1.Effect of strontium ranelate on bone resorption

BACKGROUND: Most of anti-osteoporosis drugs can only inhibit bone resorption, but can not improve the bone formation. So far, Strontium ranelate is the only anti-osteoporosis drug that has a dual effect on bone resorption and bone formation. Through the retrieval of related studies, the present review article evaluates the current understanding of strontium ranelate effects on bone resorption based on the pharmacologic experiments, clinical studies, action mechanisms. Recombinant human bone morphogenetic protein-2 (rhBMP-2) has been widely used on clinic; however, there are still few reports addressing rhBMP-2-induced osteogenesis in intervertebral disc.
2.Role of osteopontin in mediating mechanical stress effect on bone

BACKGROUND: After tooth extraction, residual ridge resorption is a complicate process, while stress is one of the main causes. Recent studies have demonstrated that osteopontin (OPN) is a kind of agent that is sensitive to mechanical stress.OBJECTIVE: To study the role of OPN in mediating mechanical stress effect on bone, and to investigate the relationship of OPN and bone rebuilding, action mechanism of OPN and cell surface receptor and the relationship of OPN and osteoblast and osteoclast, in order to reveal the molecule mechanism of residual ridge resorption mainly caused by stress, and also to be a basic theory for clinical molecule therapy or gene therapy.RETRIEVE STRATEGY: PubMed was searched from January 1985 to June 2007 for English literatures with the searching words of "OPN, mechanical stress and bone resorption" and China Journal Full-text Database was also searched from January 1995 to June 2007 for Chinese literatures with the searching words of "OFN, mechanical stress and bone resorption". Fifty-two literatures were retrieved. The data were primarily selected. Inclusive criteria: being relative to role of OPN in mediating mechanical stress effect on bone. Exclusive criteria: repetitive studies.LITERATURE EVALUATION: Among the 52 literatures, including 6 Chinese and 46 English, 5 were rejected for objective independent from this study, 13 for repetitive contents, and so 34 literatures were involved for further analysis on the role of OPN in mediating mechanical stress effect on bone, 6 being review and the others being clinical or basic studies.DATA SYNTHESIS: ①After tooth extraction, residual ridge resorption is a complicate process. It also influences the effect of artificial denture, while stress is the main cause. ② OPN is widely distributed throughout the body, and has many physiologic functions. Recent studies have demonstrated that OPN is a kind of agent that is sensitive to mechanical stress. ③Bone is a special kind of biological tissue, bone resorption and bone rebuilding both can take place when it is lack of mechanical stress (atrophy of disuse),or endures mechanical stress. ④Bone rebuilding can be seen as the results of these molecule events. OPN, as a main protein, participates in the bone resorption and bone rebuilding. ⑤These OPN can be seen as a chemotatic factor to guide prosoma osteoclast to migrate to the surface of the bone, and mediate it to combine with the integrin on the surface of osteoclast so to promote the resorption of bone. So, OPN is a necessary factor in mechanical stress induced bone loss. ⑥OPN is a signal of transmission between osteoblast and osteoclast. ⑦OPN can be seen as a long acting feedback inhibition signal to inhibit bone mineralization.CONCLUSION: OPN is a kind of agent that is sensitive to mechanical stress. It plays an important role in mediating mechanical stress effect on bone.
3.Botulinum toxin A in treatment of tic disorder

Objective To evaluate the efficacy of Chinese botulinum toxin A(CBTX-A)injection in the treatment of motor tics in patients with tic disorder.Methods Twenty-five patients(20 male,5 female)were treated with CBTX-A in the sites of their most problematic motor tics.Before treatment the data of Yale Global Tic Severityb Scale(YGTSS)was collected.All patients were assessed at least 3 times after 1 week,4 weeks and within 12-month period.At each visit the following data were collected:YGTSS,Jankovic clinical rating scale,time to response,duration of response,global impression of changes by patients,presence of premonitory sensory tic component and side effects.Results Total number of treatment sessions was 29.The mean response time was 5.0 days.The mean duration of response was 4.6 months.The mean peak effect response in Jankovic clinical rating scale was 3.4.Based on YGTSS,62.1% (18/29)was rated as marked improvement,34.5%(10/29)as moderate improvement and only 3.4% (1/29)as no effect.85.7%(18/21)patients had less premonitory sensation symptoms(mean benefit 72.4%(20%-100%)).Weakness in the site of injection and rash were the main side effect. Conclusion CBTX-A is an effective and safe treatment for motor tics associated with tic disorder.

Gut microbiota means a highly diverse and dense microbiota which inhabits in the gastrointestinal tract of humans.It plays a vital role in many important physiological processes including exerting nutritional and metabolic activities,regulating the immune system,and protecting against pathogens.Recent studies suggested that the gut microbiota was associated with the development of many lung diseases,such as pneumonia,asthma and tuberculosis.There is a gut-lung axis in human body.Now,the relationship between the gut microbiota and the gut-lung axis is reviewed.
5.Management of hematomyelia during microendoscopic discectomy: A preliminary discuss

Objective To explore the emergent measures for hematomyelia during microendoscopic discectomy (MED). Methods Different measures, such as electrocoagulation, compression, irrigation, or hemostatic administration, were directed against different types of hematomyelia during MED. Results No conversions to open surgery for hematomyelia were required in all the 287 cases in the study. A follow-up for 4~30 months (mean, 17 months) was carried out. According to the MacNab criteria, excellent results were achieved in 219 cases, good results in 59 cases, fair in 7, and poor in 2. Conclusions Measures for hematomyelia during MED include the decrease of abdominal pressure, strict hemostatic performance, keeping off varicose veins when the protruded intervertebral disc is resected, compression by using gelfoam or styptic cotton, and intravenous administration of thrombin. These measures can effectively improve the surgical safety and the curative effect.

Patients who complained of pain, tenderness mass in neck, fever, limitation of mouth opening, pharyngalgia,dysphagia,with lab showing increased Neu, remarkably decreased PLT, upsurging Glu, urine-Glu, ALT, T-Bil, AFP, were finally diagnosed to suffer from deep neck abscesses after CT scanning. What was worth mentioning was that diabetes, hepatitis C, thrombocytopenia, and fungus infection added to the complexity of the treatment.

8.Experimental Study on the Phaeohyphomycosis Caused by Ochroconis gallopava

Objective To study the morphology, physiology, biochemistry and molecular biologic characteristics of Ochroconis gallopava which was firstly isolated from a pulmonary phaeohyphomycosis in China and compare it with a standard strain to assist in the clinical diagnosis and treatment. Methods The clinical isolate and standard strain were inoculated on SDA, PDA, CMA, CDA, BHIA and OA, respectively to observe the colony morphology and growth rate, and slide cultures were prepared and were examined under light microscope and electron microscope. Thermo-tolerance study, cycloheximide resistance test, API 20C, antifungal susceptibility test and PCR for the internal transcribed spacer of rDNA were also performed. Results Ochroconis gallopava grew rapidly on most media. The colonies appeared to be brown in color with smooth surface. There were aerial mycelia on SDA and PDA. Microscopic observations revealed pale brown and septate hyphae with club-shaped conidia. The growth of O. gallopava was optimal at 35 ℃ and was inhibited by cycloheximide. The two strains assimilated the same 14 glycogens, but 1 different, in API 20C AUX. MICs of fluconazole, itraconazole, amphotericin B and terbinafine were 32 ~ 64 ?g/mL, 1 ?g/mL, 2 ?g/mL and 0.25 ~ 0.5 ?g/mL, respectively. PCR results showed that the ITS segment was 743 bp with 3 bp difference in both strains. The length of 18S region was 506 bp, their sequences were the same entirely. Conclusions The clinical isolate is proved as a domestic strain of Ochroconis gallopava at the level of morphology, physiology, biochemistry and molecular biologic characteristics.
9.Association between Expression of HDAC1 Protein and HPV16/18 Infection in Cervical Intraepithelial Neoplasia and Cervical Squamous Cell Carcinoma

Objective To explore the anomalous expression of HDAC1 in cervical intraepithelial neoplasia(CIN)and cervical carcinoma and its relationship with the infection of HPV16/18. Methods The expressions of HDAC1 protein and HDAC1 infection were detected by in situ hybridiza?tion(ISH)and immunohistochemistry staining(SP)in tissues from patient with chronic cervicitis(the chronic cervicitis group),CIN(the CINⅠgroup,the CINⅡgroup and the CINⅢgroup)and cervical squamous carcinoma(the cervical carcinoma group). Results The infection rate of HPV16/18 was 8.33%,34.78%,48.00%,65.21%,78.00%in patients with chronic cervicitis,CINⅠ,CINⅡ,CINⅢand cervical carcinoma re?spectively. There was significant difference between groups(P<0.05). The positive expression rate of HDAC1 was 0,13.04%,32.00%,47.82%and 76.00%in chronic cervicitis,CINⅠ,CINⅡ,CINⅢand cervical carcinoma,respectively. The differences were statistically significant(P<0.05). The positive expression of HDAC1 in the HPV16/18 infected patients was significantly higher than those uninfected with HPV16/18(P<0.05). The HDAC1 positive expression had no relationship with clinical stages of cervical carcinoma,however,it was related to the tissue differentia?tion,stromal invasion and pelvic lymph node metastasis(all P<0.05). Conclusion The infection of HPV16/18 may play an important role in the carcinongenesis of SCC through increasing the expression of HDAC1.
10.Studies on Susceptibility of Nine Antifungal Agents Against Dematiaceous Fungi by NCCLS M38-P Method

Objectives To evaluate the application of M38-P method recommend by NCCLS to dematiaceous fungi, and detect the susceptibility of itraconazole, fluconazole, miconazole, econazole, flucytosine, clotrimazole, nystatin, amphotericin B and terbinafine against dematiaceous fungi. Methods Using M38-P method forty-five strains of dematiaceous fungi were studied, which included 2 strains of Bipolaris spp., 1 strain of Drechslera spp., 1 strain of Curvularia spp.,14 strains of C. carrionii, 8 strains of F. pedrosoi, 3 strains of F. compacta and 16 strains of P. verrucosa. The media containing the fungi in the concentrations of (0.4 ~ 5) ? 106 CFU/mL were incubated at 35℃ for 5 ~ 7 days. Results The tested dematiaceous fungi were highly susceptible to itraconazole and terbinafine. Some of them were resistant to itraconzole, fluconazole, flucytosine and amphotericin B. Conclusions The data suggest that M38-P standards could be applied as the susceptibility testing of dematiaceous fungi, and it is necessary to monitor the resistant strains in antifungal treatment.
