordance with the values recommended in European guideline.The AGD and IQF could provide an effective way for performance assessment and constancy checks for digital mammography systems.
10.Effects of hypertonic saline on CD14/CD16 expression by monocytes and the levels of anti-inflammatory cytokines in patients sustaining traumatic hemorrhagic shock

Objective To investigate the expression of CD14/CD16 by monocytes and the anti-inflammatory effects of hypertonic saline plus dextran (HSD) in adult blunt trauma patients in hemonhagic shock. Method A total of 30 adult patients were eligible for inclusion in the study if they sustained blunt trauma from March to October 2007 and had at least one recorded episode of hypotension (systolic blood pressure ≤ 90 mm Hg) with clear evidence of blood loss (external or internal including the thorax, abdomen or retroperitoneum). Patients were excluded if they refused to participate, were admitted ≥ 6 hours after injury, were pregnant, or had chronic disease. The enrolled patients were randomly divided in a double-blinded manner into an HSD group which was administered 7.5% Nad plus 6% dextran - 70, and a control group which was administered 0.9% NaCl. A single 250 ml dose of either HSD or NaO was immediately administered to the patients in each of the two groups while they were in the emergency room. The primary outcomes were to measure the changes in CD4/CD16 expression by monocytes and the levels of anti-inflammatory cytokines, including tumor necrosis factor-alpha (TNF-α), interleukin (IL)-lra and IL-10. Patient demographics, fluid requirements, organ dysfunction, infection and death were recorded. Results A total of 28 patients were enrolled with no significant differences in their clinical measurements. Hyperosmolarity was modest and transient. HSD altered the shock-induced monocyte redistribution pattern by reducing the drop in the "classic" CD14 ++ subset and remarkably affecting the expansion of the "pro-inflammatory" CD14+CD16+ subsets. In parallel, HSD significamly reduced pro-inflammatory TNF-α production while increasing anti-inflammatory IL-lra and IL-10 production. Conclusions This human trial demonstrates that HSD has anti-inflammatory and immunologic properties for trauma patients in hemorrhagic shock. HSD exerts profound immunomodulatory effects, promoting more balanced pro-/anti-inflammatory responses and reducing post-traumatic complications. Therefore, it could be useful in attenuating post-trauma multiorgan dysfunction (MOD).
